중풍환자의 혈압 상승에 대한 대추혈 사혈의 혈압 강하 효과 (Decreasing Effect of Wet-Cupping at Daechu (대추, Dazhui:GVl4)-point on the Increase of Blood Pressure in Patients with Stroke)

  • Objectives : Daechu (Dazhui:GV14 (Governor Vessel 14))-point is located between the spinous process of the 7th cervical vertebra and that of the 1st thoracic vertebra. GV14 has been used to treat hypertension, high fever, neck pain, common cold, headache and so on. Hypertension often negatively affects the improvement of stroke patients. We investigated whether wet-cupping at GV14 had any effect of decreasing blood pressure (BP). Methods : In this study, 81 stroke patients were studied, from Oct. 2000 to Sept. 2001. They were composed of a Sample group (n=36) and Control group (n=45). The Sample group (n=36) was divided into Sample-Normal (n=9), Sample-Mild (n=7), Sample-Moderate (n=l1), and Sample-Severe group (n=9). The Control group (n=45) was divided into Control-Moderate (n=34) and Control-Severe group (n=ll). We checked blood pressure 5 times (just before treatment, then after 30,60, 90, and 120min. (2hrs.)) in each sample group and 3 times (baseline, after 30, and after 120min. (2hrs)) in the control group. Results : In a comparison of before-treatment BP and after-treatment BP in the sample group, SBP significantly decreased as time passed, while DBP slightly decreased but not significantly. In comparison of BP differences in each sample subgroup, BP slightly increased in the Sample-Normal group, but significantly decreased in the Sample-Mild, Sample-Moderate, and Sample-Severe groups. In comparison of BP differences between the sample and control groups, SBP and DBP of the sample group definitely decreased more than those of the control group, but not significantly. Conclusions : This study suggests that wet-cupping at GV14 has significant decreasing effectson blood pressure in stroke patients. Accordingly, we hope that this measure will be used more widely as an emergent treatment for increased blood pressure.

Pedicled Anterolateral Thigh Flaps for Reconstruction of Recurrent Trochanteric Pressure Ulcer

  • The reconstruction of recurrent pressure sores is challenging due to a limited set of treatment options and a high risk of flap loss. Successful treatment requires scrupulous surgical planning and a multidisciplinary approach. Although the tensor fascia lata flap is regarded as the standard treatment of choice-it provides sufficient tissue bulk for a deep trochanteric sore defect-plastic surgeons must always consider the potential of recurrence and accordingly save the second-best tissues. With the various applications of anterolateral thigh (ALT) flaps in the reconstructive field, we report two cases wherein an alternative technique was applied, whereby pedicled ALT fasciocutaneous island flaps were used to cover recurrent trochanteric pressure sores. The postoperative course was uneventful without any complications. The flap provided a sound aesthetic result without causing a dog-ear formation or damaging the lower-leg contour. This flap was used as an alternative to myocutaneous flaps, as it can cover a large trochanteric defect, recurrence is minimized, and the local musculature and lower-leg contour are preserved.

Evaluation of Non-Thermal Decontamination Processes to Have the Equivalence of Thermal Process in Raw Ground Chicken

  • The present study was aimed at examining the antibacterial effects of nonthermal decontamination processes, which are equivalent to thermal treatment, to ensure microbiological safety of raw ground chicken. Escherichia coli or Salmonella were inoculated into 25 g of raw ground chicken samples. The raw ground chicken samples were non-treated or treated with high hydrostatic pressure (HHP) at 500 MPa (1-7 min), light-emitting diode (LED) irradiation at 405 nm wavelength (30-120 min), and heat at 70℃, 90℃ (1-60 min), and 121℃ (1-15 min). E. coli and Salmonella cell counts were enumerated after treatments. Moreover, the color parameters of treated raw ground chicken were analyzed. HHP treatment reduced E. coli and Salmonella cell counts by more than 5 Log CFU/g and more than 6 Log CFU/g after 7 min and 1 min, respectively; these effects were equivalent to those of thermal treatment. However, LED irradiation reduced Salmonella cell counts by only 0.9 Log CFU/g after 90 min of treatment, and it did not reduce E. coli cell counts for 90 min. Compared with those of the non-treated samples, the ΔE (total color difference) values of the samples treated with HHP were high, whereas the ΔE values of the samples treated with LED irradiation were low (1.93-2.98). These results indicate that despite color change by HHP treatment, HHP treatment at 500 MPa could be used as a non-thermal decontamination process equivalent to thermal treatment.

