The Effect of Ethical Management and Positive Psychological Capital on Organizational Effectiveness in Hospitals (의료조직의 윤리경영과 긍정심리자본이 조직유효성에 미치는 영향)

  • Background: In this study, state-owned medical institutes, industrial accident hospitals, veteran hospitals, and private medical clinics including 16 university medical institutes in the Seoul metropolitan area were selected to examine the causality of ethical management, positive psychological capital, and organizational effectiveness. Methods: The study analyzed 1,056 valid questionnaires to which a total of 1,325 nurses, medical technicians, doctors, and administrative staff in 34 healthcare organizations answered over two months from June to August 2015. The study also utilized a 'structural equation model,' and a 'hierarchical linear model' to conduct the analysis. Results: It was first found that ethical leadership, ethical management systems, and organizational ethics values, which are the three factors of ethical management, had significant influence on organizational commitment, and behavior. These are the three factors of employee organizational effectiveness. Second, ethical management, ethical leadership, ethical management systems, and organizational ethics values had significant influence on positive psychological capital. Third, positive psychological capital had significant influence on organizational commitment, turnover intention, and organizational citizenship behavior. Positive psychological capital presented an indirect effect on the relationship between the ethical management and organizational effectiveness of employees. The effect of positive psychological capital consisting of self-efficacy, hope, resilience, and optimism was confirmed in the healthcare organizations. Fourth, in relations among ethical management variables, ethical leadership showed a significant impact on ethical management systems, which had significant impacts on organizational ethics values, which had significant impacts on ethical leadership.

Does computer-aided diagnostic endoscopy improve the detection of commonly missed polyps? A meta-analysis

  • Background/Aims: Colonoscopy is the gold standard diagnostic method for colorectal neoplasia, allowing detection and resection of adenomatous polyps; however, significant proportions of adenomas are missed. Computer-aided detection (CADe) systems in endoscopy are currently available to help identify lesions. Diminutive (≤5 mm) and nonpedunculated polyps are most commonly missed. This meta-analysis aimed to assess whether CADe systems can improve the real-time detection of these commonly missed lesions. Methods: A comprehensive literature search was performed. Randomized controlled trials evaluating CADe systems categorized by morphology and lesion size were included. The mean number of polyps and adenomas per patient was derived. Independent proportions and their differences were calculated using DerSimonian and Laird random-effects modeling. Results: Seven studies, including 2,595 CADe-assisted colonoscopies and 2,622 conventional colonoscopies, were analyzed. CADe-assisted colonoscopy demonstrated an 80% increase in the mean number of diminutive adenomas detected per patient compared with conventional colonoscopy (0.31 vs. 0.17; effect size, 0.13; 95% confidence interval [CI], 0.09-0.18); it also demonstrated a 91.7% increase in the mean number of nonpedunculated adenomas detected per patient (0.32 vs. 0.19; effect size, 0.05; 95% CI, 0.02-0.07). Conclusions: CADe-assisted endoscopy significantly improved the detection of most commonly missed adenomas. Although this method is a potentially exciting technology, limitations still apply to current data, prompting the need for further real-time studies.

Design and Development of an EHR Platform Based on Medical Informatics Standards (의료정보 표준에 기반한 EHR 플랫폼의 설계 및 개발)

  • As the ARRA enacted recently in the United States, the interest in EHR systems have been increased in the field of medical industry. The passage of the ARRA presents a program that provides incentives to office-based physicians and hospitals adapting the EHR systems to guarantee interoperability with various medical standards. Thanks to the incentive program, a great number of EHR systems have been developed and lots of office-based physicians and hospitals have adapted the EHR systems certified by CCHIT. Keeping pace with the rapid changes in the market of healthcare, some enterprises try to push in to the United States healthcare market based on the experience acquired by developing EHR systems for hospitals in Korea. However, the developed system must be customized because of the different medical environment between Korea and the United States. In this paper, therefore, we design and develop an integrated EHR platform to guarantee the interoperability between different medical information systems based on medical standard technologies. In the developed platform, an integrated system has been composed by integrating various basic techniques such as data transmission standards and its methods, medical standard terminologies and its usage, and knowledge management for medical decision-making support. Moreover, medical data can be processed electronically by adapting an HL7 interface engine and the terminologies for exchanging medical information and the standardization of medical information. We develop SeniCare, an EHR system for supporting ambulatory care of the office-based physicians, based on the platform, and we verify the usability of the platform by confirming whether SeniCare satisfies the criteria of "meaningful use" issued by CMS or not.

  • In recent years, because of the development of ubiquitous technology in healthcare research is actively progress. Especially, healthcare service area is change to home for the elderly or patients from hospital. The technology to identify residents in a home is crucial for smart home application services. However, existing researches for resident identification have several problems. In this case, residents are needed to attach of various sensors on their body. Also relating private life, it is difficult to apply to resident's environment. In this paper, we used constraint-free sensor and unconscious sensor to solve these problems and we limited using of sensor and indoor environment in the way of working economical price systems. The way of multi-resident identification using only these limited sensors, we selected elements of personal identifications and suggested the methods in giving the weight to apply these elements to systems. And we designed the SABA mechanism to tract their location and identify the residents. It mechanism can distinguish residents through the sensors located each space and can finally identify them by using the records of their behaviors occurred before. And we applied the mechanism designed for applications to approve this location tracking system. We verified to the location tracking system performance according to the scenario.

