현미경적 다발혈관염을 동반한 폐섬유증 1예 (A Case of Pulmonary Fibrosis with Microscopic Polyangiitis)

  • A 65-year-old woman was admitted due to poor oral intake and a dry cough over the previous 3 months. The physical examination was remarkable for bibasilar crackles, and plain chest radiography showed reticulation in both lower lung fields. A pulmonary function test demonstrated a restrictive pattern with a reduced diffusing capacity of the lung for carbon monoxide. High resolution computed tomography showed reticulation and honey-combing in both peripheral lung zones, which was consistent with usual interstitial pneumonia pattern. Her skin showed livedo reticularis. The erythrocyte sedimentation rate and C-reactive protein level were elevated, and hematuria was noted on urinary analysis. A serologic test for auto-antibodies showed seropositivity for Myeloperoxidase-Anti-neutrophil cytoplasmic antibody (MPO-ANCA). A kidney biopsy was performed and showed focal segmental glomerulosclerosis. She was diagnosed as having pulmonary fibrosis with microscopic polyangiitis (MPA) and treated with high dose steroids. Here we report a case of pulmonary fibrosis coexistent with microscopic polyangiitis.

An Analysis of Stress Pattern in the Coracoclavicular Ligaments with Scapular Movements: A Cadaveric Study Using Finite Element Model

  • Background: Acromioclavicular (AC) stability is maintained through a complex combination of soft-tissue restraints that include coracoclavicular (CC), AC ligament and overlying muscles. Among these structures, the role of the CC ligament has continued to be studied because of its importance on shoulder kinematics, especially after AC injury. This study was designed to determine the geometric change of conoid and trapezoid ligaments and resulting stresses on these ligaments according to various scapular motions. Methods: The scapuloclavicular (SC) complex was isolated from a fresh-frozen cadaver by removing all soft tissues except the AC and CC ligaments. The anatomically aligned SC complex was then scanned with a high-resolution computed tomography scanner into 0.6- mm slices. The Finite element model of the SC complex was obtained and used for calculating the stress on different parts of the CC ligaments with simulated movements of the scapula. Results: Average stress on the conoid ligament during anterior tilt, internal rotation, and scapular protraction was higher, whereas the stress on the trapezoid ligament was more prominent during posterior tilt, external rotation, and retraction. Conclusions: We conclude that CC ligament plays an integral role in regulating horizontal SC motion as well as complex motions indicated by increased stress over the ligament with an incremental scapular position change. The conoid ligament is the key structure restraining scapular protraction that might occur in high-grade AC dislocation. Hence in CC ligament reconstructions involving only single bundle, every attempt must be made to reconstruct conoid part of CC ligament as anatomically as possible.

병리특이적 형태분석 기법을 이용한 HRCT 영상에서의 새로운 봉와양폐 자동 분할 방법 (A Novel Method for Automated Honeycomb Segmentation in HRCT Using Pathology-specific Morphological Analysis)

  • 봉와양폐(Honeycomb)는 직경 2~10mm 정도의 크기가 같지 않은 낭포(Cyst)가 경계가 명확한 섬유질(Fibrosis)로 이루어진 벽에 둘러싸여 밀집된 형태로 이루어져 있다. 봉와양폐가 발견될 경우 급성악화의 발생 빈도가 높으며 따라서 봉와양폐의 관찰 여부와 측정은 임상에서 중요한 지표가 된다. 따라서 본 논문에서는 봉와양폐 영역의 정량적 측정을 위하여 봉와양폐의 특징을 이용한 형태학적 기법과 군집성 평가 기법을 통해 자동 구획 방법을 제안하였다. 첫 번째로 영상의 잡음을 제거하기 위하여 가우시안 필터링을 적용하고, 모폴로지 기법 중 팽창 기법을 이용하여 폐 영역을 구획하였다. 두번째로, 주변 8방향 검사를 통해 봉와양폐를 구성하는 낭포의 후보군을 찾고, 영역 확장과 외곽선 검사를 통해 비 낭포들을 제거하였다. 마지막으로 군집화 검사를 통해 최종적으로 봉와양폐를 구획하였다. 제안한 방법은 80장의 고해상도 컴퓨터 단층촬영 영상에서 실험한 결과, 89.4%의 민감도와, 72.2%의 양성 예측도를 보였다.

