최적화된 노인여가시설 구축을 위한 서비스디자인 기초연구 (A Basic study of Service Design for Optimization of the Public Elderly Recreational Place)

  • 본 연구의 목적은 고령화 시대를 대비하고 노인들이 행복한 여가시설을 구축하는 것이 중요한 과제라 인식하고 이에 물리적으로 행복한 노인 여가시설을 구축함을 위한 기초 연구로서 현재 노인들이 공공기관에서 제공하는 여가 시설에 대하여 활용성이 떨어지는 요인에 대하여 연구하였다. 구체적인 연구 방법 및 내용으로 첫째, 국내에서 가장 성공적인 공공 노인 여가시설의 모텔로 선정된 대구의 시니어스타워를 방문하여 단순설문 및 관찰을 통하여 장단점에 대한 요소들을 정리 및 KJ기법 및 그라프기법 (Graph Theory)을 적용하여 물리적 공간에 대한 한계성의 요소에 대한 우선순위 그리고 상호관계성을 발췌하여 우선순위 가이드라인을 구축하였다. 둘째, 실제 노인들이 여가시설에서 느끼는 만족정도를 알아보기 위하여 부산시 사상구 노인여가 시설에 방문 및 현지 노인들을 대상으로 설문을 실시하였고, 발췌한 우선순위법(Priority Method) 결과와 비교한 내용을 기반으로 최종적으로 개선 할 사항을 정리 하였다. 본 연구를 통하여 첫째, 물품 및 기기를 노인의 여가공간에 효율적으로 배치하기 위한 노력과 이를 실질적으로 다룰 수 있는 전문성도 중요하다는 것을 알 수 있었다. 둘째, 물리적 기기 및 프로그램을 활용한 노인여가 시설도 중요하지만 노인의 신체적, 정신적 특성을 고려하여 자발적으로 참여하기 위한 참여 형 공간도 중요하다는 것을 알 수 있었다.

Comparative analysis of energy intake and physical activity according to household type and presence of metabolic syndrome in middle-aged men based on data from the 7th Korea national health and nutrition examination survey (KNHANES) (2016-2018)

  • [Purpose] This study analyzed data from the 7th Korean National Health and Nutrition Examination Survey (2016-2018) to compare the differences in energy intake and physical activity (PA) levels in middle-aged Korean men aged 40-59 years according to household type and the presence of metabolic syndrome (MetS). [Methods] Data from 2,266 young adults (aged >39 years and <60 years) were obtained from the KNHANES. We analyzed the differences in energy intake and PA levels according to household type and MetS. The presence or absence of MetS was determined by measuring waist circumference, blood pressure, fasting blood glucose, triglyceride, and HDL-C levels. [Results] Regarding total nutritional intake, no relationship with MetS was observed regardless of household type (interaction between MetS and household type: p = 0.875). No differences in total PA were observed between multiple- and single-person households (interaction between MetS and household type: p = 0.122). The relationship between MetS and MetS components according to nutritional intake showed that participants with a higher energy intake had a 27% lower prevalence of low high-density lipoprotein cholesterol (HDL-C) (p < 0.05) and a 36% higher prevalence of high blood pressure (p < 0.01). The relationship between MetS and MetS components according to the PA level showed significantly reduced prevalence of MetS by approximately 39%, 35%, and 43% (p < 0.01, 0.01, and 0.001) in subjects who were somewhat active, active, and very active, respectively, compared to inactivity. [Conclusion] Middle-aged men showed no difference in nutritional intake and PA between multiple- and single-person households. However, the risk of MetS was significantly higher in participants with approximately 1.5 times the energy intake compared to the group with the lowest energy intake. Moreover, higher levels of PA had a marked positive effect on the risk factors and prevalence of MetS. Therefore, we suggest that reducing the total energy intake and increasing total PA are important for preventing MetS in middle-aged men regardless of household type.

다중감각 되먹임 장치를 이용한 자가 일어서기 훈련이 편마비 환자의 균형능력과 일어서기 동작 수행능력에 미치는 영향 (The Effects of Self-Sit-to-Stand Training Using Multi-Sensory Feedback Device on Balance Ability and Sit-to-Stand Ability in Hemiplegic Stroke Patients)

