  • This study was conducted to investigate the subacute transdermal toxicity of Syndella gel, a new topical drug containing deproteinized dialysate of calf's blood and micronomicin sulfate in Sprague-Dawley rats. Three doses (1.97, 3.94, 7.88 g/kg) of Syndella gel was daily treated transdermally to male and female rats for 30 days. No death was occurred in either control or treated rats. No significant toxic clinical signs and body weight change were not observed at any doses in the male or female rats treated. There were no significant alterations in hematologic and biochemical parameters in both sexes, however slight increase of potassium concentration was observed in 3.94g/kg and 7.88 g/kg female groups. No significant necrotic changes were not observed in examined organs. This study showed that up to 7.88g/kg Syndella gel did not induce subacute transdermal toxicity.

Korean Red Ginseng Protects Oxidative Injury Caused by Lead Poisoning

  • Lead (Pb) is a metal that is generally considered to be toxic to the cardiovascular system. Pb-exposed animals display the evidence of increased oxidative stress and hypertension. The current study was designed to examine whether Korean red ginseng (KRG) has protective effects against Pb-induced hypertension and oxidative stress in Pb-exposed rats. Male Sprague-Dawley rats were randomly assigned to Pb exposure or control groups. KRG was administered in drinking water at a concentration of 100 mg/kg/day; the control group received plain drinking water. Animals in the Pb-exposed groups were provided with drinking water containing 100 ppm Pb acetate for 12 weeks. Blood pressure, plasma glutathione, blood Pb concentration, and hematologic data, such as red blood cell quantity, were determined. Pb poisoning was assessed by measuring the blood Pb concentration. Pb exposure (100 ppm) for 12 weeks resulted in a marked rise in systolic blood pressure and blood Pb concentration, as well as a significant reduction in plasma glutathione levels and red blood cell quantity. Other measurements, such as heart rate, body weight, and white blood cell quantity, were unchanged. Treatment with KRG significantly lowered blood pressure, raised plasma glutathione and increased red blood cell numbers in Pb-exposed animals; it also had no effect on heart rate, body weight, or white blood cell quantity. However, the elevated blood Pb concentration was not reduced by treatment with KRG (100 mg/kg). Taken together, these data indicate that treatment with KRG in Pb-exposed animals can reduce oxidative stress and lower blood pressure, suggesting that KRG might be protective against Pb-exposed hypertension and oxidative stress.

  • Wilson's disease is an autosomal recessive disorder of copper metabolism consequence of which leads to accumulation of copper in the liver, brain, cornea and other tissues. The manifestations are more likely to be hepatic in the early childhood and neurological in the adolescents. In addition, the abnormalities that develop during disease progression may result in other manifestations such as hematologic, endocrine, or renal findings. We report a thirteen year-old girl who manifested tetany shortly after the initial diagnosis of Wilson's disease. Despite aggressive calcium, magnesium and vitamin D replacement, the hypocalcemia and hypomagnesemia did not respond to the therapy promptly. It took more than three weeks for blood levels of the minerals to be normal. We concluded that tetany occurred in our patient because of hypoparathyroidism as a rare complication of Wilson disease, vitamin D deficiency resulting from various conditions, and inconclusive hypomagnesemia.

  • The phenotypes of hemoglobin, albumin and transferrin of 3U2 Jindo dogs in Jindo area were studied by starch gel electrophoresis for hemoglobin and albumin, and by polyacrylamide gradient gel electrophoresis for transferrin. The results obtained were as follows: 1. In the hemoglobin phenotypes, three phenotypes, HbAA, HbAB and HbBB, which were controlled by two allelic genes, $Hb^A$ and $Hb^B$, were observed and their frequencies of appearance were 1.65%, 10.60% and 87.75% respectively. The distribution of gene frequency was calculated as 0.0695 in $Hb^A$ and 0.9305 in $Hb^B$. 2. In the albumin phenotypes, three phenotypes, Alb FF, Alb FS and Alb SS, which were controlled by two allelic genes, $AIb^F$ and $AIb^S$ were observed and their frequencies of appearance were 12.59%, 25.56% and 61.85% respectively. The distribution of gene frequency was calculated as 0.2537 in $AIb^F$ and 0.7463 in $AIb^S$. 3. Analysis of transferrin phenotypes showed 6 different types which were controlled by three allelic genes, $Tf^B$, $Tf^C$ and $Tf^D$ and their frequencies of appearance were 54.04% in TfBB, 17.54% in TfBC, 9.82% in TfBD, 8.07% in TfCC, 7.37% in TfCD and 3.16% in TfDD. The distribution of gene frequency was calculated as 0.6772 in $Tf^B$, 0.2053 in $Tf^C$ and 0.1175 in $Tf^D$.

