자세 조절 운동이 심장이식환자의 심박수 상승지연에 미치는 영향 -단일사례연구- (Effects of Postural Control Exercise on the Delayed Heart Rate Increase in Heart Transplant Patients -A Case Study-)

  • Purpose: This study aimed to investigate the effects of postural control exercise on the delayed heart rate increase in heart transplant patients. Methods: The subject was a female heart transplant recipient who had a delayed increase in heart rate during exercise. The intensity of exercise was performed at MBorg level 4. The A-B-A' and A-B-A'-B' designs were used to identify the changes in heart rate during active-assisted exercise, lower limb postural control exercise, and upper limb postural control exercise. Experiments were performed for four weeks. The heart rates at pre- and post-exercise were compared, and the time to reach MBorg 4 was measured. Results: In the active-assisted exercise, the average heart rates at pre- and post-exercise and after 10 min of exercise were 88, 89, and 87.7 bpm, respectively. In the repetitive comparison of pre- and post-exercise in the lower limb postural control exercise, the difference in the mean heart rate was 3.5 and 3 bpm in stable support and 14 and 14.5 bpm in unstable support, respectively. In the repetitive comparison of pre- and post-exercise in the upper limb postural control exercise, the difference in the mean heart rate was 6 and 4 bpm in stable support and 4 and 4.5 bpm in unstable support. The time required to reach MBorg 4 was short when both the upper and lower postural control exercises were performed in an unstable state. Conclusion: We suggest that combining proper postural control exercise with strength exercise and aerobic exercise, among others, may be effective in rehabilitating patients in the recovery stage after a heart transplant.

Cystatin C for managing diuretic-induced kidney dysfunction in MMVD dogs

  • Cystatin C, a low-molecular-weight protein synthesized by cells, is being explored as a valuable biomarker for assessing renal function in veterinary medicine. Although the relationship between cystatin C and heart disease remains unclear, some studies suggest a possible association. This retrospective case-control study aimed to investigate the role of cystatin C as a biomarker for heart disease and its correlation with diuretic use in veterinary clinical practice. A total of 39 dogs were included in this study, comprising 9 control dogs without a predisposition to heart disease and 30 dogs in the study group diagnosed with heart disease. Among the 30 dogs with heart disease, 18 exhibited symptoms indicative of heart failure. Results showed significantly higher cystatin C levels in the heart disease group compared to the control group (P<0.05). However, no significant differences were observed among different stages of heart disease severity in the control group. Furthermore, cystatin-C showed statistically positive correlations with BUN (r=0.478, P<0.01), creatinine (r=0.506, P<0.01), and furosemide (r=0.338, P<0.05). Diuretics are essential for managing congestive heart failure, and the observed associations between cystatin C and furosemide suggest potential impacts of diuretic use on renal function in dogs with heart failure. Monitoring renal function markers, such as cystatin C, can aid in predicting and managing potential renal complications, ultimately improving the overall health and quality of life of dogs with heart disease.

완전이식 인공심장을 위한 제어시스템의 개발에 관한 연구 (Development of a microcontroller-based control system for a total artificial heart)

  • For use in patients with severe forms of heart disease for which no surgical repair is possible, development of artificial hearts has many importance in point of economics, medical and industrial applications. To provide a sufficient cardiac output to the physiological demands of circulatory systems is the objective of control systems for an electromechanical artificial heart, which is based on the stable controller design for the motor in the artificial heart. In this paper, an implantable microcontroller-based brushless DC motor control system with the implantability, reliability, and stability is introduced. The developed control system for the artificial heart has the following advantages: (1) It is possible to be implanted in a body by realizing the fundamental functions such as a motor speed detection, proportional-intergral control, timer, and PWM generation through a software programming. (2) Thus, the power consumed in the controller is reduced. (3) The reliability and stability are improved through the reduction of electronic parts and line connetions at the controller. The performance of the artificial hearts and control system developed was evaluated through a series of mock circulatory experiments and a reliability test for one and half years. A sheep with the artificial heart and control system was survived for three days.

