Background: The purpose of this study is to investigate the effect of the intensive functional electrical stimulation(FES) on the improvement of the gait pattern of the chronic hemiplegic patients. Method: Six hemiplegic patients, who could walk independently but have equinovarus deformity during the gait cycle, participated in this study. The affected peroneus longus and tibialis anterior muscles of all subjects were stimulated for 2 weeks period (20 minutes duration, 6 times/day). We measured the activities (mean voltage) of those muscles during the walking, using dynamic EMG. Results: After treatment, there were significant improvements in the strength of peroneus longus and tibialis anterior muscles and the gait speed, but there was no interval change of the spasticity of plantar flexor. The mean voltages of two muscles are significantly increased in all the patients (p<0.05). Conclusion: The results showed that the intensive FES on affected peroneus longus and tibialis anterior muscles in chronic hemiplegic patients could be useful for the improvement of functional gait.

Background: Hemiplegic patients have the problems of the balance and weight shifting to the affected leg in walking. The aim of this study was to investigate the effect of unilateral stepping exercise combined with auditory feedback on balance and walking ability of the hemiplegic patients. Methods: Thirty hemiplegic patients were allocated in study group (n=15) or control group (n=15). General exercise and weight supporting exercise were conducted for the control group, and general exercises and unilateral stepping exercise combined with auditory feedback were conducted for the study group. Exercise were conducted three times a week for six weeks. Balance ability was measured by Berg Balance Scale (BBS), postural assessment scale for stroke (PASS), and performance-oriented mobility assessment (POMA). Walking ability was measured by time up and go test (TUG), 10m walk test (10mWT), and six minutes walk test (6MWT). Results: Balance and walking ability were significant increased in both group (p<.05). Balance and walking ability of the study group were more increased than that of the control group (p<.05). Conclusions: Unilateral stepping exercise combined with auditory feedback is more effective than weight supporting exercise to increase on balance and walking ability for the hemiplegic patients.
Purpose: Circumference of the chest and waist can be one of clinical indicator to reflect respiratory function in children with cerebral palsy. In this study, we compared to differences in the chest/waist circumference and maximal phonation time between children with spastic diplegia and hemiplegia. Methods: Seventeen children with spastic diplegic and hemiplegic cerebral palsy were recruited, who were matched to gender, age, height, weight, and body mass index for control of the known factors affected to respiratory function. The chest/waist circumference and were measured in each group, when children took a breath at rest and at maximal voluntary inspiration/expiration. Results: No significant differences were found in the chest and waist circumference and expansion between the two groups. However, only in the waist expansion, children with diplegic CP were significantly lower extensibility of lung, compared to the other group. In comparison of the maximal phonation time, a significant lower score was shown in children with spastic diplegic CP, compared to children with hemiplegic CP. Conclusion: Our results indicated that children with spastic diplegic CP had smaller chest wall and waist, compared to children with spastic hemiplegic CP. In addition, they showed a shorter time for sustaining phonation than spastic hemiplegic CP did. Therefore, spastic diplegic CP will be required for careful monitor regarding respiratory function in rehabilitation settings.
Little information is available about the characteristics in revolution gait of hemiplegic elderly, which is frequently represented in daily life. It is also hard to elucidate purely the characteristics due to hemiplegia because of no consideration of aging factors. The aim of study is to identify the alteration characteristics of lower extremity joint angles in both straight and revolution gaits together due to hemiplegia through comparing healthy with hemiplegic elderly. Following Institutional Review Board approval, twelve healthy and hemiplegic elderly were participated and the center of body mass (COM) and lower extremity joint angles were measured during straight and revolution gaits using a computer-aided video motion capture system. The results showed that the gait characteristics were generally altered in both straight and revolution gaits due to hemiplegia (p < 0.05). The gait characteristics were then different between the straight and revolution gaits each other. This study may be valuable by identifying for the first time the alterations of the lower extremity joint angles in both straight and revolution gaits due to pure hemiplegia through comparing healthy elderly with hemiplegic elderly.
Purpose : The purpose of this study was to analyze the effect of closed kinematic chain exercise and proprioceptive neuromuscular facilitation exercise on the static dynamic balance performance of hemiplegic patients in order to suggest them therapeutic intervention methods. Methods : The subjects of this study were 18 hemiplegic patients grouped into 2 subgroups according to the exercise program. one group of closed kinematic chain exercise carried out sit to stand, Hooklying with pelvic lift(bridging) and stair-up & down by a hemiplegic leg. The other group of proprioceptive neuromuscular facilitation exercise carried out leg flexion-extension pattern in supine position, leg flexion pattern in standing and stabilizing reversal exercise in stating position. Each exercise was carried out over 3 sets of 10reps. Results : The results of this study were summarized as follows: 1. For both groups, there were statistically significant changes in the static balance (FICSIT-4) performance after exercise program (p<.05). 2. For both groups, there were statistically significant changes in the dynamic balance (FSST, TUG, FRT) performance after exercise program (p<.05). 3. In the comparison between both groups, there was no statistically significant difference in the static dynamic balance performance (FICSIT-4, FSST, TUGT, ER) after exercise program. Conclusion : As the results of the study shows closed kinematic chain exercise and proprioceptive neuromuscular facilitation exercise affect the improvement of hemiplegic patients'' static dynamic balance performance, it is supposed that these exercises could be therapeutic exercise program in clinical situations.
