Construction of Multi-agent-based Distributed Framework for Application Services of u-Hospital Information Systems (u-병원 정보 시스템의 응용 서비스를 위한 멀티에이전트 기반 분산 프레임워크 구축)

  • As the hospital environment has been increasingly changed into a ubiquitous environment, the application services for the hospital environment are also faced with new requirements. In particular, the emergence of various mobile devices and the introduction of a wireless sensor network technology have accelerated the realization of the u-healthcare. The multi-agent paradigm has been introduced for satisfying both the integration of information and the various application scenarios established from various environments. This paper describes the software structure and u-applications for u-hospital information system based on ubiquitous environments. And it suggested the construction of a multi agent based distributed framework for supporting u-hospital information system. The suggested framework includes the JADE and distributed object group framework. And it implemented u-application services for supporting doctors and nurses, which provides the patient's health information and ward environment information. Especially, it is using a dynamic security mechanism on the security situation, which has not been emphasized in existing researches, and this paper shows the results for each user through the GUI.

  • In recent years, the developments of medical technology and emergency medical services have been changed to home from the hospital. In this regard, the researches for the prevention or early diagnosis have become actively. In particular, bio-signal monitoring is applied to a variety of u-healthcare application services. The proposed system in this paper is to provide a security technology to protect the medical information measured from the various sensors. Especially, bio-signal information is privacy-sensitive personal information that must be protected. We applied a two-dimensional code technology, QR code, for the protection and management. In the client side, it can analyze the QR code and confirm the results on devices. Finally, with this proposed platform, we show the results of application service to verify the creation and distribution of integrated image file between the bio-signal and medical image information.

  • In recent, hospital information systems are widely used to electronically record, manage and share the data collected in hospitals. Such systems have contributed greatly to improving the work efficiency in modern hospitals, however, the collected data concerning the patients should be appropriately processed and reused to provide the healthcare service providers with decision supports. Especially, this paper proposes the patient flow monitoring system for the operations management of the outpatient department for patients with chronic diseases, and discusses the related issues. The proposed system visualizes the standard process model extracted from the patient history data and various performance measures, and this enables the managers to evaluate and enhance the operations of the outpatient clinic. In this paper, the patient flow monitoring system is applied to the rheumatology clinic, and the prototype system optimized for I-pad is illustrated.

  • It is currently being used or being developed mobile fitness application systems associated with the most obese. These systems give or notify the user to reduce the weight, check your momentum to inform your calories burned. However, the accuracy is low because it provides and manages the common data without considering the individual characteristics or the environment of the user. In this paper analyzes the disadvantage of mobile fitness system. To solve this problem it presents the data sharing mechanism to alert you to the exercise equipment with the other users belonging to the BMI group, such as yourself. And it proposes the design of a mobile fitness system which is based on the data sharing mechanism.

The Effect of Organizational and Individual Capabilities on Job Satisfaction and Work Performance in Healthcare Institutions (의료기관에서 조직역량과 개인역량이 직무만족 및 업무수행에 미치는 영향)

  • This study examines the effects of the organizational and individual capabilities on employee' job satisfaction and work performance. The proposed research model and developed hypotheses were tested using structural equations modeling based on data collected from 243 care team members (doctor and nurse) in hospitals with more than 500 beds in South Korea. The results of study confirm the positive effects of organizational capabilities on job satisfaction and work performance, but individual capabilities did not significant effects on job satisfaction. In addition, there is a positive relationship between job satisfaction and work performance. The study provides valuable insights to hospital top management and administrators, institutions, and policy-makers about the importance of developing organizational and individual capabilities.

  • The need of Clinical Decision Support System (CDSS) in healthcare setup is increasing day by day. EHR Meaningful Use advocates CDSS as an important component of EHR/EMR systems. CDSS can be ranged from a simple to a very sophisticated system. The more complex CDSS systems need more attention to develop because of many reasons including its Knowledge Base (KB) structure/maintenance/evolution, inference capabilities and usability. Above all the KB maintenance and evolution is very crucial and important from the perspective of useful decision capabilities. Also the richness of the KB is important to cover the decision gaps handling a particular situation in the course of patient care. It cannot be expected from the clinicians to remember everything in regard to patient diagnosis and treatment. Similarly, it is also crucial for clinicians to keep themselves updated with the new research in the area. That is the reason they frequently require accessing to the online knowledge resources. Literature proved that online knowledge resources are capable providing answers to questions that might not be answered rely only on clinician wisdom and experience. This paper provides the theme of meaningful utilization of online knowledge resources in the context of diagnosis and treatment process for cancer patients more specifically Head and Neck cancer.

