A Case of Muscular Sarcoidosis diagnosed by Gallium-67 Scintigraphy and Magnetic Resonance Imaging (갈륨 신티그라피와 자기공명영상으로 진단한 근육 유육종증)

  • Gallium-67 scintigraphy is helpful in the assessment of active extrapulmonary sarcoidosis. Muscular involvement of sarcoidosis is often asymptomatic or nonspecific, and laboratory examinations do not provide convincing evidence of muscular involvement We report a case of muscular sarcoidosis, which was detected by gallium-67 scintigraphy. In a patient who was suffering from fever and arthralgia of knee joint, gallium-67 scintigraphy showed mediastinal and hilar involvement of sarcoidosis with unexpected extensive muscular uptake. Magenetic resonance imaging revealed the detailed depiction of intramuscular infiltration of sarcoid granuloma. Gallium-67 scintigraphy is useful in detecting inflammatory muscular involvement of sarcoidosis as well as other multiorgan involvement.

A Case of Intrapulmonary Neurilemmoma (폐실질안의 신경초종 체험 1례)

  • In this paper, we present an extremely rare case of a primary intrapulmonary neurogenic tumor, in which localization of S-100 protein was investigated using immunohistochemical staining. The patient, who was a 47 year old man, experienced no symptoms, however, a routine chest X-ray revealed a round tumor like shadow in the hilar area of left lung. To confirm and cure the mass, surgery was performed. Histopathological examination of the excised tumor revealed it to be a primary intrapulmonary neurilemmoma. Immunohistochemical staining demonstrated the presen e of S-100 protein in the tumor cells.

  • Sixty six patients who were operated as lung cancer during the period from Mar. 1991 to Sep. 1993 at the department of Thoracic and cardiovascular surgery, were reviewed retrospectively and the accuracy of regional lymph node in preoperative CT were compared with histopathologlc report obtained from operation. The age ranged from 30 to 72 years old (mean age : 56.5), and 51 patients were male and 15 patients were female. The author analysed the true positive, true negative, false positive and false negative and sensitivity, specificity, positive predictive index, negative predictive index and accuracy of each nodes. The result is that there were differences between seven nodal groups in specificity, sensitivity, positive predictive Index, negative predictive index and accuracy. The range of each nodal group is from 81.7 to 98.3% The nodes of the most poor accuracy are aortopulmonary area and hilar area.

  • Subaortic stenosis in a 2.9kg, 3-month-old pug dog and a 11 kg, 4-month-old Rott -weiler dog showing signs of dyspnea, tachypnea, anorexia, exercise intolerance and depression was diagnosed with electrocardiography, thoracic radiography and ultrasonographic evaluation. In electrocardiography, PR interval widening, R wave amplitude increase in lead II in case 2, ST segment depression and left axis deviation in case 1 and 2 were identified. In thoracic radiographs, enlarged left ventricle and atrium, caudal displacement of left crus of diaphragm and caudal part of cranial mediastinum widening were identified in ventrodorsal view. In lateral view, left ventricular enlargement, secondary dorsal deviation of trachea and post-stenotic dilation of aorta were seen. Also, edematous pulmonary changes were shown in peri-hilar area. In ultrasonographic examination, left ventricular dilatation, pericardial effusion and subaortic valvular ridge were seen with real time B-mode in right parasternal long axis view. Aortic valve regurgitation was identified in color-Doppler mode.

  • Benign hyperplasia of hilar lymph glands is rare. Most of the lesions were intrathoracic cavity. The lesions were discovered most often on routine roentgenograms of the chest or because. of pressure symptoms or the presence of a palpable mass if outside the thorax. Diagnosis is. made after removal of gland, a procedure which may also have therapeutic value. They have been divided into 2 histol0gic types: the hyaline-vascular lesions, which were, most numerous, were characterized by small hyaline-vascular follicles and interfollicular capillary proliferation ;the plasma cell lesions were characterized by large follicles with! intervening sheets of plasma cells. We experienced one case of benign hyperplasia of lymph gland in left hilum, which were. most numerous, characterized by small hyaline-vascular follicles and interfollicular capillary proliferation-hyaline-vascular type. This 29 years old male patient was treated by right upper lobectomy with excision of lesion. The postoperative courses was uneventful.