회복 시 심박수의 역할: 심장질환과 운동프로그램 적용 가능성 (The Role of Heart Rate Recovery: Possibility of Heart Disease and Exercise Program Application)

  • Heart rate is a relatively simple and non-invasive method that is used as an important physiological indicator in many studies and has a close relationship with heart structure and function, cardiovascular disease and sudden cardiac death. In general, appropriately low heart rate during resting means effective heart function and cardiovascular fitness; heart rate at recovery is an important indicator of health and disease condition. We found a beagle dog (Dog_1) with a high heart rate in the previous preliminary experiment. Therefore, purpose of this study was to compare the heart rate response of the Dog_1 with the control group during 12 weeks of interval exercise, to evaluate the structural and functional abnormalities of the heart and to verify the applicability of exercise program. Heart rate was checked during 12 weeks of interval exercise, and after the exercise was over, imaging examination and hematological and serum biochemistry were performed. As a result, Dog_1 (165.6 ± 1.5) showed significantly higher heart rate in low intensity session of interval exercise than control group (133.3 ± 0.5, p < 0.01). In addition, Dog_1 (181.2 ± 1.4) showed significantly higher heart rate than control group (155.1 ± 0.9) in high intensity session (p < 0.01). The heart rate (30 sec, 60 sec) during recovery state was higher in Dog_1 (30 sec: 156.8 ± 4.0, 60 sec: 166.8 ± 5.8) than in the control group (30 sec: 111.2 ± 2.5, 60 sec: 104.0 ± 5.1, p < 0.01). The results of the imaging examination of Dog_1 with high heart rate confirmed that the heart had no functional and structural abnormalities. All beagles with the interval exercise program did not show maladjustment, and in the hematological and serum biochemistry results, all the parameter were within the reference range. If the interval exercise program of this study is used in the future, it is expected to be used as an important basic data to achieve the purpose of health, welfare, and physical fitness improvement of dogs.

압반사 제어모델을 이용한 심혈관시스템 모델링 및 시뮬레이션 (Modeling and Simulation of the Cardiovascular System Using Baroreflex Control Model)

  • In this paper, we consider the aortic sinus baroreceptor, which is the most representative baroreceptor sensing the variance of pressure in the cardiovascular system, and propose heart activity control model to observe the effect of delay time in heart period and stroke volume under the regulation of baroreflex in the aortic sinus. The proposed heart activity baroreflex regulation model contains electric circuit sub-model. We constituted the time delay sub-model to observe sensitivity of heart activity baroreflex regulation model by using the variable value to represent the control signal transmission time from the output of baroreflex regulation model to efferent nerve through central nervous system. The simulation object of this model is to observe variability of the cardiovascular system by variable value in time delay sub-model. As simulation results, we observe three patterns of the cardiovascular system variability by the time delay, First, if the time delay over 2.5 second, aortic pressure and stroke volume and heart rate is observed nonperiodically and observed. Finally, if time delay under 0.1 second, then heart rate and aortic pressure-heart rate trajectory is maintained in stable state.

Decrease in $Ca^{2+}$ Storage in the Cardiac Sarcoplasmic Reticulum of Diabetic Rat

  • In order to elucidate the molecular mechanism of the intracellular $Ca^{2+}$ overload frequently reported from diabetic heart, diabetic rats were induced by the administration of streptozotocin, the membrane vesicles of junctional SR (heavy SR, HSR) were isolated from the ventricular myocytes, and SR $Ca^{2+}$ uptake and SR $Ca^{2+}$ release were measured. The activity of SR $Ca^{2+}-ATPase$ was $562{\pm}14$ nmol/min/mg protein in control heart. The activity was decreased to $413{\pm}30$ nmol/min/mg protein in diabetic heart and it was partially recovered to $485{\pm}18$ nmol/min/mg protein in insulin-treated diabetic heart. A similar pattern was observed in SR $^{45}Ca^{2+}$ uptakes; the specific uptake was the highest in control heart and it was the lowest in diabetic heart. In SR $^{45}Ca^{2+}$ release experiment, the highest release, 45% of SR $^{45}Ca^{2+}$, was observed in control heart. The release of diabetic heart was 20% and it was 30% in insulin-treated diabetic heart. Our results showed that the activities of both SR $Ca^{2+}-ATPase$ and SR $Ca^{2+}$ release channel were decreased in diabetic heart. In order to evaluate how these two factors contribute to SR $Ca^{2+}$ storage, the activity of SR $Ca^{2+}-ATPase$ was measured in the uncoupled leaky vesicles. The uncoupling effect which is able to increase the activity of SR $Ca^{2+}-ATPase$ was observed in control heart; however, no significant increments of SR $Ca^{2+}-ATPase$ activities were measured in both diabetic and insulin-treated diabetic rats. These results represent that the $Ca^{2+}$ storage in SR is significantly depressed and, therefore, $Ca^{2+}-sequestering$ activity of SR may be also depressed in diabetic heart.

