한국 성인 음주 남성의 고위험 음주 관련 요인: 2차 분석 연구 (Factors associated with high-risk drinking among men in Korea: a secondary analysis)

  • Purpose: This study was conducted to identify factors associated with high-risk drinking in Korean men. Methods: This study was a secondary analysis using data from the Korea National Health and Nutrition Examination Survey (KNHANES VIII-2), 2020. Data were downloaded from the KNHANES website. The subjects of this study were 1,653 alcohol-drinking men between 19 and 64 years of age. Data were analyzed using the Rao-Scott chi-square test and complex sample logistic regression statistics. Results: The high-risk drinking rate among alcohol-drinking men was 27.1%. High-risk drinking was more common in men who smoked (odds ratio [OR] = 2.11, p < .001), men with a middle school education or lower (OR = 1.91, p = .016), men who lived with a spouse (OR = 1.61, p = .025), men who slept less than 6 hours on weekends (OR = 1.51, p = .016), and men who perceived stress (OR = 1.30, p = .044), while it was lower in men who were underweight (OR = 0.19, p = .006). Conclusion: To reduce the rate of high-risk drinking, it is necessary to provide an intervention that considers factors associated with high-risk drinking among men. In particular, smoking was the most important factor associated with high-risk drinking, implying that the integrated management of drinking and smoking is necessary to reduce high-risk drinking.

Association between drinking behaviors and components of metabolic syndrome in subjects in their 20s and 30s: data obtained from the Korea National Health and Nutrition Examination Survey (2016-2018)

  • BACKGROUND/OBJECTIVES: Numerous studies have examined the relationship between drinking behaviors and metabolic syndrome (MetS) for adults, but these include very few studies for young adults. This study therefore undertook to investigate the association between drinking behaviors and components of MetS among adult drinkers aged 20-30 years. SUBJECTS/METHODS: Using the 2016-2018 Korea National Health and Nutrition Examination Survey data, drinking behaviors of adults in the age group 20-30 years were divided into 4 groups: 1) group A, good drinking habits; 2) group B, frequent binge drinking but not frequent drinking; 3) group C, frequent drinking but not frequent binge drinking; 4) group D, frequent drinking and binge drinking. The association between MetS components and drinking behaviors was analyzed by applying multiple logistic regression analysis. RESULTS: We determined the prevalence risk compared to group A. In men, the prevalence risk of high triglyceride (TG) increased 2.051-fold in group C and 1.965-fold in group D. Moreover, in group D, the prevalence risk of low high density lipoprotein cholesterol (HDL-C) increased 0.668-fold, high blood pressure (BP) increased 2.147-fold, and MetS increased 1.567-fold. In women, there was an increased prevalence risk of low HDL-C (0.353-fold) and MetS (3.438-fold) in group C, whereas group D showed increased prevalence risk of abdominal obesity (2.959-fold), high TG (1.824-fold, and low HDL-C (0.424-fold). CONCLUSIONS: Our study indicates that frequent drinking increases the risk of high TG, whereas frequent and binge drinking increases the risk of high TG, low HDL-C, high BP, and prevalence of MetS in men. In women, frequent drinking without binge drinking increases the risk of low HDL-C and MetS, whereas frequent and binge drinking increases the risk of abdominal obesity, high TG, and low HDL-C. We propose that improvements in the drinking behaviors can reduce the prevalence of MetS.

Factors Related to Regional Variation in the High-risk Drinking Rate in Korea: Using Quantile Regression

  • Objectives: This study aimed to identify regional differences in the high-risk drinking rate among yearly alcohol users in Korea and to identify relevant regional factors for each quintile using quantile regression. Methods: Data from 227 counties surveyed by the 2017 Korean Community Health Survey (KCHS) were analyzed. The analysis dataset included secondary data extracted from the Korean Statistical Information Service and data from the KCHS. To identify regional factors related to the high-risk drinking rate among yearly alcohol users, quantile regression was conducted by dividing the data into 10%, 30%, 50%, 70%, and 90% quantiles, and multiple linear regression was also performed. Results: The current smoking rate, perceived stress rate, crude divorce rate, and financial independence rate, as well as one's social network, were related to the high-risk drinking rate among yearly alcohol users. The quantile regression revealed that the perceived stress rate was related to all quantiles except for the 90% quantile, and the financial independence rate was related to the 50% to 90% quantiles. The crude divorce rate was related to the high-risk drinking rate among yearly alcohol users in all quantiles. Conclusions: The findings of this study suggest that local health programs for high-risk drinking are needed in areas with high local stress and high crude divorce rates.

