A five-year-old, male, Great Dane weighing 107 kg was presented with anorexia, abdominal distension, and dyspnea for 5 days. Physical examination, blood works, radiography, electrocardiography (ECG), and echocardiography were performed. Based on severely low fractional shortening (FS) and marked four chamber enlargement in echocardiography, continuous atrial fibrillation and occasional ventricular premature complex (VPC) on ECG, the dog was diagnosed as dilated cardiomyopathy (DCM) concurrent with congestive heart failure. Pleural effusion and ascites were modified transudate. In accordance with DCM scoring system recommended by European Society of Veterinary Cardiology (ESVC), DCM score was 13/15 in this case. Concentrations of cTnI and NT-pro-BNP were 1.0 ng/mL and 693 pmol/L, respectively. Since the former and the latter were remarkably high values, it was certain that the patient had grave prognosis. Intensive care was performed for the dog and the clinical signs as well as the radiographic abnormalities were resolved. However, when he presented serious dyspnea again at 25 days post therapy, the dog was dead. In case of canine DCM, the scoring system for the diagnosis and cardiac biomarkers including NT-pro-BNP and cTnI could be useful to advise owners on the status and prognosis of their dog with DCM.

Background: Postinfarction ventricular septal defects (pVSDs) are a serious complication of acute myocardial infarctions. The aim of this study was to analyze the clinical outcomes of the surgical treatment of pVSDs. Methods: The medical records of 23 patients who underwent operations (infarct exclusion in 21 patients and patch closure in two patients) to treat acute pVSDs from 2001 to 2011 were analyzed. Intra-aortic balloon counterpulsation was performed in 19 patients (82.6%), one of whom required extracorporeal membrane support due to cardiogenic shock. The mean follow-up duration was $26.2{\pm}18.6months$. Results: The in-hospital mortality rate was 4.3% (1/23). Residual shunts were found in seven patients and three patients required reoperation. One patient needed reoperation due to the transformation of an intracardiac hematoma into an abscess. No patients required reoperation due to recurrence of a ventricular septal defect during the follow-up period. The cumulative survival rate was 95.5% at one year, 82.0% at five years, and 65.6% at seven years. Conclusion: The use of a multiple-patch technique with sealants appears to be a reliable method of reducing early mortality and the risk of significant residual shunting in patients with pVSDs.

This study aimed to describe the prevalence of comorbidities associated with chronic obstructive pulmonary disease (COPD) and their relation with relevant outcomes. A systematic review based on the PRISMA methodology was performed from January 2020 until July 2021. The MEDLINE, Lilacs, and Scielo databases were searched to identify studies related to COPD and its comorbidities. Observational studies on the prevalence of comorbidities in COPD patients and costs with health estimates, reduced quality of life, and mortality were included. Studies that were restricted to one or more COPD pain assessments and only specific comorbidities such as osteoporosis, bronchitis, and asthma were excluded. The initial search identified 1,409 studies and after applying the inclusion and exclusion criteria, 20 studies were finally selected for analysis (comprising data from 447,459 COPD subjects). The most frequent COPD comorbidities were: hypertension (range, 17%-64.7%), coronary artery disease (19.9%-47.8%), diabetes (10.2%-45%), osteoarthritis (18%-43.8%), psychiatric conditions (12.1%-33%), and asthma (14.7%-32.5%). Several comorbidities had an impact on the frequency and severity of COPD exacerbations, quality of life, and mortality risk, in particular malignancies, coronary artery disease, chronic heart failure, and cardiac arrhythmias. Comorbidities, especially cardiovascular diseases and diabetes, are frequent in COPD patients, and some of them are associated with higher mortality.
