흰쥐의 적출심장에서 HTK 용액의 심근보호 효과 (The Effect of the Histidine-Tryptophan-Ketoglutarate (HTK) Solution on Myocardial Protection in Isolated Rat Heart)

  • 배경: HTK 용액은 우수한 심근보호 효과를 나타내는 것으로 알려져 왔다. HTK 용액은 비교적 낮은 농도의 칼륨으로 심정지를 유도하고, 저농도의 나트륨을 함유하며, 혈액보다 우수한 완충능을 제공하는 생물학적 완충제인 histidine을 다량 포함하는 것이 특징이다. HTK 용액의 심근보호 효과는 우수하여 1회 주입으로 상당한 시간(120분) 허혈로부터 심근을 보호한다고 보고되었다. 이 연구는 두 가지 다른 심근온도(10∼12$^{\circ}C$, 22∼24$^{\circ}C$)하에서 심근 허혈이 120분을 초과할 때 HTK 용액의 심근보호 효과를 평가하고자 하였다. 대상 및 방법: 체중 300∼340 g의 Sprague-Dawley계 흰쥐의 심장을 적출 하여 Krebs-Henseleit용액으로 100 cm $H_2O$의 압력으로 관류시켰다. 안정화 후에 심박동수, 좌심실내압, 관 관류량을 측정하였다. 상행대동맥을 통하여 HTK 용액을 1회 주입 후 적출 심장을 각기 다른 4가지 방법으로 보존하였다. 제1군(n=10)은 10∼12$^{\circ}C$의 저온에서 2시간, 제2군(n=10)은 22∼24$^{\circ}C$의 저온에서 2시간, 제3군(n=10)은 10∼12$^{\circ}C$의 저온에서 3시간, 제4군(n=10)은 22∼24$^{\circ}C$의 저온에서 3시간 보존하였다. 보존을 끝낸 후 적출 심장을 다시 관류 장치에 연결하여 재관류 후 15분, 30분, 45분에 각 각 심박동수, 좌심실내압, 관 관류량을 측정하였다. 미세구조 변화를 평가하기 위하여 심근을 생검하여 사립체 점수를 측정하였다. 결과: 심박동수는 제1군과 2군, 제1군과 3군의 비교에서 차이가 없었다. 제4군에서 재관류 후 15분에서 심박동수가 제1군에 비하여 유의하게 감소하였으나(p<0.05 by ANCOVA), 30분, 45분에서 심박동수는 회복되었고 제1군과 유의한 차이가 없었다. 좌심실내압은 제4군에서 재관류 후 15분, 30분, 45분에서 제1군에 비해 유의하게 감소하였다(p<0.001 by ANCOVA). 관 관류량은 제4군에서 재관류 후 15분, 30분, 45분에서 제1군에 비해 유의하게 감소하였다(p<0.001 by ANCOVA). 미세구조 변화의 평가에서 심근의 평균 사립체 점수는 제1군 1.02$\pm$0.29, 제2군 1.52$\pm$0.26, 제3군 1.56$\pm$0.45, 제4군 2.22$\pm$0.44였다. 걸론: HTK 용액은 2시간의 심근 허혈에 대하여 심근 온도에 관계없이 우수한 심근보호 효과를 나타내나, 허혈시간이 2시간을 초과하면 심근의 혈역학적 기능과 미세구조의 변화는 중등도의 저온(22∼24$^{\circ}C$)에서 유의하게 악화되었다. 이 같은 결과로 볼 때 심근 허혈시간이 2시간을 초과한다면 심근 온도를 낮추어야 할 것으로 판단된다.

허혈성 전조건화 유발이 심근보호에 미치는 영향에 관한 실험적 연구 (The Experimental Study for Myocardial Preservation Effect of Ischemic Preconditioning)

