단체급식 안전관리에 대한 영양사 인식 조사(II) - 안전시스템 및 안전교육 현황 - (Dieticians' Perception of Safety Supervision in Institutional Foodservices (II) - Status of Safety System and Safety Education -)

  • The purpose of this study was to investigate the status of the safety system and safety education in institutional foodservices in the Changwon and Masan areas. The survey was conducted from February 1 to March 31, 2009 via questionnaires that were sent to 300 dietitians, and 142 dietitians responded. It was determined that most of the safety managers were 'dietitians (87.1%)', whereas facilities/equipment managers consisted of 'dietitians (45.7%)', 'department of facilities management in the organization (36.4%)' and 'outsourced company of facilities management (17.9%)'. Out of the 11 safety practices, seven safety practices showed less than 50% of total implementation ratio, which meant that the safety systems were not functioning properly. Except for 'non-skid shoes (85.9%)', other safety equipment was seldom used. The survey respondents recognized that safety education was very necessary (4.47 points); however, they responded so-so (3.46 points) to the question of whether or not the actual frequency and time spent on safety education were enough. The average time spent on safety education was 28 minutes 11 seconds. Regarding the difficulties in performing safety education, 'not many safety education materials and media (3.44 points)', 'not many varieties in the subjects and contents for safety education (3.40 points)', and 'not much organizational support on safety education (3.33 points)' showed higher scores than 'lack of education time due to workload (3.20 points)'. The following were cited as education materials that should be developed as it is currently difficult to obtain relevant information and data: 'root causes of musculoskeletal injury and preventive measures (15.8%)', 'healthcare and disease preventive exercises for employees who do simple and repetitive works (14.9%)', and 'instruction on safe handling of chemicals (12.7%)'

의료기관에서의 환자 중심 교육 시스템 연구 (Study on Clinical Patient-oriented Education System for Medical Organizations)

  • 환자의 입원전후, 입원생활, 수술전후, 퇴원후 정보교육 및 식사 영양지도 등 의료 기관에서의 적재 적시적 환자교육 정보의 부재, 지연과 오류는 임상적 예후는 물론 의료의 질 저하와 의료분쟁 등 심각한 문제를 야기시키고 있다. 따라서 최근 의료선진국을 중심으로 의료의 질 관리, 상호소통 및 의료분쟁의 화두와 함께 맞춤형 환자교육 체계의 중요성이 크게 부각되고 있다. 의료진, 원무행정 및 임상 실험실들을 중심으로 환자교육이 네트워크기반으로 활발하게 이뤄질 때 의료 질이 향상되고, 궁극적으로 병원의 경영이익창출에도 도움이 되는 것으로 S병원 사례에서 유의미하게 나타났다. 이러한 동기에서 본 논문에서는 CPES 개념의 연구를 바탕으로 웹기반 맞춤형 환자교육 시스템을 제안하고 S 병원을 대상으로 설계와 구현을 진행하였다. 더불어 S병원에서의 실증적 계량분석을 통해 ?춤형 환자 중심 교육 시스템의 도입 효율성과 효과성을 분석하였다.

뇌혈관질환 사망자 의료이용 변이의 분석 (A Study on the Variation in Meical Service Utilization of The Dead by Cerebrovascular Diseases Patients in Korea)

  • This study focused on finding the variation of medical service utilization, paths of medical service utilization and medical payments of the patients died by cerebrovascular diseases. For this study, data of the one-year episodes of the health insurance subscribers died in 2004 were selected. The frequency of medical visits, the lengths of stays, the days of outpatient visits, the total period of medical services and the total medical payments were compared by the characteristics of the suppliers and utilizers. This study is useful in reviewing the equity of medical service utilization because it analyzed variance in utilization by episodes. In oder to collect accurate data of the patients died by cerebrovascular diseases in 2004 the 2004 reimbursement data of all medical institutions were matched to the data of funeral fee payment by the National Health Insurance Corporation from January 2004 to May 2005. The major results of the study are as follows. The variation of medical service utilization of cerebrovascular diseases was influenced by supplier factors suppliers, such as types and locations of medical institutions and user factors such as sex and age. It was suspected that the reimbursement by fee-for-service contributed to the variation quite a lot, but we could not compare the variation between the different reimbursement systems in Korea. On the basis of analyzing results this study suggests that the factors of suppliers and utilizers should be reviewed to reduce the under use and over use expressed by variations of medical service. The processes of care, effective communication and management system should be investigated for the equity of medical service utilization and also. alternative medical services would be recommended to reduce the high medical payment. Additionally to find other causes of variation further in depth study controling the severity of diseases, socio-economic status of the users and the system factors is required.

