디지털헬스케어 시장과 O2O서비스 소비자분쟁 및 보호방안 (A Study on the Consumer Disputes and Protection Measures of the Digital Healthcare Market and O2O Service)

  • The O2O services in the healthcare sector have only been in full swing for about three years, and unlike existing O2O consumer goods, the scale and scope of the dispute are more complicated due to restrictions on medical treatment. In this study, O2O service platform operators and medical institutions' roles and responsibilities were redefined as a countermeasure for resolving disputes in healthcare O2O services and the laws for changing the transaction environment. A change in institutional mechanisms was proposed. This study looked at the types of consumer disputes related to healthcare O2O services as insufficient information problems, problems in the course of medical service implementation, problems with immunity provisions for platform operators, cancellations, and non-compliance with refunds. All the information generated during transactions in the healthcare sector was extensive in scale and included the most sensitive information among personal information, stressing the importance of ensuring security. The area that started in the O2O range before the medical institution visit also proposed a plan to establish a system for the delivery of proven information as a pre-medical person. The scale and growth will grow faster, given that consumers can experience the information they want anytime, anywhere they want. However, the platform broker's role, a link player, will become more important because consumers who use the service will have their first meeting with non-face-to-face product providers. On the other hand, service providers may have side effects of misleading consumers by providing false information or misleading consumers through exaggerated advertisements. The O2O service market is expected to expand beyond distribution and dining out to the entire industry. However, since it is challenging to check accurate statistics on the detailed market, various disputes and consumer protection measures will be required for each detailed market, and comprehensive leading solutions will be essential in the future.

  • Background: The aim of this study is to analyze determinants of welfare attitudes toward healthcare services in South Korea, using three main theories: self-interest (positive welfare attitudes if the policy fulfills people's personal interests), symbolic attitudes (positive welfare attitudes if the person is politically progressive or egalitarian), and sociotropic perception (positive welfare attitudes if the person experienced positive aggregated collective experiences of societal events and trends regarding the policy). Although the definition of the welfare attitude is rather ambiguous in literatures, in this investigation, we operationalize the concept as the 'willingness to pay higher taxes to improve the level of health care services for all people in Korea' which shows individuals' actional propensity. Methods: We used the health module from the International Social Survey Program 2011 for the analysis (N= 1,391). Five logistic regression models were built successively using two variables for each theory to measure key concepts of self-interest, symbolic attitudes, and sociotropic perceptions as independent variables. Results: The result showed self-interest and symbolic attitudes factors to be strong determinants of welfare attitudes towards healthcare services in South Korea, whereas sociotropic perception factors have inconsistent effects. Conclusion: For a more politically elaborated healthcare reform in South Korea and elsewhere, there needs to be further research on various dimensions and determinants of welfare attitudes to understand popular basis of welfare expansion, especially in the era of inequality.

  • 현재 많은 연구자들이 저비용, 저 전력을 필요로 하는 센서 네트워크를 활용하여 사람의 건강 상태를 실시간으로 모니터링 할 수 있는 u-Healthcare(ubiquitous Healthcare) 시스템을 구축하는데 심혈을 기울이고 있다. u-Healthcare 시스템은 센서 네트워크로부터 수집된 대량의 생체신호를 신속히 처리 분석하여 의료진에게 전달함으로써 시간과 장소에 관계없이 환자에게 적절한 의료 서비스를 제공할 수 있다. 기존의 u-Healthcare 시스템을 통해 환자의 건강상태 모니터링이 가능하지만 수집된 생체 신호를 신속히 분석하여 의학적으로 의미 있는 결과를 도출하는 것은 아직 어려운 상황이다. 본 논문에서는 대량의 생체신호를 고속으로 연산할 수 있는 그리드 컴퓨팅 기술을 센서 네트워크와 결합하여 환자의 생체신호를 측정하여 의학적으로 의미 있는 결과를 도출하고자 한다. 서로 다른 프로토콜을 사용하는 두 네트워크의 연동을 위해 게이트웨이가 필요하며, 게이트웨이에는 효과적인 u-Healthcare서비스 제공을 위해 센서 네트워크의 효율적 관리 및 제어, 생체신호 실시간 모니터링, 그리드 네트워크와 연계된 통신 서비스 등의 기능이 포함되어야 한다. 본 논문에서는 진보된 u-Healthcare 시스템을 구축하기 위하여 센서 네트워크와 그리드 네트워크를 유연하게 연동할 수 있는 센서그리드 게이트웨이를 설계하고 구현된 결과를 제시한다.

