Treatment of hearing loss due to temporomandibular joint disorders: Case Report (턱관절 장애로 인한 청각장애의 치료: 증례보고)

  • Introduction : Patients with temporomandibular disorder may have various ear-related symptoms. If an excessive load is applied to the ear area due to the skeletal shape of the mandibular condyle or malposition of the disc, an auditory problems may occur. Case report : The patient was referred to our clinic due to the suspicion of temporomandibular disorder from the local otorhinolaryngology clinic. A few days ago, his right ear could not be heard. MRI showed that the left TMJ disc was anterior displacement with reduction, the right TMJ disc was anteromedial displacement without reduction. Also Right mandibular condyle showed sclerotic bone change, subchondral cyst and was compressing the frontal wall of the ear on MRI view. Right TMJ arthroplasty was done under the diagnosis of right TMJ osteoarthritis and osteochondroma. Postoperative intermaxillary fixation was done with SAS screw and elastics for 2 weeks. One month after the operation, hearing and TMJ discomfort were recovered without any complications. Conclusions As seen in this case, hearing loss due to benign tumor-like lesions of the temporomandibular joint should be treated surgically to restore the TMJ function and hearing.

A Case Report of Yeoldahanso-tang on Sudden Hearing Loss and Tinnitus after Trigeminal Schwannoma Surgery (열다한소탕의 삼차신경초종 수술후 돌발성 난청 및 이명 치험 1례)

  • Objective The case report showed that sudden hearing loss and tinnitus after trigeminal schwannoma surgery were improved with Yeoldahansotang-gamibang. Method The patient was diagnosed with Taeeumin interior heat (Ganyeol) disease based on the original symptoms of Taeeumin. He was treated with herbal medicine and acupuncture treatment. Puretone audiometry (PTA), speech audiometry, tinnitus handicap inventory (THI) and the original symptoms were investigated before and after the treatment. Results Right side PTA score was significantly reduced, speech audiometry was improved in speech reception threshold (SRT), word recognition score (WRS) and most comfortable level (MCL) and THI score decreased from 40 to 0, which showed normalizing hearing function. And the patient revealed improvements in sleeping, digestion, stooling, perspiration and facial sensation after treatment. Conclusion This study suggests that Yeoldahansotang-gamibang is effective on sudden hearing loss and tinnitus after surgery by correcting the imbalanced energy of Taeeumin.

Effects of Cognitive Impairment on Self-reported Hearing Handicap in Older Adults with Early-stage Presbycusis (초기 노인성 난청자에서 인지장애가 일상생활 듣기 어려움에 미치는 영향)

  • Everyday hearing handicap caused by presbycusis ultimately reduces quality of life in older adults. The aim of this study was to explore effects of cognitive impairment on self-reported hearing handicap in older adults with early-stage presbycusis. We compared K-HHIE scores between 40 elderly subjects with mild cognitive impairment (MCI) and age- and hearing-threshold matched 40 cognitively normal elderly (CNE) subjects. The results are as follows: 1) The MCI group scored significantly higher than the CNE group on the social/situational and emotional sections, and in total. 2) The MCI group scored significantly higher than the CNE group on all four subscales, and the most significant group difference was on the first subscale relating to interpersonal relationships and social handicaps. 3) Both groups scored highest on the item 8 (problems hearing whispering sounds) and item 15 (problems hearing TV or radio sounds). Besides those two items, the MCI group also scored high on the item 21 (problems hearing in a restaurant), item 6 (problems hearing when attending a party), item 3 (avoiding groups of people), and item 20 (personal or social restrictions). Our findings suggest that, among older adults with early-stage presbycusis, older adults with cognitive impairment tend to report greater everyday hearing handicap than their peers with normal cognitive function. Especially, they show significant problems hearing in background noise or multi-talker situations, which cause social restrictions and social/emotional loneliness.

