Priority Setting for the Healthy City Program in Busan Using the Analytic Hierarchy Process (계층 분석법을 적용한 부산시 건강도시 사업의 우선순위 설정)

  • Objectives: Busan had the highest mortality and the shortest life expectancy at birth among 16 provinces in Korea in 2008 and there were considerable health inequalities within the region. This study was performed to build up a priority setting framework in Healthy City Busan project. Methods: Analytic hierarchy process was used to determine the relative priority weight for different strategic and program dimensions along with the consistency of response. An on-site workshop-based meeting (calculating importance) and online survey (calculating risk) were conducted to obtain data from 8 experts. Results: The results showed that in strategic criteria "active health promotion & diseases prevention" and "building infrastructure for the Health City project" were two most important factors. In program criteria, considering both importance and risk scores, "making a healthy community" and "building community health centers" in disadvantaged areas were a top priority group. In addition, "enacting an ordinance for the Healthy City", "building the infrastructure for health impact assessment" and "making health care safety net for vulnerable population" were also higher priorities group. Conclusions: Our findings suggest that the Healthy City project in Busan should be focused on strengthening health equity and building infrastructure for sustainability of the project.

Comparison of Weight Control Behavior and Self-esteem between Healthy Weight and Obese Children (건강체중아 및 비만아의 체중조절 경험, 식습관, 운동습관 및 자아존중감 비교)

  • The study compared eating and physical activity related behaviors and self-esteem between healthy weight and obese children by presenting 175 primary school students in Busan City and Gyeongsang province with a self-reported questionnaire and Coopersmith's self-esteem inventory. The questionnaire was composed of 25 items, weighted primarily by a Likert scale. The self-esteem inventory presented to the students comprised 25 'Yes' or 'No' response questions to different statements. The study found obese children were more likely to think they always had to control their weight (p=0.000), reportedly measuring their weights significantly more than the healthy weight children. Also the study found that obese children are significantly more likely than healthy weight children to go on a diet, however neither group were successful in losing weight as the duration of the diet in 79.5% of the total sample lasted no longer than one week. In comparison to healthy weight children, obese children reported that they consumed fewer snacks during the day, avoided snacking subsequent to an evening meal and exercised more frequently for as long as physically possible. Interestingly, we found no difference of reported self-esteem between groups, though the obese group were more likely to answer that their parents did not understand them (p=0.055) Based on these findings, we concluded that the obese children who participated in the study were more aware of their body weights than the healthy weights children. It may be necessary to investigate further the relationship between self-esteem and participants' weights while considering other variables such as personality and body image.

Major Dietary Patterns and Their Associations with Socio-Demographic, Psychological and Physical Factors Among Generally Healthy Korean Middle-Aged Women (건강한 한국 중년 여성에서의 주요 식이패턴과 인구.사회적, 심리적, 신체적 요인 간의 연관성)

  • This study was conducted to identify major dietary patterns of generally healthy Korean middle-aged women and to examine associations of the dietary patterns with socio-demographic, psychological, and physical characteristics. Data were drawn from the 2001 Korean National Health and Nutrition Examination Survey (KNHANES), which is an ongoing national surveillance system. Healthy female subjects aged 40-64 years and provided the health interview examination and food frequency questionnaire (FFQ) data were included. We conducted factor analysis based on daily mean intake frequency of 26 food groups and identified 3 major dietary patterns, healthy, convenient, and alcohol-animal. The healthy pattern was characterized by higher intakes of fish, seafood, poultry, mushroom, seaweed, vegetable, tomato, potato, and fruit, the convenient pattern by processed meat and fish, ramen, noodle and rice cake, sweet snack, egg, bread, and fast food, and the alcohol-animal pattern by alcohol, poultry, meat, and fish. The healthy pattern was significantly related with higher socio-economic status, more stable family structure, healthier mentality, and sounder status of anthropometric and biochemical variables. Those with higher factor scores for the convenience pattern were found to be younger and having higher socio-economic status, higher subjective levels of health, and lower subjective levels of worry on health. Several physical characteristics significantly improved as the levels of the convenience pattern increased. The alcohol-animal pattern was significantly associated with several socio-demographic characteristics including a low education level, a young age, a small family size, and blue-collar jobs. Public health policy makers can utilize the study findings to select prior target populations with higher needs and to tailor dietary behavioral messages to lower chronic disease risks among Korean middle-aged women.

