Analysis of Dietary Risk Factors of the Colorectal Cancer Patients in DaeguㆍKyungpook Area, Korea (I) - A Study on Lifestyle and Eating Behaviors of the Colorectal Cancer Patients - (대구ㆍ경북지역 대장직장암 환자의 식사관련 위험인자의 분석 (I) -대장직장암 환자의 일상생활 패턴 및 식행동 특성-)

  • The purposes of this study were to investigate the characteristics of lifestyle and dietary habits of the colorectal cancer patients in Daegu and Kyungpook areas and to collect the data useful for nutrition education for the prevention of colorectal cancer in this community. The case subjects of the study were 123 patients diagnosed recently as colorectal cancer at Kyungpook National University Hospital. The control subjects were 182 persons who did not have any gastrointestinal diseases, including the patients from the department of orthopedic surgery and healthy volunteers. The survey covering general characteristics, life style, dietary habits, eating behaviors, and food intake frequency was administered by individual interviews using questionnaires. The results of the study suggest that high BMI, daily life stress, pessimistic personality, lack of physical activities, and familial cancer history might be the possible risk factors for the incidence of colorectal cancer. Dietary factors suspected as risk factors for colorectal cancer in the present study included strong preferences to meats, salty and fatty taste foods, low intake of water, alcohol drinking, smoking, coffee intake and irregular eating habits. A high consumption of seaweeds, green-yellow vegetables, light-colored vegetables, and green tea was suggested as a preventive factor for colorectal cancer. It is recommended to conduct more extensive and systematic surveys in the near future to reconfirm the risk factors of colorectal cancer in consideration of the characteristic food culture in this community. Relative Risk Ratio of Residents Living Near the Municipal Solid Waste Landfill Site at Some Province in South Korea

  • The purposes of this study were to investigate the symptoms, diseases and deaths of residents living near the municipal solid waste landfill site, and to compare the relative risk ratio of their adverse health effects with control group. In self-evaluation, the scores were especially severe lowest in residents of v2 and v3 villages(which were located about 500 m toward under the landfill site) such as 32.2 and 16.7 for village-environment, 24.8 and 16.0 for management of landfill site, and 23.5 and 16.5 for confidence of environmental policy, respectively. On symptoms, relative risk ratios were also highest as 3.53 and 3.55 for breathing difficulty, and 3.36 and 3.00 for respiratory symptom in v2 and v3 villages, respectively. On morbidity, they were slightly high as much as 1.39 and 1.24 in v5 and v2 villages, respectively. On mortality, relative risk ratios were $1.15{\sim}2.46$ in experimental villages. They were especially high as much as 2.46 in v3 village where located near under the landfill site, and also 2.14 in v5 village where located at area affected with the landfill site, but near the sea. The rate of cancer causing death was average 35.2% of total deaths. It was very highest as much as 61.1 % in v2 village, where was closely located near under the landfill site. Cancers causing death in this village were lung cancer(3 cases), larynx cancer(2 cases), stomach cancer(2 cases), pancreatic cancer(1 case), thryoid cancer(1 case), leukemia(1 case) and other(1 case). Our data, although based on limited number of cases and geographical coverage, suggest that residents living near landfill site have the increasing relative risks of various symptoms and mortality causing cancer. No causal mechanisms are available to explain these findings. But the possibility of a causal association between the increased adverse health effects and the municipal solid waste landfill site cannot be fully excluded.

Occupational risk factors influencing subjective oral symptoms in hospital facility Temp·Contract Workers (병원 시설 파견·용역 근로자들의 주관적 구강 증상에 영향을 미치는 직무 위험 요인에 관한 연구)

  • The purpose of this study was to examine the occupational risk factors that affect oral symptoms in hospital facility workers. This study surveyed 627 hospital facility temp·contract workers in the metropolitan area from November 17, 2020 to May 20, 2021. The results of the study indicate that oral symptoms were higher among workers with high risk of musculoskeletal disorders and injuries. Also, symptoms of oral mucosa, temporomandibular joint disorder, and dry mouth were high in the areas of job insecurity and organizational injustice. As the occupational risk factors and occupational stress of workers increased, the risk of oral symptoms increased. In the future, this study can be used as basic data for improving oral health policies to better the oral health of hospital facility workers.