  • 개심술 후 나타나는 심기능 저하는 수술 중 허혈로 인하여 생긴 심근손상에 의한 것으로 심근 허혈 상태가 반복되는 경우에 허혈 상태에 의한 심근기능의 손상이 축적되지 않고 오히려 먼저 온허혈 상태에 의하여 심근세포가 일종의 조건화 상태가 되어 허혈에 견디는 힘이 증가하여 장시간의 허혈이 오는 경우에 허혈에 의한 심근손상을 줄일 수 있다고 하여 이를 허혈성 전조건화라고 하는데, 허혈성 전조건화의 효과는 심근손상을 감소시키고, 재관류 시 심근회복에 좋은 영향을 미치며, 부정맥의 빈도를 감소시킨다고 하였다. 이러한 허혈성 전조건화를 본원에서 사용하는 중외1호 심정지액을 흰쥐의 적출 심장에 이용하여 심근보호 효과를 알아보고자 본 연구를 실시하였다. 대상 및 방법: 실험동물은 Sprague-Dawley계 수컷 흰쥐를 사용하였으며, 케뉼라를 삽관한 후 modified isolated working heart model에 부착 고정하였다. 관류과정은 비작업성 순환과 작업성 관류로 나누어 실시하였으며, 작업성 관류를 20분간 실시하고 이 때 심박동수, 대동맥압, 대동맥관류량, 관관류량을 측정기록 하였다. 대조군은 적출심장을 작업성 순환 20분 후 대동맥 차단과 동시에 심정지액을 주입하여 60분간 보존 후 재관류를 실시하여 회복시킨 군이며, 비교군은 허혈성 전조건화로 관류액 자체를 저산소증으로 만들어 허혈을 유발시킨 후 심정지액을 주입하여 60분간 보존시킨 군(제I군), 작업성 관류 20분 후 대동맥 차단을 실시하여 허혈을 유발시킨 후 재관류 없이 심근보호액을 45초(제II-1), 1분(제II-2군), 3분(제II-3군) 주입한 군과, 작업성 관류 20분 후 45초(제III-1군), 1분(제III-2군) 및 3분(제III-3군)간 대동맥 차단을 실시한 후 2분간 재관류를 실시하여 심장을 회복시킨 후 다시 동일 방법으로 2회 실시 후 심근보호액을 주입한 군으로 나누었으며, 모든 군에서 60분간 보존시킨 후 재관류를 실시하여 회복정도를 혈역학적 성적만을 측정 비교하였다. 결과: 자연 심박동 출현시간은 대조군에 비해 제I군, 제II-3군, 제III-2군 및 제III-3군에서 매우 늦은 출현시간을 보였고(p<0.01), 제II-1군과 제III-1군에서도 늦은 출현은 보였지만 통계적 유의성은 없었다(p=NS). 심박동수의 비교에서는 대조군에 비해 제III-1군이 가장 좋은 회복을 보였고(p<0.05), 비교군 간의 비교에서도 제III-1군이 제II-1군보다 좋은 회복을 보였다(p<0.05). 대동맥 수축기압에서도 제III-1군(p<0.05)에서 좋은 회복을 보였고, 대동맥 차단 군간에서도 제III-1군이 제II-1군에 비해 가장 좋은 회복을 보였다(1<0.01). 심박출량에서는 대조군에 비해 제III-1군이 좋은 회복률을 보였고(p<0.05), 비교군 간에서는 제III-1군이 제II-1군보다 좋은 회복을 보였으나 통계적 유의성은 없었지만(p=NS), 제III-2군이 제II-2군에 비해 좋은 회복을 보였다(p<0.05). 심부종의 평가에서는 대조군에 비해 제I군(p<0.01)과 제II-3군(p<0.05)에서 심부종이 심한 것을 알 수 있었다. 결론: 적출 심장만으로는 다른 장기의 영향을 배제한 경우에 심근보호액 자체보다 허혈 전조건화를 부여한 심근보호액 군에서 허혈 전조건화 시 심박동의 이상 징후가 출현하기 직전까지 짧은 시간 동안 허혈을 실시한 후 재관류시킨 뒤 심정지액을 주입하여 심장을 보호하는 것이 심기능 회복에 효과가 있는 것으로 생각되며, 앞으로도 계속적인 연구가 필요할 것이다.