A Study on the Development of the Goals and Contents System of Healthy Dietary Education Program for After-School Care in Lower Grade in Elementary School (초등 저학년 돌봄 교실 건강식생활 교육프로그램의 목표 및 내용체계 개발 연구)

  • Objectives: The study purpose is to develop a content system for a healthy dietary education program for after-school care in lower grade in elementary school. Methods: The contents of healthy dietary education in the 2015 revised curriculum and textbooks and the major education programs related to dietary life that are currently used in elementary school education were analyzed. Focus group interviews were held with field experts related to lower grade in elementary care class. Accordingly, the structuring of the education area and the detailed education contents were systematized. Results: From the analysis results, the contents of curriculum, textbook, and administrative department were classified as hygiene safety, health, and culture. The goal of the educational content system was divided into three areas: nutritional dietary life, food hygiene and health, and food culture. The subjects consisted of dietary balance, healthy body weight, digestion and absorption, food hygiene, Korean agricultural products, traditional food, and table manners. The curriculum was composed of 12 content elements. Conclusions: In order to ensure that after-school care students can grow into healthy, growth-oriented and creative talents, the role of the caring guide is important, and associated guidelines are needed in the future.

Can Dining Alone Lead to Healthier Menu Item Decisions than Dining with Others? The Roles of Consumption Orientation and Menu Nutrition Information (혼밥이 건강한 메뉴 선택에 미치는 영향: 소비 목적 지향과 메뉴 영양 정보 표시의 역할)

  • Objectives: Driven by a growth of single-person households and individualized lifestyles, solo dining in restaurants is an increasingly recognizable trend. However, a research gap exists in the comparison of solo and group diners' menu-decision making processes. Based on the self-control dilemma and the temporal construal theory as a theoretical framework, this study compared the ordering intentions of solo vs. group diners with healthy vs. indulgent (less healthy) entrées. The mediating role of consumption orientation and the moderating role of amount of menu nutrition information were further explored to understand the mechanism and a boundary condition. Methods: A scenario-based online survey was developed using a 2 (dining social context: solo vs. with others) × 3 (amount of menu nutrition information: no nutrition information vs. calories vs. calories/fat/sodium), between-subjects, experimental design. Consumers' level of nutrition involvement was controlled. A nationwide survey data (n = 224) were collected from a crowdsourcing platform in the U.S. Data were analyzed using multivariate analysis of covariance, independent t-test, univariate analysis of covariance, and moderated mediation analyses. Results: Findings reveal that solo (vs. group) diners have less (vs. more) intentions to order indulgent menu items due to a more utilitarian (vs. more hedonic) consumption orientation in restaurant dining. Findings also show that solo (vs. group) diners have more (vs. less) intentions to order healthy menu items when the restaurant menu presented nutrition information including calories, fat, and sodium. Conclusions: The findings contribute to the literature of foodservice management, healthy eating, and consumer behavior by revealing a mechanism and an external stimuli of solo vs. group diners' healthy menu-decision making process in restaurants. Furthermore, the findings provide restauranteurs and health professionals with insights into the positive and negative impacts of menu nutrition labelling on consumers' menu-decisions.

Factors Influencing Healthy Living Practice by Socio-ecological Model (사회생태학적 모형에 의한 건강 생활 실천 관련 요인)

  • The purpose of this study is to provide basic data for resolving individual and regional health inequalities by identifying factors that affect healthy living practices, and to protect the access to health equity and the access to health equity and the people's right to health. Raw data from the 2019 Community Health Survey were used, and descriptive statistical analysis and multivariate logistic regression analysis were performed using SAS 9.4 and IBM SPSS ver. 21. The healthy living practice rate was 33.8% overall, and there was a difference of 11~20% by region. In terms of individual factors, healthy living practices were significantly different in gender, age, occupation, sleep time, subjective health status, and subjective stress level. In the interpersonal factor, there was a difference in social activity for healthy living practice, and in the community factor, positive attitude toward the local physical environment, annual unsatisfied medical care, and use of health institutions were significant. In order to increase the practice of healthy living by region based on the research results, comprehensive policies and cooperative measures that can be approached at the individual, social and national level should be implemented along with specific strategies.

