Characteristics of Vibration Response Imaging in Healthy Koreans

  • Background: Vibration response imaging (VRI) is a new technology that records energy generated by airflow during the respiration cycle. Analysis of lung sound using VRI may overcome the limitations of auscultation. Objectives: To set a VRI standard for healthy Koreans, we conducted a clinical assessment to evaluate breath sound images and quantification in healthy subjects and compared the findings with reported breath sound characteristics. Methods: Recordings were performed using the VRIxp. Eighty subjects took a deep breath four times during a 12-second interval while sitting upright. The quantitative aspect was analyzed using the VRI quantitative lung data (QLD) for total left lung, total right lung and for six lung regions: left upper lung (LUL), left middle lung (LML), left lower lung (LLL), right upper lung (RUL), right middle lung (RML), right lower lung (RLL). The qualitative aspect was provided through image assessments by three reviewers. Results: In all regions the left lung had significantly higher QLD than the right lung (P<0.005, paired t-test). The inter-rater agreement was 0.78. 84% of the images were found normal by the final assessment. Among the 16% (n=13) of images with abnormal final assessment, the most common flawed features were dynamic image (77%, n=10) and maximum energy frame (MEF) shape (77%, n=10). No significant differences were found between males and females for QLD but there were significant differences in qualitative aspects including dynamic images, MEF shape, and missing LLL. Conclusion: The characteristics of healthy Koreans are similar to those of Western subjects reported previously. VRI is easy to use and objective, and so is helpful to diagnose patients with respiratory diseases and to monitor the progress of diseases after medical treatments.

The acute effect of maximal exercise on plasma beta-endorphin levels in fibromyalgia patients

  • Background: This study aimed to investigate the effect of strenuous exercise on ${\beta}$-endorphine (${\beta}$-END) level in fibromyalgia (FM) patients compared to healthy subjects. Methods: We enrolled 30 FM patients and 15 healthy individuals. All study participants underwent a treadmill exercise test using modified Bruce protocol (M.Bruce). The goal of the test was achieving at least 70% of the predicted maximal heart rate (HRMax). The serum levels of ${\beta}$-END were measured before and after the exercise program. Measurements were done while heart rate was at least 70% of its predicted maximum. Results: The mean ${\pm}$ the standard deviation (SD) of exercise duration in the FM and control groups were $24.26{\pm}5.29$ and $29.06{\pm}3.26$ minutes, respectively, indicating a shorter time to achieve the goal heart rate in FM patients (P < 0.003). Most FM patients attained 70% HRMax at lower stages (stage 2 and 3) of M.Bruce compared to the control group (70% versus 6.6%, respectively; P < 0.0001). Compared to healthy subjects, FM patients had lower serum ${\beta}$-END levels both in baseline and post-exercise status ($Mean{\pm}SD$: $122.07{\pm}28.56{\mu}g/ml$ and $246.55{\pm}29.57{\mu}g/ml$ in the control group versus $90.12{\pm}20.91{\mu}g/ml$ and $179.80{\pm}28.57{\mu}g/ml$ in FM patients, respectively; P < 0.001). Conclusions: We found that FM patients had lower levels of ${\beta}$-END in both basal and post-exercise status. Exercise increased serum the ${\beta}$-END level in both groups but the average increase in ${\beta}$-END in FM patients was significantly lower than in the control group.

정상인에서 측정한 바이오피드백의 정신생리학적 특징 (Psychophysiological Response Patterns Measured by a Biofeedback System in Healthy People)

  • 본 연구는 정상인을 대상으로 바이오피드백 시스템으로 측정된 정신생리적 반응의 특징을 파악하는 것을 목적으로 시행되었다. 연구 결과 20대에서 피부전도반응이 다른 연령대에 비해 유의하게 높았으며, 여성에서 전두부 근전도가 남성에 비해 유의하게 높았다. 한편 정상인에서 바이오피드백 측정치와 자가 평가한 기분 변수와는 관련이 없는 것으로 나타났다. 본 연구의 결과는 향후 바이오피드백 시스템 기기를 이용하여 평가를 할 때 성별, 연령별 특징을 고려해야 함을 시사한다. 본 연구에서 제시된 정신생리변수에 관한 중요한 특징들은 향후 동종의 바이오피드백 시스템을 이용한 연구결과를 비교하는 데 도움이 될 것으로 기대된다.

