Allergy Prevention Effect of Oral Administration of Quercetin on Picryl Chloride-induced Contact Dermatitis in BALB/c Mice (BALB/c mice에서 quercetin의 경구투여가 picryl chloride로 유발된 접촉성 피부 알레르기의 예방에 미치는 영향)

  • The number of patients with immune- mediated dermatitis such as contact dermatitis is increasing year by year. Allergic contact dermatitis is a complex phenomenon that involves resident epidermal cells, fibroblasts, and endothelial cells, as well as invading leukocytes that interact with each other under the control of a network of cytokines and lipid mediators. It is a cell-mediated immune reaction, which occurs after susceptible individuals are exposed to sensitizing chemicals, and characteristic eczematous reaction is seen at the point of contact with an allergen. In this study, we investigated the allergy prevention effects of quercetin on mast cell-mediated allergic inflammation in BALB/c mice. BALB/c mice were sensitized with 40 ${\mu}l$ of 1.5% picryl choloride (PCL) to the left and right ear each. Total serum IgE levels and histamine levels were measured by the sandwich ELISA method using mouse IgE, histamine measuring kit. For histopathological examination, paraffin sections were stained with hematoxylin and eosin(HE) or toluidine blue(TB). Ear swelling responses were much weaker in the high-dose group (100 mg/kg) than the control group (0 mg/kg). The number of mast cells showed a significant decrease in the high-dose group (100 mg/kg) compared to the control group (0 mg/kg). Degranulation of mast cells was also confirmed by Toluidine Blue (TB) staining method. Both total serum IgE and histamine levels were significantly decreased in the high-dose group (100 mg/kg) compared to other groups. These findings suggest a certain relationship between the elevation of IgE, histamine levels and the degranulation of mast cells. These results show that the pharmacological actions of quercetin indicate its potential activity for prevention of allergic inflammatory diseases through the down-regulation of mast cell activation.

Histopathology and residues in fresh water fish exposed to acute and chronic copper and mercury toxicity

  • A total number of 668 apparently healthy fish were obtained from farm to study the effect of two heavy metals (Copper and Mercury) on histopathology of liver, kidney, spleen, gills and muscles also residues in muscles. The $LC_{50}$/96 hr. of Cu and Hg were estimated and fish exposed to 1/2 $LC_{50}$ for 7 days and for 1/10 $LC_{50}$ for 8 weeks from each product separately. Histopathological findings in acute and chronic mercuric chloride toxicity revealed degeneration and necrosis in the glomeruli, interstitium tissue and epithelium lining renal tubules. The tubular epithelium became necrotic at several places. Eosinophilic hyaline droplets is exist in the cytoplasm of the necrosed cells. Degenerative changes and hyperactivity in melanomachrophage center was seen in the spleen together with some necrotic areas. Necrosis and aggregation of melanomachrophage were seen in the hepatic cells, Hepatic cells showed vacuolar degeneration in the hepatic cells. Gills showed loss in the lamellae of the filaments associated with edema, inflammatory cells infiltration and haemorrhages in the arch. The sarcoplasm of the bundles of the skeletal muscle showed granular degeneration and focal inflammatory cells infiltration between the hyalinized bundles. Mercury residues obtained from these studies in the acute toxicity were 0.22 ppm/gm in the 2nd day, 0.411 ppm/gm in the $5^{th}$ day ended with 0.96 ppm/gm in the $7^{th}$ day. In chronic toxicity it was 1.1320, 1.7140, 2.3620 and 3.5640 ppm/gm respectively from the $2^{nd}$ to the $8^{th}$ week of exposure. In acute and chronic copper toxicity, there was degenerative changes in renal tubules. Melanophores aggregation in the wall of the blood vessels of the spleen and depletion of some of the melanophores in the melanomachrophage were seen together with necrosis in some areas. Congested Mvs (Micro vessels) and vacuolation of hepatocytes were observed. Some areas of hemorrhage and melanophores vacuolar degeneration in the liver were seen. There was mitosis in some areas with displesia of hepatopancreatic cells and eosinophilic granular cells aggregation. Zymogen granules disappeared and there were dyplastic hepatocytes. Congestion in the blood vessels of the gill filaments, associated with massive number of granular eosinophilic cells infiltration were seen in the base of the filaments. There were sever vacuolization and hyalinization in the skeletal muscle bundles. Detection of residues of copper sulfate revealed increase of the amount of copper measured in ppm/gm comparing to the normal control starting from 0.60 ppm/g in the $2^{nd}$ day, 0.67 ppm/g in the $5^{th}$ day and 0.67 ppm/g in the $7^{th}$ day. Result obtained in chronic copper sulfate toxicity revealed gradual increase of the amount of copper which ranged from 0.18 ppm/g at the $2^{nd}$ week to 0.21 ppm/g in the $8^{th}$ week of exposure.

