Purpose: Children with cerebral palsy generally have a high incidence of respiratory problem, resulted from poor coughing, airway clearance problem, respiratory muscle weakness, kyphoscoliosis and so forth. The purpose of this study is to investigate the possible factors that can be affected to forced vital capacity (FVC) in children with cerebral palsy. Methods: Total thirty six children with diplegic and hemiplegic cerebral palsy were recruited in this study. They were evaluated by general demographic data (i.e., age, gender, body mass index (BMI)) and variables related to respiratory functions (i.e., chest mobility, waist mobility, maximal phonation time, and maximum inspiratory/expiratory pressure (MIP/MEP)). The correlation between forced vital capacity and the rested variables were analyzed, and multiple regression with stepwise method was conducted to predict respiratory function, in terms of FVC as the dependent variable, and demographic and other respiratory variables as the independent variable. Results: FVC showed a significant correlation with waist mobility (r=0.59, p<0.01), maximal phonation time (r=0.48, p<0.05), MIP (r=0.73, p<0.01), and MEP (r=0.60, p<0.01). In addition, the multiple regression analysis model indicated that FVC could be predicted by the assessment of each waist mobility and MIP. Conclusion: These finding suggest that respiratory function is related to body size and respiratory muscle strength, and that BMI, waist mobility, and MIP can be predictable factors to affected respiratory function in term of FVC.

Purpose : The purpose of this study was to find out the effect of closed kinetic chain exercise with functional electrical stimulation(FES) of the gluteus medius on gait in stroke. Methods : 30 hemiplegic patients voluntarily participated in this study. Subjects were divided into experimental group(n=15) and control group(n=15). Experimental group was given closed kinetic chain exercise with FES of the gluteus medius and control group was given only closed kinetic chain exercise for 4 weeks. All subjects were measured 10m-walking speed, cadence, functional walking category(FAC) and modified motor assessment scale(MMAS) before and after intervention. Results : In experimental group, gait velocity, cadence, FAC and MMAS showed significant difference between pre and post test(p<.05). In control group, gait velocity, cadence and FAC showed significant difference between pre and post test(p<.05). Before intervention, gait velocity, cadence, FAC and MMAS were not significant difference between experimental group and control group(p>.05), but after intervention, gait velocity, FAC and MMAS were significant difference(p<.05). Conclusion : This study show that closed kinetic chain exercise with functional electrical stimulation(FES) of the gluteus medius is beneficial intervention for increase the wlking ability in stroke.
Purpose: The purpose of this study was to determine the effects of 5 weeks self-help management program developed by Kim et al. (2000-b) and reinforced with music therapy for post stroke patients. Method: This was pre-experimental study and the subjects were 14 post stroke hemiplegic patients at home who were registered at the Gunsan Health Center. The program was applied for five weeks and two hours for each session composed of the preparation step, the main step and the finishing step. The contents of the program were ROM exercise, daily activity training, risk factors and aftereffects, nutrition management and stress management. To collect data, all subjects were questioned before and after the application of the program. Collected data were analyzed through frequencies, percentages, Wilcoxen signed rank test and Cronbach's alpha using SPSS-WIN program. Result: After the application of the self-help management program, a statistically significant increase was observed in the subjects' ability to perform activities of daily living (p=.039), ability to perform instrumental activities of daily living (p=.005), self-efficacy (p=.001), self-care behavior (p=.001) and quality of life (p=.001), and a statistically significant decrease was observed in depression (p=.012). Conclusion: The present self-help management program was found to be helpful in improving the subjects' physical and psychological functions after they were attacked by stroke, so to be an effective nursing intervention strategy for post stoke patients. Future researches need to reinforce and materialize music therapy and to develop and apply a self-help management program that includes not only post stroke patients but also their families. In addition, it is necessary to expand the scope of subjects and apply follow-up management in order to continue self-help meetings.