Design of a Personal-Led Health Data Management Framework Based on Distributed Ledger (분산 원장 기반의 개인 주도적 건강 데이터 관리 프레임워크 설계)

  • After the 4th industrial revolution, the healthcare industry is striving to find new business models through new technologies. Among them, blockchain technology is one of the technologies that have great interest in the healthcare industry. Most providers of personal health record systems have difficulty in securing marketability due to various problems. Therefore, they try to integrate blockchain technology to develop new systems and gain marketability. However, blockchain has limitations in solving the problems of the personal health record system. In this study, we have designed a personalized health data management framework that enables information subjects to acquire full ownership rights of individual's health data, based on distributed ledger technology. For the framework design, we refer to the structure of R3 Corda. It was designed with a different network structure than the existing blockchain systems so that the node can be operated on the personal user's mobile device. This allows information subjects to directly store and manage their own data and share data with authorized network members. Through the proposed system, the information utilization of the healthcare industry can be improved and the public health promotion and medical technology development can be realized.

  • Small and Medium sized hospitals are exposed to severe managerial environments recently. Around 8.0% of the hospitals are bankrupted every year. The adverse managerial environment does not only come from external factors such as patients' preference for larger hospitals, regulations on the medical charges; more serious problems come from the way the medium and small sized hospitals deal with those exogenous changes including lack of management skills, lack of change management skills, lack of managerial decision support systems, etc. This paper aims to support managers to make decisions regarding the exogenous changes. This paper can be interpreted as an attempt of a merge of the two techniques; BSC and system dynamics. Starting with a BSC system, the development of a system dynamics model can take advantages of the BSC information.

  • 본 논문에서는 현재 보건소에서 시행중인 오프라인 비만관리 시스템을 벗어난 온라인으로도 효율적으로 참여자를 늘릴 수 있는 스마트폰의 GPS와 Accelerometer 센서를 이용한 실내외 운동과 식단관리를 통한 효율적이 비만관리 시스템을 설명한다. 이 시스템은 직장인, 비만계층처럼 오프라인 보건소 프로그램에 참가하기 힘들거나 자신감 결여로 참여가 힘든 계층에게 보건소의 프로그램과 동일한 프로그램을 온라인으로 제공하여 비용을 최소로 줄이고 최대의 효율을 가능하게 한다.

Personalized Context-Aware System for Chronic Low Back Pain (만성 요통에 대한 맞춤형 상황 인지 시스템)

  • Treatment and management of chronic low back pain (CLBP) should be tailored to the patient's individual context. However, there are limited resources available in which to find and manage the causes and mechanisms for each patient. In this study, we designed and developed a personalized context awareness system that uses machine learning techniques to understand the relationship between a patient's lower back pain and the surrounding environment. A pilot study was conducted to verify the context awareness model. The performance of the lower back pain prediction model was successful enough to be practically usable. It was possible to use the information from the model to understand how the variables influence the occurrence of lower back pain.

Correlation of Unmet Healthcare Needs and Employment Status for a Population over 65 Years of Age (65세 이상 인구의 고용형태와 의료요구 미충족 경험률의 관련성)

  • The present study aimed to investigate the rate of unmet healthcare needs for elderly over the age of 65 years, as well as analyze the relevance between employment status and unmet healthcare needs due to financial reasons. With regard to the study method, a logistic regression analysis was performed to investigate the correlation between employment status and unmet healthcare needs due to financial reasons, targeting 5,528 subjects over the age of 65 years. The results showed that the rate of unmet healthcare needs was 18.9%, in which the rate of unmet healthcare needs due to financial reason was 8.1%. The rate of unmet health needs was higher for temporary workers(ORs=1.75) than for retirement workers. However, the rate of unmet healthcare needs caused by financial reasons was higher among day workers(ORs=1.92). In conclusion, in order to prevent unmet healthcare needs for senior Korean patients, it is necessary to not only improve the income security system for the elderly, but also improve the occupational form and level of income of these economically active citizens, considering the increase in average life expectancy. Moreover, it is also necessary to reinforce health insurance coverage systems for settling medical expenses.

  • In the era of the Fourth Industrial Revolution, how well the explosive information and data are handled and used is recognized as a problem directly related to the competitiveness of the industry. In particular, the introduction of artificial intelligence technology in the medical field can be said to have a great social impact on its use, and this research was conducted to understand the trends of artificial intelligence according to the range of use case. In this study, the application of artificial intelligence in the healthcare field is divided into four scopes, (1) hospital solutions, (2) personal health care, (3) insurance, and (4) new drug development. Based on various cases and trends in artificial intelligence technology, this study tried to give directions on how to develop artificial intelligence in Korea. In this study, we wanted to find out the use cases of artificial intelligence in various areas of healthcare industry and describe the latest issues in healthcare to help the overall medical industry. The development of artificial intelligence-based medical systems has made it easier to manage the chronic patients, increased the accuracy of cancer or disease diagnosis, and helped developing new drugs faster and more efficiently. Through this study, the medical industry we wanted to give a direction to the future development of artificial intelligence in Korea.