Purpose: This study was conducted to study the acceptance of disability and influential factors between hemiplegic elderly and non-elderly after stroke. Methods: Data were collected with questionnaires from 104 elderly and 134 non-elderly with hemiplegia. Data were analyzed by $x^2$-test, t-test, ANOVA, ANCOVA, Pearson correlation coefficient, and multiple regression. Results: Significant predictors of acceptance of disability were family support, activities of daily living, and age, and these factors accounted for 24.2% of variance in acceptance of disability in the hemiplegic elderly. On the other hand, the significant predictors were family support and employment, and these factors accounted for 32.3% of variance in acceptance of disability in the non-elderly. Family support was the most influential variable in both the elderly and the non-elderly. Conclusion: An acceptance of disability program for the hemiplegic elderly should be designed differently from that for the non-elderly.
The purpose of this study was to compare a pelvic tilt angle between sound side and affected side in hemiplegic patients and the changing affected pelvic posterior tilt angle was measured at intervals of 3, 6, 9 weeks after Bobath approach. The subject for the study were 10 hemiplegic patients(mean age of 54.1 years)without orthopedic disability on pelvic bone. The data were analyzed by t-test, one-way ANOVA. The results of this study were as follows. There was a significant difference in the pelvic tilt angle between sound side and affected side in hemiplegic patients. There was a significant difference in affected pelvic posterior tilt angle between pre-treatment and post-treatment(9 weeks).
This study was undertaken to identify the influence which affect on gait speed and energy consumption regarding putting on arm sling during gait of the 40 hemiplegic patients selected from University Hospital and rehabilitation center in seoul during two months. The analysis of data was performed using the paired samples ttest to compare the differences of gait velocity, heart rate, oxygen consumption and oxygen cost in gait of preand post- arm sling. The results of this study were as follows; 1. When comparing the result before putting on arm sling in the gait of hemiplegic patient, gait velocity after putting on arm sling was statistically significantly increased(p<.05). 2. When comparing the result before putting on arm sling in the gait of hemiplegic patient, heart rate after putting on arm sling was statistically significantly decreased(p<.05). 3. When comparing the result before putting on arm sling in the gait of hemiplegic patient, oxygen consumption per weight after putting on arm sling was statistically significantly decreased(p<.05). 4. When comparing the result before putting on arm sling in the gait of hemiplegic patient, oxygen consumption rate per weight after putting on arm sling was statistically significantly decreased(p<.05). When putting together the above result, the gait with arm sling in comparison with the gait without arm sling was to increase gait velocity, decrease heart rate, decrease oxygen consumption and was finally to decrease energy consumption in the gait of hemiplegic patient.
This study investigated therapeutic effects of pelvic tilt exercise (PTE) on weight bearing and body sway during sit-to-stand (STS) on 18 hemiplegic patients who had visited the Hanyang University Seoul Hospital and Injae University Sanggyebek Hospital physiotherapy rooms. The study compared the patients with 18 normal adults. The subjects were sampled out from those who could get up independently, maintain a standing posture more than 10 seconds, understand the movements of this study and have no difficulty in performing the tasks. By executing STS in a natural way with habitual movements before and after PTE, the weight bearing was measured by using Mediance II. In order to compare the difference of weight distribution, weight bearing and body sway on affected and nonaffected sides during STS before and after PTE, the Wilcoxon Signed Ranks Test was used. The statistical significance level was based on p<.05. The results revealed that the difference of weight distribution in the hemiplegic group was significantly decreased (p<.05), whereas there was no significant difference in the healthy group (p>.05). Weight bearing loaded on the affected side was $42.53{\pm}7.65%$ and $44.20{\pm}6.32%$, respectively, in the hemiplegic group during STS before and after PTE. Weight bearing during STS after PTE is increased significantly, as compared with weight bearing before PTE (p<.05). Body sway in the hemiplegic group was significantly decreased (p<.05). As mentioned, PTE proved to be effective for improvement in weight bearing on the affected side during STS of hemiplegic patients.
The Posterior Leaf Spring (PLS) has been used for hemiplegic patients in order to help their walking and to increase their balance function. Past studies have mainly focused on the PLS's influence on patients' walking without taking balance function into consideration. The purpose of this study was to identify the immediate effect of PLS on the standing balance in hemiplegic patients. Fifteen hemiplegic patients participated in this study: 10 men and 5 women, with an average age of 53.8 years. Standing balance was measured using a computerized dynamic posturography device under three conditions namely bare-foot standing, standing in shoes without PLS, and standing in shoes with PLS. The results were as follows: 1) The composite equilibrium scores of patients who stood in shoes only and stood in shoes with PLS were higher than those of patients who stood bare-foot. 2) There were no statistical difference in the composite equilibrium scores between the standing condition of patients who stood bare foot and that of patients who stood in shoes with PLS. In conclusion, it is not clear whether or not the PLS affects the standing balance of hemiplegic patients. Further study is required to determine the precise effect of the PLS on standing balance in hemiplegic patients who are not wearing shoes. This is particularly relevant in Korea culture where custom demands the removal of shoes when entering any house or even many restaurants.