Purpose : This study examines the effects of pre-eccentric exercise and stretch ing to bicepsbrachii to prevent delayed onset muscle soreness and recovery of muscular function depending on the training intensity with 28 normal adults in their twenties. Methods : The subjects were divided into a control group, a group without any previous eccentric exercise, and a stretching group. Pre-eccentric exercise group conducted exercise with the intensity of 25% of maximal voluntary contraction. Pre-eccentric exercise and stretching was applied before to induce delayed onset muscle soreness and after, 24 hour post, 48 hour post, and 72 hour post. Measurements were conducted to examine pain and muscular function changes before, immediately after, and after inducing delayed onset muscle soreness. After inducing delayed onset muscle soreness, measurements were taken at the 24th hour, 48th hour, and 72nd hour. Results : The pre-eccentric exercise group and stretching group showed a significant difference from the control group by isometric contract ion power and mechanical pain threshold as a result of measuring delayed onset muscle soreness. Conclusion : From these results, electrical stimulation using presynaptic inhibition mechanism of transcutaneous electrical stimulation (TES) had positive effects for walking ability on inhibition of muscle tone in lower extremity. The motor level stimulation group experienced a more significant effect than the sensory level stimulation group. Therefore, the transcutaneous electrical stimulation (TES) is considered to be effective on walking ability increasing through inhibition of muscle tone in lower extremity for rehabilitation of post stroke hemiplegic patients.
The purpose of this study is to identify influence that eye movement have an effect on postural control and balance performance by plegia side of the impaired patients of central nervous system. Subjects are patients with the impairment of central nervous system and experimental and control groups are forty people and thirty people seperatively, Total subjects were selected to seventy persons, but twelve persons of experimental group quit during the experimental periods and eight persons of control group did not measure after exercise. Therefore, twenty eight persons of experimental group and twenty two persons of control group were selected in this study and experimental group peformed eye movement for eight weeks according to the exercise program of this study. The measurement of dependent variable is right static balance, left static balance, bilateral static balance, and bilateral dynamic balance before exercise and after eighth week of exercise, examiner again measured and analyzed the results. The results were as follows: 1. As the result of comparing balance performance ability after test of the experimental and control group with right hemiplegia and control group, all of static balance performance ability of right side (p < 0.01), static balance performance ability of left side (p < 0.01), static balance performance ability of bilateral side(p < 0.01), dynamic balance performance ability of bilateral side(p < 0.05) of experimental group were statistically greater improved than those of the control group. 2. As the result of comparing balance performance ability after test of the experimental and control group with left hemiplegia and control group, all of static balance performance ability of right side (p < 0.01), static balance performance ability of left side (p < 0.01), static balance performance ability of bilateral side(p < 0.05), dynamic balance performance ability of bilateral side(p < 0.05) of experimental group were statistically greater improved than those of the control group.
Purpose: The purpose of this study was to compare the blocked practice and random practice of task-oriented training in patients with chronic stroke to determine the effect of lower extremity muscle activity and balance ability. Methods: The thirty participants were randomly assigned to either the block practice group (BP) group or the random practice group (RP) and received the training three times per week, 30 minutes per day, for six weeks. Surface electromyography was used for measurement of lower extremity muscle activity. Static balance was to measured the stability index (SI) and weight distribution index (WDI) using the Tetrax. The four square step test (FSST) was used to measure dynamic balance. The paired t-test was used for determination of differences before and after intervention, and the independent t-test was used for determination of differences between groups. Results: Lower extremity muscle activity, RA and GCM was improved in the RP group after intervention and between groups. TA was significantly improved in the RP group compared with the BP group. In comparison of before and after interventions, SI was reduced in BP and RP. WDI in OS was reduced in comparison of BP and RP before and after intervention. CS was reduced in BP and RP. The OS and CS was improved in RP compared with BP. In comparison of before and after intervention, FSST was improved in BP and RP. Conclusion: Task-oriented training methods using random practice was found to be effective in promoting lower extremity muscle activity and balance ability in chronic stroke patients.
Objective : The purpose of this clinical study was to investigate the effects of moxibustion on functional recovery in stroke patients. Methods : Forty two stroke patients were randomized into either the standard physiotherapy treatment combined with moxibustion group or a control group with standard physiotherapy alone. They were 8 weeks from onset to the start of this study. Moxibustion was applied at 合谷(LI14), 外關(TE5), 曲池(LI11), 太衝(LIV3), 懸鍾(絶骨,G39), 足三里(S36) in hemiplegic upper and lower extremity, once a day for 6 weeks. The effect of treatment on functional recovery was assessed using the Functional Independence Measure scale. Statistical significance was achieved if the probability was less than 5%(p<0.05). Result : These 2 groups had comparable clinical characteristics; sex, age, lesion, and pre-treatment FIM score. After 6 weeks, patients in the moxibustion group performed better on FIM. The differences were significant(P=0.001). Conclusion : These results suggest that moxibustion is an effective treatment for functional recovery in stroke patients.

