While most of regulatory communities in abroad recognize ' 'natural attenuation " to include degradation, dispersion, dilution, sorption (including precipitation and transformation), and volatilization as governing Processes, regulators prefer "degradation" because this mechanism destroys the contaminant of concern. Unfortunately, true degradation only applies to organic contaminants and short- lived radionuclides, and leaves most metals and long-lived radionuclides. The natural attenuation Processes may reduce the potential risk Posed by site contaminants in three ways: (i)contaminants could be converted to a less toxic form througy destructive processes such as biodegradation or abiotic transformations; (ii) potential exposure levels may be reduced by lowering concentrations (dilution and dispersion); and (iii) contaminant mobility and bioavailability may be reduced by sorption to geomedia. In this review, authors will focus will focul on "sorption" among the natural attenuation processes of hazardous inorganic contaminants including radionuclides. Note though that sorption and transformation processes of inorganic contaminants in the natural setting could be influenced by biotic activities but our discussion would limit only to geochemical reactions involved in the natural attenuation. All of the geochemical reactions have been studied in-depth by numerous researchers for many years to understand "retardation" process of contaminants in the geomedia. The most common approach for estimating retardation is the determination of distrubution coefficiendts ($K_{d}$) of contaminants using parametric or mechanistic models. As typocally used in fate and contaminant transport calculations such as predictive models of the natural attenuation, the $K_{d}$ is defined as the ratio of the contaminant concentration in the surrounding aqueous solution when the system is at equilibrium. Unfortunately, generic or default $K_{d}$ values can result in significant error when used to predict contaminant migration rate and to select a site remediation alternative. Thus, to input the best $K_{d}$ value in the contaminant transport model, it is essential that important geochemical processes affecting the transport should be identified and understood. Precipitation/dissolution and adsorption/desorption are considered the most important geochemical processes affecting the interaction of inorganic and radionuclide contaminants with geomedia at the near and far field, respectively. Most of contaminants to be discussed in this presentation are relatively immobile, i.e., have very high $K_{d}$ values under natural geochemical environments. Unfortunately, the obvious containment in a source area may not be good enough to qualify as monitored natural attenuation site unless owner demonstrate the efficacy if institutional controls that were put in place to protect potential receptors. In this view, natural attenuation as a remedial alternative for some of sites contaminated by hazardous-inorganic components is regulatory and public acceptance issues rather than scientific issue.
A retrospective analysis was peformed on 23 patients with pineal region tumors treated with radiation from 1979 through 1985 at the Department of Therapeutic Radiology, Seoul National University Hospital. Histologic confirmation was done in only one case by surgical removal, and in the remaining 22 patients, the diagnosis was based on clinical and radiological findings. The radiation volume was the primary tumor site in 1 case, whole brain in 14 cases, and the whole craniospinal axis En 8 cases. The overall 5 year survival was $71.5\%$. The 5 year survival was $69.3\%$ for whole brain treated group and $73.3\%$ for craniospinal axis treated group. The survival for the two groups did not differ significantly. In two cases sites of recurrence were detected, one in supratentorial area, and the other in the lung. The results from this retrospective analysis and the review of other reports indicate that routine use of prophylactic spinal irradiation is not warranted in pineal region tumor, and the craniospinal irradiation is recommended in cases with high risk for subarachnoid seeding such as positive CSF cytology, surgical removal or biopsy.