A Clinical Study of Hearing Disturbance and Middle Ear Pathology in Chronic Otitis Media (만성중이염의 교실내 병변과 청력에 관한 임상적 연구)

  • The degree of hearing impairment of chronic otitis media will indicate the nature and severity of middle ear pathology especially condition of ossicular chain, size of ear drum perforation and location of granulation tissue in the middle ear cavity. The subjects were 189 ears of tympanoplasty for chronic otitis media and divided into four groups as follows: Normal ossicular chain with only ear drum perforation (group I), normal ossicular chain with granulation tissue only around the ossicles regardless of any other region (group II), ossicular ankylosis or fixation of handle of malleus to promontory with or without granulation tissue around the ossicle (group III) and ossicular interruption by partial or complete destruction(groupf IV). The results were concluded as follows: 1) The average hearing threshold of chronic otitis media was 44.6 dB and hearing threshold was closely related to the condition of ossicular chain. Hearing threshold became greater in order of normal ossicular chain, ankylosis and interruption. 2) The average hearing threshold of ossicular interrupted group was 49.1 dB and it was greater in the cases of total destruction than that of partial destruction. 3) The hearing loss in the cases of normal ossicular chain with only tympanic membrane perforation was within 45 dB and this level was presumed to represent normal ossicular function. The degree of hearing loss was in proportion to the size of ear drum perforation and when over 45 dB, other middle ear pathology was suggested. 4) In the cases of small ear drum perforation with normal ossicular function, the hearing threshold was within 30 dB. 5) In the type of audiogram, flat type was 30.2% and ascending type 35.4%. Descending type was more frequent in the cases of normal ossicular mobility with granulation tissue around the ossicle and flat type was observed frequently in the cases of ossicular ankylosis. 6) Carhart's notch was seen in 14 cases (7.4%) and observed mainly in ossicular ankylosis. 7) There was no relation between hearing threshold and histopathological type of granulation tissue in chronic otitis media. However the degree of hearing impairment was related to the location of granulation tissue in the middle ear cavity. 8) Authors recognized the granulation tissue compensated the function of interrupted ossicular chain.

Fixed-point Optimization of a Multi-channel Digital Hearing Aid Algorithm (다중 채널 디지털 보청기 알고리즘의 고정 소수점 연산 최적화)

  • In this study, multi-channel digital hearing aid algorithm for low power system is proposed. First, MDCT(Modified Discrete Cosine Transform) method converts time domain of input speech signal into frequency domain of it. Output signal from MDCT makes a group about each channel, and then each channel signal adjusts a gain using LCF(Loudness Compensation Function) table depending on hearing loss of an auditory person. Finally, compensation signal is composed by TDAC and IMDCT. Its all of process make progress 16-bit fixed-point operation. We use fast-MDCT instead of MDCT for reducing system complexity and previously computed tables instead of log computation for estimating a gain. This algorithm evaluate through computer simulation.

A Comparative Study on Brainstem Auditory-Evoked Response between Dogs and Cats

  • Hearing assessment is critical in dogs and cats. Hearing loss in dogs and cats may be congenital or secondary to a central nervous system disorder or ear disease. The brainstem auditory-evoked response (BAER) test has been developed as an electrophysiological test for auditory function assessment. Modern BAER equipment is based on a computerized system. Thus, auditory function assessment can be performed using this objective, safe, and noninvasive method. No study has yet investigated the interspecies differences between BAER test results of dogs and cats. Therefore, the present study aimed to analyze the differences in BAER test results between dogs and cats. The test was conducted on four healthy adult dogs and four healthy adult cats. Regarding latency, lower values were obtained for all waveforms above 50 dB in cats compared to dogs. Regarding amplitude, cats showed higher values than dogs at intensities above 50 dB. Through a comparative analysis in this study, it was concluded that the two species had statistically significant differences. The BAER data of dogs cannot be applied to cats, and vice versa.

Remote Articulation Training System for the Deafs (청각장애자를 위한 원격조음훈련시스템의 개발)

  • In this study, remote articulation training system which connects the hearing disabled trainee and the speech therapist via B-ISDN is introduced. The hearing disabled does not have the hearing feedback of his own pronuciation, and the chance of watching his speech organs movement trajectory will offer him the self-training of articulation. So the system has two purposes of self articulation training and trainer's on-line checking in remote place. We estimate the vocal tract articultory movements from the speech signal using inverse modelling and display the movement trajectoy on the sideview of human face graphically. The trajectories of trainees articulation is displayed along with the reference trajectories, so the trainee can control his articulating to make the two trajectories overlapped. For on-line communication and ckecking training record the system has the function of video conferencing and tranferring articulatory data.