High-impact chronic pain: evaluation of risk factors and predictors

  • Background: The concept of high-impact chronic pain (HICP) has been proposed for patients with chronic pain who have significant limitations in work, social life, and personal care. Recognition of HICP and being able to distinguish patients with HICP from other chronic pain patients who do not have life interference allows the necessary measures to be taken in order to restore the physical and emotional functioning of the affected persons. The aim was to reveal the risk factors and predictors associated with HICP. Methods: Patients with chronic pain without life interference (grade 1 and 2) and patients with HICP were compared. Significant data were evaluated with regression analysis to reveal the associated risk factors. Receiving operating characteristic (ROC) analysis was used to evaluate predictors and present cutoff scores. Results: One thousand and six patients completed the study. From pain related cognitive processes, fear of pain (odds ratio [OR], 0.92; 95% confidence interval [CI], 0.87-0.98; P = 0.007) and helplessness (OR, 1.06; 95% CI, 1.01-1.12; P = 0.018) were found to be risk factors associated with HICP. Predictors of HICP were evaluated by ROC analysis. The highest discrimination value was found for pain intensity (cut-off score > 6.5; 83.8% sensitive; 68.7% specific; area under the curve = 0.823; P < 0.001). Conclusions: This is the first study in our geography to evaluate HICP with measurement tools that evaluate all dimensions of pain. Moreover, it is the first study in the literature to evaluate predictors and cut-off scores using ROC analysis for HICP.

Classification of Microsatellite Alterations Detected in Endoscopic Biopsy Specimens of Gastric Cancers (단순반복염기서열의 변이 형태에 따른 위암 내시경 조직의 유전자형 분류)

  • Purpose: Individual gastric cancers demonstrate complicated genetic alterations. The PCR-based analysis of polymorphic microsatellite sequences on cancer-related chromosomes has been used to detect chromosomal loss and microsatellite instability. For the purpose of preoperative usage, we analyzed the correspondance rate of the microsatellite genotype between endoscopic biopsy and surgical specimens. Materials and Methods: Seventy-three pairs of biopsy and surgical specimens were examined for loss of heterozygosity and microsatellite instability by using 40 microsatellite markers on eight chromosomes. Microsatellite alterations in tumor DNAs were classified into a high-risk group (baselinelevel loss of heterozygosity: 1 chromosomal loss in diffuse type and high-level loss of heterozygosity: 4 or more chromosomal losses) and a low-risk group (microsatellite instability and low-level loss of heterozygosity: 2 or 3 chromosomal losses in diffuse type or $1\∼3$ chromosomal losses in intestinal type) based on the extent of chromosomal loss and microsatellite instability. Results: The chromosomal losses of the biopsy and the surgical specimens were found to be different in 21 of the 73 cases, 19 cases of which were categorized into a genotype group of similar extent. In 100 surgical specimens, the high-risk genotype group showed a high incidence of nodal involvement (19 of 23 cases: $\leq$5 cm; 23 of 24 cases: >5 cm) irrespective of tumor size while the incidence of nodal involvement for the low-risk genotype group depended on tumor size (5 of 26 cases: $\leq$5 cm; 18 of 27 cases: >5 cm). Extraserosal invasion was more frequent in large-sized tumor in both the high-risk genotype group ($\leq$5 cm: 12 of 23 cases; >5 cm: 23 of 24 cases) and the low-risk genotype group ($\leq$5 cm: 7 of 26 cases; >5 cm: 16 of 27 cases). The preoperative prediction of tumor invasion and nodal involvement based on tumor size and genotype corresponded closely to the pathologic tumor stage (ROC area >0.7). Conclusion: An endoscopic biopsy specimen of gastric cancer can be used to make a preoperative genetic diagnosis that accurately reflect the genotype of the corresponding surgical specimen.