연구배경: 재택산소요법의 건강보험 급여 혜택이 있기 이전에 이뤄진 국내 보고는 특수한 일부 환자군만을 대상으로 하였을 가능성이 높아 재택산소 사용의 정확한 실태를 반영하지 못했을 가능성이 높다. 저자들은 재택산소요법의 보험 혜택 이후 시점부터 본 병원에서 재택산소처방을 받은 환자군의 임상적 특성, 산소사용의 실태 및 순응도에 대해 조사하여 이전의 국내 보고들과 비교해 보고자 하였다. 방 법: 자가산소요법의 건강보험 급여 혜택이 시작된 2006년 11월부터 2008년 9월까지 인제대학교 백병원 호흡기 내과를 방문하여 재택산소처방을 받고 외래에서 추적관찰 중인 환자를 대상으로 하여 의무기록 검토와 설문조사를 하였다. 결 과: 대상 환자수는 총 105명, 평균 나이 69세이고, BMI 19.1 $kg/m^2$, 남녀의 비는 60:45명이었다. 산소를 처방하게 된 기저 질환은 COPD 64명, 폐암 14명, 결핵으로 파괴된 폐가 13명, 기관지확장증 3명이고 그 외 폐성심, 기관식도루, 천식, 울혈성 심부전, 폐렴이 있었다. 평균 동맥산소분압은 54.4 mmHg, 산소포화도는 86.3%, 평균 헤모글로빈은 11.9 g/dL이었고, 평균 심실구혈률은 61.1%, 우심실수축기압력 42.7 mmHg이었다. 88명에서 시행한 폐기능검사에서 평균 FVC 2.05 L (66.5%), $FEV_1$ 1.12 L (52.0%), $FEV_1$/FVC 55.6이었다. 평균 산소처방기간은 8.2개월이었고, 하루 평균 산소시간은 9.8시간이었다. 그 중 평균외출시간 5.4${\pm}$3.7시간 동안은 산소를 사용하지 못했다. 산소사용을 중단한 사람은 66명으로 62.8%를 차지하였고 그 원인으로 사망이 33명(50%), 임의중단 24명(36%), 저산소증 호전 8명(12%), 입원한 경우가 1명 있었다. 임의 중단 환자는 산소처방 후 평균 7.0${\pm}$4.7개월 의 시점에 산소를 중단하였다. 임의중단 사유는 증상호전 11명(45%), 사용불편 6명(25%), 증상호전 없음 4명(17%), 경제적 비용부담 3명(13%) 순이었다. 결 론: 재택산소 처방 후 충분한 산소사용 시간 확보 및 산소의 지속적인 사용을 유지하려면 산소사용에 대한 환자 및 보호자에 대한 교육, 일반인을 대상으로 한 홍보, 경제적 부담 완화 등의 추가적인 노력이 있어야겠고, 야외활동 중에도 사용이 가능한 휴대용 산소장치에 대한 보험 혜택 확대가 필요하다고 하겠다.
심장 질환 가운데에서 부정맥은 방치할 경우에 뇌졸중, 심장 마비, 심부전과 같은 심각한 합병증이 발생할 수 있기 때문에 지속적이고 정확한 심전도 관리에 의한 건강 상태의 확인은 임상적 치료에 매우 중요한 요소이다. 그러나, 심전도(Electrocardiogram; ECG) 데이터의 정확한 해석은 전적으로 의료 전문가에 의존하기 때문에 부가적인 시간과 비용을 요구한다. 따라서 본 논문에서는 라이프로그 기반의 비정상적인 맥파 파형의 분석을 통한 의료 플랫폼 개발을 목적으로 부정맥 인식 모듈을 제안한다. 제안하는 방법은 ECG 데이터를 시계열 데이터가 아닌 이미지 형식으로 처리하여 시각적 패턴 인식 기술을 적용한 후, CNN 모델을 이용하여 부정맥을 탐지하는 방법을 제안한다. 본 논문에서 제안한 ECG 데이터의 이미지 타입 변환에 의한 CNN 모델의 부정맥 분류의 유효성 검증하기 위해 MIT-BIH 부정맥 데이터셋을 사용한 결과, 97%의 정확도를 보였다.