A Study on Rehabilitation in Hearing Impaired Children (청력손실아동의 재활에 관한 고찰)

  • The human baby appears to be born with preexistent knowledge of language. This specialized neural structure in the brain awaits auditory experience with language to trigger it into functioning. The auditory-linked acquisition of language is a time-locked function related to early maturational periods in the infant's life. The longer auditory language stimulation is delayed, the less efficient the language facility will be. It is for these reasons that it is urgent to fight the hearing problems of children with all the skill, knowledge and insights of which we are capable, the so called 'rehabilitative process'. An understanding of the timetable and the origin of prenatal to early life development of auditory mechanism will help in planning the aural rehabilitation. Further interests and studies are needed to establish the systematic structure of rehabilitative audiology.

The Protective Effect of Orally Ingested Korean Red Ginseng on the Noise Induced Hearing Loss in Mice (마우스에서 고려 홍삼의 구강내 섭취를 통한 소음성 난청의 예방효과)

  • It is well known that the saponin of Korean red ginseng (KRG) has an anti-oxidant effect and could suppress the accumulation of lipid peroxidation. The aim of the present study was to observe the inhibitory effect of KRG on mice with noise-induced hearing loss, and to determine its optimal dose. BALB/c mice with a normal hearing level and normal Preyer's reflexes were used in the study. The mice in the permanent-threshold-shift (PTS) group were exposed to noise (120-dB SPL, white noise band) in a noise booth for 3 h a day, for three consecutive days. The mice in the experimental group were given heat-processed red-ginseng extract (50 mg/kg, 100 mg/kg, and 200 mg/kg), and those in the control group were given normal saline alone during their noise exposure. The mice in the temporary-threshold-shift (TTS) group were exposed to noise (120 dBSPL, white noise band) in a noise booth for 3 h. The mice in the experimental group were given heat-processed red-ginseng extract (50 mg/kg, 100 mg/kg, and 200 mg/kg), and those in the control group were given normal saline alone before their noise exposure. The hearing levels of the mice were measured through auditory brainstem response (ABR) immediately and I, 3, 5, 7, and 14 days after their noise exposure. Cochleae were removed from the mice 14 days after their noise exposure. lmmunochemical and immunofluorescent staining were performed to observe the expression of 8-oxoG in cochlea. In the PTS group, the hearing function of the mice in all the groups was not recovered after their noise exposure. In the TTS group, however, the hearing function of the mice in all the groups was recovered within 14 days. Reduced hearing impairment and early recovery were observed in the mice that were given 200 mg/kg KRG, and early recovery was observed in the mice that were given 100 mg/kg KRG The immunopositive staining of 8-oxoG was detected in the stria vascularis in the control group but was diminished in the mice that were given 200 mg/kg KRG The ingestion of more than 100 mg/kg KRG demonstrated a protection and recovery effect on the noiseinduced-TTS group. Since KRG has been reported to be a safe compound even up to hundreds of mg/kg, a higher concentration of it may effectively protect and recover TTS.

Modeling of Sensorineural Hearing Loss for the Evaluation of Digital Hearing Aid Algorithms (디지털 보청기 알고리즘 평가를 위한 감음신경성 난청의 모델링)

  • Digital hearing aids offer many advantages over conventional analog hearing aids. With the advent of high speed digital signal processing chips, new digital techniques have been introduced to digital hearing aids. In addition, the evaluation of new ideas in hearing aids is necessarily accompanied by intensive subject-based clinical tests which requires much time and cost. In this paper, we present an objective method to evaluate and predict the performance of hearing aid systems without the help of such subject-based tests. In the hearing impairment simulation(HIS) algorithm, a sensorineural hearing impairment medel is established from auditory test data of the impaired subject being simulated. Also, the nonlinear behavior of the loudness recruitment is defined using hearing loss functions generated from the measurements. To transform the natural input sound into the impaired one, a frequency sampling filter is designed. The filter is continuously refreshed with the level-dependent frequency response function provided by the impairment model. To assess the performance, the HIS algorithm was implemented in real-time using a floating-point DSP. Signals processed with the real-time system were presented to normal subjects and their auditory data modified by the system was measured. The sensorineural hearing impairment was simulated and tested. The threshold of hearing and the speech discrimination tests exhibited the efficiency of the system in its use for the hearing impairment simulation. Using the HIS system we evaluated three typical hearing aid algorithms.