Factors Affecting Unmet Healthcare Needs of Working Married Immigrant Women in South Korea

  • Purpose: This study was conducted to identify the factors affecting on unmet healthcare needs of married immigrant women, especially who are working in South Korea. Methods: It is designed as a cross-sectional descriptive study. We analyzed data from 8,142 working married immigrant women to the 'National Survey of Multicultural Families 2015.' Based on Andersen's health behavior model, logistic regression was conducted to determine the predictors of unmet healthcare need. Results: The prevalence of unmet healthcare needs among the subjects was 11.6%. In multivariate analysis, significant predictors of unmet needs included existence of preschooler, country of origin, period of residence in predisposing factors, monthly household income, helpful social relationship, social discrimination, Korean proficiency, working hour per week in enabling factors, and self-rated health, experience of grief or desperation in need factors. Conclusion: The association between labor-related factors and unmet healthcare needs of marriage immigrant women currently working was found from nationally representative sample. Support policies for immigrant women working more than legally defined hours and having preschooler should be supplemented to reduce unmet healthcare needs. In addition, eradicating discrimination in workplace, enlarging social relationship, and developing culturally competent nursing services tailored to health problems caused by labor are needed.

의사들의 유헬스케어 서비스에 대한 인식과 사용의도 (Doctors' Perception and Intention of the U-healthcare Service)

  • 유헬스케어 서비스는 오랫동안 만성질환 관리에 대한 효과적인 대안으로 부각되고 있으나 아직까지 활성화되지 못하고 있다. 본 연구는 유헬스케어 서비스 도입에 있어 중요 의사결정자인 의사들의 유헬스케어 서비스에 대한 인식과 사용의도에 영향을 주는 요인에 대해 고찰해 보고자 한다. 이를 위해 유헬스케어 서비스 제공자인 의사들을 대상으로 설문조사를 실시하여 유헬스케어 서비스 사용의도에 따른 집단별 특성을 비교하고, 사용의도에 영향을 주는 요인을 고찰하기 위해 로지스틱 회귀분석을 실시하였다. 분석 결과 의사들의 유헬스케어 서비스 사용경험은 16.0%로 낮으나 사용의도는 70.1%로 높게 나타났다. 또한 유헬스케어 서비스 적용은 만성질환과 예방분야가 적절하다고 응답하였으며, 적용이 적합한 질환은 당뇨병과 고혈압등의 순으로 나타났다. 의사 특성에 따른 사용의도의 차이에 관한 로지스틱 분석 결과 유헬스케어 서비스 사용의도는 대학병원에 비해 비대학병원 의사들의 사용의도가 3.7배 높았다. 본 연구는 유헬스케어 서비스에 대한 의사들의 인식 그리고 사용의도에 대한 집단 간 차이를 파악함으로써 유헬스케어 서비스의 활성화와 효과적인 비즈니스 모델 개발에 기여할 것이다.

