Factors Associated with Suicide Risk in Advanced Cancer Patients: A Cross-Sectional Study

  • The study aimed to find out to what degree suicidal thoughts and associated factors affect the suicide risk of advanced cancer patients. The frequency of suicidal thoughts among patients with cancer, especially in the advanced stages, is about 3 times greater than the adult average in South Korea. We recruited 457 participants with four types of cancers (colon, breast, cervical, and lung) using stratified sampling. Data collection was carried out through one-on-one interviews by trained nurses using a structured questionnaire. Advanced cancer patients with high, vs. low, levels of anxiety and pain had a higher suicide risk. In contrast, having one's spouse as the primary care provider was associated with a low suicide risk. Overall, the three factors of anxiety, pain, and the primary caregiver being one's spouse explained 17.2% of the variance in suicide risk. In conclusion, we derived influencing factors of suicide risk using a sample of patients with various types of advanced cancer. The results provide systematic baseline data for preparing nurse-led interventions to prevent suicidal thoughts and suicide attempts among advanced cancer patients.

우리나라 중.고등학생의 자살 생각 관련요인 분석 (The Analysis on Factors related to Suicide Ideation of Middle and High School Students in Korea)

  • Purpose: By analyzing factors related to suicide ideation of Korean middle and high school students, the purpose of this study was to multi-dimensionally understand suicide ideation of Korean middle and high school students and arrange the plans to prevent suicide. Methods: This study used and analyzed the data on ‘The 3rd (Year2007) Korea Youth Health Risk Behavior Web-based Survey (KYHRBWS)' conducted by the Korea Centers for Disease Control and Prevention. In this study, we analyzed 74,698 respondents in total. SPSS 12.0 Program was used to analyze the survey. Descriptive statistics, Chi-Square Test and multivariate logistic regression were also used as research methods. Results: 23.8% of the Korean middle and high school students had suicide ideation during the last one year. Girls had higher possibility of suicide ideation by 1.51 times compared to boys. Students with poor academic achievement had more suicide ideation. In terms of health characteristics, students who perceived themselves as not healthy, and students with short weekday sleeping hours, more drinking experience and more smoking experience had more suicide ideation than others. Furthermore, students who perceived themselves as overweight or obese had more suicide ideation than other students. In terms of mental characteristics, students with high level of stress, low level of happiness and depression had more suicide ideation. Conclusion: As a result of this study, we found out that the factor that had the biggest influence on suicide ideation of Korean middle and high school students was depression. Consequently, active supports and policies to decrease suicide ideation of Korean middle and high school students are necessary. Therefore, active interviews, education for suicide prevention, and periodical screening inspection should be introduced in schools.

Depressive Symptoms on the Geriatric Depression Scale and Suicide Deaths in Older Middle-aged Men: A Prospective Cohort Study

  • Objectives: Prospective evaluations of the associations between depressive symptoms and suicide deaths have been mainly performed in high-risk populations, such as individuals with psychiatric disorders or histories of self-harm. The purpose of this study was to prospectively examine whether more severe depressive symptoms assessed using the Geriatric Depression Scale (GDS) were associated with a greater risk of death from suicide in a general-risk population. Methods: A total of 113 478 men from the Korean Veterans Health Study (mean age, 58.9 years) who participated in a postal survey in 2004 were followed up for suicide mortality until 2010. Results: Over 6.4 years of follow-up, 400 men died by suicide (56.7 deaths per 100 000 person-years). More severe depressive symptoms were associated with greater risk of suicide death (p for trend <0.001). The unadjusted hazard ratios (HRs) in comparison to the absence of depression were 2.18 for mild depression, 2.13 for moderate depression, 3.33 for severe depression, and 3.67 for extreme depression. After adjusting for potential confounders, men with a potential depressive disorder had an approximate 90% higher mortality from suicide (adjusted HR, 1.92; 95% confidence interval [CI], 1.38 to 2.68; p<0.001) than men without depression. Each five-point increase in the GDS score was associated with a higher risk of death by suicide (adjusted HR, 1.22; p<0.001). The value of the area under the receiver operating characteristics curve of GDS scores for suicide deaths was 0.61 (95% CI, 0.58 to 0.64). Conclusions: Depressive symptoms assessed using the GDS were found to be a strong independent predictor of future suicide. However, the estimate of relative risk was weaker than would be expected based on retrospective psychological autopsy studies.