Commonest Cancers in Pakistan - Findings and Histopathological Perspective from a Premier Surgical Pathology Center in Pakistan

  • Context: There are no recent authoritative data about incidence and prevalence of various types of cancers in Pakistan. Aim: To determine the frequency of malignant tumors seen in our practice and provide a foundation for building a comprehensive cancer care strategy. Materials and Methods: 10,000 successive cases of solid malignant tumors reported in 2014 were included. All cases had formalin fixed, paraffin embedded specimens available and diagnosis was based on histological examination of H&E stained slides plus ancillary studies at the Section of Histopathology, Department of Pathology and Laboratory Medicine, Aga Khan University Hospital, Karachi. The latest WHO classifications were used along with the latest CAP protocols for reporting and the most updated TNM staging. Results: There were 9,492 (94.9%) primary tumors while 508 (5.1%) were metastatic. Some 5,153 (51.5%) were diagnosed in females and 4,847 (48.5%) in males. The commonest malignant tumors in females were breast (32%), esophagus (7%), lymphomas (6.8%), oral cavity (6.7%) and ovary (4.8%), while in males they were oral cavity (13.9%), lymphomas (12.8%), colorectum (7.9%), stomach (6.9%) and esophagus (6.6%). Malignant tumors were most common in the 5th, 6th and 7th decades. About 8% were seen under 20 years of age. Conclusions: Oral cavity and gastrointestinal cancers continue to be extremely common in both genders. Breast and esophageal cancers are prevalent in females. Lung and prostate cancer are less common than in the west. Ovarian cancer was very common but cervix cancer was less so.

Clinicopathological and Histopathological Findings of Experimental Lead Poisoning in Dogs (실험적 납 중독견의 임상병리학적 및 조직병리학적 소견)

  • The study of present study was to determine the valuable laboratory tests for the differential diagnosis of lead poisoning in dogs. Sixteen mongrel dogs were divided into 2 experimental groups (A and B) and a control group (C). The A and B groups were administered orally 2 mg and 20 mg of lead per kilogram of body weight for 49 days, respectively. In addition to clinical observation, blood, urine and hair samples were collected on appointed day and examined for hematological changes, lead content of serum, whole blood and hair, and urinary $\delta$-aminolevulinic acid concentrations. All dogs were necropside on 49th day and examined for the lead content and histological changes of organs. The results obtained were summarized as follows: The group B showed digestive and nervous signs, and weight loss. The group A showed no significant hematological changes except polychromatophilla on the 7th day. But group B showed polychromatophilia as well as mild anemia and nucleated erythrocyte on the 7th and 35th day. Basophlic stippling erythrocytes were observed in some of the group B on the 14th day. The lead content of whole blood was increased significantly in both A and B groups on the 21the day. The urinary $\delta$-aminolevulinic acid content was increased in both A and B groups on the 7th day. The hair lead content of A and B groups was increased significantly on the 49th and 21th day, respectively. The lead contents of organs including liver, kidney, spleen, muscle and bone were increased significantly in group B. Histopathologic changes were characterized by hemorrhages, necrosis and intranuclear inclusion body in the epithelial cells of convoluted tubles of kidney, cloudy swelling and degeneration and/or necrosis of liver, enlargement of Virchow-Robin space, and swelling of endothelial cells and hyperplasia of the pericytes of brain. From these results it may be concluded that examination of nucleated erythrocyte/polychromatophilia, urinary $\delta$ -aminolevulinic acid, and whole blood and hair lead contents is a reliable clinico-pathological diagnostic methods, and that examination of the Virchow-Robin space, endothelial cells and pericytes of brain as well as intranuclear inclusion body in the epithelial cells of convoluted tubles of kidney is valualble postmortem diagnostic methods for lead poisoning in dogs.

