The Study on the Development and Effectiveness of Parent Role Education Program (영아건강증진을 위한 부모역할교육 프로그램의 개발 및 그 효과에 관한 연구)

  • Purpose: This study was conducted to evaluate the effectiveness of the maternal role education program for primiparas in mother-infant interaction, childrearing environment, and infant development. Method: A Non-equivalent control group time-series design was used. For the intervention group, programmed parenting education focusing on mother-infant interaction, home environment for infant development, and parent counseling and support was provided via home visits or telephone for twelve months. Result: Significant differences were found in the mother-infant interaction feeding scale at one and three months, but no differences were found in the teaching scale at six and twelve months between the intervention and control groups. Also, the difference in childrearing environment (HOME) between the two groups was significant at three, six, twelve months. In addition, the intervention group showed higher GQ in the Griffiths mental development scale at three and six months. In multiple regression analysis, 22.6% to 43.6% of infant development was explained by HOME, mother-infant interaction, and previous development. Conclusion: The maternal role education program proved to be effective in promoting mother-infant interaction, organizing the childrearing environment, and fostering infant development.

The Follow-up Study of Changes in Frailty in Elderly Receiving Home Health Care of the Public Health Center

  • Purpose: The purpose of this study was to follow-up the frailty of the old who received home health care by Registered Nurse in Public Health Center over 8 years. Methods: We used the second wave data which was a comprehensive longitudinal data set, Public Health Information System of a public health center located in Seoul from 2010 to 2018. For statistical analysis, a mixed model of repeated measures by R program was used. Results: Frailty (range: 0~31) was getting worse significantly from 5.38 on registration to 6.54 on 4th year, 7.40 on 7th year, 7.69 on 8th year with adjustment for age, sex, economic status, the number of family, and the number diseases. The coefficient of parameters with frailty change was serviced year (β=0.29, p<.001), age (70~79 to 60~69; β=0.98, p=.018) and sex (female to male; β=2.55, p<.001). Conclusion: This study showed that the home visiting health service needs to take attention to aged 70s and over, female. The home health care of public health center need to be extended more practical and effective services in terms of 'community care'and 'ageing in place'.

Wound Care Needs of Hospital-based Home Care Nursing at Elderly Care Facilities (노인요양시설의 간호인력과 비간호인력간 상처관리에 대한 병원중심 가정간호 요구도 비교)

  • Background: The purpose of this study is to investigate the needs for wound care services in the elderly care institutions by analyzing hospital-based home care nursing in select facilities in Korea. Methods: A total of 92 staffs at the elderly care institutions, located throughout the country, completed self-report questionnaires between February 1 and November 31, 2009. SPSS ver. 17.0 was used for data analysis, regarding frequency and chi square. The 5% level of significance was the critical level for acceptance of the study's hypotheses. Results: Results were as follows; group A and group B were rated as 'high' and 'very high', respectively, in care of simple pressure ulcer, care of complicated pressure ulcer, stitch out, care of simple surgical wound, ostomy care, complicated ostomy care, simple dressing and complicated dressing. Conclusion: Political decisions intended to promote visits by hospital- based home care nurses to the elderly care institutions would be a prudent course.

A Study on Home Care Service and the Level of Client Satisfaction in Community (일 지역사회의 가정간호서비스 내용 및 만족도 조사)

  • Purpose : This study was done to examine the home care service provided by home care nurses and the level of client satisfaction in community. Method : Data were collected from 120 clients who received the home care service at the community home care service center in Pusan from 2th May. 2003 to 31th May 2003. The tool for measurement of satisfaction was composed of 16 items and was 4 score scale by Kim. Data were analyzed by using SPSS/WIN 10.5 program. Result: 1) 67.5% among 120 clients was over sixty years old and 30.6% of clients received home visits 20 times by home care nurse. 2) The total number of home care services was 15,783. And most of the clients received therapeutic nursing care. 3) The mean score of satisfaction on provided home care services was 3.31, among 16 items, "The home care nurses kept up the secret of patients and family" was the highest($3.67{\pm}0.48$). the total level of satisfaction of home care services was very high, with mean score $3.31{\pm}0.32$. Conclusion : The satisfaction level of home care services was very high. so we can expect that the prospect of home care services is very challengeable. Therefore we should try to expand the service recipients with promotional education to assure close ties with the public health centers for the continuous home care service linking and make more efforts to improve the quality of the service.