A Novel Transmission Scheme for Compressed Health Data Using ISO/IEEE11073-20601

  • In view of personal health and disease management based on cost effective healthcare services, there is a growing need for real-time monitoring services. The electrocardiogram (ECG) signal is one of the most important of health information and real-time monitoring of the ECG can provide an efficient way to cope with emergency situations, as well as assist in everyday health care. In this system, it is essential to continuously collect and transmit large amount of ECG data within a given time and provide maximum user convenience at the same time. When considering limited wireless capacity and unstable channel conditions, appropriate signal processing and transmission techniques such as compression are required. However, ISO/IEEE 11073 standards for interoperability between personal health devices cannot properly support compressed data transmission. Therefore, in the present study, the problems for handling compressed data are specified and new extended agent and manager are proposed to address the problems while maintaining compatibility with existing devices. Extended devices have two PM-stores enabling compression and a novel transmission scheme. A variety of compression techniques can be applied; in this paper, discrete cosine transformation (DCT) is used. And the priority of information after DCT compression enables new transmission techniques for performance improvement. The performance of the compressed signal and the original uncompressed signal transmitted over the noisy channel are compared in terms of percent root mean square difference (PRD) using our simulation results. Our transmission scheme shows a better performance and complies with 11073 standards.

  • A flood of information has occurred with the rise of the internet and digital devices in the fourth industrial revolution era. Every millisecond, massive amounts of structured and unstructured data are generated; smartphones, wearable devices, sensors, and self-driving cars are just a few examples of devices that currently generate massive amounts of data in our daily. Machine learning has been considered an approach to support and recognize patterns in data in many areas to provide a convenient way to other sectors, including the healthcare sector, government sector, banks, military sector, and more. However, the conventional machine learning model requires the data owner to upload their information to train the model in one central location to perform the model training. This classical model has caused data owners to worry about the risks of transferring private information because traditional machine learning is required to push their data to the cloud to process the model training. Furthermore, the training of machine learning and deep learning models requires massive computing resources. Thus, many researchers have jumped to a new model known as "Federated Learning". Federated learning is emerging to train Artificial Intelligence models over distributed clients, and it provides secure privacy information to the data owner. Hence, this paper implements Federated Averaging with a Deep Neural Network to classify the handwriting image and protect the sensitive data. Moreover, we compare the centralized machine learning model with federated averaging. The result shows the centralized machine learning model outperforms federated learning in terms of accuracy, but this classical model produces another risk, like privacy concern, due to the data being stored in the data center. The MNIST dataset was used in this experiment.

Analysis on the Assist Characteristics for the Knee Extension Motion of Lower Limb Orthosis Using Muscular Stiffness Force Feedback (근육 강성도 힘 피드백을 이용한 하지 보조기의 무릎 신전 운동 보조 특성 분석)

  • The lower limb orthosis with a pneumatic rubber actuator, which is intended for the assistance and the enhancement of muscular activities of lower limbs was developed in this study. Compared to other knee extension assistive devices being developed by other researchers, our device is designed especially for the elderly people and intended only for slight assistance so that the subjects can keep their muscular strength. For the effectiveness of system, muscular activities of major muscles in lower limbs during sit-to-stand (STS) and squat motion were measured and analyzed. Subjects were performed the STS and squat motion with and without lower limb orthosis. We made comparison muscular activities between with and without lower limb orthosis. Lower limb orthosis was controlled using muscular stiffness force feedback that is controlled by muscular activities of the measured muscle from force sensor. For analysis of muscular activities, electromyography of the subjects was measured during STS and squat motion, and these were measured using MP 150(BIOPAC Systems, Inc.). Muscles of interest were rectus femoris(RF), vastus lateralis(VL), vastus medialis(VM) and vastus intermedius(VI) muscles in lower limbs of the right side. A biodex dynamometer was used to measure the maximal concentric isokinetic strength of the knee extensors of wearing and not wearing orthosis on right side. The test were performed using the concentric isokinetic mode of test with the velocity set at 60°/s for muscles around the knee joints. The experimental result showed that muscular activities in lower limbs wearing orthosis using muscular stiffness force of a vastus medialis muscle was reduced and knee extension torque of an knee joint wearing lower limb orthosis was increased. With this, we confirmed the effectiveness of the developed lower limb orthosis.

Improved Access Control using Context-Aware Security Service (상황인식 보안 서비스를 이용한 개선된 접근제어)

  • As the ubiquitous technology has penetrated into almost every aspect of modern life, the research of the security technology to solve the weakness of security in the ubiquitous environment is received much attention. Because, however, today's security systems are usually based on the fixed rules, many security systems can not handle diverse situations in the ubiquitous environment appropriately. Although many existing researches on context aware security service are based on ACL (Access Control List) or RBAC (Role Based Access Control), they have an overhead in the management of security policy and can not manipulate unexpected situations. Therefore, in this paper, we propose a context-aware security service providing multiple authentications and authorization from a security level which is decided dynamically in a context-aware environment using FCM (Fuzzy C-Means) clustering algorithm and Fuzzy Decision Tree. We show proposed model can solve typical conflict problems of RBAC system due to the fixed rules and improve overhead problem in the security policy management. We expect to apply the proposed model to the various applications using contextual information of the user such as healthcare system, rescue systems, and so on.