The effect of antioxidant on pulmonary lesions induced by bleomycin in rats (항산화제가 bleomycin에 의해서 유발되는 랫트 폐장병변에 미치는 영향에 관한 연구)

  • The purpose of this study was to investigate the effect of antioxidants (vitamin E, selenium, and coenzyme $Q_{10}$) on the bleomycin-induced pulmonary lesions in male rats. Sprague-Dawley male rats were divided into 4 treatment groups ($T_1$, $T_2$, $T_3$, $T_4$) and 4 control groups ($C_1$, $C_1$, $C_3$, $C_4$). The treatment groups of rats weie given a single intratracheal dose of bleomycin (1.5 units/rat) and control groups of rats were given a single intratracheal dose of normal saline (0.15ml/rat). The rats in the $T_1$ group and $C_1$, group were dosed with normal saline (0.5ml/kg/day), the rats in the $T_2$ group and $C_2$ group were dosed with vitamin E (50mg/kg/day), the rats in the $T_3$ group and $C_3$ group were dosed with sodium selenite (3mg/kg/day) and the rats in the $T_4$ gronp and $C_4$ group were dosed with coenzyme $Q_{10}$ (2.5mg/kg/day) intraperitoneally for 7 days or 14 days, respectively. Animals were killed at 7th and 14th day after dosing with bleomycin or saline. The results obtained were as follows: 1. Lung wet weight of treatment groups of rats was increased significantly while body weight gain of them was decreased significantly in comparison with that of control groups of rats (p<0.01). 2. The ratio(%) of lung wet weight to final body weight of treatment groups of rats was increased significantly in comparison with that of control groups of rats (p<0.01). 3. The main histopathological findings of lungs observed in rats at 7th day after dosing with bleomycin were proliferation of the type II alveolar epithelial cells and fibroblasts, increased invading of macrophages into lesions, round cell infiltration and perivascular edema. 4. Lung fibrous tissues were markedly increased in rats observed at 14th day after dosing with bleomycin. 5. Pumonary lesions observed in rats dosed with bleomycin and antioxidants(vitamin E, selenium, coenzyme $Q_{10}$) were not significantly different from those of rats given bleomycin alone.

The Safety and Efficacy of Mediastinoscopy in Non-small Cell Lung Cancer (비소세포폐암에서 종격동경검사의 안전성과 유용성)

  • Background: Mediastinal lymph node metastasis is an important factor for staging and prognosis of non-small cell lung cancer (NSCLC), so accurate diagnosis is essential for treatment. Mediastinoscopy provides histopathological diagnosis of mediastinal lymphnode metastasis in NSCLC. The efficacy of mediastinoscopy was investigated. Material and Method: From Jun, 1999 to Aug, 2005, mediastinoscopic lymph node biopsy was performed to 348 patients with NSCLC. Patients characteristics, radiologic findings, mediastinoscopic results and pathologic stages were evaluated for investigation of safety and efficacy of modiastinoscopy in NSCLC. Result: There was 263 male and 85 female patients and the mean age was $62.1{\pm}8.5$ years. By radiologic study for mediastinal lymph node metastasis, 203 patients were negative and 145 patients were positive. Mean procedure time was $55.5{\pm}16.5$ minutes and biopsy was peformed at $2.2{\pm}1.0$ lymph node stations. There were only transient complications (1.7%) during the procedure, without other complication and mortality. There was 7.8% of false negative result in mediastinoscopy. Sensitivity (77.5% vs 71.9%, p=0.012), specificity (100% vs 74.4%, p=0.00), and accuracy (92.2% vs 73.6%, p=0.00) of mediastinoscopy were more superior than that of radiologic study for the diagnosis of mediastinal lymph node metastasis in NSCLC. Conclusion: Mediastinoscopy is a safe and effective modality for diagnosis of mediastinal lymph node metastasis in NSCLC.