  • Purpose: The aim of this research was to investigate the effects of self-sit-to-stand training on balance ability and sit-to-stand ability in hemiplegic stroke patients using a multisensory feedback device. Methods: A total of 19 stroke patients participated in this study, and they were divided into two groups: 10 underwent self-sit-to-stand training using a multisensory feedback device, and 9 underwent sit-to-stand training with a physical therapist. In both groups, sit-to-stand training was performed for 30 min, 3 times a week, for 6 weeks. The subjects also underwent physical therapy twice a day for 30 min, 10 times a week, for a total of 60 sessions. Balance ability was evaluated using the AFA-50 and Berg Balance Scale. Sit-to-stand ability was evaluated using the five times sit-to-stand test. Results: Sway length, pressure, and total pressure all significantly increased in both groups, and there was no difference between the two groups. The Berg Balance Scale results showed that balance ability significantly increased in both groups, and there was no difference between the two groups. The five times sit-to-stand test results showed that sit-to-stand ability significantly increased in both groups, and there was no difference between the two groups. It was found that the self-sit-to-stand training using a multisensory feedback device had a positive effect on balance control and sit-to-stand ability. When the two groups were compared, no difference in balance ability or sit-to-stand ability was observed. Conclusion: The findings of this study indicate that self-sit-to-stand training using a multisensory feedback device is as effective as sit-to-stand training with a physical therapist. Hence, self-sit-to-stand training using a multisensory feedback device could be an effective home-based exercise protocol for hemiplegic stroke patients to improve their balance and sit-to-stand abilities.

고등학생의 체중조절에 대한 관련 행동 및 태도 (The Behavior and an Attitude for Weight Control of High-School Students)

  • The purpose of this study was based on students in high school to find out how interest eating and controlling weight on normal time by sex to let them get a better sense's of view on weight controlling so they can get healthy, and healthy school life and concentrate on studying, As a result of analyzing the data collected for the subject of high school students, the conclusions were as follows; First, out of 325 students, 44.6% were male students, and 179 were female students, Compared to the BMI, normal mass were 50.8%, which were 165 students, less weighted were 39.7%, which were 129 students, and over weighted were 9.5%, which were 31 student. Also average height for male students were 173.8 cm, for female students were 161.5 cm, average weight were 67.2 kg for male students, and for female students were 53.2kg. Using BMI analysing the results and the male students had an average of 22.2%, and the female students had an average of 20.3%, so male students were a little higher than the female students. Second, the interest rate for weight control were 82.2%, that's 267 students for, 'interested' and 7.8%, which were 58 student for 'not interested', so most students were interested, in controlling weight. Interest rate were 83.6% for male students, 81.0% were female students. BMI told that group of less weighted were 82.9%, group of normal were 79.4% and group of over weighted were 93.5%. The rate were all high not relating to BMI. Third, compared to the past, more people said 'normal'(41.3%), people who said 'a little fat'(36.3%) decreased, but they still think they are fat even though they are not, also when they are less weighted they still think they are fat. Fourth, for 'weight control, and food' both male and female said they were related, and for 'weight control and exercising' they also said they were related, but more male said that they were related, However for relations between' controlling weight and school's physical education class' the answers were usually disagree. Fifth, for the 'satisfaction of their present weight', both BMI and the students answered and this results were mostly same as the past result, so most students prefer to lose weight. Also, both male and female think that the reason they have this weight now is, because of 'the amount of exercises' and 'the amount of food they eat', so they find that it's related to each other. Sixth, for the experience on weight control, both male and female had experiences, and they answered 'exercising and food treatment' is the good way to control weight. Also for 'the reason they started to control there weight', both male and female answered, 'they thought there weight were not normal'. Seventh, 'Do you pick on food to control weight?' and 'Do you feel nervous before you eat?' and for last 'control of drinking water' the answer was all different, and both male and female answered negatively. Eighth, time wasted on exercising per day, for less than 30 minute were 81.5%, the form of exercises that students did were 'not much or walking on the way to school and way to home'. Usually for their free time, male students spend on exercising however female student did not. Also both female and male students showed that they like to exercise, but majority of female student disliked to exercise. To everyone's point of view 'like'(32.9%), 'like a lot'(20.9%), so everyone agrees. The knowledge information on controlling weight, they answered, usually found from commercials, newspaper or magazines and from parents or friend. From the past many high school students wanted to control their weight, so there should had been a good education on this, however there wasn't any of those education and still it doesn't exist. Also most school's education are arranged to entrance pressure so they have a lot of knowledge and informations to it, but for real they don't have any activity or actions on it. Through this research, we felt that, we should correct students with wrong understanding on controlling weight and wrong knowledge. Also we suggest to make an activity program for this.