The Analysis of Research on the Home-Based Pulmonary Rehabilitation Program (가정 호흡재활 프로그램 관련 연구 논문 분석)

  • Purpose: The purpose of this study was to analyze the trend of research on the use of home-based pulmonary rehabilitation. Method: Using Medline and CINAHL search engine, experiemental research, titled as 'home-based pulmonary rehabilitation' and 'chronic lung disease', statistics were collected. The total 29 experimental studies published between 1981 to 2000 were selected and analyzed according to the ATS guidelines. Result: 1) The study samples in research were mostly of men, whose lung function was moderate to severe. 2) The total period of the intervention program varied from 5 to 12 week. The program included excercise intervention and educational intervention. Psychosocial intervention was minimal. 3) The outcome of the home-based pulmonary rehabilitation had been measured mostly by physical aspects. Quality of life and cost were less frequently measured. 4) In terms of effectiveness of the home based pulmonary rehabilitation program, physiologic outcome, such as lung function and hematologic markers, there was more 'no effect' than 'positive effect', whereas there was more 'positive effect' in decreasing dyspnea, improving excercise capacity, and improving quality of life. Conclusion: Based on these findings, future research on home-based pulmonary rehabilitation should be emphasized and provide standardized protocol, including psychosocial intervention, and analyses on cost and quality of life.

Prognostic Value of Hematologic Parameters in Patients with Metastatic Renal Cell Carcinoma Using Tyrosine Kinase Inhibitors

  • Background: The prognostic significance of the neutrophil-to-lymphocyte ratio for progression free survival in patients with metastatic renal cell carcinoma is unclear. Materials and Methods: We retrospectively reviewed 45 patients diagnosed with metastatic RCC previously treated with tyrosine kinase inhibitors from two centers, Akdeniz University Hospital and Afyon Kocatepe University. The prognostic value of the pretreatment neutrophil-tolymphocyte ratio, and other clinical and laboratory parameters were assessed by univariate and multivariate analysis. Results: Median progression free survival (PFS) was 13.9 months [95% CI for HR (6.88-20.91)] and overall survival figure of 16.6 months [95% CI for HR (7.23-26.03)] Univariate analysis revealed that PFS was significantly affected by hemoglobin level [p=0.013 (95% CI for HR (0.71-0.96))], eosinophil count [p=0.031 (95% CI for HR (0.20-0.92))], ratio of neutrophil lymphocytes (NLR) [p=0.007 (95% CI for HR (1.47-11.74))] and calcium level [p=0.006 (95% CI for HR (0.15-0.73))]. However, only NLR [p=0.031 (95% CI for HR (1.15-18.1))] and calcium levels [p=0.018 (95% CI for HR (0.20-18.1))] retained significance with multivariate analysis. Median PFS was 23.9 vs 8.6 months in patients with NLR ${\leq}2$ vs NLR >2 (Log rank; p= 0.040). Conclusions: This study showed that increased pretreatment NLR is an independent prognostic factor for patients with metastatic RCC using tyrosine kinase inhibitors.

Hematologic Toxicity in Patients Undergoing Radical Anti-cancer Therapy: A Cross-Sectional Analysis of Patients in an Oncology Ward in India

  • Burden of cancer is progressively increasing in developing countries like India which has also led to a steep rise in toxicity due to anti-cancer therapy. A cross-sectional analysis was here conducted for patients with different malignancies (except leukaemia) who while undergoing radical anti-cancer therapy were admitted to our oncology ward from January-July 2013. In a total of 280 patients, the total number of toxicity events was 473. Nine patients expired over this time period. Among the events, grade 2 anaemia the most common (n=189) while the most common grades of neutropenia and thrombocytopenia were grade 4 (n=114) and grade 2 (n=48), respectively. Among the tracable microbial etiologies, gram negative bacteria were the most commonly found pathogens. Treatment interruptions took place in 240 patients (median duration=8.8 days). Prolonged hospital admission, intensive care and artificial ventilation support was needed to be given in 48, 7 and 13 patients respectively. Advanced NSCLC, KPS <70, pancytopenia and artificial ventilation requirement were found to have a significant impact on death. Such studies show the prevailing practice from institutes of our country and may guide us formulating a guideline for managing such toxicities for this part of the world.

