국민건강증진을 위한 영양 부문의 2000년 잠정목표와 전략 (Provisional National Nutrition Targets and Strategies for Health Promotion of Koreans)

  • This study attempts to propose a priority of national nutrition targets and strategies for health promotion by the year 2000 in Korea, as a part of the task set for national health promotion objectives and strategies. Among all of the important health issues raised, ten were chosen, nutrition was one priority area. In the first part, the current status of the nutrition-related health problems and risk factors are reviewed, in conjunction with the newly arisen health phenomena, such as changes in prevalence of lifestyle disease and causes of death, changes of food consumption patterns in our country. In the second section this study suggests six feasible national nutrition targets, eight implementing strategies and current major tasks on the basis of the assessment of present status and in consideration of the other health promotion goals and strategies, with reference to that of other developed countries. The main targets and strategies are suggested as follows ; Firstly, the national nutrition monitoring and surveillance system should be established for identifying the nutritional problems for our people, and current National Nutrition Survey is a strong need for improvement to a more comprehensive and reliable one. Secondly, effective administrative mechanism should be operation at national level for the development of nutrition policy. Ministry of Health and Welfare (MOHW) as well as local health department must be remarkably renewed and strengthened the nutrition section. And it is recommended that MOHW organize and operate “The Council of Nutrition”, in which all government authorities related with foodstuffs and nutrition would incorporated. The Council of Nutrition would act as an adjustor as well as a coordinator in nutrition related policy-making. Thirdly, healthy eating pattern will be supported by activities of introducing a nutrition labeling for providing consumers with the necessary information and skills for food selection. Fourthly, nutrition education, and nutrition intervention programs will be carried out in various settings such as health centers, schools, and clinical fields and workplace. Fifthly, the current dietary guidelines shall be continuously improved in detail, and publicly circulated to particular levels of people by age group and by health condition. And finally, researches and epidemiological studies particularly in regard to diet for development of chronic diseases are needed for more investigation and up-to-date national health and nutrition data should be collected with the support and cooperation from the various medical professional teams . Cardiovascular Health Metrics and All-cause and Cardiovascular Disease Mortality Among Middle-aged Men in Korea: The Seoul Male Cohort Study

  • Objectives: This study estimated the association of cardiovascular health behaviors with the risk of all-cause and cardiovascular disease (CVD) mortality in middle-aged men in Korea. Methods: In total, 12 538 men aged 40 to 59 years were enrolled in 1993 and followed up through 2011. Cardiovascular health metrics defined the following lifestyle behaviors proposed by the American Heart Association: smoking, physical activity, body mass index, diet habit score, total cholesterol, blood pressure, and fasting blood glucose. The cardiovascular health metrics score was calculated as a single categorical variable, by assigning 1 point to each ideal healthy behavior. A Cox proportional hazards regression model was used to estimate the hazard ratio of cardiovascular health behavior. Population attributable risks (PARs) were calculated from the significant cardiovascular health metrics. Results: There were 1054 total and 171 CVD deaths over 230 690 person-years of follow-up. The prevalence of meeting all 7 cardiovascular health metrics was 0.67%. Current smoking, elevated blood pressure, and high fasting blood glucose were significantly associated with all-cause and CVD mortality. The adjusted PARs for the 3 significant metrics combined were 35.2% (95% confidence interval [CI], 21.7 to 47.4) and 52.8% (95% CI, 22.0 to 74.0) for all-cause and CVD mortality, respectively. The adjusted hazard ratios of the groups with a 6-7 vs. 0-2 cardiovascular health metrics score were 0.42 (95% CI, 0.31 to 0.59) for all-cause mortality and 0.10 (95% CI, 0.03 to 0.29) for CVD mortality. Conclusions: Among cardiovascular health behaviors, not smoking, normal blood pressure, and recommended fasting blood glucose levels were associated with reduced risks of all-cause and CVD mortality. Meeting a greater number of cardiovascular health metrics was associated with a lower risk of all-cause and CVD mortality.