한반도 평화통일 기반구축을 위한 군비통제 추진방향 (Direction of Arms Control to Establish Foundation for Peaceful Reunification in Korean Peninsula)

  • 한반도에서 평화통일의 기반을 구축하기 위해서는 비군사분야에 머물고 있는 남북교류협력 영역을 군사분야로 확대하여 군비통제를 적극 추진해야 할 것이다. 그동안 남북 간에 군비통제가 추진되지 못한 이유는 (1)남북 간 신뢰구축의 한계, (2)군비통제 자체의 기능적 한계, (3)남북 간 제도 구조적 한계, (4)국내 외 환경적 한계 등 태생적 요인이 자리 잡고 있기 때문이다. 첫째, 군비통제를 추진하기 위해 남북 고위급회담 수준의 정치적 대화가 선행되어야 하며, 협상 및 추진과정에서 '전략적 유연성'을 발휘해야 한다. 이를 위해 상황과 여건에 부합한 '신축적 상호주의'를 적용해야 한다. 둘째, 기존의 '선 신뢰구축, 후 군축' 원칙에서 탈피하여 새로운 절충적 방법으로 '신뢰구축 및 군축의 동시추진' 원칙을 모색해 나가야 한다. 즉, 군비통제의 낮은 수준이라 할 수 있는 군사적 신뢰구축과 합리적 충분성에 입각한 제한된 군축을 병행하는 방안을 추진할 필요가 있다. 셋째, 국무총리실 직속으로 군비통제 전담기구를 설치하여 국가정책 전략 차원에서 군비통제 문제를 적극적으로 추진해야 한다. 또한 남북 고위급회담이 성사되면 '남북 군비통제 공동추진위원회(가칭)'을 구성 운영할 필요가 있다. 넷째, 평화통일을 위한 군비통제의 필요성에 대한 국민적 공감 형성과 유리한 국제적 환경조성을 위하여 보다 능동적인 외교역량을 발휘해야 할 것이다. 특히 국제사회와 함께 북한의 핵문제를 해결해 나가고, 한미동맹과 한중협력관계의 균형을 어떻게 유지해 나갈 것인가를 더욱 고민해야 할 것이다.

허약노인을 위한 방문재활 프로그램의 장애발생예방 효과에 대한 연구 (Effects of Visiting Prehabilitation Program against Functional Decline in the Frail Elderly: A Prospective Randomized Community Trial)

  • 본 연구는 허약노인의 장애발생예방을 위하여 개발된 지역사회 방문재활 프로그램의 효과성을 평가하고, 허약노인 대상의 방문재활사업이 공공부문의 보건기관에서 실제 수행될 수 있도록 그 근거를 마련하는 것을 목적으로 하고 있다. 2009년 4월부터 8월까지 맞춤형 방문건강관리사업에 등록된 저소득 취약계층 노인들 중 110명의 허약노인을 선정하여, 이들을 대상으로 지역사회 중심의 실험연구를 수행하였다. 연구대상자는 무작위 방식으로 실험군(n=50)과 대조군(n=60)으로 나뉘어, 이 중 실험군은 3개월 동안 상·하지 근력강화, 보행능력 강화, 균형능력증진 등에 초점 맞추어진 허약노인 방문재활 프로그램에 참여하게 하였고, 대조군은 가정방문을 통한 낙상예방교육을 받게 하였다. 프로그램의 평가를 위해 1차 결과지표로서 신체기능척도, 간이신체기능 평가를 프로그램 사전·사후에 측정하였고, 그밖에 운동수행능력검사, 영양지표, 정서기능, 입원 및 낙상경험 등을 측정하였다. 연구결과 방문재활 프로그램에 참여한 대상자에서 신체기능척도와 간이신체기능평가 등 주요 노인기능지표의 유의한 향상이 관찰되었다. 신체기능척도에 대해서 실험군은 1.3 ± 3.8점의 상승이 있었던 반면, 대조군에서는 1.1 ± 5.4점의 감소가 있었다 (p=.020). 간이신체기능평가에 대해서도 실험군은 2.4 ± 2.0점의 상승이 있었던 반면, 대조군에서는 0.3 ± 1.5점의 상승만이 관찰되었다 (p<.001). 그밖에 허약노인을 위한 방문재활 프로그램은 운동수행능력, 정서기능, 반복낙상 횟수 및 낙상 후 기능저하 발생에 대해서도 유의한 효과를 보여주었다 (p .002-.038). 향후 우리나라 노인들의 장애발생을 예방하고 이로 인한 중증 장기요양단계로의 진입을 억제하기 위하여 허약노인 대상의 방문재활 프로그램과 이에 근거한 건강관리사업이 보다 적극적으로 확대·보급되어야 할 것이다.