A Preliminary Study on the Classification of Visiting Nursing Service Recipients and the Development of Standardized Visiting Nursing Service Pathways Based on Public Health Center (대도시 보건소 동단위 방문간호 대상자의 군분류 및 표준 방문간호서비스 경로 개발을 위한 기초연구)

  • Purpose: The purpose of this research is to develop and classify district visiting nursing standards and to standardize visiting nursing service pathways. Method: This research was conducted as a focus group study and analyzed visiting nursing records. We surveyed 201 recipients at urban health centers, who were selected through convenient sampling, from April 2003 to November 2003. Result: First, visiting nursing service recipients were classified into four groups according to household and financial characteristics, existence of disease, ability of self-care, and existence of home care service needs. Standardized pathways of the selected items were assessment. nursing care plan, disease management and promotion of self-care ability for Level I (mean=12.2 visits). For Level II (7.3 visits) were offered assessment. disease management. health education. and health promotion services. For Level III (5.2 visits) were offered assessment. disease management. health education and health promotion services, and for Level IV (2.7 visits) were offered thorough assessment, education for self-care and health promotion. Conclusion: The visiting nursing service pathways identified in this research need to be developed further as basic materials applicable to quality assurance and agency evaluation. For this, we suggest repeated research and test to apply the derived standardized visiting nursing services pathways in visiting nursing programs.

Home Care Services Utilization and Satisfaction for Clients with Cancer (항암화학요법 환자의 가정간호 이용 양상과 간호 만족도)

  • This study was to identify the utilization of home care service and home care nursing satisfaction for clients with cancer who had completed secondary and tertiary chemotherapy session. Raw data was collected by reviewing charts and questionnaire of 23 clients with cancer between September, 2002 and November 2002 at an university hospital located in Seoul. Korea. The result are as follows: 1. Characteristics in the use of home care: After applying for home care service, patients were visiting an average of 1.88 days later. The major purpose of using home care service was to help recovery after hospitalization or to maintain present health. The average period for home care service was 14.6 days, with 4 visits over this period, for 35.78 minutes per visit. 2. Content of home care service: A total of 47 items of service were provided - basic nursing care, education and counseling, and therapeutic nursing care. On every visit, an average of 19 items of home care service were provided, and the majority (7 items) were therapeutic nursing care. 3. Satisfaction with home care services: Satisfaction was very high, an average of 3.88 on a scale of 4 points. Both patients and families expressed high satisfaction with all sub-domains of nursing care: guidance by the home care nurses, knowledge, skill, attitude, interpersonal relationships, emotional support, and accessibility. In conclusion, home care served basic nursing care, teaching and counseling, and therapeutic nursing to clients with cancer going through secondary or tertiary chemotherapy. Therefore they changed knowledge and attitude to disease and treatment which were difficult to change.

A Study on Home Care Service and the Level of Client Satisfaction (일 대학병원의 가정간호서비스 내용 및 만족도 조사)

  • This study was done to examine the home care service provided by home care nurses and the level of client satisfaction. Data were collected from 110 clients who received the home care service at the P-hospital in Pusan from 23th April, 2001 to 30th November, 2002. The tool for measurement of satisfaction was composed of 16 items and was 4 score scale. Data was analyzed by using SPSS/WIN 10.5 program, the results of this study are summarized as follows; 1) 77.3% among 110 clients were over sixty years old. By the disease pattern. subjects were distributed into cancer(52.7%), cerebro-vascular disease(22.7%) and the others. 36.4% of clients were received home visits from 10 to 20 times by home care nurse. 2) The total number of home care services was 20,828. And most of the clients received the health education and training. 3) The mean score of satisfaction on provided home care services was $3.36\pm0.45$, out of 4. among 16 items. 'The home care nurses were kind enough' was highest ($3.59\pm0.49$), the total level of satisfaction of home care services was very high. with total mean score $53.84\pm7.16$. As mentioned above. the satisfaction level of home care services was very high. so we can except that the prospect of home care services is very challengeable. Therefore. we should try to expand the service recipients with promotional education to the home care clients under the cooperation with medical staff and make more efforts to develop the management system about clients' information. to improve the quality of the service, to assure close ties with the public health centers for the continuous home care service linking.