고혈압 환자들의 건강형태 (Health Behaviors in Hypertensive Patients)

  • This study was conducted to examine health behaviors in hypertensive patients and the factors that would affect these healthy life-styles. and to provide basic data for effective and continuous hypertension control in private medical facilities. Among those patients diagnosed as hypertension who had visited the Department of Internal Medicine of Yeungnam University Hospital during the two month period from August 2 to September 30, 1999, the present study included 222 patients who were in more than one month after the initial diagnosis of hypertension and those who had hypertension for less than 10 years. Using a structured questionnaire, the present study was conducted by a self-administered survey method, and the obtained data were analyzed with t-test, $x^2$-test and one-way analysis of variance using the SPSS statistical program. The scores on knowledge related to hypertension were higher as the education level of a patient was higher(p〈0.05). As for occupation, those who performed office or management jobs showed higher scores than those who were into manufacturing jobs, and as for economic status, although those patients who were in middle class showed highest scores, however, no statistical significance was observed. The scores of knowledge on hypertension was higher when the family history of hypertension was present(p〈0.05). The scores of health-related behaviors were higher with higher education level and higher economic status, higher in those with family history of hypertension, and higher in those with office or management jobs than those who had manufacturing jobs(p〈0.05). Blood pressure measurement on regular basis was performed most frequently in those who were between 50-59 years old with 83.3% and was least frequent in those who were older than 70 years old with 50%(p〈0.05). The frequency of regular blood pressure measurement was higher with higher education level, higher economic status, higher in those with family history of hypertension, and the highest in those with management position with 93.5%, however, on statistical significance was observed. Changes in health-related behaviors after hypertension diagnosis were higher with higher education level, higher economic status, and in those patients performed office or management work. In particular, diet change was observed in female and higher economic status and smoking cessation was observed in 60-69 years old. Housewives and office workers or managers have taken exercise more regularly and those who had management jobs and had high scores on knowledge related to hypertension would participate health education program more actively. Thus, for improving health-related behaviors for continuous management of hypertension, changes in health-related behaviors can be followed through conduction health education to improve understanding of knowledge related to hypertension as the method of helping to improve changes in health life-styles in those with little education and those in low economic status.

유아의 우유군의 섭취가 식이의 다양성과 영양소 섭취의 적정도에 미치는 영향 (The Effects of Milk Group Intake to Dietary Diversity Score and Nutrient Adequacy Ratio among Toddler)

  • This research was conduced to 1) examine the influence of milk group intake on the dietary, considering dietary diversity and nutrient adequacy, 2) identify which foods truly contribute to the intake of specific nutrients 3) point out the dietary problems of this age group. The subjects were 664(male 357, female 307) healthy toddlers of the ages of 12-36 months, who had neither been taking any medicine-type supplements nor had any diseases that affect the dietary intakes, chosen among the outpatients of university and private hospitals in Seoul and Kyonggi province during Mach-August, 1999. The 24 hour recall method and CAN Pro(food analysis program) were used to analyze the amount of nutrient intake. The grouping of the subjects for the analysis by monthly age was based on the 98 Korean Child growth standard. The results that the milk group intake showed significant negative correlations with the grain group(r=-0.194, p<0.01), the vegetable group(r=-0.115, p<0.01) and the fruit group(r=-0.125, p<0.01). It also showed that the more the milk group intake, the lower the Dietary Diversity Score(r=-0.131, p<0.01), and the more the intake of raw milk, the more dificient the intake of iron(r=-0.211, p<0.01), vitamin A(r=-0.169,p<0.01), vitamin B$_1$(r=-0.078, p<0.05) and vitamin C(r=-0.187, p<0.01). Foods contributory to nutrient intake for of 12-14 months of were mostly infant formula, and the contribution ratio was high. In the age of 15-17 months, the subjects obtained most of the nutrients from raw milk., liquid-type yoghurt, soybean milk, cooked rice, etc. As for iron intake, the age of 21-23 month obtained the most from ionic drinks, and 24-29 months from raw milk, chocolate cakes, ionic drinks, etc. Though the NAR(Nutrient Adequacy Ratio) and MAR(Mean Adequacy Ratio) generally tended to increase accordingly with the DDs, as for nutrients Ca, vitamin A, vitamin B$_1$, and vitamin C the NAR marked the highest when the DDS marked the lowest(due to infant formula). And for iron, even when the DDS was 5 the NAR value was low. Im conclusion, the more the milk group intake, the lower was the dietary diversity. High dependency on infant formula and others from milk group over the age of 12 months proved to result in a decrease of nutritious solid food intake, iron deficiency and a low dietary diversity score, which can lead to an unbalanced diet. Therefore, nutrition education towards mothers/caretakers of toddler is of urgent need, in order to reduce the fluid milk group intake of low nutritional density and to provide various solid foods the children. 한국 여자의 연령별 골밀도에 영향을 미치는 영양요인 분석 (Nutritional Factors Related to Bone Mineral Density in the Different Age Groups of Korean Women)