Improved estimates of populations at risk for quick and effective response to natural and man-made disasters require spatial disaggregation of zonal population data because of the spatial mismatch problem in areal units between census and impact zones. This paper implements a dasymetric surface model to facilitate spatial disaggregation of the population of a census block group into populations associated with each constituent pixel and evaluates the performance of the surface-based spatial disaggregation model visually and statistically. The surface-based spatial disaggregation model employed geographic information systems (GIS) to enable dasymetric interpolation to be guided by satellite-derived land use and land cover data as additional information about the geographic distributor of population. In the spatial disaggregation, percent cover based empirical sampling and areal weighting techniques were used to objectively determine dasymetric weights for each grid cell. The dasymetric population surface for the Atlanta metropolitan area was generated by the surface-based spatial disaggregation model. The accuracy of the dasymetric population surface was tested on census counts using the root mean square error (RMSE) and an adjusted RMSE. The errors related to each census track and block group were also visualized by percent error maps. Results indicate that the dasymetric population surface provides high-precision estimates of populations as well as the detailed spatial distribution of population within census block groups. The results also demonstrate that the population surface largely tends to overestimate or underestimate population for both the rural and forested and the urban core areas.
Background: Sperm-associated antigen 9 (SPAG9) has been recently proposed as a novel biomarker for early diagnosis of several human tumors, including ovarian, cervical and breast cancers. Its clinical value remains to be clarified for endometrial cancer (EC). In this study, we investigated the utility of serum SPAG9 levels in diagnosis of EC and its association with important clinicopathological parameters. Materials and Methods: This cross-sectional study was performed at a tertiary women's referral center in Ankara, Turkey. Preoperative serum samples were collected from patients surgically treated for endometrial cancer between June 2012-April 2013. Similar aged women with a biopsy proven benign endometrium were used as controls. Serum SPAG9 levels were measured with an enzyme-linked immunosorbent assay (ELISA) method and assessed for links with clinicopathological factors. Receiver operating characteristic (ROC) curve analysis was performed to assess power of SPAG9 levels for EC prediction. P values less than 0.05 were considered statistically significant. Results: A total of 63 women with EC and 27 with benign endometrium were included in the study. Mean age in the EC group was $58.7{\pm}1.1$. Median SPAG9 levels in the EC and control groups were 18.3 (range, 12.7-53.8) and 14.1 (range, 4.3-65.3), respectively (p<0.001). A cut-off value of 17 ng/ml for SPAG9 predicted presence of malignant endometrium with 74% sensitivity and 83% specificity [Area under curve (AUC)=0.82, p<0.001]. SPAG9 levels did not demonstrate any significant association with histological type, FIGO stage, tumor grade, size, myometrial invasion, lymphovascular space invasion, cervical involvement, adnexal involvement, peritoneal cytology or lymph node status (all p>0.05). Conclusions: Testing for SPAG9 may be useful for early detection of EC in asymptomatic high-risk women. Its role in post-treatment follow-up and early detection of recurrence should be assessed in future trials.
Although tobacco use has been known as one of the biggest risk factors on periodontal health, little is known about the effect of smoking cessation on it. The aim of this study was to investigate the change of concentration of matrix metalloproteinase (MMP)-8, MMP-9 and interleukin (IL)-$1{\beta}$ in gingival crevicular fluid (GCF) of 11 quit-smokers for 1 year after smoking cessation. Eleven male subjects to maintain quit-smoking for 1 year participated the oral examination, GCF and saliva collection without periodontal treatments at baseline, after 2 weeks, 2 months, 4 months, 6 months and 1 year. To confirm quit-smoking, nicotine and cotinine concentrations in saliva were measured by high performance liquid chromatography. MMP-8, MMP-9 and IL-$1{\beta}$ concentrations in GCF of upper anterior teeth area were measured by enzyme-linked immunosorbent assay. Change of MMP-8 in GCF during smoking cessation showed fluctuation with decrease (5 subjects) or increase (2 subjects) or maintenance tendency (4 subjects). Changes of MMP-9 were decrease (6 subjects), or increase (2 subjects), or maintenance (3 subjects). Change of IL-$1{\beta}$ also showed fluctuation with decrease (5 subjects) or increase (3 subjects) or maintenance tendency (3 subjects). The subjects with increase tendency had the relatively smaller amount concentration of MMP-8 and MMP-9 at the baseline. It was unclear smoking cessation without periodontal treatment could affect MMP-8, MMP-9, and IL-$1{\beta}$ in GCF. Fluctuation of periodontal biomarkers during smoking cessation might result from feedback interaction between environmental factors and periodontal cells.