The microsurgical reconstruction is necessary for elderly patients to treat severe trauma and head and neck tumor. The aim of this study is to analyze the risks of microvascular surgery and whether or not happening of more complication in elderly patients who are older than 60 years old and to suggest the solution of the complication. The retrospective study included 41 elderly patients who underwent treatment of 44 microsurgical reconstructions among total 271 cases of microsurgical reconstruction from July, 1988 to December, 1998. Their ages ranged from 61 years to 79 years. There were 26 males and 15 females. The involved sites were 23 head and necks, 13 upper gastrointestinal tracts, 3 lower extremities, 1 chest and 1 sacral region. The causes of microsurgical reconstruction were 36 head and neck tumors, 2 radionecrosis, 2 traumas and 1 melanoma in lower limb. The used flaps were 14 radial forearm flaps, 13 jejunal flaps, 10 latissimus dorsi muscle flaps, 3 rectus abdominis muscle flaps, 2 lateral arm flaps, 1 scapular flap, and 1 iliac osteocutaneous flap. They had medical problems which were 29 tobacco abuse, 14 hypertensions, 13 alcohol abuse, 10 chronic obstructive pulmonary diseases, 7 diabetes mellituses, 3 ischemic heart diseases. All patients have had successful results without specific complications except 3 cases of free flap failure and 3 perioperative death. The causes of 3 flap failures were 2 flap necrosis due to arterial insufficiency and 1 flap loss due to secondary infection. All of these cases were treated with secondary free flap surgery. However 3 patients died perioperatively due to 2 respiratory arrests and 1 sepsis. It was not related to operate microsurgical reconstruction itself, but was correlated with the complication of postoperative care after head and neck surgery. We conclude that plastic surgeons consider the importance of prevention of expected complication as thorough analysis of operative risk factor and appropriate treatment. We had to select the donor and recipient vessel appropriately to perform successful microsurgery in elderly patients and consider vein graft and end-to-side anastomosis to reduce complication if necessary. In addition, we emphasize the importance of pre, peri and postoperative care in head and neck cancer patients to reduce postoperative complication and morbidity.
배경 : 최근 중재적 시술과 심부전에 대한 내과적 치료의 발전으로 관상동맥우회술의 대상이 되는 허혈성 심근질환을 가진 환자들이 늘어나고 있다. 우리는 좌심실 기능이 감소된 이러한 환자들에 시행 된 관상동맥우회술의 결과를 후향적으로 분석하여 중단기 결과를 알아보고자 한다. 대상 및 방법: 2001년 1월부터 2005년 6월까지 관상동맥우회술을 시행 받은 1,143 명의 환자 중 죄심실박출지수가 35% 이하인 환자는 144명이었다. 인공심폐기를 사용하지 않고 관상동맥우회술(‘off-pump’ coronary artery bypass grafting, OPCAB) 을 시행한 경우가 66예(45.8%), 인공심폐기를 사용하고 뛰는 심장에서 관상동맥우회술(on-pump beating heart coronary artery bypass grafting)을 시행한 경우가 34예(23.6%), 전통적인 관상동맥우회술(conventional coronary artery bypass grafting)을 시행한 경우가 44예(30.6%) 였다. 동반된 심장수술로는 승모판륜 성형술을 포함하여 35명(24.3%)의 환자들에서 48건이 시행되었다. 결과: 환자당 원위 문합수는 $3.5{\pm}1.3%$개였고 술 후 중환자실 체류기간과 재원기간의 중간값은 각각 2일과 8일이었다. 조기 사망은 6명(4.2%)으로 심실성 빈맥으로 인한 경우가 5명, 소장경색이 1명이었다. 평균 추적기간은 $21{\pm}14%(2{\sim}54$개월)였다. 1년 생존율은 $95{\pm}2%$, 3년 생존율은 $83{\pm}7%$였고 심장관련 event-free 1년, 3년 생존율은 각각 ${88{\pm}3%,\;69{\pm}7%}$였다. 결론: 만족할 만한 중단기 결과에 따라 허혈성 심근질환에서도 적극적으로 관상동맥우회술이 시행되어야 할 것으로 생각되며 술 후 심실성 빈맥에 대한 치료를 집중적으로 시행한다면 더 좋은 결과를 보이리라 생각된다.