  • Nutritional factors affecting bone mineral density (BMD) in the different age groups of Korean women were investigated to obtain baseline data for maintaining bone health. Information on diet and anthropometry were collected in 80 elementary school children (height 127.2 cm, weight 27.3 kg), 84 high school students (height 161.6 cm, weight 52.4 kg), 100 adults aged 25 to 35 years (height 159.4 cm, weight 52.7 kg) and 120 elderly people over 60 years of age (height 150.9 cm, weight 55.6 kg). Data for nutrient intake were obtained by 24-hour recall method. BMDs of lumbar spine (L$_2$-L$_4$) and femoral neck were measured by dual energy x-ray absorptiometry. The relationship between BMD of femoral neck and nutritional factors were analyzed. The average BMD of femoral neck for females was 0.61 g/$\textrm{cm}^2$ in children, 0.88 g/$\textrm{cm}^2$ in adolescents, 0.90 g/$\textrm{cm}^2$ in adults, 0.64 g/$\textrm{cm}^2$ in elderly people. Among the adult subjects, 11.0% was classified as osteopenia in the femoral neck. For the elderly, the prevalence of osteopenia and osteoporosis were 34.2% and 47.5% of the subjects. It was shown the intake of energy, protein, plant protein, animal protein, fat, carbohydrate, Ca, P, Fe, vitamin A, thiamin, riboflavin, niacin, vitamin C, carbohydrate energy percent and fat energy percent influenced bone health status in all age groups. In the MAR on bone health status, children, adult and elderly subjects were significantly different among groups classified by bone health status and the MAR of the groups with good in bone health was higher. The RDA percent of each nutrient was influence factor on BMD. Nutrient intake of energy, protein, P, Fe, thiamin, niacin were lower BMD on below 75% of Korean RDA. Stepwise multiple regression analysis revealed that several dietary factors were influence on BMD. MAR on femoral neck BMD of children and elderly subjects was the highest influence factor. Beyond this, the most influential dietary factors on BMD were the vitamin A, total Ca and vegetable Ca. The above results have confirmed that dietary factors influence BMD in various age groups. Energy, protein, Ca, P, Fe, thiamin, riboflavin, niacin, vitamin C as well as MAR were important dietary factors influencing BMD. The results of this study revealed that people who received sufficient nutrients intake showed healthy bone status. The MAR mainly influenced the bone health status.

Relationships between milk consumption and academic performance, learning motivation and strategy, and personality in Korean adolescents