3차원 전산화단층찰영 영상을 이용한 얼굴 연조직 두께 계측 (Measurement of facial soft tissues thickness using 3D computed tomographic images)

  • Purpose : To evaluate accuracy and reliability of program to measure facial soft tissue thickness using 3D computed tomographic images by comparing with direct measurement. Materials and Methods : One cadaver was scanned with a Helical CT with 3 mm slice thickness and 3 mm/sec table speed. The acquired data was reconstructed with 1.5 mm reconstruction interval and the images were transferred to a personal computer. The facial soft tissue thickness were measured using a program developed newly in 3D image. For direct measurement, the cadaver was cut with a bone cutter and then a ruler was placed above the cut side. The procedure was followed by taking pictures of the facial soft tissues with a high-resolution digital camera. Then the measurements were done in the photographic images and repeated for ten times. A repeated measure analysis of variance was adopted to compare and analyze the measurements resulting from the two different methods. Comparison according to the areas was analyzed by Mann-Whitney test. Results : There were no statistically significant differences between the direct measurements and those using the 3D images (p>0.05). There were statistical differences in the measurements on 17 points but all the points except 2 points showed a mean difference of 0.5 mm or less. Conclusion : The developed software program to measure the facial soft tissue thickness using 3D images was so accurate that it allows to measure facial soft tissues thickness more easily in forensic science and anthropology.

Utility of Multidetector Computed Tomographic Angiography as an Alternative to Transesophageal Echocardiogram for Preoperative Transcatheter Mitral Valve Repair Planning

  • BACKGROUND: Three-dimensional (3D) transesophageal echocardiogram (TEE) is the gold standard for the diagnosis of degenerative mitral regurgitation (dMR) and preoperative planning for transcatheter mitral valve repair (TMVr). TEE is an invasive modality requiring anesthesia and esophageal intubation. The severe acute respiratory syndrome coronavirus 2 pandemic has limited the number of elective invasive procedures. Multi-detector computed tomographic angiography (MDCT) provides high-resolution images and 3D reconstructions to assess complex mitral anatomy. We hypothesized that MDCT would reveal similar information to TEE relevant to TMVr, thus deferring the need for a preoperative TEE in certain situations like during a pandemic. METHODS: We retrospectively analyzed data on patients who underwent or were evaluated for TMVr for dMR with preoperative MDCT and TEE between 2017 and 2019. Two TEE and 2 MDCT readers, blinded to patient outcome, analyzed: leaflet pathology (flail, degenerative, mixed), leaflet location, mitral valve area (MVA), flail width/gap, anterior-posterior (AP) and commissural diameters, posterior leaflet length, leaflet thickness, presence of mitral valve cleft and degree of mitral annular calcification (MAC). RESULTS: A total of 22 (out of 87) patients had preoperative MDCT. MDCT correctly identified the leaflet pathology in 77% (17/22), flail leaflet in 91% (10/11), MAC degree in 91% (10/11) and the dysfunctional leaflet location in 95% (21/22) of patients. There were no differences in the measurements for MVA, flail width, commissural or AP diameter, posterior leaflet length, and leaflet thickness. MDCT overestimated the measurements of flail gap. CONCLUSIONS: For preoperative TMVr planning, MDCT provided similar measurements to TEE in our study.

전자선 단층 촬영을 이용한 관상동맥 우회로 개존의 비침습적 평가 (Noninvasive Evaluation of Coronary Artery Bypass Graft Patency by Electron Beam Tomography)