For this study, data of the one-year episodes of the health insurance subscribers died in 2004 were selected. The results of the study suggested that tile factors of suppliers and utilizers should he reviewed to reduce the under use and over use expressed by variations of medical service utilization. The processes of care, effective communication and management system should be investigated for the equity of medical service utilization. Additionally, prospective payment could he recommended to reduce the high variation of medical service Use. To find the variation caused by under use and over use, further study need to control the severity of diseases, socio-economic status of the users and the system factors.

  • 최근 의료기술의 발전으로 인하여 응급의료 서비스가 병원에서 가정으로 변화되고 있다. 이와 관련하여 예방 또는 조기진단을 위한 연구가 활발해지고 있다. 특히, 생체신호를 모니터링하여 다양한 u-헬스케어 응용 서비스에 적용하고 있다. 본 논문에서 제안하는 시스템은 다양한 센서로부터 측정된 의료정보 보호 및 보안 기술을 제안하고자 한다. 특히, 생체신호는 개인의 주요 정보이면서 프라이버시와 관련된 정보이기 때문에 보호 및 관리를 위해 2차원 코드화 기술인 QR 코드를 적용하였다. 클라이언트 단말에서는 QR 코드를 분석하여 확인할 수 있도록 하였다. 끝으로 제안한 플랫폼 상에서 의료영상정보와 생체신호의 통합 이미지 파일 생성과 배포를 확인하는 응용서비스를 통해 수행 결과를 보였다.

  • From the last past decade, the Internet of Thing (IoT) area has attracted a lot of attention from researchers. It is said to be a promising technology with great impact in people life, since it redefines the relationship objects have with Human and between themselves. It allows objects to gather data from the real world and communicate with others through the internet. This enabled many opportunities for service providers, companies, factories, environmental monitoring, healthcare, smart cities, and soon. Therefore, today, IoT is densely used in various domains of life, and knows an exponential growth. However, although many advancements have been achieved, several challenges keep causing issues and still need to be overcome. This paper gives an overview on the current trend issues in IoT on which researchers are focusing. It's also explores different proposed frameworks to allow the application of cognitive computing as an integrated process of an Internet of things (IoT) systems, to bring a great advanced in the way machine may communicate with human and their surroundings. This is known as cognitive IoT (CIoT), which allows machines to produce a human-like behavior, then providing enhanced level of capabilities to IoT.

Evaluating Direct Costs of Gastric Cancer Treatment in Iran - Case Study in Kerman City in 2015

  • Background: Gastrointestinal cancers are common malignancies associated with high mortality rates. Healthcare systems are always faced with high costs of treatment of gastrointestinal cancers including stomach cancer. Identification and prioritization of these costs can help determine economic burden and then improve of health planning by policy-makers. This study was performed in 2015 in Kerman City aimed at estimating the direct hospital costs for patients with gastric cancer. Materials and Methods: In this cross-sectional study, the medical records of 160 patients with stomach cancer admitted from 2011 to 2014 to Shafa Hospital were examined, the current stage of the disease and the patients' health status were identified, and the direct costs related to the type of treatment in the public and private sectors were calculated. SPSS-19 was used for statistical analysis of the data. Results: Of the patients studied, 103 (65%) were men and 57 (35%) were women. The mean age of patients was 65 years. Distribution into four stages of the disease was 5%, 20%, 30%, and 45%, respectively. Direct costs in four stages of the disease were calculated as 2191.07, 2642.93, 2877, and 2674.07 USD (63,045,879, 76,047,934, 82,783,019, and 76,943,800 IRR), respectively. The highest percentage of costs was related to surgery in Stage I and to medication in Stages II, III, and IV. According to the results of direct costs of treatment for stomach cancer in Kerman, the mean total cost of treating a patient in the public sector was estimated at 74,705,158 IRR, of which averages of 60,141,384 IRR and 14,563,774 IRR were the shares of insurance and patients, respectively. Conclusions: The high prevalence and diagnosis of disease in old age and at advanced stages of disease impose great costs on the patients and the health system. Early diagnosis through screening and selecting an appropriate treatment method might largely ameliorate the economic burden of the disease.