자살성향 측정척도들의 자살예측력에 대한 고찰 (A Review on Predictive Validity of Suicide Assessment Measures)

  • The increasing suicide rate is a serious problem in Korea. Because of increased awareness of suicidality as a problem and because completed suicide is the fourth leading cause of death, it is very important to assess the risk of suicide. The purpose of this review is to provide a systematic examination of predictive validity of measures of suicidal ideation and behavior. A number of instruments are described as useful for identifying individuals "at high risk" for suicidal behavior. However, the predictive validity for most suicide measures has not been established. The present review only includes suicide assessment instruments with published predictive validity. In addition to evaluating the suicide assessment with respect to predictive validity, the present review describes and summarizes the psychometric properties of each measure. In conclusion, because of the complexity of studying the risk of suicide and the paucity of well-designed studies, it is extremely difficult to compare and generalize these findings. In addition, only a few instruments, such as the Scale for Suicide Ideation, Suicide Intent Scale and the Beck Hopelessness Scale, have been found to be significant risk factors for completed suicide. Another problem in the field involves that there have been few suicide measures designed for elderly populations. Clearly, future research is needed to investigate the predictive validity of standardized measures for completed suicide, especially targeting elderly populations.

효과적인 자살예방 캠페인을 위한 메시지 프레이밍 연구 : 자살생각이 있는 집단과 없는 집단을 대상으로 (Study on the Message Framing for Effective Campaigns to Prevent Suicides : A Group Suicide Ideation and without Suicide Ideation)

  • 본 연구는 자살예방 메시지를 획득프레이밍과 손실프레이밍의 두 가지 방식으로 제시했을 때 메시지 프레이밍의 효과 차이가 있는지를 탐색하였다. 또한 자살생각의 유무, 성별에 따라 메시지 프레이밍의 효과 차이가 있는지 탐색하였다. 본 연구의 대상은 총 98명의 대학생으로, 자살생각을 해본 적이 있는지의 유무에 따라 집단을 구분하였다. 이들에게 획득프레이밍과 손실프레이밍의 자살예방 메시지를 제시한 후 메시지에 대한 태도를 측정하였다. 그 결과 자살 생각을 해본 적이 있는 집단에서는 획득프레이밍 메시지와 손실프레이밍 메시지에 대한 호의적 태도가 유사하게 나타났다. 반면, 자살 생각을 해본 적이 없는 집단은 손실프레이밍에 비해 획득프레이밍 메시지가 더 호의적인 것으로 나타났다. 또한 여성은 남성에 비해 획득프레이밍 메시지에 대해 더 호의적이었으며, 설득 효과도 컸다. 이러한 결과는 자살예방 메시지를 제시할 때 표적 집단에 따라 효과적인 프레이밍이 다르다는 것을 시사한다. 마지막으로 본 연구의 제한점과 추구 연구방향에 대해 논의하였다.

일 종합병원 입원 환자의 자살 위험도 및 정서 상태에 영향을 미치는 요인 연구 (Factors Affecting Emotional Status and Suicidal Risk in Hospitalized Medical Patients)

  • 연구목적 본 연구는 일 종합병원 입원 환자를 대상으로 우울, 불안, 디스트레스 등을 비교 분석하여 자살 고위험군을 선별하고, 이들의 임상적 특징을 파악하고자 하였다. 방 법 일 종합병원에 2016년 1월로부터 2017년 12월까지 입원한 전체 환자를 대상으로 하여, 최종 41,249명의 정서상태, 통증, 기저질환 및 기능수준과 관련된 의무기록을 분석하였다. 정서상태 평가를 위해 디스트레스 온도계, 병원 불안-우울 척도를 사용하였다. 자살에 대한 고위험군은 디스트레스 온도계 점수 4점 이상이면서, 병원 불안-우울 척도의 불안, 우울 하위 척도 중 하나라도 8점 이상인 환자로 정의하였다. 결 과 대상자 중 자살에 대한 고위험군 기준에 부합하는 환자는 3,603명(8.7%)였다. 자살 고위험군은 일반군에 비해 더 심한 통증을 경험하고 있었으며, 기저질환을 가진 경우가 더 많은 것으로 나타났다. 통증을 보고한 환자들에서는 디스트레스 온도계 점수, 병원 불안-우울 척도의 불안, 우울 하위 척도 모두에서 통증이 없다고 보고한 환자들에 비해 더 높은 점수를 보였다. 결 론 자살 고위험군은 더 심한 통증과, 더 많은 기저질환을 갖는 것으로 나타났다. 따라서 입원 시 환자의 위험도에 따른 선별 및 선별된 환자들을 대상으로 하는 적극적인 자살 예방 방안이 필요하다.

고등학생의 자살위험성, 자아존중감 및 영적 안녕의 관계 (The Relationships among the Suicidal Risk, Self-Esteem, and Spiritual Well-being of High School Students)

  • Purpose: This study was carried out to investigate the relationships among suicidal risk, self-esteem, and spiritual well-being of high school students, and to provide basic data for developing suicide prevention programs. Method: The subjects of this study were 1.176 high school students from six academic high schools and two technical high schools in Daegu City. Data were collected from the 13th to the 25th of September 2004. The research tools were the Suicide Probability Scale developed by Go. Kim and Lee (2000), the Self-Esteem Inventory modified by Choi and Jeon (1993), and the tool of Spiritual Well-Being modified by Yoo (2002). Data were analyzed with SPSS Windows 11.0. Result: Variables that showed statistically significant difference in suicidal risk were school type, whether to have the best friends, the number of close friends, current relationship with close friends, experience of school violation, respect for the teacher, grades, financial condition, parents marital status, father's education level, mother's education level, home atmosphere, conversation with father, conversation with mother, smoking, drinking, experience of counseling for problems, whether to have physical illnesses, experience of thinking about suicide, experience of attempting suicide. Suicidal risk was in a significantly negative correlation with self-esteem and spiritual well-being. Conclusion: The researcher identified many different variables that affect the suicidal risk of high school students. Self-esteem and spiritual well-being were found to be in a significant correlation with suicidal risk. Therefore, the results of this study can be used as basic data and information for suicidal prevention programs.