한국 여성의 고위험 음주 영향요인 분석 -위계적 회귀분석을 이용하여- (A Study on the Factors Affecting High-Risk Drinking in Korean Women -Using hierarchical regression-)

  • 본 연구는 여성의 고위험 음주에 관한 영향요인을 분석하고자 시도되었다. 본 연구는 제7기 국민건강영양조사 자료를 활용하였으며, 연구대상자는 만 19세 이상 여성 3,619명이었다. 자료 분석을 위해 대상자를 국민건강영양조사의 고위험 음주율 지표에 따라 일반음주여성과 고위험음주여성으로 구분하여 인구사회학적 특성, 신체건강관련 특성, 정신건강관련특성의 차이 및 고위험음주 영향요인을 파악하였다. 자료 분석은 IBM SPSS 25.0 프로그램을 이용하여 복합표본설계를 적용하였다. 연구결과, 우리나라 여성의 고위험 음주율은 8.8%로 나타났고, 고위험 음주의 영향요인은 20대부터 40대까지의 낮은 연령, 흡연을 하는 경우, 우울감을 경험한 경우로 나타났다. 특히 흡연과 우울감 여부는 고위험 음주에 대한 위험도를 각 4.5배[(95% confidence interval(CI)=2.804-7.372, p<0.001)], 7.0배[(95% confidence interval(CI)=2.918-16.855, p<0.001)] 높이는 것으로 나타나 중요한 위험요인인 것으로 분석되었다. 또한, 고위험 음주여성의 음주시작연령이 15세에서 19세에서 높은 비율로 나타나 청소년기와 초기 성년기 여성 대상의 음주 교육 프로그램의 개발 및 시행이 시급함을 시사하고 있다. 상기와 같은 본 연구의 결과는 여성 고위험 음주율을 낮추기 위한 보건정책 및 건강증진 프로그램에 활용 될 수 있을 것으로 기대된다.

부모의 고위험 음주 행동이 자녀의 건강행동에 미치는 영향 (Impact on the Health Behaviors of Children by High Risk Alcohol Drink Behavior of Parent)

  • 이 연구는 2009년 지역사회 건강조사 자료를 활용하여 부모의 고위험 음주행동이 자녀의 건강행동에 미치는 영향을 조사하여 가족의 건강문제의 예방 및 관리를 위한 기초자료를 제공하고자 하는 목적에서 이루어 졌다. 대상은 19세 이상의 자녀를 둔 세대 중 고위험 음주에 노출된 16,383명을 선택하여 각 ID별로 부모가 고위험 음주자인 916명을 분류하여 빈도분석, 카이제곱 검정과 로지스틱 회귀분석을 실시하였다. 고위험 음주 부모의 자녀가 정상인 음주부모의 자녀에 비해 고위험 음주행동이(p<0.01)이 높았으며 부정적 건강행태에 영향을 미쳤다. 그러므로 고위험음주들에 대한 가족형 보건교육이 제도적으로 필요함을 고려하여 음주로 인한 문제의 해결이 가정에서부터 이루어져야 함을 제안한다.

Drinking Patterns Among Korean Adults: Results of the 2009 Korean Community Health Survey

  • Objectives: In Korea, the proportion of deaths due to alcohol is estimated at 8.9%, far exceeding the global estimate of 3.8%. Therefore, this study was performed to examine the factors associated with low-risk, moderate-risk, and high-risk drinking patterns in Korean adults and to identify target populations for prevention and control of alcohol-related diseases and deaths. Methods: We analyzed data from 230 715 Korean adults aged 19 years and older who participated in the 2009 Korean Community Health Survey. Multinomial logistic regression analysis was used to examine associations between socio-demographic and health-related factors and patterns of alcohol use. Results: A substantially larger proportion of men than women engaged in high risk (21.2% vs. 3.4%) and moderate-risk alcohol use (15.5% vs. 8.2%). In both sexes, moderate- and high-risk uses were associated with younger age, higher income, being currently employed, smoking, being overweight/obese, and good self-rated health. Conclusions: Given the large proportion of the population that is engaging in moderate- and high-risk drinking and given the social norms that support this behavior, public health policies and campaigns to reduce alcohol consumption targeting the entire population are indicated.

성인 1인 가구의 성별에 따른 고위험 음주 영향요인에 관한 연구 (A Study on the Influencing Factors of High Risk Drinking by Gender in Single Adult Households)