담정격 및 담승격 자침이 정상 성인의 맥파변화에 미치는 영향 (The Effects of Gall Bladder Jeonggyeok and Seunggyeok on Radial Pulse Variation in Healthy Human Subjects)

  • Objectives : The purpose of this study is to find parameters to compare the effects of Sa-Am acupuncture with gall bladder jeonggyeok and seunggyeok on radial pulse in healthy subjects. Methods : 60 healthy subjects participated in this study and divided into the gall bladder jeonggyeok group (GB+ group), the gall bladder seunggyeok group (GB- group) and the control group. Radial pulse was measured by a 3 dimensional pulse imaging system (DMP-3000) before, immediately after, 30 minutes after and 60 minutes after acupuncture on the Cun, Guan and Chi in the GB+, GB- group and at the same time for the control group. Results : 1. The GB+ and GB- group exhibited significantly increased T, M4/M1 and decreased T1/T, T2/T, T5/T, M1, M2 compared to the control group. 2. The GB+ and GB- group exhibited significantly increased H4, Ad/Ap, RAI/HR and decreased As/Ap, Aw/Ap in the right Guan and decreased pulse power volume/min in the left Guan compared to the control group. 3. The GB- group exhibited significantly decreased M3, M3/M1 and increased M4/M1, M5, M5/M1 compared to the GB+ group. The GB+ group exhibited significantly decreased T4/(T-T4) compared to the GB- group. 4. The GB- group exhibited significantly decreased H1 in the right Guan, Chi, H4, pulse area in the right Chi, H5, pulse power volume/min in the left Chi, right Chi, and increased main peak angle in the right Guan, Chi compared to the GB+ group. Conclusion : The effect of gall bladder jeonggyeok and seunggyeok in healthy humans may be observed on various parameters. The parameters analyzed in this study may be used to differentiate effects between gall bladder jeonggyeok and seunggyeok on radial pulse. Further studies on the effects of Sa-Am acupuncture using radial pulse are needed.

고혈압 환자와 건강인의 맥상과 사상체질에 관한 연구 (A Comparative Study on the Pulse Wave Variables and Sasang Constitution in Hypertension Patients and Healthy Subjects)

  • 1. Objectives We studied the characteristics of pulse wave variables in hypertensive patients and healthy subjects and would develope the program diagnosing Sasang Constitution. 2. Methods Healthy subjects and hypertensive patients were measured with pulse analyzer. In each of healthy and hypertensive group, means of pulse analyzer variables were compared among Constitution. On these bases of these results, decision tree composed of several rules were developed. 3. Results and Conclusions Decision tree with 72.13% accuracy were developed. 한국인 남녀에서 이상지질혈증 관련 건강생활 실천 규명 -국민건강영양조사 2012년 자료 이용- (Investigation of Healthy Life Practices among Korean Males and Females in relation to Dyslipidemia Using data from the 2012 Korea National Health and Nutrition Examination Survey)

  • 본 연구의 목적은 이상지질혈증군과 정상군을 구분하여 한국인 남녀에서 이상지질혈증 발생과 건강생활 실천의 영향을 파악하기 위함이다. 연구대상은 제5기(2010년~2012년) 국민건강영양조사에서 공개된 제3차년도(2012년) 자료 중에서 건강 설문 자료를 분석대상으로 하였으며, 대상자는 우리나라의 질병 발생 양상 및 의료 환경 등을 고려해 만든 한국지질 동맥경화학회(KSLA: Korean Society of Lipidology and Atherosclerosis, 2009)의 이상지질혈증 치료지침을 적용하여 만 19세 이상 이상지질혈증군 2,092명(남자 1,042명, 여자 1,050명)과 정상군 1,944명(남자 740명, 여자 1,204명)인 4,036명이었다. 본 연구는 SPSS 18.0 프로그램을 이용하여 서술적 통계의 t-test와 ${\chi}^2-test$, 로지스틱 회귀분석을 사용하였다. 본 연구의 결과를 토대로 이상지질혈증 관련 건강생활 실천 프로그램은 혈청 지질 농도에 차이를 보이고 있는 한국인 남녀의 특성을 고려하여 적용하여야 하며, 이를 위해 지속적인 연구 및 건강생활 실천의 효과를 적용한 체계적인 국가 차원의 맞춤교육이 필요하다고 사료된다.