  • BACKGROUND/OBJECTIVES: A healthy diet has been reported to be associated with physical development, cognition and academic performance, and personality during adolescence. This study was performed to investigate the relationships among milk consumption and academic performance, learning motivation and strategies, and personality among Korean adolescents. SUBJECTS/METHODS: The study was divided into two parts. The first part was a survey on the relationship between milk consumption and academic performance, in which intakes of milk and milk products and academic scores were examined in percentiles among 630 middle and high school students residing in small and medium-sized cities in 2009. The second part was a survey on the relationships between milk consumption and learning motivation and strategy as well as personality, in which milk consumption habits were collected and Learning Motivation and Strategy Test (L-MOST) for adolescents and Total Personality Inventory for Adolescents (TPI-A) were conducted in 262 high school students in 2011. RESULTS: In the 2009 survey, milk and milk product intakes of subjects were divided into a low intake group (LM: ${\leq}60.2g/day$), medium intake group (MM: 60.3-150.9 g/day), and high intake group (HM: ${\geq}151.0g/day$). Academic performance of each group was expressed as a percentile, and performance in Korean, social science, and mathematics was significantly higher in the HM group (P < 0.05). In the 2011 survey, the group with a higher frequency of everyday milk consumption showed significantly higher "learning strategy total," "testing technique," and "resources management technique" scores (P < 0.05) in all subjects. However, when subjects were divided by gender, milk intake frequency, learning strategy total, class participation technique, and testing technique showed significantly positive correlations (P < 0.05) in boys, whereas no correlation was observed in girls. Correlations between milk intake frequency and each item of the personality test were only detected in boys, and milk intake frequency showed positive correlations with "total agreeability", "organization", "responsibility", "conscientiousness", and "intellectual curiosity" (P < 0.05). CONCLUSION: Intakes of milk and milk products were correlated with academic performance (Korean, social science, and mathematics) in Korean adolescents. In male high school students, particularly, higher milk intake frequency was positively correlated with learning motivation and strategy as well as some items of the personality inventory.

계란의 콜레스테롤은 심장질환과 무관 : 총설 (Egg Cholesterol is not related with Heart Disease : A Review)

  • 본 총설은 논란이 되고 있는 계란 콜레스테롤이 심장질환과 관련성이 없다는 것을 증명하기 위하여 지난 50여 년간의 연구 결과를 조사하였다. 그 결과, 식이로 섭취된 콜레스테롤은 혈액 콜레스테롤 농도에 큰 영향을 미치지 않는 것이 밝혀졌다. 더욱이 고콜레스테롤혈증이 있는 중년 남녀에게 1일 2개의 계란을 더 섭취한 실험에서도 혈액 콜레스테롤이 증가되지 않는 것으로 나타났다. 식이 콜레스테롤 섭취가 증가하면 LDL-콜레스테롤 뿐 아니라, HDL-콜레스테롤도 같이 증가하여 심장질환 위험의 중요 지표인 LDL : HDL 비율은 거의 변화가 없었다. 특히 노인에게서 더 좋은 지표로 밝혀진 HDL-콜레스테롤이 증가하므로, 콜레스테롤 섭취는 심장질환 예방에 좋은 것으로 나타났다. 따라서 계란의 섭취는 건강에 나쁘다고 알려진 인식은 사실이 아니며, 오히려 심장질환 예방에 효과적인 것으로 밝혀졌다. 또한, 계란 섭취량에 관한 공식적인 기준도 없는 것으로 나타났다. 따라서 이와 같은 증거들은 더 이상 계란에 대한 오해와 불신이 없기를 제안한다.

Study on the Changes in Enzyme and Insulin-like Growth Factor-1 Concentrations in Blood Serum and Growth Characteristics of Velvet Antler during the Antler Growth Period in Sika Deer (Cervus nippon)

  • This study was conducted to investigate changes in blood enzyme parameters and to evaluate the relationship between insulin-like growth factor-1 (IGF-1), antler growth and body weight during the antler growth of sika deer (Cervus nippon). Serum enzyme activity and IGF-1 concentrations were measured in blood samples collected from the jugular and femoral veins at regular intervals during the antler growth period. Blood samples were taken in the morning from fasted stags (n = 12) which were healthy and showed no clinical signs of disease. Alfalfa was available ad libitum and concentrates were given at 1% of body weight to all stags. The experimental diet was provided at 9 am with water available at all times. There were no significant differences in alkaline phosphatase, aspartate aminotransferase, and alanine aminotransferase during antler growth, but alkaline phosphatase concentrations increased with antler growth progression, and the highest alkaline phosphatase concentration was obtained 55 days after antler casting. Serum IGF-1 concentrations measured from blood samples taken from the jugular vein during antler growth, determined that levels of IGF-1 was associated with body weight and antler growth patterns. Serum IGF-1 concentrations were higher at the antler cutting date than other sampling dates. Antler length increased significantly during antler growth (p<0.001), and there was a similar trend to between right and left beams. Body weight increased with antler growth but was not significant. Consequently it appeared that serum alkaline phosphatase concentration was related to antler growth and both antler growth and body weight were associated positively with IGF-1 concentrations during antler growth.