배경: 경피적 심폐 보조장치(PCPS)는 체내의 가스교환과 혈액의 관류를 간접적으로 도와줌으로써, 장기를 회복시키기 위한 다른 치료법의 사용을 용이하게 하고, 여러 다양한 임상 상황에서 출혈의 위험성을 최소화하면서 심각한 심폐부전에 빠진 환자의 생명을 구해준다. 본 저자들은 허혈성 심질환자들에서 발생한 심인성 쇼크 혹은 심정지를 PCPS로 치료한 경험을 보고하고자 한다. 대상 및 방법:1999년 5월부터 2005년 6월까지 발생한 심정지 혹은 심인성 쇼크 환자들을 대상으로 연구가 진행되었고, 환자들 중 7명에서 원심펌프인 Biopump (Medtronic inc., Minneapolis, MN), 나머지 13명에게는 자가 priming이 되는 헤파린-코팅된 EBS (Terumo, Japan)를 사용하였다. 삽관은 모든 환자에서 대퇴동맥과 대퇴 정맥을 통해서 이루어졌으며, 동맥용 케눌라는 17 Fr에서 21 Fr까지, 정맥용은 21 Fr에서 28 Fr까지 의 DLP (Medtronic inc., Minneapolis, MN) 또는 RMI (Edwards lifescienc LLC, Irvine, CA)의 비교적 긴 정맥용 케눌라를 사용하였다. 결과: 소생을 목적으로 PCPS가 시도되었던 총 20명의 환자들 중에서 심정지가 13명이었으며, 심인성 쇼크였던 환자가 7명이었다. 이들 중 11명에서 관동맥 우회술이 시행되었고, 나머지 9명의 환자에서 PCPS한 상태에서 경피적 관동맥 중재술이 시도되었다. 평균 PCPS가동 시간은 38$\pm$42시간이었으며, 총 20명의 환자들 중 PCPS 이탈이 가능하였던 11명($55\%$)의 환자 중 총 8명($40\%$)의 환자가 특별한 합병증 얼이 평균 27$\pm$17일만에 퇴원하였다. 퇴원한 환자는 현재 모두 생존하여 치장 31개월째 외래 추적관찰 중이다. 결론: PCPS를 시행함으로써 혈역학적 회복을 가져옴으로써 다른 방법으로는 살리기 힘들었던 심정지 또는 심인성 쇼크로부터 환자를 구할 수 있고, 일단 회복된 환자들은 장기 생존율을 보인다.