  • Currently, approx. 10,532 turnouts were installed on the KR line, of which 3,644 turnouts were installed in the main line where high speed operation is performed. This shows that it is necessary to improve the performance of turnout as one of vulnerable areas for the safe operation of a train. Even though the number of railway accidents is decreasing every year due to the renewal of railway facilities, there are still many factors that cause the occurrence of an accident. In particular, an incident in turnout area does not only have a high risk of a serious accident but also affect the operation of a train on the adjacent track in most cases, and consequently a big social loss is expected due to the delay in train operation as well as the loss of life and property. The objective of this study is to examine the accidents occurred in turnout area that is one of typical vulnerable areas over the past ten years, on the basis of the results, and to take an appropriate measure by finding out the major cause of accidents in turnout area so that a systematic safety system can be established to prevent or reduce accidents in turnout area.

Simultaneous Analysis of 13 Pesticides in Groundwater and Evaluation of its Persistent Characteristics

  • For this study, groundwater samples for 3 years from 2011 through 2013 were collected at 106 groundwater monitoring site in Korea. These groundwater samples were analyzed for 13 pesticides such as cabofuran, pentachlorobenzene, hexachlorobenzene, simazine, atrazine, lindane (gamma-HCH), alachlor, heptachlor, chlordane (total), endosulfan (1, 2), dieldrin, endrin, 4,4-DDT. The objectives of this study were to determine the detection frequency and their concentrations of 13 pesticides and evaluate the health risk level considering ingestion, inhalation, and skin contact using concentrations of 13 pesticides in groundwater samples. An analysis was used for the simultaneous determination for 13 pesticides using GC-MS. GC-MS was performed on HP-5ms, using helium ($1ml\;min^{-1}$) as carrier gas. The average recoveries of the pesticides were from 92.8% to 120.8%. The limits of detection (LODs) were between $0.004{\mu}g\;L^{-1}$ and $0.118{\mu}g\;L^{-1}$ and the limits of quantification (LOQs) were between $0.012{\mu}g\;L^{-1}$ and $0.354{\mu}g\;L^{-1}$. 106 groundwater wells were selected. 54 wells were from well to monitor background groundwater quality and 52 wells were from well to monitor groundwater quality in industrial or contamination source area. Eight pesticides including pentachlorobenzene, lindane (Gamma-HCH), heptachlor, chlordane (total), Endosulfan (1, 2), dieldrin, endrin, and 4,4-DDT were not detected in groundwater samples. The detection frequency for hexachlorobenzene, alachlor, carbofuran and simazine was 23.4%, 11.4%, 7.3%, and 1.0%, respectively. Atrazine was detected once in 2011. The average concentrations were $0.00423{\mu}g\;L^{-1}$ for carbofuran, $0.000243{\mu}g\;L^{-1}$ for alachlor, $0.00015{\mu}g\;L^{-1}$ for simazine, and $0.00001{\mu}g\;L^{-1}$ for hexachlorobenzene. The detection frequency of hexachlorobenzene was high, but the average concentration was low. In the contaminated groundwater, the detection frequency for hexachlorobenzene, alachlor, carbofuran, simazine and atrazine was 26.1%, 21.3%, 7.1%, 1.9% and 0.3%, respectively. In the uncontaminated groundwater, detection frequency for hexachlorobenzene, carbofuran and alachlor were 20.2%, 7.5%, and 1.9% respectively. Simazine and atrazine were not detected at uncontaminated groundwater wells. According to the purpose of groundwater use, atrazine was detected for agricultural groundwater use. Hexachlorobenzene showed high detection frequency at agricultural groundwater use area where the animal feeding area and golf course area were located. Alachlor showed more than 50% detection frequency at cropping area, pollution concern river area, and golf course area. Atrazine was detected in agricultural water use area. By land use, the maximum detection frequency of alachlor was found near an orchard. For human risk assessment, the cancer risk for the 5 pesticides was between $10^{-7}$ and $10^{-10}$, while the non-cancer risk (HQ value) was between $10^{-4}$ and $10^{-6}$. For conclusion, these monitoring study needs to continue because of the possibility of groundwater contamination based on various purpose of groundwater use.