  • 최근 혈관 질환의 진단을 위한 비침습적 영상이 발달하면서, 기존의 도자술은 중재적 치료에 국한되는 실 정이다. 그러나 관상동맥이나 우회로는 작은 직경, 심박동 움직임 등으로 도자술이 아직도 진단에 필수적이 며, 비침 응\ulcorner영상 진단의 마지막 도전 영역이다. 전자선 단층 촬영기는 높은 시간 해상능으로 심장 영상을 얻을 수 있다. 전자선 단층 촬영을 이용하여 모관상 동맥 협착이나 관상동맥 우회로 이식술 후 개존성의 평 가가 시도되고 있으며, 이중 관상동맥 우회로술 평가의 정확도는 매우 높아서 임상 적용이 가능하다. 저자와 다른 연구자의 경험에 의하면 복재 정맥은 넓은 직경, 비교적 짧고 직선적인 경로, 심박동에 덜 영 향 받음으로써 EBT조영술의 정확도가 높았다. 전향적 민감도와 특이도가 각각 92%, 97%를 보였다. 그러나 위양성과 위음성을 보인 두 예는 후향적으로 분석 할 때 경험 부족에 의한 초기의 판독 오류로 사료되어 복 재 정맥의 경우 후향적으로는 100%의 정확도를 보였다. 반면 내유동맥 이식혈관은 작은 내경과 주변의 수술 클립에 의한 인공산물로 개존성을 확인하기가 대체로 어려웠고, 역동적 검사를 병행하여 우회로내 혈류를 확인하는 것이 필요하다. 내유동맥의 경우 상대적으로 정확도가 낮아 민감도, 특이도가 각각 100%, 80%를 보였으며, 위양성을 보인 2예는 후향적으로도 개존을 확인할 수 없었다. 전자선 단층 촬영 혈관 조영술은 관상 동맥 우 막\ulcorner이식술 후의 우회 혈관 개존성의 평가, 특히 복재 정맥 우회로의 경우 매우 정확도가 높은 비침습적 검사로써, 임상 적용이 기대된다. 앞으로 촬영 기기와 영상 재구성 software의 발달로 정확도를 더욱 높일 수 있는 잠재성이 있다.

  • 폐쇄 세기관지염은 아데노바이러스, 인플루엔자, 파라인플루엔자, 홍역바이러스, 폐렴미코플라스마, 호흡기 세포융합바이러스 등에 의해서 생길 수 있다. 특히 3, 7, 21형 아데노바이러스의 경우에는 급성기의 심한 폐증상을 일으킬 뿐만 아니라 만성적인 합병을 남길 수 있다. 이에 7형 아데노바이러스에 의한 심한 폐렴 후, 만성적인 기침 및 호흡기 증상을 가진 환아를 HRCT를 통해 폐쇄 세기관지염으로 진단하였고, 이후 흡입 스테로이드와 흡입 기관지확장제를 통해 증상을 조절한 1례를 경험한 바 문헌 고찰과 함께 보고하는 바이다.

전폐 세척술로 부분 관해 후 GM-CSF 투여로 치료된 특발성 폐포단백증 1예 (A Case of Idiopathic Pulmonary Alveolar Proteinosis Treated with Granulocyte-Macrophage Colony Stimulating Factor (GM-CSF) after Partial Response to Whole Lung Lavage)

  • Idiopathic pulmonary alveolar proteinosis (PAP) is a rare disorder characterized by surfactant component accumulation in the alveolar space. Idiopathic PAP has recently been recognized as a autoimmune disease of impaired alveolar macrophage function caused by autoantibodies against granulocyte-macrophage colony-stimulating factor (GM-CSF). While whole lung lavage has been the standard treatment, not every patient shows a complete response. Subcutaneous injection or inhalation of GM-CSF is another promising treatment option for PAP. A 45-year-old patient visited our hospital for dyspnea, he was diagnosed as PAP and underwent whole lung lavage. Eighteen months later, the patient had not achieved complete remission in despite of initial response. After then he was administered with GM-CSF (5 ${mu}g/kg/day$, subcutaneous injection) for fivetimes a week during 2 months. Nine months later, the abnormal shadows in high-resolution computed tomography (HRCT) decreased and the patient fully recovered in forced vital capacity. After 60 months, the HRCT scan showed complete remission of PAP.

  • 전산화단층촬영에서 조영제를 자동주입기로 주입하는 과정에서 발생하는 혈관외유출은 조직의 괴사 및 손상의 원인이 되고 있다. 대량의 혈관외유출은 구획증후군으로 발전하여 근막절개술을 시행하는 경우가 발생한다. 혈관외유출이 발생한 환자를 대상으로 혈관외유출 범위 및 부위 정도를 평가하기 위해 CT 검사에서 경정맥에 조영제을 주입하는 과정에서 혈관외유출이 발생한 환자를 대상으로 하였다. 조영제에 의한 혈관외유출 범위 및 손상을 확인하기 조영제 주입부위의 목 및 가슴의 부위를 MDCT를 이용하여 스캔하였다. 경정맥 주사부위의 혈관외유출부위를 MPR, MIP와 볼륨 렌더링의 3차원영상을 3D 프로그램을 이용하여 묘출하였다. 3차원 재구성 영상은 높은 해상력과 정확도를 제공하여 혈관외유출 범위 및 부위를 확인 하여, 환자의 예방의 필요성 및 사후 조치 및 적절한 치료와 수술 계획에 유용하게 이용될 것으로 기대된다.