Sense of community and community participation for health promotion in urban areas of Korea (건강증진을 위한 지역사회 참여와 지역사회 공동체의식: 대도시 지역을 중심으로)

  • Objectives: This study aims at examining the association of sense of community with community participation for health promotion in urban areas of Korea. Methods: We analyzed data from 'Community Capacity for Healthy Gangdong Communites' Survey' in 2007. The survey was based on self-reported questionnaires, which were distributed to 1,800 community residents over the age of nineteen in five administrative communities of Gangdong-gu, Seoul, in Korea by using proportionate probability sampling method. We measured 'Sense of community' with four indicators including 'Good neighborhoods', 'Perceived possibility of cooperation', 'Pride of community' and 'Possibility of development' by using 5-point Likert scales. Community participation was measured with the experience rate or the extent of participation by 5-point Likert scales in seven community actions or activities including voting, community program planning, social actions, etc. We examined the association of sense of community with community participation by using regression analyses. Results: This study has shown that sense of community was associated with and made positive impacts on community participation in diverse community actions or activities in urban communities. Conclusions: For promoting community health in urban areas, we can increase community participation more effectively with the efforts of enhancing sense of community.

A Study on The Health Status of Island Community People in Island (도서지역 주민의 건강상태에 관한 연구)

  • During the past 10 years, concern for community people's health has increased together with the changes. Public health policies and studies for community people's health, however, have influenced those in childmaternal health care to want more general coverage and studies of health and wellness. Particularly, the study of community people's health in the extent an island area is almost rare as that personal and the material benefits in this area are lacking of community people's work is large, and the basic elements of living, such as diet and elimination, are irregular due to the schedule of the tide. Thus, there are many potential health problems. In this regard, the study attempted to understand the health problems of island community people and to provide a basis for developing health promotion and health education programs. In collecting data for the study, face to face interviews were made through a structured questionnaire from October 1 to December 30, 1996. Collected data were analyzed with the SAS statistics program, descriptive statistics, t-test and ANOVA. Subjects' health status was examined by classifying into such categories as their health perception, complaints of health problem, related lifestyle, psychosocial health staus, the result of examination is as follows; 1. For subjects' health perception, 26.9% of the subject answered not sick, but not so healthy'; 30.9% thought they were healthy, while 22.9% answered that they were not healthy. 2. For the health problem complaints many complained of pains in their muscles and skeletal system, especially knee joint pain. Women's health problems related with breast and the reproductive system included 52.3% of cases doing breast self examination, while 56.55% received the cervical cancer screening test. In men's health problems, 44.2% of subjects answered that they have moderate to severe BPH(Benign Prostatic Hypertrophy) symptom. 3. There were statistically a significant difference in the degree of physical health according to marital status(p=0.0028), occupation(p=0.0442), income(p=0.0357). 4. For stress status, 17.2% was to need the intervention, 50.2% was to need observation. 5. The mean score of self-esteem was 27.7 showing a relatively high score. 6. For the rate of smoking, 37.7% used to smoke, while 28% used to take alcohol. 7. The rate of substance abuse was 45.9% of subjects. 8. Most of subjects' health behaviors included most of the acupuncture (52%). 9. The rate of subjects receiving comprehensive medical testing was 34.36% while 34.78% did after care managing behavior. 10. For the obesity grade, 53% is normal weight, low-weight 32.8%, obesity 33%. 11. For nutrition status, 78.7% illy balanced to need intervention of nutritional education. 12. For 78.7% of subjects, muscle strength and 40.7% of stretching were not good enough to need health education on physical exercises. Therefore, based on the results, appropriate health education programs need to be developed to promote health of community people on an island.
