Design of a Smart Application for Remote Diagnosis in Ubiquitous Computing Environment (유비쿼터스 컴퓨팅 환경에서 원격진단을 위한 스마트 응용의 설계)

  • With the rapid growth of the up-to-date smartphone and wireless network technologies, huge and various types of smart applications using these technologies are actively developed recently. Especially, multiplex types of smart applications using smartphone are developed and diffused with the rapid development of the ubiquitous sensor network technology using various sensors and the mobile computing technology that enables us to get network services at any time in any places. In this paper, we design a smart application that can accurately diagnose and process the current state of the local environment, objects, and persons remotely based on the context information such as local ecology, circumstances, medical or healthcare records and realtime sound or motion pictures using up-to-date samrtphone technology on the USN based mobile computing environment.

Measuring Out-of-pocket Payment, Catastrophic Health Expenditure and the Related Socioeconomic Inequality in Peru: A Comparison Between 2008 and 2017

  • Objectives: Describe out-of-pocket payment (OOP) and the proportion of Peruvian households with catastrophic health expenditure (CHE) and evaluate changes in socioeconomic inequalities in CHE between 2008 and 2017. Methods: We used data from the 2008 and 2017 National Household Surveys on Living and Poverty Conditions (ENAHO in Spanish), which are based on probabilistic stratified, multistage and independent sampling of areas. OOP was converted into constant dollars of 2017. A household with CHE was assumed when the proportion between OOP and payment capacity was ≥0.40. OOP was described by median and interquartile range while CHE was described by weighted proportions and 95% confidence intervals (CIs). To estimate the socioeconomic inequality in CHE we computed the Erreygers concentration index. Results: The median OOP reduced from 205.8 US dollars to 158.7 US dollars between 2008 and 2017. The proportion of CHE decreased from 4.9% (95% CI, 4.5 to 5.2) in 2008 to 3.7% (95% CI, 3.4 to 4.0) in 2017. Comparison of socioeconomic inequality of CHE showed no differences between 2008 and 2017, except for rural households in which CHE was less concentrated in richer households (p<0.05) and in households located on the rest of the coast, showing an increase in the concentration of CHE in richer households (p<0.05). Conclusions: Although OOP and CHE reduced between 2008 and 2017, there is still socioeconomic inequality in the burden of CHE across different subpopulations. To reverse this situation, access to health resources and health services should be promoted and guaranteed to all populations.

  • In recent years, there has been an increasing demand for healthcare services that can periodically monitor health status and maintain health by increasing the weight training population. However, injuries in the absence of trainer are increasing with the increase in the number of members in the fitness training center. Therefore, there is a need for a system that can periodically monitor the user's exercise state and assist in systematic and safe exercise even when the trainer is absent. In this study, we developed an auto training system that can effectively manage the exerciser while supporting the strength movement. The auto training system consists of a cable mount module, a control module, and a training information monitoring module. In order to evaluate performance of the developed system, the assistant force tests are carried out. Experimental results showed that the assistant force works well when the exerciser is out of power.

Development of Smart Stick Using Motion Sensing and GPS for Elderly Users' Safety (모션센서 및 GPS를 활용하는 고령 사용자 안전을 위한 스마트 지팡이 개발)

  • This paper describes the development of smart sticks as walking assistance for elderly people that incorporate motion sensing, hear-beat sensing, data processing, and communication functions. Our sticks communicate with users' smart phones and upon a detection of falling-off, an alarm is generated and propagated to multiple guardians registered in advance. In addition, the sticks provide smart healthcare functionalities for elderly people thus suggest an health platform that is empowered by various health-related informations. The heartbeat sensors and motion sensors mounted on the sticks enable various additional functions. Our smart sticks are designed to function as stable walking assistance as well as to support elderly people by providing useful services in the convergence with information technologies.

Effect of the Long-term Care Insurance Policy on Medical Expenditures for the Elderly (노인장기요양보험제도가 노인진료비에 미치는 영향)

  • Background: The purpose of this study was to analyze the medical expense change and influencing factors after introducing longterm care insurance system. The study period was 2 years before and after introduction of the system. Methods: We analyzed data collected from two divided group lived in Incheon. Four hundred and eighty-five elderly who received long-term care wage for one year were selected for experimental group. For control group, 1,940 elderly were selected by gender and age stratified random sampling. Difference-In-difference analyses was used for evaluating policy effectiveness. Also multiple regression analyses were conducted to identify the factors associated with total medical expenditures. The control variables were demographic variables, economic status, diseases, and medical examination variables. Results: Difference-in-difference analyses showed that total average medical expenses among long-term patients has decreased by 61.85%. Of these, the hospitalization expenses have decreased by 91.63% and the drug expenses have increased by 31.85%. Multiple regression analyses results showed that total average medical expenses among long-term patients have significantly decreased by 46.5% after introducing the long-term care insurance. The hospitalization expenses have significantly decreased by 148.5%, whereas the drug expenses have increased by 53.6%. And outpatient expenses have increased by 10.4%, but the differences were not statistically significant. Conclusion: The results showed that total medical expenses and hospitalization expenses have decreased after introducing the long-term care insurance. These results could support the opinion that the health insurance spending among long-term patients will be reduced gradually by long-term care insurance through changing medical demand.