Outcomes of Home Care Service for Coronary Artery Bypass Graft (관상동맥우회술 환자의 가정간호 효과)

  • This study was to compare the functional status. complication and readmission rates. and client satisfaction with nursing care of home-based care and hospital-based care for clients with Coronary Artery Bypass Graft. Raw data were collected by interviewing and reviewing charts of 41 clients with Coronary Artery Bypass Graft between June 2001 and July 2002 at an university hospital located in Seoul. Korea. Out of 41 clients. 15 were in home care group and 26 were in hospitalization group. The baseline characteristics of the groups were almost identical. Mean age was 61.7 and 75.6% of clients being male. For home care group. the data collection was made at discharge and at termination of home care. and for hospitalization group. at discharge and at the first visit of outpatient department. Complication and readmission rates were investigated at one month after operation. Collected data were then analysed by conducting Chi-square test. Wilcoxon rank sum test. and Wilcoxon signed ranks test with SPSS program. The level of significance was .05. The results of the study are summarized as follows: 1. Postoperative length of stay of the home care group was shorter than that of hospitalization group by 1. 14 days(8.45 days vs. 9.59 days). On average. 1.8 home visits per client were observed. 2. The functional status (Barthel Index) at the termination of home care was significantly increased from that at discharge. For hospitalization group. a significant increase was observed between the functional status at the discharge and that at the first visit of outpatient department. The differencies in incremental of the scores. between the groups. were however not significant. 3. Complication and readmission rates; no statistically significant difference between the groups was observed. 4. The client satisfaction with nursing care (CSS) at termination of home care was significantly higher than that at hospital discharge. In conclusion. the outcomes of the analysis suggest that the home care benefits clients with Coronary Artery Bypass Graft. Client satisfaction with nursing care rises at termination of home care as compare to that measured at hospital discharge. Meanwhile. there was no significant differences in functional status. and complication and readmission rates. Further. home care reduced the length of stay in hospital.

Evaluation of a Community-Based Rehabilitation Program in Wonju City (원주시 지역사회중심 재활사업 평가)

  • This study evaluated the community-based rehabilitation services provided by the Wonju Public Health Center from Jan. 2000 to Dec. 2002. Ninety-four persons with disabilities dwelling in the community participated and the surveys were completed in an interview during home visits. The respondents' demographic, socio-economic, and medical characteristics, rehabilitation service received, willingness to receive home-visit rehabilitation services, and satisfaction with the rehabilitation services were analyzed by frequency and percentage. A Likert scoring system consisting of five agreement-disagreement categories was applied to each item, consisting of Very Satisfied, Satisfied, So-So, Poorly Satisfied, and Very Poorly Satisfied. The major findings were as follows: 1) The rehabilitation services used included medical rehabilitation (26.9%), followed by social assistance (23.5%), diagnosis by a physician at home (17.3%), medical examination (12.3%), housekeeping services (6.2%), and vocational and educational rehabilitation (3.5%). 2) Of the medical services, the respondents desired physical therapy at home and free rental of rehabilitation equipment, such as wheelchairs, canes, walkers, the most, followed by home visit occupational therapy, nursing services, and oriental medicine service in descending order. 3) Some of the respondents expressed so-so satisfaction (50.0%) or dissatisfaction (16.9%) with the rehabilitation services provided by the Wonju Public Health Center. These findings should prove useful when planning or extending community-based rehabilitation programs for the homebound disabled in the community.

Effects of Holistic Healthcare Home Visit Intervention Program for Multi-cultural Couples

  • This study was to identify the effects of home visit intervention program for Holistic Healthcare of multi-culture Couples. It used a non-equivalent control group pretest-posttest design quasi-experimental research. The subjects of this study consist of 20 couples for experimental and control groups. Multi-cultural couples comprised of foreign women married to Korean men in farm and fishery areas. The research tools were used stress index by SA-3000P (Medicare co. Ltd. KOREA), Center for Epidemiologic Studies-Depression Scale (CES-D), and World Health Organization Quality of Life (WHOQOL-Bref). The experimental group of the wives had lower stress index than the control group, the experimental group of the husband showed lower stress index than the control group, (t=-3.14, p=.002). The wife (t=-3.75, p=.001) and husband (t=-4.20, p=.001) of the experimental group showed lower depression scores than the control group. Both the wife (t=3.86, p=.001) and husband (t=5.28, p=.001) showed higher scores for quality of life compared to the control group. It was found that this holistic healthcare home visit program is an effective program to make improvements on stress, depression and quality of life for multi-cultural couples. Therefore, as the intervention program developed in this study is home visits for the holistic health of multi-cultural couples, and it is judged that it can be implemented at the local social health centers or healthcare centers.