  • 현대사회의 정보화 기반 시스템은 매우 빠른 속도로 변하고 있으며, 무선통신과 센서 네트워크 분야 또한 변하고 있다. 센서 네트워크 분야에서는 RFID 태그를 이용하여 홈 네트워크, 각종 제어 시스템, U-헬스케어 시스템과 물류 유통에까지 그 영역을 확장하고 있다. 이 중 RF태그를 이용한 제어 분야에 대한 연구는 본격적인 서비스 인프라를 지향하기 위해 진행되고 있으며, 각종 이력 정보와 주변 환경 정보를 센싱하고 제어하는 지능형 센서네트워크 구축을 위한 핵심 영역이 되고 있다. 본 논문에서는 이러한 취지의한 기반으로 UHF 대역의 RFID 태그 기술을 선박에 적용하여 선박 내 인원에 대한 관리의 효율성을 높이기 위한 선행 연구로서 선박 내에서의 태그의 안전성 확보와 구현한 프로세서의 동작을 확인하고 이를 이용한 출입의 통제 및 안전사고 방지를 위한 선박 내 위한 인원 관리 시스템을 설계하고 구현하였다.

An Automated Draft Report Generator for Peripheral Blood Smear Examinations Based on Complete Blood Count Parameters

  • Background: Complete blood count (CBC) results play an important role in peripheral blood smear (PBS) examinations. Many descriptions in PBS reports may simply be translated from CBC parameters. We developed a computer program that automatically generates a PBS draft report based on CBC parameters and age- and sex-matched reference ranges. Methods: The Java programming language was used to develop a computer program that supports a graphical user interface. Four hematology analyzers from three different laboratories were tested: Sysmex XE-5000 (Sysmex, Kobe, Japan), Sysmex XN-9000 (Sysmex), DxH800 (Beckman Coulter, Brea, CA, USA), and ADVIA 2120i (Siemens Healthcare Diagnostics, Eschborn, Germany). Input data files containing 862 CBC results were generated from hematology analyzers, middlewares, or laboratory information systems. The draft reports were compared with the content of input data files. Results: We developed a computer program that reads CBC results from a data file and automatically writes a draft PBS report. Age- and sex-matched reference ranges can be automatically applied. After examining PBS, users can modify the draft report based on microscopic findings. Recommendations such as suggestions for further evaluations are also provided based on morphological findings, and they can be modified by users. The program was compatible with all four hematology analyzers tested. Conclusions: Our program is expected to reduce the time required to manually incorporate CBC results into PBS reports. Systematic inclusion of CBC results could help improve the reliability and sensitivity of PBS examinations.

Occupational Health Protection for Health Workers in China With Lessons Learned From the UK: Qualitative Interview and Policy Analysis

  • Background: Healthcare settings have been recognized among the most hazardous places to work. Based on the five categories of occupational hazards that were identified by the ILO and WHO, this study aimed to analyze policy framework relevant to occupational health protection of health workers (HWs) in public health service in China, then discussed how to share the experience of the National Health Service (NHS) England for improvement. Methods: Based on policy learning theories, policy analysis and qualitative interview were used in this study. Results: In the Chinese public health service, at least five laws related to the regulation of occupational health protection for HWs; however, enforcement of relevant laws was separated and multi-centered; the national monitoring system, which targeted to occupational hazards and health outcome for HWs in China, had yet to be developed; the top three priorities were workplace violence, bloodborne pathogens, and musculoskeletal disorders; national strategies included Security Hospital, and Healthy China 2030. In NHS England, three laws were fundamental; several monitoring systems had been set up, including NHS Staff Survey, Commissioning for Quality and Innovation incentive scheme; mental health, musculoskeletal problem, and nutrition disorder and overweight were raised great concern; Health and Safety, and NHS Healthy Workforce Program were critical nationwide strategies. Conclusion: There were several similarities as well as differences between the Chinese public health system and NHS England, which laid foundation of learning by China. Recommendations of improving occupational health policies in China were provided, based on the lessons learned from the NHS England.