만성질환 노인의 경제적 부담, 물리적, 시간적 제약으로 인한 미충족 의료 요인 (Unmet Healthcare Needs due to the Economic, Physical, and Time Burden among Older People with Chronic Diseases)

  • Background: The purpose of this study is to analyze the factors affecting the unmet healthcare needs of older people with chronic diseases in Korea and provide a basic research report to strengthen their access to medical care. Methods: In the 2020 older people survey data, 8,182 older people aged 65 or older who were diagnosed with one or more chronic diseases were the final subjects of the study. According to Andersen's behavioral model used in unmet healthcare needs, independent variables were composed of predisposing factors, possible factors, and necessary factors, and whether or not unmet healthcare needs was set as dependent variable. Results: Of the older people with chronic diseases, 1.6% experienced unmet healthcare needs, of which 55.9% experienced unmet healthcare needs for reasons related to economic burden, 31.6% physical constraints, and 12.5% time constraints. As a result of the analysis, older people with chronic diseases were more likely to experience unmet healthcare needs if they were relatively low in age, low in education level, no spouse, low in household income, poor subjective health, complex chronic diseases, and functional restrictions. However, by major reasons for experiencing unmet healthcare needs, living in rural areas were more likely to experience unmet healthcare needs due to physical constraints, and those who participated in economic activities and who had were more likely to experience unmet healthcare needs due to time burden. These results were not derived when only unmet healthcare needs was set as the dependent variable. Conclusion: This study emphasizes the need for an approach by cause of unmet medical occurrence by suggesting that there are differences in influencing factors by reason for experiencing unmet healthcare needs.

구조방정식을 이용한 모바일 헬스케어 서비스에 대한 사용의도 영향요인 연구 (A Study on Influence Factors of Mobile Healthcare Service Using Structural Equation Modeling)

  • 본 연구는 스마트폰을 사용하는 일반인을 대상으로 구조방정식을 이용한 모바일 헬스케어서비스의 특성에 따른 사용의도 요인을 파악하기 위해 시도되었다. 자료수집은 2014년 03월 10부터 04월 08일까지 500명 대상으로 이루어 졌으며, 수집된 자료는 SPSS WIN 23.0과 AMOS 18.0 이용하여 Path analysis, Structural equation modeling analysis로 분석하였다. 연구결과 외부변수에 해당되는 서비스 품질, 혁신성은 지각된 유용성에 통계적으로 유의한 영향을 미쳤으며, 이 두 요인은 모바일헬스케어서비스 사용의도에 긍정적인 영향을 미친 것으로 나타났다. 이용편의성 또한 지각된 유용성에 유의한 영향을 미치고 있고 또한 콘텐츠 특성과 비용합리성은 이용 편의성에 유의한 영향을 미쳤다. 유용성 또한 사용의도에 직접적인영향을 미침으로 다양한 요인이 모바일헬스케어서비스의 사용에 영향을 주는 것을 알 수 있었다. 최근 증가하고 있는 의료비 상승의 대안으로 스마트폰을 이용한 모바일헬스케어에 대한 인식의 확산과 앱을 개발하는 업체의 다양한 시도가 있어야 하며, 정부는 건강보험의 적용 등 건강관리서비스에 대한 접근성 향상을 위한 정책적 노력이 필요하다. 또한 향후 모바일 헬스케어 대상에 대한 차별적인 서비스 개발과 이에 대한 사용의도를 확인하는 후속적인 연구가 지속적으로 이루어지기를 기대한다.

상급종합병원의 4대 중증질환 의료 서비스 품질과 보호받을 권리 및 존엄성 유지에 관한 연구 (A Study on the Quality of Healthcare Services for Four Critical Illnesses and the Maintenance of Right to Protection and Dignity in a Senior General Hospital)

