Clinical Experience of Carbomedics Prosthetic Heart Valve (Carbomedics 인조 심장판막의 임상적 경험)

  • The CarboMedics Medical valve has become our mechanical valvular prosthesis of choice because of favorable hemodynamic results that associated with marked clinical improvement and low incidence of thromboembolism after it,s uses. The data for this study was collected from August 1988 to July 1993,five years period. There were total of 57 patients[female 40,male 17 in this series with 4 isolated aortic valve,26 isolated mitral valve,11 double valve and a triple valve replacement. The mean follow up time was 32 months. Postoperatively,58% of cases were in New York Heart Association[NYHA functional class I,and mild and moderate symptoms[NYHA class II were present in 36% and there were very few patients remaining in higher functional classes. In postoperative echocardiographic study, showed marked improved cardiac function. The overall early mortality was 3.5% and the late mortality was one case after triple valve replacement due to sudden death. The causes of early death were attributed to early prosthetic valve endocarditis and heart failure.

  • The object of this study is to develop a model of the cardiovascular system capable of simulating the short-term transient and steady-state hemodynamic responses such as hypotention and disequilibrium syndrome during hemodialysis or hemofiltration. The model consists of a closed loop 12 lumped-parameter representation of the cardiovascular circulation connected to set-point models of the arterial and cardiopulmonary baroreflexes and 3 compartmental body fluid and solute kinetic model. The hemodialysis model includes the dynamics of sodium, urea, and potassium in the intracellular and extracellular pools, fluid balance equations for the intracellular, interstitial, and plasma volumes. We have presented the results of many different simulations performed by changing a few model parameters with respect to their basal values.

  • Hemodialysis is essential for patients with end stage renal failure. It is important to improve the patency rate and to minimize occurrence of the stenosis. Also, the blood flow to the artificial kidney can affect the blood flow characteristics through arteriovenous graft. Thus, the delivered dose are important factors for analyzing hemodynamic characteristics during hemodialysis access. In this study, the numerical analysis was performed for the effect of the delivered dose during hemodialysis access on the blood flow through the graft. As a result, The adverse pressure gradient occurred in case of a larger delivered dose through a catheter than standard dose and the flow instability increased. Also the circulation flow appeared largely at anastomotic site of the vein when the delivered dose was exceeded about half blood flow of inlet blood flow.

  • This study was undertaken to hemodynamically determine the differences of myocardial protective effect between crystalloid and blood cardioplegic solution. Twenty nine children undergoing cardiac operations due to cyanotic congenital heart diseases were randomized into two groups receiving crystalloid or blood cardioplegia. Cardiac indices and other hemodynamic datum were examined postoperatively. Although there was no statistical differences between groups, postoperative stroke volume indices and left ventricular stroke work indices were slightly better with blood cardioplegia. We also found that postoperative left atrial pressures[p=0.0003], central venous pressures[p=0.004], and heart rates[p=0.014] were significantly lower with blood cardioplegia. The fact that relatively lower ventricular preloads [left atrial pressure and central venous pressure] were required to provide adequate cardiac output in blood cardioplegia group suggested superior myocardial protective effect of blood cardioplegic solution.

  • Relationships between biochemical phenomena and hemodynamics on human endothelial cells are very important to study the mechanism of atherosclerotic formation and development. The objective of this study is to investigate the flow phenomena around the endothelial cell model by the PIV experiment. The microscopic images of endothelial cells were acquired by a CCD camera to fabricate the shape of endothelial cell. The cell models were fabricated by using a photoforming process. Two consecutive particle images were captured by the CCD camera for the image processing. Conifer powder as the tracing particles was added to water to visualize the flow field. The cross-correlation method was applied fer the image processing of the flow visualization. Pressure and wall shear stress variations on the surfaces of the endothelial cells were calculated to investigate the effects of hemodynamic forces on the morphological changes.

  • We experienced a series of animal experimental studies of the total artificial heart in 1988. So called, "Korean Heart* was used in this study, which is developed and fabricated in the Department of Biomedical Engineering, College of Med., S.N.U.. "Korean Heart" is a Rolling-Cylinder Motor-Driven type which is a newly developed electromechanical heart over the shortcomes of the previous artificial hearts, especially pneumatic type. The advantages of the "Korean Heart" are total implantability, quiet and smooth movement, small size fittable in oriental people, etc. The animal experiments were performed two times, as an assist device in sheep and total artificial heart implant experiment in calf weighing 100 kg. After total implantation, the artificial heart was well functioned in movement and hemodynamic control. So that, the calf was recovered excellently, which was able to stand up by herself and take an oral intake. Total survival time was 100 hours and the cause of death was a sudden pumping failure [electrical connection problem]. Several postoperative laboratory results almost within normal limits and no hemolysis, but in autopsy, the multiple thromboembolic findings were seen at the lung and kidney.n at the lung and kidney.

Computational Study to Understand the Cardiac Electromechanical Responses in LBBB and RBBB to the Application of CRT and LVAD

  • the aim of this study was to observe the combined effect of the CRT and LVAD on electromechanical cardiac behavior under LBBB and RBBB conditions computationally. We performed simulation by using advanced electromechanics model of failing ventricle combined with lumped model represents circulatory system, CRT and LVAD. We analyzed seven failing ventricle model including normal sinus rhythm, LBBB, LBBB coupled with CRT, LBBB coupled with CRT and LVAD, RBBB, RBBB coupled with CRT, and RBBB coupled with CRT and LVAD. We compared the effect from CRT and the effect from combined CRT and LVAD to both under LBBB and RBBB conditions. The results showed that the combined CRT and LVAD contributed a better hemodynamic compared to single CRT. This combined system synchronized the electrical activation greatly under LBBB and slightly under RBBB. It also shortened mechanical activation time which resulted short electromechanical delay. More importantly, the combined system produced better mechanical responses under both LBBB and RBBB conditions.

  • Objectives The aim of this study is to develop a cardiovascular simulator that can reproduce blood pressure pulse and blood flow similar to those of the human body. Methods In order to design a system similar to the human cardiovascular system, the required performances were determined by investigating the hemodynamic characteristics of the heart and the arterial system. Main organ to be imitated is heart in simulator. The rest of the system was minimally designed. Also, a blood pressure and blood flow measurement system was developed for measuring the results. Results The developed system showed blood pressure pulse at similar range of the human aorta. The result waveform include primary wave caused by ventricular systole except reflected wave. Conclusions The blood pressure and blow flow patterns were replicated by the simulator. These patterns were similar to those of the human body. The system will play an important role in studying pulse diagnostics.

  • 50 months experience with St-Jude Medical Cardiac Valve Prosthesis The St. Jude Medical valve has become our mechanical valvular prosthesis of choice because of favorable hemodynamic results that associated with marked clinical improvement and low incidence of thromboembolism. The data for this study was collected from April 1986 to May 1990, four years period. There were total of 110 patients[female 53, male 58] in this series with 22 isolated aortic valve, 66 isolated mitral valve, 20 double valve, 2 tricuspid valve replacement. The mean follow up time was 23 months. Postoperatively, 77% of cases were in New York Heart Association[NYHA] functional class I, and mild and moderate symptoms[NYHA II ] were present in 20% and there were very few patients remaining in higher functional classifications. In postoperative echocardiographic study showed marked improved cardiac function. The overall early mortality was 5.4% and was higher after double[13.3%] and mitral valve replacement[5.6%] and the late mortality was one case after mitral valve replacement due to endocarditis. The cause of death in early mortality was attributed to heart failure, acute renal failure, sepsis, etc.