  • 본 연구는 성인 1인 가구의 고위험 음주에 관한 영향요인을 분석하고자 시도되었다. 본 연구는 2018년 지역사회 건강조사 자료를 활용하였으며, 연구대상자는 만 19세 이상의 음주경험이 있는 1인 가구 32,389명이었다. 자료 분석을 위해 대상자를 지역사회건강조사의 고위험 음주율 지표에 따라 고위험음주군을 추출하여 인구사회학적 특성, 건강관련 특성의 차이 및 고위험음주 영향요인을 파악하였다. 자료 분석은 IBM SPSS 25.0 프로그램을 이용하여 복합표본설계를 적용하였다. 연구결과, 우리나라 1인 성인 가구의 고위험 음주율은 15.0%(남성 25.8%, 여성 5.8%)로 나타났고, 고위험 음주의 영향요인은 연령과 고등학교 이하의 교육수준, 서비스직 종사, 흡연을 하는 경우, 우울감을 경험한 경우, 고혈압을 진단받은 경우, 아침식사를 규칙적으로 하지 않는 경우가 남성과 여성의 공통요인으로 나타났으며, 남성에게만 영향을 주는 요인은 스트레스, 여성에게만 영향을 주는 요인은 당뇨 진단을 받은 경우로 나타났다. 연령의 경우 남성은 장년층에서, 여성은 20-30대 청년층에서 고위험 음주 위험성이 높은 것으로 나타났다. 국가 경제력의 원동력인 청장년층의 고위험 음주율이 높고, 특히 스트레스나 우울감 경험 등의 정신적인 요인이 고위험 음주 영향요인으로 나타난 본 연구결과는 1인 가구의 고위험 음주율을 낮추기 위해서는 건강행태 및 생활습관의 세밀한 분석을 통한 맞춤형 보건정책과 함께 정신건강 프로그램의 연계가 필요함을 시사한다.

The Relationship between Alcohol Consumption and Diabetes in Korean Adults

  • The prevalence of diabetes continues to increase worldwide, and the problem is also important in Korea, and about 14% of Korean adults have diabetes. Alcohol consumptions are increasing rapidly around the world and are recognized as one of the major problems in the country. Alcohol consumption is an environmental factor previously known to be associated with the risk of developing diabetes. Alcohol consumption can cause problems in the endocrine system and gastrointestinal function, and oxidative stress of acetaldehyde, an ethanol metabolite, can damage beta cells in the pancreas. In the present study, we found the effect of alcohol intake on fasting blood sugar and the difference between men and women in the risk of diabetes according to alcohol intake. In men, the high-drinking group had a higher risk of diabetes than the non-drinking group (OR, 1.41; P=0.058). In women, the Moderate-drinking group had a lower risk of diabetes compared to the non-drinking group (OR, 0.42; P=0.039). The high-drinking group had a higher risk of diabetes than the non-drinking group (OR, 2.97; P=0.034). The group that consumed more than the WHO's daily alcohol intake risk standard tended to have a higher risk of diabetes than the group that did not (OR, 5.48; P=0.001). This study suggest that moderate alcohol consumption reduce the risk of developing diabetes, and high alcohol consumption increase the risk of developing diabetes.

한국 성인의 문제 음주 유병률 및 위험요인 (Prevalence and related risk factors of problem drinking in Korean adult population)

  • 본 연구는 한국 성인의 문제음주 유병률과 문제음주에 영향을 주는 위험요인을 살펴보고자 시행하였다. 연구 대상자는 2007년과 2008년에 수행된 국민건강영양조사에 참여한 19세 이상 성인 7,867명을 대상으로 하였다. 자료 분석을 위해 대상자의 인구사회학적 특성, 건강관련 자료, 알코올 사용 장애 선별검사(Alcohol Use Disorders Identification Test, AUDIT) 자료를 국민건강영향조사 데이터베이스에서 추출하였고 AUDIT 점수별로 문제음주 저위험군, 중간위험군, 고위험군으로 분류하여 분석하였다. 연구결과, 문제 음주 중간위험군은 21.1%, 고위험군은 12.2%로 나타났고 각 군에서 AUDIT 점수의 평균치를 비교한 결과 남성, 젊은 연령층, 낮은 교육수준, 흡연하는 경우 높은 평균 점수를 보였다. 로지스틱 회귀분석을 통해 고위험군의 음주 위험도에 영향을 미치는 요인을 살펴본 결과 연령, 성별, 교육수준, 흡연으로 확인하였으며 특히 이러한 위험 인자 중 흡연이 고위험 음주에 대한 위험도를 4.78배[(95% confidence interval (CI)=3.75-6.10, p<.0001)] 높여 제일 중요한 위험요인인 것으로 분석되었다. 본 연구 결과는 여러 가지 사회문제를 야기 시키고 많은 질병 부담의 원인이 되는 문제 음주에 대한 개선 및 적절한 음주 소비를 위한 다양한 음주관리 보건정책 및 건강증진 프로그램에 활용될 수 있을 것이다.

연령군별 성인 음주자의 고위험음주 관련 요인 (Factors Related to High Risk Drinking in Adult Drinkers by Age Group)

  • Purpose: The purpose of this study was to identify factors associated with high risk drinking in adults. Methods: Multi-variate logistic regression was used to analyze the data of 15,949 adults age 19 years or older from the sixth (2013-2015) Korea National Health and Nutrition Examination Survey. Results: Factors associated with high risk drinking included education level, employment and smoking status among the age group of 19-39. In the 40-59 year age group, the associated factors were gender, employment, smoking, obesity, and depressive mood. In the over 60 year age group, related factors included gender, employment, smoking, abdominal obesity, and subjective health status. Conclusion: The result of this study suggests that early risk factor screening may be helpful to avoid the progression to high-risk drinking. An individualized approach for each age group can be used as a preventive measure.