The Effect of Lactose and Calcium on the Acute Lead Poisoning in Rats (白鼠에서 乳糖과 칼슘이 急性 납중독에 미치는 영향)

  • This study was performed to investigate the effect of lactose in 4 different concentrations against the protective effect of calcium on the acute lead poisoning in rats after 4 weeks treatment. In this animal experiment, 70 albino male weanling rats (50-70g of body weight) of Sprague-Dawley strain were used. Lead was dissolved in the distilled water and intubated at the dose of 400mg lead (as acetate)/ kg of body weight/day. Calcium and lactose were administered in drinking water ad libiturn dissolved with the solution of 0.7% calcium gluconate mixed with 40, 80, 160 and 320mM lacotse respectively. The results obtained were summarized as follows: 1. The rate of body weight gain in all treated groups turned out to be lower than that in the control group during 4 weeks treatment. The slow-down of body weight gain was the most significantly observed in the group treated with lead only ( p < 0.05). 2. The relative spleen weight in lead only treated group was significantly lower than that of lead + calcium, lead + calcium + 80mM lactose treated group ( p < 0.05). 3. The value of RBC, WBC, Hb and Hct showed a decreasing tendency in the group treated with lead only ( p < 0.05), however, a significant decrease was not observed in the group treated with lead + calcium. On the other hand, the protective effect of calcium was deteriorated in the group treated with lead + calcium + lactose. 4. The activity of $\delta$-aminolevulinic acid dehydratase ($\delta$-ALAD activity) showed the same tendency as No. 2. 5. The lead concentration in the blood (PbB) showed an increasing tendency and the interrelation among the different groups was also identical with No. 2. 6. With a statistical approach, it was found out that the activity of $\delta$-ALAD and the lead concentration in the blood show a relation of inverse proportion(r=-0.7301). The diagram was interpreted with the logarithmic equation InY = 5.5357-0.0251X (X:PbB, Y:$\delta$-ALAD activity). 7. In the histopathological findings of the kidney, the protective effect of calcium was observed. However, the protective effect of calcium was restricted in the group treated with lead + calcium + lactose. As a conclusion, the intensity of the acute ingested lead poisoning was obviously reduced by calcium, however, the protective effect of calcium was deteriorated in proportion with the concentration of the lactose to be administered. On the other hand, it was also noted that the deterioration was lightly restrained in the group treated with the physiological concentration of 80mM lactose than the results shown in the groups treated with lactose of other concentrations.

Histopathological Study of Gastric Adenoma (위선종에 관한 병리조직학적 연구)

  • A total of 53 gastric adenomas from endoscopically biopsied gastric mucosa were examined histopathologically. The average age at the time of endoscopic biopsy was 59 years, and gastric adenomas were found to be more frequent in the aged, particularly above the age of 50. The majority of adenomas occurred at the antrum. Concerning the shape of the adenomas, Yamada type II was more frequent(55%). All adenomas were accompanied by varying degree of intestinal metaplasia, and this findings suggest that gastric adenoma develops from intestinal metaplasia. In adenomas with severe atypia(grade III), endocrine cells(argyrophil and argentaffin cells)were markedly decreased or absent. Gastric adenocarcinomas coexistent with adenoma were seen in 5(9.4%) out of 53 cases, and were more frequent in male than female patients(sex ratio, 4 : 1) and the average age was 61.4 years. It is suggested that there is a necessity of thorough follow-up study for definitive correlation between gastric adenoma and adenocarcinoma.

Comparison between Early and Late Onset Breast Cancer in Pakistani Women Undergoing Breast Conservative Therapy: is There any Difference?

