A Case of Infantile Fungal Urinary Tract Infection

  • Urinary tract infection is common in the pediatric population. The most common causative agents are bacteria, among which Escherichia coli is the most frequent uropathogen. Although fungal urinary tract infection is rare in the healthy pediatric population, it is relatively common among hospitalized patients. Fungus may be isolated from the urine of immunocompromised patients or that of patients with indwelling catheters. The most common cause of funguria is Candida albicans. Although more than 50% of Candida isolates belong to non-albicans Candida, the prevalence of non-albicans candiduria is increasing. Herein, we report a case of community-acquired candiduria in a 4-month-old immunocompetent male infant who had bilateral vesicoureteral reflux and was administered antibiotic prophylaxis. He was diagnosed with urinary tract infection caused by Candida lusitaniae and was managed with fluconazole.

A rare case report of neonatal calcinosis cutis induced by distant and delayed extravasation of intravenous calcium gluconate

  • A 3,480 g male neonate showed tachypnea symptom with a serum ionized calcium level of 0.66 mmol/L by routine clinical analysis. He was injected calcium gluconate intravenously through femoral vein catheter to treat the hypocalcemia. On second day after the injection, he started to show erythema in the flank area. The lesion became firm and changed into whitish crust consist of small crystals. Abdominal X-ray and ultrasonography showed the accumulation of calcium deposit in the subcutaneous layer of the lesion. Surgical debridement was performed to remove the crust with calcium deposit and acellular fish skin graft rich in omega-3 (Kerecis) was applied to the defect site for secondary intention of the defect wound. After 2 months, the skin and soft tissue defect were fully covered with healthy normal skin without depression or contracture. This report is a first case of iatrogenic calcinosis cutis without extravasation symptom.

  • This study was carried out to investigate the effects of wearing disposable diaper with the endothermic agent on thermoregulatory response of infant. Five healthy female infants aged about 5 months were taken as a subject of this experiment. Experimental diapers were six kinds of disposable diaper constructed of nonwoven tissue, fluff pulp, super absorbent polymer, back sheet film, leg elastic, and 0g urea(A) 1g urea(B) , 2g urea(C), 3g urea (D), 4g urea(E) 5g, urea(F) respectively. Urea(98% or over purity) was used as an endothermic agent. Experiment was proceeded while infants were sleeping at 27.5$\pm$0.5$^{\circ}C$, 50$\pm$5% R.H, 0.04m/sec. Each disposable diaper's properties was tested. Growth and Tolerance Assessment of a Lutein-fortified Infant Formula

  • Purpose: To evaluate safety, gastrointestinal tolerance, and growth of a new experimental starter formula (NESF) fortified with lutein, prebiotics, probiotics, nucleotides and beta-carotene, fed to infants within the first months of life. Methods: This was a non-randomized, open, uncontrolled study conducted from December 2010 to May 2011. Twenty-three healthy infants aged 10 days to 2 months old were enrolled. Outcomes included gastrointestinal tolerance, physical growth and safety. Prior to the initiation of the trial, the Scientific Research Institute of Nutrition of the Russian Academy of Medical Sciences confirmed that the NESF met all safety and nutritional parameters. Results: NESF was well tolerated. The majority of infants fed this formula passed semi-liquid, yellow or yellow-brown. The mean stool frequency/day was $2.5{\pm}0.4$ on study-day 14 and $1.8{\pm}0.5$ on study-day 28. The mean daily weight gain was $30.9{\pm}3.8$ grams and the mean length gain during the 28 days of follow up was $3.1{\pm}0.8cm$, corresponding to the average physical growth normally seen in the first months of life in Russian infants. Six children left the study: one refused to drink the formula, one left the study as parents changed residence; and one child's parents have recalled their informed consent due to adverse event unrelated to the product. Three infants presented adverse events possibly related to the product (rash; colic and abdominal pain; constipation). Seventeen infants completed the trial. Conclusion: This study demonstrated that lutein-fortified formula is safe, well-tolerated and supported physical growth of evaluated infants.