고압처리에 의한 Alliinase의 불활성화가 마늘의 풍미에 미치는 영향 (High Pressure Inactivation of Alliinase and Its Effects on Flavor of Garlic)

  • 고압처리에 의한 효소반응의 조절을 연구하기 위하여 마늘에 압을 적용하여 alliinase의 압력에 의한 활성변화를 해석하였다. 마늘을 $25^{\circ}C$에서 10분간 고압 처리한 결과 alliinase 활성은 150 MPa 및 300 MPa에서 변화가 없었고, 500MPa에서는 완전히 불활성화되었다. 한편 정제된 alliinase는 150MPa에서 30%, 300 MPa에서 80%, 500MPa에서 100% 불활성화되어, 효소의 상태에 따라 압력에 의한 불활성화 정도가 다르게 나타나는 것을 알 수 있었다. 또한 고압처리에 의한 alliinase의 불활성화는 상온보다는 고온 또는 저온에서 효과적인 것으로 나타났다. $50^{\circ}C$에서 500MPa로 고압처리한 마늘은 allicin이 거의 생성되지 않아 매운 맛을 느낄 수 없었고, 휘발성 화합물의 50%이상이 제거되어, 마늘의 무취화에 고압처리를 적용할 수 있음을 확인하였다.

천공분지에 기저를 둔 도서형 회전 근막 피부 피판을 이용한 압박궤양의 치료 (Perforator-Based Fasciocutaneous Island Rotation Flap in Treatment of Pressure sore)

  • The reconstruction of deep soft tissue defect such as pressure sore has difficult problems. Myocutaneous flaps have been used commonly as the best coverage method for pressure sore. But, they have several drawbacks such as sacrifice of functional muscle, high morbidity of the donor sites and bulkiness at the recipient site. The concepts of perforator flap has recently developed and widely used to overcome these disadvantages. Between March 2005 to July 2006, we have treated 9 patients who had pressure sore using perforator based fasciocutaneous island rotation flap. Preoperative unidirectional Doppler was used in all cases. Mean number of perforator vessels was 3.8 and flap sizes were from $7{\times}5\;cm$ to $14{\times}13\;cm$. Rotation angles of flap were from 90 degree to 180 degree. In all cases, donor sites were closed primarily. All flap survived completely and postoperative complications were wound dehiscence in 1 case, wound infection in 3 cases. The mean postoperative follow up period was 15.7 months and recurrence was not reported. We could decrease donor site morbidity and cover wound sites easily by using flap rotation and get robust blood supply without sacrifice of functional muscle. Fasciocutaneous perforator island rotation flap would be very useful for various pressure sore treatment.

고압처리가 발아벼의 항산화 성분과 활성에 미치는 영향 (Effects of High Pressure Treatment on Antioxidant Compounds and Activity of Germinated Rough Rice (Oryza sativa L.))