The pomegranate (Punica granatum L.) is a promising source of functional food, which contains several phytochemicals that perform important roles in reducing the risk of pathological diseases. Chemical compositions, such as the total sugar, uronic acid, polyphenols, and anthocyanin contents, and radical scavenging activity were determined and compared among PEs from six different cultivation areas. Total anthocyanin contents (TAC) and total polyphenol contents (TPC) from various growing areas were detected in the following order, respectively: Spain (19.08 ${\mu}g$/mL)> Turkey (12.91 ${\mu}g$/mL)> Iran-A (6.67 ${\mu}g$/mL)> Taiwan (4.77 ${\mu}g$/mL)> Uzbekistan (1.88 ${\mu}g$/mL)> Iran-B (0.76 ${\mu}g$/mL) and Turkey (639.52 ${\mu}g$/mL)> Uzbekistan (502.19 ${\mu}g$/mL)> Spain (306.40 ${\mu}g$/mL)> Iran-B (249.20 ${\mu}g$/mL)> Taiwan (162.78 ${\mu}g$/mL)> Iran-A (143.93 ${\mu}g$/mL). PEs from six different cultivation areas were determined to vary in ellagic acid content from 8.90 ${\mu}g$/mL to 332.52 ${\mu}g$/mL. The amounts of total sugars in PE from Iran-A evidenced relatively high total sugar contents, but low uronic acid levels (11.92 mg/mL). DPPH (1,1-diphenyl-2-picrylhydrazyl) radical scavenging activities were as follows: Turkey> Uzbekistan $\gg$ Spain> Iran-B> Iran-A> Taiwan. ABTS [2,20-azinobis(3-ethylbenzothiazoline-6-sulfonic acid)] radical scavenging activities were detected in the following order: Turkey$\geq$ Uzbekistan$\gg$ Spain> Iran-B> Iran-A> Taiwan. These results indicate that the PEs from Turkey with higher levels of TPC, TAC, ellagic acid, and higher radical scavenging activity may constitute a promising functional source for the prevention of several degenerative diseases.
A year has passed since the nuclear power plant accident in Fukushima on March 11, 2011, and a survey for public perception on radiation by Korean people has been made. The methodological design was based on a quantitative survey and a frequency analysis was done. The analysis objects were survey papers (n=2,754pcs) answered by random ordinary citizens chosen from all over the country. The questionnaires, and study tool, were directly distributed and collected. A total of 40 questionnaires using a coefficient of Cronbach's ${\alpha}$ per each area was 'self perception of radiation' (0.620), 'radiation risk' (0.830), 'benefit from radiation' (0.781), 'radiation controlled' (0.685), 'informative source of radiation' (0.831), 'influence degree from Fukushima accident' (0.763), showing rather high score from all areas. As the result of the questionnaires, the knowledge of radiation concept was 69.50 out of 100 points, which shows a rather significant difference from the result of 'know well about radiation' (53.7%) and 'just know about radiation' (37.40%). According to the survey, one of the main reasons why radiation seems risky was that once exposed to radiation, it may not have negative impacts presently but, the next generation could see negative impacts (66.1%). About 41% of our respondents showed a negative position against the government's report on radiation while 39.5% of respondents said that we should stop running nuclear power in light of Fukushima nuclear power plant accident. This study was done for the first time by Korean people's public perception on radiation after the Fukushima nuclear power plant accident. We expect this might have significant contributions to the establishment of the government's policy on radiation.