  • Background: Early onset breast cancer is associated with poor outcomes but variable results have been reported. It is a significant problem in Pakistani women but remains under reported. Breast conservation plays an important role in surgical management of this younger patient group. The objective of this study was to determine the outcome of breast conservative therapy in patients with early onset breast cancer in our population and compare it with their older counterparts. Materials and Methods: A review of patients with invasive breast cancer who underwent breast conservation surgery at Shaukat Khanum Cancer Hospital from 1997 to 2009 was performed. Patients were divided into two groups i.e. Group I age ${\leq}40$ and Group II >40 years. A total of 401 patients with breast cancer were identified in Group I and 405 patients in Group II. Demographics, histopathological findings and receptor status of the two groups were compared. The Chi square test was used for categorical variables. Outcome was assessed on basis of 10 year locoregional recurrence free survival (LRRFS), disease free survival (DFS) and overall survival (OS). For survival analysis Kaplan Meier curves were used and significance was determined using the Log rank test. Cox regression was applied for multivariate analysis. Results: Median follow up was 4.31 (0.1-15.5) years. Median age at presentation was 34.6 years (17-40) and 51.9 years (41-82) for the two groups. Groups were significantly different from each other with respect to grade, receptor status, tumor stage and use of neoadjuvant therapy. No significant difference was present between the two groups for estimated 10 year LRRFS (86% vs 95%) (p=0.1), DFS (70% vs 70%) (p=0.5) and OS (75% vs 63%) (p=0.1). On multivariate analysis, tumor stage was an independent predictor of LRRFS, DFS and OS. Conclusions: Early onset breast cancer is associated with a distinct biology but does not lead to poorer outcomes in our population.

Incidentally Detected Adenocarcinoma Prostate in Transurethral Resection of Prostate Specimens: a Hospital Based Study from India

  • Background: Awareness about prostate cancer has increased in the community, and prostate cancer screening examinations, including prostate specific antigen (PSA) assays, are now widely available. Prior to the PSA era, up to 27% of prostate cancers were detected incidentally at the time of transurethral resection of prostate (TURP). After PSA testing became widely available, the incidence of incidentally detected carcinoma prostate in TURP specimens without prior diagnosis reduced to 5-13%. However, the incidence of incidentally detected carcinoma prostate has been reported to vary across the globe since various factors can influence the identification of this malignancy in TURP specimens. In this paper, we focus on rates of incidentally detected prostate cancer in TURP specimens in our hospital and correlate it with various parameters. Materials and Methods: This retrospective study of histopathological findings of biopsy specimens was conducted for patients undergoing TURP during a period of 5 years from April 2010. The inclusion criteria were patients diagnosed with benign prostatic hyperplasia (BPH) (digital rectal examination (DRE) not showing any abnormally hard areas and normal age adjusted PSA values). Patients with elevated PSA, abnormal DRE, documented urinary tract infection and proved adenocarcinoma prostate (CaP) were excluded from the study. The total weight of prostatectomy specimen, occurrence of carcinoma prostate in the chips, percentage of total tissue resected showing malignancy and Gleason's scores were recorded. Results: A total of 597 patients belonging to the inclusion criteria were studied. The incidence of occult CaP in the study group was 5.2 % (31/597). Out of these, 8 belonged to T1a and 23 belonged to T1b stages. The age group 70 - 79 years had the maximum incidence of occult CaP. It was observed that the clinical grading of prostate did not have a bearing on the incidence of occult CaP whereas the weight of resected specimen correlated with the incidence of CaP. The incidence of occult CaP was greater with low volume prostates (<20 g). (P=0.15). Conclusions: The rate of incidentally detected adenocarcinoma prostate in patients undergoing TURP for clinically diagnosed BPH was found to be only 5.2 % in our study which is low when compared with similar studies done elsewhere. The age of the patient and weight of the resected specimen correlated with incidence of occult prostate cancer. The clinical grading of prostate by DRE however, demonstrated no correlation.