Risk Factors for Suicidal Ideation and Attempts in Adolescents

  • Objective: Although suicide is a serious public health concern for adolescents, there is a lack of studies that explore its risk factors in the Republic of Korea. The present study aims to investigate risk factors associated with suicidal behaviors in Korean adolescents. Methods: Participants consisted of 2258 middle and high school students who completed a series of questionnaires regarding suicide ideation or attempts, non-suicidal self-injuries, depression, impulsivity, drinking behaviors, and negative life events, including peer bullying. Results: Among the participants, 8.3% of students reported suicide ideation, while 3.2% reported a history of a suicide attempt in the past 12 months. Depression, peer-victimization, internet-related delinquency, and positive attitudes toward suicide were associated with suicidal ideations and attempts. Adverse life events were also associated with suicide ideation, but not attempts, while not living with both parents and poor family relationships were associated with suicide attempts, but not ideations. Non-suicidal self-injuries were associated with both suicide ideations and attempts. Students with suicidal ideations and attempts can be differentiated depending on the presence of self-injury. Conclusion: In addition to depression or behavioral problems, non-suicidal self-injuries and lack of support from family may also play significant roles in suicide attempts in adolescents. To facilitate the prevention of suicide in adolescents, longitudinal studies should be followed to confirm the risk factors identified in this study.

대학생의 자살위험도와 MMPI-2의 관계 (Suicide Risk and the MMPI-2 Findings Among College Students)

  • Objectives : The Minnesota Multiphasic Personality Inventory (MMPI) is a commonly used psychological test measuring personality and psychopathology in both clinical and non-clinical population. This study was to evaluate characteristic MMPI-2 profile associated with the risk of suicide among college students. Methods : We analyzed the survey response of 2,964 college students who participated in a health survey from a school health center at a national university in 2011. Those who endorsed any of six items on the suicidaity module of MINI were classified as a suicide risk group and remaining students who did not as a control group. Then we compared the Patient Health Questionnaire (PHQ-9) score, the MMPI-2 Clinical scales and Restructured Clinical (RC) scales. To evaluate the correlation RC scales with suicidality score, Pearson correlation analysis was performed. Results : The suicide risk group was 464 students, and the control group was 2,500. The classification result of suicidality, 255 (8.6%) students were low-risk group, 149 (4.8%) students were moderate-risk group and 60 (2.0%) students were high-risk group. In the suicide risk group, VRIN, F scale, Clinical scale and RC scales were significantly higher. In the control group L, K and S scales were significantly higher. Suicidality score has significant correlation with all RC scales. Conclusion : In the suicide risk group, overall psychopathology was higher than the control group. Taken together, features of depressive symptom, antisocial behavior, aggressiveness, introversion may indicate the risk of suicide in college students. These results display both clinical and public health implications for clinicians and school health professionals.

Suicide Attempts and Contributing Factors among South and North Korean-Family Youth Using the Korean Youth Risk Behavior Web-based Survey

  • Objectives: There is an increasing number of North Korean adolescents in South Korea. These adolescents need clinical attention as they experience a high risk of suicidal behavior because of the childhood adversity and acculturative challenges that they face before and after arriving in South Korea. This study assessed the risk of suicide attempts among North Korean adolescents compared to South Korean adolescents, and investigated the contributing factors for each group. Methods: We used data from the Korean Youth Risk Behavior Web-Based Survey (KYRBS) spanning 2011 to 2018, in which 404 adolescents had a father and/or mother who was a North Korean native. Data on 1,212 propensity-matched South Korean adolescents were extracted from the 2011 to 2018 KYRBS. Prevalence was calculated and compared by group. Separate odds ratios were calculated by group. Results: The North Korean group had a significantly higher suicide attempt rate [unadjusted odds ratio (OR)=8.27; adjusted OR=8.45]. Multivariate analysis indicated that having a low or high socioeconomic status and depressive symptoms were significantly associated with suicide attempts in North Korean adolescents, while being female, having a high socioeconomic status, alcohol use, and depressive symptoms were significantly related to suicide attempts in South Korean adolescents. Conclusion: The results found similarities and differences in the factors associated with the likelihood of suicide attempts in the two groups. From these results, different approaches are needed when planning interventions for each group.