  • 본 연구에서는 발아기간 및 고압처리 시간에 따른 항산화 성분 및 항산화 활성을 조사함으로써 발아벼의 항산화 활성에 미치는 고압처리공정의 효과에 대해 연구하였다. 발아기간은 6일까지로 하였고, 기간별로 발아된 벼는 30 MPa의 압력 하에서 24시간 및 48시간 동안 처리하였다. 총 폴리페놀함량은 48시간 고압처리를 실시한 6일차 발아벼가 5.15 mg/g으로 고압처리를 실시하지 않은 6일차 발아벼의 1.11에 비하여 증가하였으며, 15종의 페놀산 중 gallic acid 외 6종의 페놀산과 총 페놀산함량이 발아기간 및 고압처리 시간이 증가함에 따라 유의적으로 증가하였다. ABTS 라디칼 소거능과 DPPH 라디칼 소거능은 대조구의 경우 발아 4일차에서 최대 값을 나타내었으며, 고압처리 24시간 처리구와 48시간 처리구 모두 대조구에 비해 높았다. 환원력과 이온킬레이팅 효과는 총 폴리페놀, 페놀산 함량과 유사하게 발아기간 및 고압처리시간이 증가함에 따라 증가하였다. 본 연구결과 발아와 가압을 병행처리 하였을 때 일반벼 및 발아벼에 비해 항산화 성분 및 항산화 활성이 증가함을 알 수 있었으며, 발아벼의 기능성을 증대시키기 위하여 고압처리공정의 적용이 효과적이라고 판단된다.

Effects of Asymmetric Airway Inertance on Mean Lung Volume During High Frequency Ventilation(HFV)

  • A possible asymmetry in airway inertance was modeled based on previously reported radiographic visualization data of the airway wall fluctuation in intact dogs. Effects of asymmetric Inertance on mean lung volume during high frequency ventilation (HFV) were evaluated in terms of mean inertive pressure drop across the airways. It was found that a negligible inertlve pressure drop was expected ($<1{\;}cmH_20$) in normal subjects, while a sig- nificant rise in mean alveolar pressure compared to mean mouth pressure by approximately $3{\;}cmH_20$ was resulted for about 40% airway fluctuation representing bronchoconstriction state by Histamine induction. These results demonstrate that asymmetric Inertance could lead patients with airway diseases to a significant lung hyperinflation (LHI), and bronchodilation treatment is recommended prior to applying HFV to prevent those patients from a possible barotrauma.

  • Subarachnoid pleural fistula (SPF) is an aberrant communication between the pleural cavity and subarachnoid space, resulting in uncontrolled cerebrospinal fluid drainage. The negative pressure of the pleural cavity creates a continuous suctioning effect, thereby impeding the spontaneous closure of these fistulas. Dural tears or punctures in cardiothoracic procedures, spinal operations, and trauma are known to cause such abnormal communications. Failure to recognize this entity may result in sudden neurological or respiratory complications. Hence, a high index of suspicion is required for early diagnosis and prompt management. Noninvasive positive pressure ventilation has been described to be effective in managing such fistulas, thus mitigating the high morbidity associated with exploratory surgery for primary repair. Herein, we describe the typical presentation of SPF and the clinical course, treatment, and follow-up of a patient who sustained SPF following anterior thoracic spinal surgery.

Purification and Characterization of Homoharringtonine from Cephalotaxus koreana

  • 한국산 개비자나무로부터 alkaloid계 항암 활성 물질인 Homoharringtonine의 분리 및 정제 공정을 개발하였다. 추출 용매로 메탄올을사용하여 biomass와 mthanol을 1:8의 비율로 분씩 3회 추출할 경우 개비자나무로부터 대부분(>99%)의 Homoharringtonine이 추출됨을 알 수 있었다. 흡착 공정에서는 활성 백토(active clay)를 사용하여 추출물에 포함되어 있는 식물유래 타르, 왁스 성분을 효과적으로 제거하였다. 크로마토그래피 공정을 통해서 52% 이상의 Homoharringtonine을 정제하였다.

Degumming of Antheraea yamamai silkworm cocoon

  • Oak silkworm, Antheraea yamamai (A. yamamai), has been used for clothing and surgical suture and considered as biomaterial due to RGD tripeptide. This paper reported the degumming conditions of A. yamamai using sodium oleate, high pressure and temperature, and sodium carbonate. Degumming ratio of A. yamamai cocoon using sodium oleate was less than 10%. High pressure and temperature treatment induced 30% weight loss of A. yamamai cocoon. The concentration, treatment temperature and time using sodium carbonate was examined and revealed the following conditions for degumming; 5% owf, 60 min at 100℃. The degummed solution was confirmed using UV and FT-IR spectrometer. Our results can be used to handle A. yamamai silkworm cocoon for further application including material processing.
