Purpose: Effective transitional care is needed to improve the quality of life in older adult patients with chronic illness and avoid discontinuity of care and adverse events. The aim of this article is to provide an overview of the key features, broader implications, and the utility of Meleis' transition theory intended for the transitional care of older adults with chronic illnesses. We present the role of nurse in the context of transitional care and propose future directions to increase the quality of nursing care. Methods: The online databases Cumulative Index of Nursing and Allied Health Literature, MEDLINE, and Science Direct were searched for relevant literature published since 1970 along with textbooks regarding nursing theory. Results: An evaluation of the usefulness of transition theory based on transitional care in older adult patients with chronic illnesses is provided. Healthy transition should be the expected standard of nursing care for older adults across all healthcare settings. Conclusion: Nurses need to contribute to the development of transitional care for vulnerable populations; however, transition theory needs to be enhanced through additional theoretical work and repeated evaluations of the applicability in areas of transitional care.
Background: This study was undertaken to evaluate how gender differences affect the sensory processing by the Adolescent/Adult Sensory Profile(ASP) which evaluates the behavioral responses of adults to everyday sensory experiences. Methods: The study period lasted one month which was June 2009, and the study subjects were 325(three hundred twenty five) people who were healthy and reside in Chungcheong province including Daejeon. Detailed information about the study was explained to the study group and the ASP questionnaires were drawn up by them personally. Results: On average, the study group scores similarly in low registration, sensation seeking, sensory sensitivity, and sensation avoiding. According to the sensory processing abilities by gender, the group obtained scores indicating performance similar to most people in low registration, sensation seeking, and sensory sensitivity, except for sensation avoiding. Furthermore, in the taste/smell processing of "I only eat familiar foods", sensation avoiding score was higher in females than males, and in the activity level of "I find time to get away from my busy life and spend time by myself", males' sensation avoiding scored higher than females. Conclusion: In the study of differences of sensory processing abilities of adults by gender using the ASP, gender differences were revealed in the category of sensation avoiding.
This study was performed to determine the effect of dietary Na levels on plasma amino acid levels. Plasma amino acid levels were compared in 20 adult healthy women subjects who were given high Na diet (290.48 mEq/day : NaCl 17g) or low Na diet (51.26 mEq/day : NaCl 3g) for subsequent 6 days. Plasma essential amino acids levels were significantly decreased (36%) while plasma non-essential amino acids levels were significantly increased (22%) when subjects were given low Na diet (p<0.001). Among essential amino acids, threonine was decreased (74%) significantly when subjects were given low Na diet (p<0.01). Among nonessential amino acids, serine (49%), proline (20%) and aspartic acid (14%) were increased (p<0.01), while arginine (48%) and glutamic acid (27%) were decreased (p<0.001). In conclusion, dietary Na contents seemed to be an important factor to affect plasma amino acid levels. It would be appropriate to decrease the dietary Na intakes level considering the various clinical effects of dietary Na on the body fluid. For the patients who need low Na diet, it would be suggested that the level of dietary proteins should be carefully considered along with dietary Na manipulation. This study was intended to investigate the anticarcinogenic effect of ginseng previously elucidated by other researches in animal studies. The sister chromatid exchange(SCE) method of human lymphocytes was used as a biomarker. Based on the literature search and the results of our laboratory, smoking was used as a parameter elevating the SCE frequency of general human population. To evaluate the smoking and ginseng effect on SCE frequency, 98 male healthy factory workers aged 23 to 58 years were divided into 4 groups : smoker with ginseng (SG), smoker control(SC), non-smoker with ginseng(NSG), and non-smoker control(NSC) groups, according to their smoking habits and ginseng intake. The mean sponteneous SCE per cell for the SG(10.8$\pm$0.3) and SC(10.4$\pm$0.3) groups were significantly higher than the NSG(9.1$\pm$0.2) and NSC(9.3$\pm$0.3) groups(p<0.05). High frequency cells (HFCs, cells with 15 SCEs) in SG and SC groups were also greater than those in NSG and NSC groups. However, the SCE levels of the SG and SC groups were not associated with the personal smoking history and the number of cigaretts smoked per day. Ginseng intake did not show any effect on the increased SCE caused by smoking. There were no correlations of the elevated SCE among smoking and ginseng types, history of ginseng intake, and consumption frequencies of ginseng intake. These results does not support the findings of other researchers that ginseng could be a protective agent to DNA damage.
Purpose: The purpose of present study was to explore relationship between physical activity (PA) behavior and self-efficacy among older Korean-Americans (OKAs) based on reviewing major theories that are used to understand PA behavior, particularly as they relate to older adults within sociocultural contexts. Methods: 151 OKAs, living in California, were recruited and interviewed once. Self-efficacy and PA level were measured by Exercise Self-Efficacy Scale, Community Healthy Activities Model Program for Seniors (K-CHAMPS) questionnaire, respectively. Results: 82.8% of OKA women and 72.4% of OKA men had relatively low ($38.8{\pm}29.3$) self-efficacy related to PA. Exercise self-efficacy was statistically significant and positively associated with frequency for moderate and greater PA (r = 0.25, p < .01), frequency for all listed PA (r = 0.21, p < .05), caloric expenditure for moderate and greater PA (r = 0.23, p < .01), and caloric expenditure for all listed PA (r = 0.31, p < .01). Conclusion: Social Cognitive Theory including self-efficacy can provide insight about how to shape strategies that encourage OKAs to adhere to regular PA because it focuses on environmental as well as personal factors. Increased efforts are needed to understand the other factors associated with being active in OKAs.