Effects of Supplementary Diet on Iron Status and Development in Infants (유아보충식 제공이 유아의 철분영양상태 및 발달에 미치는 효과)

  • This study was carried out to evaluate the effects of supplementary diet in infants. Influence of appropriate dietary habits on infants was also examined by being applicable to diets fortified chewiness as a means of intervention. The iron supplementary diet was supported to the healthy infants twice a day for three months. Measures of hemoglobin, hematocrit, RBC count, serum iron, TIBC, ferritin, development examination, and dietary intake patterns of experimental group (n = 25) and control group (n = 20) were performed before and after the intervention. The amount of iron intake from the supplementary diet in the experimental group was $1.77{\pm}0.80 {\cal}mg/day$. After the intervention period, the experimental group not only had increased intakes of grains also decreased intakes of milk. Outcomes observed in infants receiving iron intervention showed that the improved trend of excessive milk intakes and the possibility as a regular diet by serving the iron supplementary diet which can apply to main dish. All measures in blood did not provide significant differences except TIBC between the experimental and the control group before the intervention. But, after the intervention, the experimental group improved most levels of measures, especially significance in hemoglobin, but serum iron. Development of two groups did not differ significantly and both groups were in the range of normal infants' development. However, the levels of MDI and PDI evaluated by BSID-II in the experimental group were slightly higher than the control. Furthermore, the development of cognitive and languistic function was associated with infant growth in the experimental group. In conclusion, this research demonstrated that the iron supplementary diet could affect the iron status and the development of infants despite low-dose supplementation of iron.

A Study of the Change of Body Weight and BMI during Some Pregnant and Postpartum Periods Based on the Sasang Constitution (일부 산모의 사상체질에 따른 임신.출산전후의 체중 및 BMI 변화에 관한 고찰)

  • Purpose: As body weight management is significantly important for women and newborn infants during pregnant and postpartum periods, there have been studies about changes of body weight during those periods. However, there are not enough studies about it which is based on the Sasang Constitution. The following is a result of the study which analyzes the changes with Sasang Constitution. Methods: This study investigated those who understand and agree this study#s purposes out of 109 primiparae who did normal delivery, who gave birth to a healthy infant and who are breastfeeding and who were treated by S postpartum care center from March 2005 to November 2005. The result of survey was collected from the questionnaires that included postpartum pattern and QSCC II. Results: There are remarkable changes of body weight and BMI based on the Sasang Constitution in pregnant and postpartum periods and they appear to be in order of Taeumin, Soyangin, Soeumin. In terms of the amount of milk according to the Constitution, there are no key changes between Soeumin and Taeumin, but there is a significance between Soyangin and Soeumin and Soyangin and Taeumin. In terms of the average body weight of infants according to the Constitution, there are no essential changes between Soeumin and Soyangin, but there is a magnitude between Soeumin and Taeumin and Soyangin and Taeumin. Conclusion: The changes of the body weight and BMI based on the Sasang Constitution appear to be the smallest for Soeumin, and the largest for Taeumin. The amount of milk from mothers did not affect on the changes and the weight of newborn infant did affect on the changes.

Study of Growth and Development of Breast-fed Infants and Parenting Experience of their Mothers (모유수유아의 성장발달 상태와 어머니의 양육 경험)

  • Purpose: The purpose of the study was to describe growth and development of breast-fed infants and to depict experiences of breastfeeding mothers. Method: The data were collected from 145 infants and mothers participating in A Healthy Breastfeeding Infant Contest. The instrument used for this study was a self-report questionnaire completed by the participants and an evaluation record completed by the examiners. Results: The more specific results of the study are as follows: the weight of the infants was much higher than the Korean Standards. The height of infants was not significantly different from the Korean Standards except for 5 month old male infants. The results of Denver II showed all 145 infants were classified as normal. The breastfeeding experiences in mothers was as follows; 31.7% of the mothers breastfed for 5 months, 48.3% wanted to breastfed as long as the baby wanted. The mothers pointed out that the difficult problem in breastfeeding was insufficient breastfeeding room in public areas. What mothers want to know related to breastfeeding was, breastfeeding duration, time to begin solid food and recommended amounts, time to discontinue breastfeeding at sleep time and appropriate methods. Conclusion: Mothers' experience in breastfeeding was positive. We should provide mothers a comfortable place in public areas for breastfeeding. Based on the result of the study it's propose that health professionals must intervene in breast care during both antepartum and early postpartum periods.