  • Purpose: The unique nature of life-and-death healthcare services sets them apart from other service industries. While many studies exist on the relationship between healthcare services and customer satisfaction, most of them focus on mildly ill patients, ignoring the differences between critically ill and non-seriously ill patients. This study discusses the actual quality of healthcare services for patients who are facing life-threatening illnesses and are on life support, as well as their right to protection and dignity. Methods: The survey conducted to 149 patients with the four major illnesses: cancer, heart disease, brain disease and rare and incurable disease, those who have experiences with senior general hospitals. Results: The basic statistics of this study are adequate to represent the four major critical illnesses, and the reliability and validity of this study's hypotheses, which were measured by multiple items, were analyzed, and the internal consistency was judged to be high. In addition, it was found that the convergent validity was good and the discriminant validity was also secured. When examining the goodness of fit of the hypotheses, the SRMR, which is the standardized root mean square of residuals that measures the difference between the covariance matrix of the data variables and the theoretical covariance matrix structure of the model, met the optimal criteria. Conclusion: The academic implications of this study are differentiated from other studies by moving away from evaluating the quality of healthcare services for mildly ill patients and focusing on the rights and dignity of patients with life-threatening illnesses in four senior general hospitals. In terms of academic implications, this study enriches the depth of related studies by demonstrating the right to protection and dignity as a factor of patient-centeredness based on physical environment quality, interaction quality, and outcome quality, which are presented as sub-factors of healthcare quality. We found that the three quality factors classified by Brady and Cronin (2001) are optimized for healthcare quality assessment and management, and that the results of patients' interaction quality assessment can be used to provide a comprehensive quality rating for hospitals. Health and human rights are inextricably linked, so assessing the degree to which rights and dignity are protected can be a superior and more comprehensive measurement tool than traditional health level measures for healthcare organizations. Practical implications: Improving the quality of the physical environment and the quality of outcomes is an important challenge for hospital managers who attract patients with life and death conditions, but given the scale and economics of time, money, and human inputs, improving the quality of interactions and defining them as performance indicators in hospital quality management is an efficient way to create maximum value in the short term.

디지털 헬스케어 생태계 활성화를 위한 라이프로그 공통데이터 참조모델 (A Lifelog Common Data Reference Model for the Healthcare Ecosystem)

  • Healthcare lifelog, a personal record relating to disease treatment and healthcare, plays an important role in healthcare paradigm shifts in which medical and information technology converge. Healthcare services based on various healthcare lifelogs are being launched domestically by both large corporations and small and medium enterprises, however, they are being built on an individual platform that is dependent on each company. Therefore, the terms of lifelog data are different as well as the measurement specifications are not uniform. This study proposes a reference model for minimum common data required for sharing and utilization of healthcare lifelog. Literature study and expert survey derived 3 domain, 17 essential items, and 51 sub-items. The model provides definition, measurement data format, measurement method, and precautions for each detailed measurement item, and provides necessary guidelines for data and service design and construction for healthcare service. This study has its significance as a basic research supporting the activation of ecosystem by ensuring interoperability of data between heterogeneous healthcare devices linked to digital healthcare platform.

Copayment Policy Effects on Healthcare Spending and Utilization by Korean Lung Cancer Patients at End of Life: A Retrospective Cohort Design 2003-2012

  • Background: In Korea, the National Health Insurance program has initiated various copayment policies over a decade in order to alleviate patient financial burden. This study investigated healthcare spending and utilization in the last 12 months of life among patients who died with lung cancer by various copayment policy windows. Materials and Methods: We performed a retrospective cohort study using nationwide lung cancer health insurance claims data from 2002 to 2012. We used descriptive and multivariate methods to compare spending measured by total costs, payer costs, copayments, and utilization (measured by length of stay or outpatient days). Using 1,4417,380 individual health insurance claims (inpatients: 673,122, outpatients: 744,258), we obtained aggregated healthcare spending and utilization of 155,273 individual patient (131,494 inpatient and 103,855 outpatient) records. Results: National spending and utilization is growing, with a significant portion of inpatient healthcare spending and utilization occurring during the end-of-life period. Specifically, inpatients were more likely to have more spending and utilization as they got close to death. As coverage expanded, copayments decreased, but overall costs increased due to increased utilization. The trends were the same in both inpatient and outpatient services. Multivariate analysis confirmed the associations. Conclusions: We found evidence of the higher end of life healthcare spending and utilizations in lung cancer patients occurring as coverage expanded. The practice pattern within a hospital might be influenced by coverage policies. Health policy makers should consider initiating various health policies since these influence the long-term outcomes of service performance and overall healthcare spending and utilization.