Computed Tomography Spending and Utilization for Inpatients with Cerebral Infarction in South Korea

  • Background: Computed tomography (CT) is one of the most efficient diagnostic methods for stroke patients. The number of CT scanners in South Korea, however, is higher than in other countries, and may cause the overuse of this tool in healthcare. We aim to study the relationship between using CT and various patient and hospital characteristics among patients with cerebral infarction. Methods: We analyzed nationwide health insurance claims data for patients due to cerebral infarction during the second half of 2013 for up to 3 months. We performed multilevel analysis, including both inpatient and hospital-level variables, to determine how factors affect CT spending and utilization. Results: The data used in our study consisted of 17,046 hospitalizations at 583 hospitals. Inpatients who visited more than one hospital had higher CT utilization numbers and cost (number: ${\geq}3$: ${\beta}$ hospitals, 2.27; p < 0.05; 2 hospitals: ${\beta}$, 0.70; p < 0.05; cost: ${\geq}3$ hospitals: ${\beta}$, 251,108; p < 0.05; 2 hospitals: ${\beta}$, 77,299; p < 0.05). People who visited a general hospital had higher numbers and cost of CT utilization than people who visited a smaller hospital. Conclusion: Increased sharing of records and improved continuity of care between hospitals are needed to help curb the overuse of CT.

Analysis of the Korea Food and Drug Administration Adverse Drug Reaction Reports (식품의약품안전청 약물유해반응 보고자료 분석)

  • In order to minimize such adverse drug reactions, governments and international organs have been on the watch for them. Also in South Korea, a system has been established in order that adverse drug reactions may be reported to Korea Food and Drug Administration(KFDA). This study is to analyze drugs to cause adverse reactions, the adverse reactions and patients concerned on the authority of the data of Korea FDA, which is expected to be the preliminary data on preventable adverse reactions. This study was conducted on the 74,037 cases of adverse drug reactions reported to Korea FDA between January 2007 and June 2010. Fentanyl, iopromide and tramadol caused adverse reactions with high frequencies. Oseltamivir showed a high frequency between 2009 and 2010 due to the influence of the new influenza A. Also, OTC drugs accounted for approximately 5% of the adverse reactions. In 2009, adverse drug reactions remarkably increased (2,106 cases; 10.1%) in infants and children due to the new influenza-A(H1N1). The patients aged between 31 and 64 accounted for approximately 55% during the given period. There was no significant intergender difference. In relation to regions, the adverse reactions most frequently occurred in the gastrointestinal system and the integumentary system for three and half years. In addition to anticancer drugs and immunosuppressive drugs that are known to cause adverse reactions frequently, not a few of OTC drugs and external preparations caused such reactions. In particular, the drugs containing specific ingredients caused adverse reactions more frequently than others from 2007 until the first half of 2010. It is advisable for prescribers to acquaint themselves with such adverse reactions and to prescribe drugs other than them. They also have need to sensibly cope with adverse drug reactions just in case they have no substitute drugs. In addition, patients also need to be trained to understand possible adverse reactions in order that they can sensibly accommodate them or choose healthcare services. The results of this study are expected to be helpful to minimize adverse drug reactions.

  • EDI(Electronic Data Interchange) works by providing a collection of standard message formats and element dictionary in a simple way for businesses to exchange data via any electronic messaging service. Open-edi is electronic data interchange among autonomous parties using public standards and aiming towards interoperability over time, business sectors, information technology and data types. The number of Internet services using XML/EDI has grown rapidly since it is easily expansible and exchangeable. To use this service, the client does not have to install EDI S/W but only needs internet browser. Consequently, it became much easier and faster to handle the trading process in an office. eBusiness SML (extensible markup language) electronic data interchange. eXedi is the service that realizes B2B of XML/EDI. eXedi can be used easily in small and medium sized companies. Companies in any place can access to eXedi using the existing Internet connection. XML/EDI provides a standard framework to exchange different types of data -- for example, an invoice, healthcare claim, project status -- so that the information be it in a transaction, exchanged via an Application Program Interface (API), web automation, database portal, catalog, a workflow document or message can be searched, decoded, manipulated, and displayed consistently and correctly by first implementing EDI dictionaries and extending our vocabulary via on-line repositories to include our business language, rules and objects.

Accurate Step-Count Detection based on Recognition of Smartphone Hold Position (스마트폰의 소지위치 인지 기반의 정확한 보행수 검출 기법)

  • As the walking exercise is emphasized in personalized healthcare, numerous services demand walking information. Along with the propagation of smartphones nowadays, many step-counter applications have been released. But these applications are error-prone to abnormal movements such as simple shaking or vibrations; also, different step counts are shown when the phone is positioned in different locations of the body. In this paper, the proposed method accurately counts the steps regardless of the smartphone position by using an accelerometer and a proximity sensor. A threshold is set on each of the six positions to minimize the error of undetection and over-detection, and the cut-off section is set to eliminate any noise. The test results show that the six position type were successfully identified, and through a comparison experiment with the existing application, the proposed technique was verified as superior in terms of accuracy.

  • Due to recent development in wireless communication technologies and mobile information devices, the services on ubiquitous computing technology without time and place restriction have been spotlighted. Moreover, the interest of the Wireless Sensor Networks (WSNs) and context-awareness technologies have largely been escalating and their technologies utilization is active in the various applications such as healthcare, farm management and so on. However, the direct adaption of the existing context-awareness technique to the WSN technology has several drawbacks as follows. First, such systems waste precious energy of sensor nodes, due to unnecessary data transmissions. Secondly, since the existing work was designed to support only specific applications, it is required to implement a new context-awareness application for a specific purpose. Therefore, we, in this paper, propose a new real-time monitoring system based on context-awareness in WSNs. Our system not only enhances energy efficiency by reducing data transmissions by doing context-awareness on a sensor node, but also is scalable in terms of supporting new context-awareness functionalities through modularization.