Influence of parents' knowledge for oral hygiene on the oral health of their infants (부모의 구강건강지식이 유아의 구강건강에 미치는 영향)

  • Objectives : The purpose of this study is to research and to analyze the relationship between oral health state of infants and oral health awareness of their parents. Methods : The study has been conducted for 196 infants ranging from the age 18 to 60 month and their parents who have visited three dental clinics in Gyeongju city. The questionnaire has been made to identify the parents' oral health knowledge and behavior, while the dentist has examined the dental care, toothbrushing, and malocclusion of infants. Results : According to the results, it has been found out that there is correlation between subject's health status and oral health status where the higher point of subject's health status lead to the point of oral health status accordingly. Also it has been additionally recognized that there is the relationship between the awareness of parents and the oral health status of their infants in the same way. Conclusions : The parents who have low oral health awareness are required by oral health professionals to be provided with information on infant oral health, in order to promote and to maintain healthy oral status of their infants. And the specific training on method how to care oral health is needed. In addition, the government should take the detailed measures by extending infant's medical benefits to draw their attention to the issue.

Isolation of Lactic Acid Bacteria Showing Antioxidative and Probiotic Activities from Kimchi and Infant Feces

  • The purpose of this study was to investigate lactic acid bacteria with antioxidative and probiotic activities isolated from Korean healthy infant feces and kimchi. Isolates A1, A2, S1, S2, and S3 were assigned to Lactobacillus sp. and isolates A3, A4, E1, E2, E3, and E4 were assigned to Leuconostoc sp. on the basis of their physiological properties and 16S ribosomal DNA sequence analysis. Most strains were confirmed as safe bioresources through nonhemolytic activities and non-production of harmful enzymes such as β-glucosidase, β-glucuronidase and tryptophanase. The 11 isolates showed different resistance to acid and bile acids. In addition, they exhibited antibacterial activity against foodborne bacteria, especially Bacillus cereus, Listeria monocytogenes, and Escherichia coli. Furthermore, all strains showed significantly high levels of hydrophobicity. The antioxidant effects of culture filtrates of the 11 strains included 2,2-diphenyl-1-picryl-hydrazyl (DPPH) radical scavenging capacity, 2.2'-azino-bis (2-ethylbenzthiazoline-6-sulfonic acid) (ABTS) radical cation scavenging activity, and superoxide dismutase activity. The results revealed that most of the culture filtrates have effective scavenging activity for DPPH and ABTS radicals. All strains appeared to have effective superoxide dismutase activity. In conclusion, the isolated strains A1, A3, S1, and S3 have significant probiotic activities applicable to the development of functional foods and health-related products. These strains might also contribute to preventing and controlling several diseases associated with oxidative stress, when used as probiotics.

A Case of Haddad Syndrome (Haddad 증후군 1예)

  • The combination of Congenital central hypoventilation syndrome with Hirschsprung's disease, also known as Haddad syndrome, belongs to the family of diseases now designated as Neurocristopathies. We have experienced a case of Haddad syndrome in a male infant who presented with repetitive abdominal distension, bilious vomiting, and sleep apnea. Following colon study and rectal biopsy disclosed the absence of the ganglion cell. And the infant could not be weaned from mechanical ventilation since birth because of the absence of effective, spontaneous respiration during sleep. As he was diagnosed as Haddad syndrome, tracheostomy and ileostomy were performed consecutively. At the age of 4 months, he was relatively healthy but remained ventilator-dependent. We report the first Korean case of Haddad syndrome with a brief review of the related literature.