연구배경 : B-type natriuretic peptide(BNP)는 주로 심장의 심실에서 분비되는 호르몬으로서 심부전의 진단, 중증도 및 예후와도 연관이 있다고 알려져 있다. 중환자의 경우 급성병색시 심부전이 올수 있는 것으로 알려져있고 이는 환자의 예후와 연관이 있을 가능성이 있다. 그래서 저자들은 우선 이전 심질환의 병력이 없는 중환자들에서 BNP를 측정하여 증가여부를 살펴보고, BNP와 중환자의 중등도 및 예후와의 관련성 여부를 살펴보기로 하였다. 방 법 : 2002년 6월부터 10월까지 본원 중환자실에 입원하였던 환자 중 환자의 중증도와는 관련이 없이 BNP가 증가될 수 있는 울혈성 심부전, 심방 세동, 허혈성 심질환, 신부전 등을 제외한 32명을 대상으로 하였고, 대조군으로는 일반 병동 및 외래 환자 32명을 무작위로 추출하였다. BNP는 Triage B-Type Natriuretic Peptide test를 이용하여 fluorescence immunoassay를 통해 측정하였다. APACHE II score와 APACHE III score 및 중환자실 사망 여부를 추적하였다. 결 과 : 남녀비는 16 : 16이었고, 연령은 $59.2{\pm}20.6$세 였다. BNP는 $186.7{\pm}274.1$ pg/mL으로 정상 대조군의 $19.9{\pm}21.3$ pg/mL 보다 유의하게 높았다. 중환자군에서 BNP 100 pg/mL이상은 14명으로 43.8%였다. APACHE II score는 $16.5{\pm}7.6$이었고, 11명이 사망하였다. BNP값은 APACHE II score및 사망과 유의한 상관 관계를 보였고(r=0.443, p=0.011, r=0.530, p=0.002), 생존군과 사망군 BNP값은 유의한 차이를 보였으나($83.2{\pm}55.8$ pg/mL vs. $384.1{\pm}401.7$ pg/mL ; p=0.033), $PaO_2/FiO_2$와는 상관 관계를 보이지 않았다. 결 론 : BNP는 이전 심질환병력이 없었던 중환자에서 증가되어 있었고, 사망률 및 중증도 점수와도 유의한 상관 관계가 있었다. 따라서 중환자들의 예후를 예측하는데 비침습적이고 빠른 방법으로 도움이 될 가능성이 있다.
Objective: The 3-hydroxy-3-methylglutaryl coenzyme A reductase inhibitors (statins) are frequently prescribed medications worldwide for the treatment of hypercholesterolemia. Statins are considered to be well tolerated; however, they have a potential for myotoxicity. Concomitant drugs that inhibit cytochrome P450 3A4 can increase the concentration of statins and thus the risk of developing myotoxicity. The purpose of this study was to evaluate risk factors associated with potential drug-drug interactions in patients receiving statins. Methods: The subjects of this study were patients aged more than 18 years who received at least one prescription of statins in a general hospital located in Chuncheon-si, Korea, between January 1, 2018, and March 31, 2018. Data regarding statin use and baseline characteristics was collected from the computerized hospital database. Logistic regression analysis was used to identify risk factors associated with potential drug-drug interactions. Results: A total of 1061 patients were finally included in the study. The incidence of potential drug-drug interactions was 45% in all subjects. According to the results of the multivariate logistic regression analysis, myocardial infarction as the indication of statin, arrhythmia or heart failure as a comorbidity, and aspartate aminotransferase levels higher than 40 IU/L were significant risk factors for potential drug-drug interactions in study subjects. Diltiazem was the most commonly co-prescribed drug that caused potential drug-drug interactions with statins. Conclusion: There was a considerable rate of potential drug-drug interactions in patients receiving statins. Health care professionals should attempt to reduce potential drug-drug interactions during statin administration.

① NDSL에서 제공하는 모든 저작물의 저작권은 원저작자에게 있으며, KISTI는 복제/배포/전송권을 확보하고
있습니다.
② NDSL에서 제공하는 콘텐츠를 상업적 및 기타 영리목적으로 복제/배포/전송할 경우 사전에 KISTI의 허락을
받아야 합니다.
③ NDSL에서 제공하는 콘텐츠를 보도, 비평, 교육, 연구 등을 위하여 정당한 범위 안에서 공정한 관행에
합치되게 인용할 수 있습니다.
④ NDSL에서 제공하는 콘텐츠를 무단 복제, 전송, 배포 기타 저작권법에 위반되는 방법으로 이용할 경우
저작권법 제136조에 따라 5년 이하의 징역 또는 5천만 원 이하의 벌금에 처해질 수 있습니다.
제 14 조 (유료서비스)
① 당 사이트 및 협력기관이 정한 유료서비스(원문복사 등)는 별도로 정해진 바에 따르며, 변경사항은 시행 전에
당 사이트 홈페이지를 통하여 회원에게 공지합니다.
② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)
회원이 이용계약을 해지하고자 하는 때에는 [가입해지] 메뉴를 이용해 직접 해지해야 합니다.
제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