Purpose: To evaluate the treatment outcome and prognostic factor after postoperative radiotherapy in retroperitoneal sarcoma. Materials and Methods: Forty patients were treated with surgical resection and postoperative radiotherapy for retroperitoneal sarcoma from August 1990 to August 2008. Treatment volume was judged by the location of initial tumor and surgical field, and 45-50 Gy of radiation was basically delivered and additional dose was considered to the high-risk area. Results: The median follow-up period was 41.4 months (range, 3.9 to 140.6 months). The 5-year overall survival (OS) was 51.8% and disease free survival was 31.5%. The 5-year locoregional recurrence free survival was 61.9% and distant metastasis free survival was 50.6%. In univariate analysis, histologic type (p = 0.006) was the strongest prognostic factor for the OS and histologic grade (p = 0.044) or resection margin (p = 0.032) had also effect on the OS. Histologic type (p = 0.004) was unique significant prognostic factor for the actuarial local control. Conclusion: Retroperitoneal sarcoma still remains as a poor prognostic disease despite the combined modality treatment including surgery and postoperative radiotherapy. Selective dose-escalation of radiotherapy or combination of effective chemotherapeutic agent must be considered to improve the treatment result especially for the histopathologic type showing poor prognosls.
Background: Recently, peroxiredoxin3 (PRDX3) was identified as a novel molecular marker for the progression of hepatocellular carcinoma (HCC). However, its potential clinical application as a serum marker for the early diagnosis and prognosis of HCC has not been investigated. Methods: PRDX3, alpha-fetaprotein (AFP), and other biochemical parameters were measured in serum samples from 297 Chinese patients, including 96 with HCC, 98 with liver cirrhosis (LC), and 103 healthy controls (HCs). Correlations between serum PRDX3 expression and clinicopathological variables and the relationship between serum PRDX3 expression and prognosis were analyzed. Results: Serum PRDX3 was significantly higher in HCC patients than in the LC and HC groups. The sensitivity and specificity of serum PRDX3 for the diagnosis of HCC were 85.9% and 75.3%, respectively, at a cutoff of 153.26 ng/mL, and the area under the curve was 0.865. Moreover, serum PRDX3 expression was strongly associated with AFP level, tumor diameter, TNM stage, and portal vein invasion. Kaplan-Meier curve analysis revealed that HCC patients with high serum PRDX3 expression had a shorter median survival time than those with low PRDX3 expression. Moreover, serum PRDX3 expression was an independent risk factor for overall survival. The inverse correlation between serum PRDX3 and patient survival remained significant in patients with early-stage HCC and in those with normal serum AFP levels. Conclusions: Serum PRDX3 can be used as a noninvasive biomarker for the diagnosis and/or prognosis of HCC.
Background: Early diagnosis of hepatocellular carcinoma (HCC) is the most important step in successful treatment. However, it is usually rare due to the lack of a highly sensitive specific biomarker so that the HCC is usually fatal within few months after diagnosis. The aim of this work was to study the role of plasma nuclear factor kappa B (NF-${\kappa}B$) and serum peroxiredoxin 3 (PRDX3) as diagnostic biomarkers for early detection of HCC in a high-risk population. Materials and Methods: Plasma nuclear factor kappa B level (NF-${\kappa}B$) and serum peroxiredoxin 3 (PRDX3) levels were measured using enzyme linked immunosorbent assay (ELISA), in addition to alpha-fetoprotein (AFP) in 72 cirrhotic patients, 64 patients with HCC and 29 healthy controls. Results: NF-${\kappa}B$ and PRDX3 were significantly elevated in the HCC group in relation to the others. Higher area under curve (AUC) of 0.854 (for PRDX3) and 0.825 (for NF-${\kappa}B$) with sensitivity of 86.3% and 84.4% and specificity of 75.8% and 75.4% respectively, were found compared to AUC of alpha-fetoprotein (AFP) (0.65) with sensitivity of 72.4% and specificity of 64.3%. Conclusions: NF-${\kappa}B$ and PRDX3 may serve as early and sensitive biomarkers for early detection of HCC facilitating improved management. The role of nuclear factor kappa B (NF-${\kappa}B$) as a target for treatment of liver fibrosis and HCC must be widely evaluated.