Malignant Fibrous Histiocytoma of the Pulmonary Artery A Case Report (폐동맥에서 발생한 악성 섬유성 조직구종 1례 보)

  • A case of malignant fibrous histiocytoma [MFH] of left pulmonary artery was reported in the 27 year-old male. He was admitted because of hemoptysis for several times. Chest x-ray revealed nodular lesions in left hilar area and left upper lobe. A left thoracotomy was performed followed left upper lobectomy. The main tumor was originated from the pulmonary arterial bifurcation and proximal portion of the left pulmonary artery. By light microscopy, there were many pleomorphic giant cells, which displayed the storiform pattern. And in those area, dilated or compressed vascular channels were involved by tumor cells, suggesting origin of This tumor. Pulmonary artery angiography and chest CT revealed this tumor was originated from pulmonary artery. It would be the first reported primary malignant fibrous histiocytoma of the pulmonary artery in our country. Postoperative prognosis was uneventful, but recently he was suffered from dyspnea, that was noticed by OVD follow-up for 1 month, probably the heart failure sign due to pulmonary arterial flow obstruction.

  • Cholangiocarcinoma, though very rare in Western countries, is one of the commonest liver malignancies in Southeast Asia, especially in Thailand. More than half of the patients present with advanced stage disease. Given the poor treatment outcomes of adjuvant therapeutic options, many patients undergo only biliary drainage for palliative treatment. Clinical characteristics and treatment outcomes after biliary stenting were here analyzed for a total of 224 uresectable cholangiocarcinoma cases, 58.9% in men. The mean age was 61.5 years. Hilar involvement was the most common location. The patients underwent biliary drainage using plastic and metallic stents equally, early stent occlusion being encountered in 21.4% and 10.7%, respectively. The median survival time was 4.93 months for patients who received plastic and 5.87 months for patients who received metallic stents.

  • Tuberculosis developed in the pericardium usually occurs as diffuse constrictive pericarditis or effusive pericarditis which contains much pericardial effusion. But types such as localized abscess or tuberculoma are very rare. Myocardial tuberculosis is also very rare and mainly extended directly from hilar lymph node or spreaded hematogenously in miliary tuberculosis. It is known to be able to make arrhythmia or heart failure by invasion of conduction system or myocardial muscle mass, but it is usually discovered by incidental postmortem autopsy and rarely concerned by clinical basis. Recently we have experienced a case of localized intrapericardial tuberculous abscess which extended to myocardium and operated that successfully. So we would report them with reference study.

Clinical Review of Metastatic Lung Tumor (전이성폐종양에 대한 고찰)

  • 52 cases of metastatic lung tumor, including 22 [42.3%] choriocarcinoma., 13[25%] liver cancer, 6 [7.5%] stomach cancer, 2 bone tumor, etc, were reviewed at Busan National University Hospital, during a 5 year period ending with 1974. The age distribution of metastatic lung tumor were same in second decade to 5th decade. The most common appearances of roentgenograms of metastatic lung tumor were that of multinodular [42.3%], diffuse acinar [21.1%],infiltrative or pneumonic[21.1%], solitary lesion [9.6%] and diffuse micronodular [5.7%]. The metastatic lesions originated from choriocarcinoma revealed multinodular and acinar in roentgenogram, and the lesions originated from liver and stomach cancer revealed infiltrative and acinar. In our series, the positive hilar lymph node enlargement was encountered in 12 cases, of which 4 were in liver cancer and 3 were in stomach cancer. Pleural effusion was also encountered in 5 cases, of which 3 were in liver cancer. The treatment of this series was almost conservative except of 2 cases of pulmonary resection, and the reason of this limited cases of surgical treatment was seemed due to the delayed direction of metastasis to lung beyond the proper indication of surgery.

Mediastinoscopy (종격동경 검사에 관한 연구: 56례 보고)

  • Fifty six consecutive cases of mediastinoscopy are analysed. The procedure was performed without any complication and mortality. It is concluded that the mediastinoscopy is a safe procedure and causes little disturbance to the patient. The value of mediastinoscopy as a diagnostic procedure for mediastinal and pulmonary hilar masses is shown. In my series, mediastinoscopy was performed in 17 cases, which could not be diagnosed by other means, for purpose of diagnostic procedure. A histologic diagnosis was obtained in 11 of the 17 patients [64.7%]. The findings are 6 cases of tuberculosis, 2 cases of metastatic carcinoma, 1 case of primary carcinoma, and one case of noncaseating granuloma. The histologic diagnosis of carcinoma in patients believed to have non-resectable tumors could be obtained in 4 of the 7 cases [57.1%]. Three cases out of 32 patients with presumably resectable bronchial carcinoma were found to have mediastinal lymph node metastases. Two patients of 29 cases refused to be operated on. The remaining 27 cases had thoracotomy and resection was undertaken in all but 9 cases [33.3%].