Prevalence of Metabolic Syndrome and Related Risk Factors of Elderly Residents in Andong Rural Area 1. Based on the Anthropometric Measurements and Health Behaviors (안동 농촌지역 중년 및 노인 주민의 대사증후군 유병율과 관련 위험요인 분석 1. 신체계측결과와 건강습관을 중심으로)

  • This study was performed to estimate the prevalence and the risk factors of metabolic syndrome in Andong rural area. A total of 1,431 people (533 males, 898 females) aged over 45 years participated in this study in 2003. The prevalence of metabolic syndrome was 38.2% (male 23.5%, female 46.9%, p<0.001). As age increased, the prevalence of the metabolic syndrome increased in female, but not in male. The major underlying components of metabolic syndrome were high blood pressure (67.1%), low HDL-cholesterol (60.6%), and abdominal obesity (39.9%). The distinctive component for male was high blood pressure (70.1%), and for female, low HDL-cholesterol (73.6%), high blood pressure (65.3%), and abdominal obesity (54.5%). Subjects having more than one component were 94.7%. The risk factors for metabolic syndrome were analyzed using the multiple logistic regression method according to gender and expressed as age-adjusted odds ratio (OR). The results of comparing female to male (OR=2.953), and of comparing obese by % body fat (M: OR=5.786, F: OR=13.498) or BMI (M: OR=3.782, F: OR=13.301) to normal body weight showed significantly higher risk for metabolic syndrome (p<0.001). Health related habits, such as smoking, alcohol drinking, and exercise, didn't show any effect on metabolic syndrome. This study revealed that the prevalence of metabolic syndrome was significantly higher in female subjects compared to both male and female, and high blood pressure was the main cause of metabolic syndrome. We suggest that the strategy for prevention or reducing the prevalence of metabolic syndrome in this area should be concentrated on reducing high blood pressure through lowering obesity and abdominal obesity.

An Experimental Study on the Fire Risk at Welding·Cutting Process (용접·절단 작업시 화재위험성에 관한 실험적 연구)

  • In this study, it was evaluated the fire risk during welding cutting tasks. Welding-cutting machines are representatively used at construction sites. Inverter AC/DC TIG welding macnine and inverter air plasma cutting machine were used in experiments. Temperature of spreaded cinders was measured using a thermal camera. Cinder sizes and spread range were measured according to the height and input current. It was also evaluated the fire risk during welding-cutting process, when flammable materials were located around the working area. There were used hay, dust fence, urethane foam, vinyl, paper and oil as flammable materials. Temperature of spreaded cinders was reached at about $450^{\circ}C$. Cinders were spread approximately 4.7 m, when a worker carried out cutting process at 2.5 m height. The possibility of a fire is very high, when flammable materials were located around the working area.

The Relationship between Internet Addiction and Health Promoting Behaviors of Elementary School Students in a Rural Area (일부 농촌지역 초등학생들의 인터넷중독과 건강증진 행위와의 관련성)

  • Purpose : To find degree of internet addiction of rural elementary school students and to assess the relationship of them to the health promoting behaviors in a rural area in chungnam province. Method : Children's grade, sex and their internet habits, and family background and internet addiction records, health promoting behavior scores of 5th and 6th grade of elementary school students. Self-recorded questionnaire surveys from 2nd December to December 20, 2008, and total 393 data were analysed. Results : The results of this study were summarized as follows: 1. The internet risk group of 5th and 6th grade of rural elementary school students were 18.3%. 2. Place of major using internet were home in 84.6% of male and using internet time were more than 2hours per day and major purpose of internet was game(72.9%) than female. 3. Significant negative correlation between the degree of internet addiction and the health promoting behavior(r=-0.245. p=0.000) 4. Health promoting behavior score was 138.24${\pm}$18.64 in internet non-addictive group and 126.66${\pm}$16.81 in potential risk group, 124.60${\pm}$19.85 in high risk group 5. As a result of multiple regression analysis, degree of internet addiction, school record, health status, concern to health of the students by the parents were related to the health promoting behavior and these provided predicted 21.7%.. Conclusion : The internet addiction risk group need for the health education including health promoting behavior and counseling for treatment of internet addiction.