흰쥐에서 출혈성 쇼크 후 회복 시 저체온법 및 수액 치료에 따른 폐장의 염증성 변화 (Inflammatory Reponse of the Lung to Hypothermia and Fluid Therapy after Hemorrhagic Shock in Rats)

  • 배경: 출혈성 쇼크는 허혈 시 발생하는 산소산화물 등에 의해 체내에 여러 가지 염증반응을 일으킴으로써 각 장기의 기능부전을 초래한다. 특히 폐장은 허혈 시 조기에 폐포 세포의 투과성이 증가하여 부종, 염증세포의 침윤 및 출혈 등이 일어나 호흡부전증을 초래한다. 또한 출혈성 쇼크 시 기본적으로 시행하는 수액 요법은 간질의 부종을 일으켜 폐장의 기능을 감소시킬 수 있는 위험 요소이다. 따라서 출혈성 쇼크 후 폐장의 기능 저하를 예방하기 위해서는 폐장의 염증 반응을 줄이고 폐부종을 최소화시키는 노력이 필요하다. 대상 및 방법: $300{\sim}350$ gm 정도의 수컷 흰쥐를 이용하여 경정맥을 통해 약 3 mL/100 g의 혈액을 제거하여 평균 경동맥압 $35{\sim}40$ mmHg의 출혈성 쇼크 상태(I단계, 60분)를 유도하고 유지한 후, 제거한 혈액을 재주입하고 수액요법을 실시하여 평균 경동맥압을 80 mmHg로 유지하는 소생 상태(II 단계, 60분)를 시행한 후 약 3시간 정도 경과를 관찰(III 단계)하였다. 실험동물은 3군으로 나누어 실험하였으며 I군(n=10)은 I 단계 시 직장체온을 $37{\pm}1^{\circ}C$로 유지하고 II 단계에서 린저액을 이용하여 수액요법을 실시하였다. II 군(n=10)은 I 단계 시 직장체온을 $33{\pm}1^{\circ}C$로 유지하고 II단계에서 린저액을 이용한 수액요법을 실시하였다. III군은 I단계 시 $33{\pm}1^{\circ}C$로 체온을 유지하였고 II 단계에서 5% 알부민액을 이용하여 수액요법을 실시하였다. 각 군 모두 실험 전, I, II, III 단계 후반에 혈류역학적 인자(심박수, 평균 경동맥압), 동맥혈 가스 분석, 혈청내 포도당과 LDH, I, II단계의 투여 수액양, 기관지-폐포 세척액의 Interleukin(IL)-8을 측정하였고, 조직검사를 통해 염증반응의 정도를 조직학적 점수로 평가하였다. 결과: I군의 4예를 제외한 26예가 III단계까지 생존하였다. 각 군 간의 평균 경동맥압의 유의한 차이는 없었다. 그러나 실험 1단계에서의 채혈량은 I군은 $3.2{\pm}0.5$ mL/100 g으로 II, III 군의 $3.9{\pm}0.8$ mL/100 g, $4.1{\pm}0.7$ mL/100 g에 비해 각각 유의하게 적었다(p< 0.05). II 단계에서의 투여 수액량은 I 군 $28.6{\pm}6.0$ mL, II 군 $20.6{\pm}4.0$ mL, III 군 $14.7{\pm}2.7$ mL로 각 군 간에 통계적인 유의성이 있었다(p<0.05). 혈청내 칼륨 농도는 I군에서 II군에 비해 소생술 후 의의 있게 높았으며(p<0.05), 포도당 농도는 II단계의 I군에서 타군과 비교하여 현저히 낮았다(p<0.05). IL-8은 I 군 $1,834{\pm}437$ pg/mL, II 군 $1,006{\pm}532$ pg/mL, III군 $764{\pm}302$ pg/mL로 I 군에서 II 및 III군과 비교하여 통계적으로 유의하게 높았으며(p<0.05), 폐조직의 조직검사를 통해 평가한 염증세포 분포 점수에서 III 군이 $1.6{\pm}0.6$으로 I 군 $2.8{\pm}1.2$에 비해 통계적으로 유의하게 낮았다(p<0.05). 결론: 압력 조절형 출혈성 쇼크 모델에서 시행한 저체온법은 정상체온을 유지하고 있는 군에 비해 쇼크 상태에서의 기초대사량을 줄여줌으로써 허혈에 의한 조직의 직접적인 손상을 억제할 수 있으리라 생각된다. 또한 저체온법은 수액의 사용량을 줄여주고 IL-8등의 싸이토카인 분비를 억제시키며 백혈구의 침윤을 줄여줌으로써 쇼크 후 폐장의 기능 회복에 도움을 준다. 그러나 저체온법을 시행한 군에서도 투여하는 수액을 달리함으로써 폐장의 염증변화나 손상이 차이를 나타낼 수 있을 것으로 생각되며 이에 대한 세심한 연구가 있어야 할 것이다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N.