PURPOSE: This study aimed to investigate the effect of emphasized initial contact gait training on balance and gait ability in hemiplegia patients. METHODS: Twenty-four hemiplegic patients were randomly allocated to an experimental group or control group. All participants received 30-min neurodevelopmental treatment. Furthermore, the experimental group received initial contact-emphasized auditory feedback gait training, whereas the control group received gait training without auditory feedback. The intervention was performed 3 times per week, 20 min per each time, for a total of 6 weeks. Balance was assessed using the center of pressure path length, center of pressure velocity, and limitation of stability path length, whereas gait ability was assessed using the 10-m walking test and functional gait assessment. RESULTS: In both groups, center of pressure path length and center of pressure velocity significantly decreased after training. Compared to the control group, the experimental group showed a 10% significant improvement (p<.05). In the limitation of stability path length of both sides, the experimental group showed a significant increase compared to that before intervention. Compared to the control group, the experimental group showed a 7% significant improvement in results of the 10-m walking test and functional gait assessment (p<.05). CONCLUSION: Emphasized Initial contact gait training is considered an effective treatment for improving gait ability and balance ability in stroke patients.

PURPOSE: This study was conducted in chronic hemiplegic patients to examine the effect of the training of the ipsilateral arm that is identical to the model performing movements and the training of the contralateral arm on the function of the arm. METHODS: The subjects were participated total 2 patients(the subject 1 with left hemiplegia and the subject 2 with right hemiplegia). The study was conducted for 4 weeks. The action observation training were repeated 10 times in 10 days during intervention period. The evaluation of the arm function such as BBT, MFT and MAL in the each subject were examined 5 times in the baseline period, 10 times during the intervention period and 5 times during the baseline regression period. RESULTS: The results of the evaluation in each subject were presented as mean values and video graphs. The arm function of the 2 subjects were improved during the intervention period in comparison with the baseline period, and the improvement was maintained even during the regression baseline period. In addition, there were large variation ratio of BBT and MAL (AOU, QOM) in comparison with subject 1. CONCLUSION: According to the results, the action observation training was more effective in improving upper limb function of stroke patients who imitate the performed behavior of paralyed parts on the same side.

PURPOSE: This study compared two different techniques of treadmill gait training, and evaluated the outcomes on gait and respiratory function in patients with hemiplegic stroke. METHODS: This was a single-blinded, randomized, controlled, comparative study, enrolling a total of 21 stroke patients in a rehabilitation hospital. Subjects were randomly assigned to either the treadmill walking training group with gradual speed increase (GSI group, n = 10), or treadmill walking training group with random speed changing (RSC group, n = 11). All participants performed 60 min of comprehensive rehabilitation therapy (5 × / week for 6 weeks). In additional, each group received either GSI or RSC treadmill walking training for 20 min (5 × / week for 6 weeks, total 30 sessions). Gait and respiratory function were measured before and after the 6-week training. RESULTS: Both groups showed significant improvements in the 10-m walking test, 6-minute walking test, timed up and go test, forced vital capacity (FVC), forced expiratory volume in 1 second, and maximal voluntary ventilation after training (p < .05). The GSI group showed a significantly greater increase in the FVC than the RSC group (+14.8 L vs. +12.5 L, p < .05). CONCLUSION: Both training methods can be effective for improving the walking and respiratory functions of stroke patients. However, our results indicate that treadmill walking training with gradual speed increase might be a more effective method for improving the respiratory function (FVC) than treadmill walking training with random speed changing.