Purpose: The purpose of this study was to identify the influencing factors on postpartum fatigue in the parturient women. Methods: The data were collected from July to September, 2006 using self-report questionnaires. The sample was 143 healthy parturient women who visited the outpatient clinic at 2 hospitals for a routine follow up after childbirth. The data were analyzed by using SPSS 12.0 Win program. Results: There were significant differences in the postpartum fatigue by age, religion, sleep hour, and complaint of perceived health problem. The postpartum fatigue were significantly correlated with infant care stress(p<0.001), sleep satisfaction(p<0.001), and feeding difficulty(p=0.016). The multiple regression analysis showed that the postpartum fatigue were significantly predicted by the infant care stress, sleep satisfaction and complaint of perceived health problem. These variables explained 31.8% of the variance of postpartum fatigue. The infant care stress was the main predictor of postpartum fatigue. Conclusions: It could be concluded that the nursing intervention for reducing the infant care stress should be developed to manage the postpartum fatigue for parturient women. Prospective studies are needed to be conducted to verify the causal relationship between infant care stress and postpartum fatigue.
Purpose: This study was to analyze the research trends centering on the theses and articles related to reflexology released in Korea. Method: This study is a descriptive study in which the researcher collected the academic degrees and theses published on the book of the academic society from 1983 to August 2005, and examined 52 domestic dissertations and articles on reflexology. Results: By academic department, the result were: the nursing theses 25(48.1%), and public health and physical education each 4(7.7%). In the type of reflexology, the result were: single therapy like hand acupuncture or hand moxibustion 27(61.4%), combined therapy 17(38.6%). As for the research design, the experimental research was most frequent (84.6%). As for the subject, the result were: healthy people 31(59.6.%), patient 11(21.1%), medical staff 7(14.9%) in order. In the theme of the theses, the effect of therapy were most frequent(84.6%). Conclusion: Researches about reflexology should be conducted and extend into interdisciplinary fields and medical areas. On the basis of preceding results, qualitative studies and international and cross-sectional comparison studies are needed to apply in practice with scientific background.
Purpose: This study was conducted, on the basis of the Theory of Planned Behavior to analyze factors influencing on undergraduate students' sexual behavior and to provide basic guidelines for the intervention of promoting sex education in school. Method: The participants of this study consisted of 284 under-graduate students at 3 colleges and 3 universities in Seoul. SPSS 10.0 win program was used and descriptive statistics, t-test, ANOVA, Pearson correlation, multiple regression and logistic regression were used to evaluate the influencing factors on undergraduate students' sexual behavior. Results: The results of this study can be summarized as follows; For undergraduate students, perceived behavioral control, and attitude of sexual behavior could explain 43% of the variance in intention of sexual behavior. However, subjective norm didn't influence sexual behavioral intention. For undergraduate students, perceived behavioral control, sexual behavioral intention were factors influencing sexual behavior, and these factors could explain 43% of the variance in sexual behavior. Conclusion: In conclusion, the interventive sex education that is upright and healthy will be needed to strengthen perceived behavioral control and sexual behavioral intention as influencing factors on sexual behavior in adolescent and young adults.
Purpose: As population of elderly people continues to grow, successful aging has risen to the top of the nursing science agenda. The successful aging includes maintenance of the cognitive and physical functions, as well as emotional well-beings. This study was carried out to evaluate effects of the environmental enrichments on cognition of institutionalized elderly. Method: The population was selected among the elderly aging over 65 residing at two of institutions. A quasi experimental design was used with non-equivalent control group. Study subjects were thirteen for each group. For the experimental group, physical, social and symbolic environmental enrichment program was provided for six weeks. The data were analyzed by repeated measure ANOVA and repeated measure ANCOVA using SPSS Win 11.0. Result: Compared to control group, the experimental group showed a significant difference on DSF(F=3.29, p=.046), and TMTA(F=4.76, p=.013) of cognitive function, and depression (F=5.56, p=.007) of emotional distress after 1 and 12 weeks of environmental enrichment program. Conclusion: Findings indicate that physical, social, and symbolic environmental enrichment was effective to partially prevent from cognitive decline, and to decrease emotional distress of elderly. As a nursing intervention, environmental enrichment program for elderly should be expanded for nursing practice to promote healthy aging and to offer support to the growing population of elderly. Further research should be conducted to evaluate the effect on the community elderly.
Purpose: This study was to examine the food intake and family support of patients with Coronary Artery Disease(CAD). Methods: 90 hospitalized patients who received CAD medical treatment were randomly selected. The food intake frequency and family support instrument were utilized. Collected data was analyzed by SPSS/WIN 14.0: t-test, $x^2$-test and Pearson's correlation coefficient. Results: The mean score of food intake frequency was 5.65(total mean score 97.50) which was relatively healthy food intake based on the CAD treatment guidelines. Undesirable food intake items were fruit, fish, milk etc. The mean score of family support was 2.44(range: 1 - 4). Food intake showed a significant(p < .001) positive correlation with family support. There was significant difference(p < .05) on family support according to exercise. There was significant difference(p < .05) on LDL level, one of the CAD risk factors, between first admission patients and readmission patients. Conclusion: These findings suggest that nursing intervention education programs should be developed to appropriately care for CHD patients for needed changes.