Two Cases of Severe Pancytopenia Associated with Low-Dose Methotrexate Therapy in Patients with Chronic Kidney Disease and Rheumatoid Arthritis (류마티스 관절염을 가진 만성신질환 환자에서 저용량 methotrexate 투여 후 발생한 중증 범혈구 감소증 2예)

  • Due to its efficacy and tolerability, low dose oral methotrexate (MTX) therapy has been widely used for treatment of rheumatoid arthritis (RA). However, it can rarely cause serious, life-threatening hematologic toxicities, such as pancytopenia. We report here on two patients with chronic kidney disease (CKD), who developed severe pancytopenia after 5 years (cumulative dose 1,240 mg) and 4 years (cumulative dose 1,320 mg) of low dose MTX therapy for treatment of RA, respectively. Both patients presented with renal insufficiency, hypoalbuminemia, concurrent use of nonsteroidal anti-inflammatory drugs, and elevated mean corpuscular volume of red blood cells (RECs), all of which are known as risk factors of MTX-induced pancytopenia. Despite receiving treatment, which included REC and platelet transfusions, antibiotic therapy, granulocyte colony stimulating factor, and leucovorin rescue, one patient died of sepsis. Based on our case study, prompt investigation of risk factors associated with MTX toxicity is required for all patients receiving MTX therapy. MTX treatment, even at a low dose, should be discontinued in patients with advanced CKD.

The Relationships among Social Support, Hope, Anxiety, and Depression in Patients Expecting to Receive Hematopoietic Stem Cell Transplantation (조혈모세포 이식 전 환자의 사회적 지지, 희망, 불안 및 우울의 관계)

  • Purpose: This study aimed to examine the relationships among social support(family support, medical team support), hope, anxiety, and depression in patients with hematologic cancers before they received hematopoietic stem cell transplantation (HSCT) to obtain baseline data for developing a nursing intervention. Methods: The participants were 70 adult patients expecting to receive HSCT from 5 university hospitals in Seoul, Gyeonggi-do, and Jeollanam-do regions. A cross-sectional survey was done using standardized instruments for social support (Tae's Family Support Scale and Professional Medical Support Scale), hope (Kim & Lee Hope Scale), anxiety and depression (Hospital Anxiety and Depression Scale). The data were analyzed by SPSS/WIN 19.0 program using frequency, percentage, item mean and standard deviation, t-test, ANOVA, and Pearson's correlation coefficient. Results: Hope was significantly correlated with social support (r=.40, p=.001), anxiety (r=-.40, p<.001) and depression (r=-.58, p<.001). Anxiety was correlated with depression (r=.54, p<.001). Conclusion: The findings of this study show greater social support for patients who expect to receive HSCT is significantly correlated to a higher level of hope, as well as low levels of anxiety and depression. In nursing practice, clinical nurses may develop a nursing intervention to reinforce social support and hope, as well as reduce anxiety and depression for patients preparing for HSCT.

The Levels of Physical Activity and Its Relationships with Depression, Health-related Quality of Life, Sleep Disturbance, and Physiological Indicators in Hemodialysis Patients (혈액투석 환자의 신체활동 수준과 우울, 삶의 질, 수면장애 및 생리적 지수와의 관련성)

  • Purpose: The purpose of this study was to identify the levels of physical activity and its relationships with depression, health-related quality of life (QoL), sleep disturbance, and physiological indicators in hemodialysis patients. Methods: The participants in this study were 139 patients undergoing hemodialysis in the hospitals in Busan and Yangsan-si. Data were collected using hematologic tests and questionnaires that contained items about individual characteristics, International Physical Activity Questionnaires (IPAQ), Center For Epidemiological Studies Depression Scale (CES-D), a 12-item Short-Form Health Survey (SF-12), and Pittsburgh Sleep Quality Index (PSQI). Data were analyzed using descriptive statistics, one way ANOVA, and correlation. Results: Mean age of the participants was $59.91{\pm}12.63$ and mean physical activity levels were $1,660.35{\pm}1,654.17$. Patients who performed higher physical activities during their daily activities reported lower levels of depression (F=4.16, p=.018) and higher levels of QOL (PCS: F=5.00, p=.008, MCS: F=8.66, p<.001) than those of the others who did not perform physical activities. Conclusion: This study showed that the levels of physical activity among hemodialysis patients was significantly associated with their depression and QOL. Developing strategies for enhancing physical activity is warranted to improve depression and QOL among hemodialysis patients.