Effects of compound organic acid calcium on growth performance, hepatic antioxidation and intestinal barrier of male broilers under heat stress

  • Objective: The aim of this study was to evaluate the effects of compound organic acid calcium (COAC) on growth performance, hepatic antioxidant status and intestinal barrier of male broilers under high ambient temperature (32.7℃). Methods: Nine hundred healthy one-d-old Cobb-500 male broiler chicks were randomly assigned into three groups with six replicates of 50 birds each. A basal diet supplemented with 0% (control), 0.4% and 0.8% COAC, respectively were fed to birds for 6 weeks. All treatments were under high ambient indoor temperature of 32.7℃, and had a constant calcium and available phosphorus ratio. Results: The results showed that, compared with control, the average daily gain of broilers in 0.4% and 0.8% was significantly increased and the ratio of feed to gain in in 0.4% and 0.8% was significantly decreased at 1 to 21, 22 to 42 and 1 to 42 days of age (p<0.05). Compared with control, 0.8% COAC slightly decreased (p = 0.093) the content of malondialdehyde in liver at 42 days of age while 0.4% COAC significantly decreased (p<0.05) the activity of alkaline phosphatase. Furthermore, 0.4% COAC significantly enhanced the intestinal barrier function via increasing jejunal and ileal ocln transcription, promoting jejunal mucin 2 transcription at 42 days of age (p<0.05), and decreasing jejunal toll-like receptor 2 (TLR-2) and ileal TLR-15, inducible nitric oxide synthase compared with control group (p<0.05). Whereas, no significant differences on the transcription of interleukin-1β in jejunum and ileum were observed among three treatments (p>0.05). Overall, heat stress caused by high natural environment temperature may induce the damage to hepatic antioxidation and intestinal barrier. Conclusion: Dietary inclusion of COAC can improve the tolerance of broilers to thermal environment through the modification of antioxidative parameters in liver and the mRNA expression of genes in intestinal barrier, resulting in an optimal inclusion level of 0.4%.

해조류(톳, 감태) 혼합물의 사료 내 첨가가 넙치의 선천성 면역과 질병저항성에 미치는 영향 (Effects of Dietary Supplementation of Alga Mixtures (Hizikia fusiformis and Ecklonia cava) on Innate Immunity and Disease Resistance Against Edwardsiella tarda in Olive Flounder (Paralichthys olivaceus))

  • We report non-specific immune responses and its disease resistance against Edwardsiella tarda by alga mixture (HE; Hizikia:Ecklonia) in olive flounder for the first time. Five isonitrogenous (44% crude protein) and isocaloric (17.1 MJ $kg^{-1}$) diets were formulated to have 0%, 2%, 4%, 6% and 8% of the alga mixture. One of five experimental diets was fed triplicate groups of fish (30 fish/group) to apparent satiation in a flow through system. After a two week feeding, blood was sampled at 3, 6, 12, 24 h after the last feeding for a kinetic measurement of nitroblue tetrazolium (NBT) activity and healthy fish with similar sizes in each tank were selected and injected with 1 mL of E. tarda suspension ($1.0\times10^7$ CFU/mL) to evaluate the disease resistance of the fish. Dietary supplementation of alga mixtures resulted in significantly higher non-specific immune responses compared with the fish fed the control diet. The cumulative mortality was significantly lower in the fish groups fed alga mixture containing diets than control group in the challenge test with E. tarda. Therefore, the results in this study indicate that dietary supplementation of Hizikia and Ecklonia mixtures enhance the non-specific immune responses and a disease resistance of olive flounder.