  • Heart diseases are critical and should be detected as soon as possible. A stethoscope is a simple device to find cardiac disorder but requires keen experiences in heart sounds. We evaluate a cardiac disorder classifier by using heart sounds recorded by a digital wireless stethoscope developed in this work. The classifier uses hidden Markov models with circular state transition to model the heart sounds. We train the classifier using two kinds of data: One recorded by using our stethoscope and the other sampled from a clean heart sound database. In classification experiments using 165 sound clips, the classifier shows the classification accuracy of 82% in classifying 6 cardiac disorder categories.

  • A model of the cardiovascular system coupling cell, hemodynamics and autonomic nervecontrol function is proposed for analyzing heart mechanics. We developed a comprehensive cardiovascular model with multi-physics and multi-scale characteristics that simulates the physiological events from membrane excitation of a cardiac cell to contraction of the human heart and systemic blood circulation and ultimately to autonomic nerve control. Using this model, we delineatedthe cellular mechanism of heart contractility mediated by nerve control function. To verify the integrated method, we simulated a 10% hemorrhage, which involves cardiac cell mechanics, circulatory hemodynamics, and nerve control function. The computed and experimental results were compared. Using this methodology, the state of cardiac contractility, influenced by diverse properties such as the afterload and nerve control systems, is easily assessed in an integrated manner.

일본국립순환기병센터형 완전 인공심장을 이용한 동물실험 (The Animal Experiments of Total Artificial Heart at the National Cardiovascular Center in Japan)

  • Recently we developed the concept of totally implantable electrohydraulic artificial heart. We tested the artificial heart which was drived by external compressive air in the calves. All three calves had pneumonia before surgery, so postoperative course was not only bad but also the results was not good. The first calf died severe pneumonia on 76th day, the second calf died from troublesome bleeding and uncertain allergic like reaction, and the third died because of bleeding. However, the performance of the artificial heart was good, and especially the blood contacting surface showed excellent hemocompatibility. The anatomic fitting was also very good even in the 35 Kg small newborn calf. During treadmill test, the first calf did not well tolerate for 1 minute but by the Full Fill Full Empty control method the artificial heart responsed well to the physiologic needs. In conclusion, the artificial heart had the very good hemocompatible surface, however, the volume of the artificial heart was a little deficient for the calf and the control algorithm needed further development.

A Novel Method to Estimate Heart Rate from ECG

  • Heart rate variability (HRV) in electrocardiogram (ECG) is an important index for understanding the health status of heart and the autonomic nervous system. Most HRV analysis approaches are based on the proper heart rate (HR) data. Estimation of heart rate is thus a key process in the HRV study. In this paper, we report an innovative method to estimate the heart rate. This method is mainly based on the concept of periodicity transform (PT) and instantaneous period (IP) estimate. The method presented is accordingly called the "PT-IP method." It does not require ECG R-wave detection and thus possesses robust noise-immune capability. While the noise contamination, ECG time-varying morphology, and subjects' physiological variations make the R-wave detection a difficult task, this method can help us effectively estimate HR for medical research and clinical diagnosis. The results of estimating HR from empirical ECG data verify the efficacy and reliability of the proposed method.
