지방의료원 수술부 영역 및 면적 구성에 관한 조사·분석연구 - 순면적을 중심으로 (A Study on the Space Zoning and Area Composition of the Operation Department in Regional Public Hospitals - Focused on Net Floor Area)

  • Purpose: To establish guidelines for local public hospitals, the survey was conducted primarily on the current status of local public hospitals, focusing on the zone configuration and the spatial area of the operating department. Methods: 3 Methods have been used in this paper. 1) Analyzing drawings to derive the net floor area of each room. 2) A literature survey about composition of the zone by infection control. 3) Observation survey for user circulation analysis. Results: The operation department consists of the major, secondary, support and connected zones according to the user's actions. The spatial area composition ratio for each zone is major zone: 41.79%, secondary zone: 10.87%, support zone: 23.10%, and connected zone: 24.24%. Implications: This study is a basic research for guideline for the operation department in local public hospitals. In this study, zones configuration of the operation departments are classified in terms of user actions and room functions. In the future, it is required to establish criteria considering infection control and copper connection aspects.

중정이 계획된 병동부 바닥면적과 간호동선 평균거리 비교 연구 (A Comparative Study of Floor Area and Average Distance of Nurse Working of Planned Courtyard in Ward)

  • Purpose: The courtyard in the ward can light and ventilate in the central space and corridor of the ward. It is possible to improve the disadvantages of the existing double-corridor ward. Also, there is the advantage in that patients are able to contact with nature indoors and cause positive activities. So, the purpose of this study is quantitatively analyze the floor area and the average distance of nurse working of the courtyard in ward. Methods: The subjects are converted through the same criteria setting. And it investigate and analyze the influence of the courtyard in the ward through the analysis of the spatial composition. Result: The factors that affected the floor area increase are the planning courtyard, the corridor type and the dispersion of the medical rooms. The average distance of nurse working is more influenced by the nursing management system of the hospital than by the courtyard. So, It can improve according to the position and number of nurse station. Implications: It can be used as a primary data for courtyard plan in the ward for positive patient environment plan considering the floor area, average distance of nurse working.

Factors Influencing the Adoption of Cloud Computing in Healthcare Organizations: A Systematic Review

  • To analyze and compare the most influencing factors on cloud computing adoption (CCA) in the healthcare organization, a systematic review and meta-analyses of studies was performed using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) and Cochrane collaboration recommendations. A search of PubMed, ScienceDirect, Springer, Wiley Online, and Taylor & Francis Online digital libraries (From inception to January 19, 2022) was performed. A total of 17 studies met the defined studies' inclusion and exclusion criteria. Statistical significance difference favoring most influencing factors on CCA were (MD 0.76, 95% CI -1.48 - 3.01, p <0.00001, I2 = 90%), (MD 1.40, 95% CI -4.76 - 7.55, p < 0.00007, I2 = 97%) (MD 0.17, 95% CI -2.69 - 3.03, p<0.00001, I2 = 96%) for technology vs. organizational, technology vs. environmental and business vs. human factors, respectively. Organizational and environmental factors had greater impacts on CCA compared with technological factors. Moreover, business factors were more influential than the human factors.

  • Qualitative research methodology has been applied with increasing frequency in various fields, including in healthcare research, where quantitative research methodology has traditionally dominated, with an empirically driven approach involving statistical analysis. Drawing upon artifacts and verbal data collected from in-depth interviews or participatory observations, qualitative research examines the comprehensive experiences of research participants who have experienced salient yet unappreciated phenomena. In this study, we review 6 representative qualitative research methodologies in terms of their characteristics and analysis methods: consensual qualitative research, phenomenological research, qualitative case study, grounded theory, photovoice, and content analysis. We mainly focus on specific aspects of data analysis and the description of results, while also providing a brief overview of each methodology's philosophical background. Furthermore, since quantitative researchers have criticized qualitative research methodology for its perceived lack of validity, we examine various validation methods of qualitative research. This review article intends to assist researchers in employing an ideal qualitative research methodology and in reviewing and evaluating qualitative research with proper standards and criteria.

Effects of Metal and Metal Oxide Nanoparticles against Biofilm-Forming Bacteria: A Systematic Review

  • Biofilm formation by bacteria poses a significant challenge across diverse industries, displaying resilience against conventional antimicrobial agents. Nanoparticles emerge as a promising alternative for addressing biofilm-related issues. This review aims to assess the efficacy of metal and metal oxide nanoparticles in inhibiting or disrupting biofilm formation by various bacterial species. It delineates trends, identifies gaps, and outlines avenues for future research, emphasizing best practices and optimal nanoparticles for biofilm prevention and eradication. Additionally, it underscores the potential of nanoparticles as substitutes for traditional antibiotics in healthcare and combating antibiotic resistance. A systematic literature search, encompassing Web of Science, PubMed, and Google Scholar from 2015 to 2023, yielded 48 publications meeting the review criteria. These studies employed diverse methods to explore the antibacterial activity of nanoparticles against biofilm-forming bacteria strains. The implications of this study are profound, offering prospects for novel antimicrobial agents targeting biofilm-forming bacteria, often resistant to conventional antibiotics. In conclusion, nanoparticles present a promising frontier in countering biofilm-forming bacteria. This review delivers a structured analysis of current research, providing insights into the potential and challenges of nanoparticle utilization against biofilm-related challenges. While nanoparticles exhibit inherent antimicrobial properties with applications spanning healthcare, agriculture, and industries, the review acknowledges limitations such as the narrow scope of tested nanoparticles and the imperative need for extensive research on long-term toxicity and environmental impacts.

Can high serum anti-M${\ddot{u}}$llerian hormone levels predict the phenotypes of polycystic ovary syndrome (PCOS) and metabolic disturbances in PCOS patients?

  • Objective: To evaluate correlations between serum anti-M${\ddot{u}}$llerian hormone (AMH) levels, phenotypes of polycystic ovary syndrome (PCOS), obesity, and metabolic parameters in patients with PCOS. Methods: A total of 175 patients with PCOS were diagnosed according to the Rotterdam Consensus were included. Exclusion criteria were age over 40, FSH>25 mIU/mL, and 17a-OHP>1.5 ng/mL. The Phenotypes of PCOS were divided into a severe form (oligo-anovulation, ANOV/hyperandrogenism/polycystic ovary morphology [PCOM]; n=59) and a mild form without HA (ANOV/PCOM, n=105). The serum AMH levels were classified into 3 groups (<5 vs. 5-10 vs. >10 ng/mL). Obesity was defined as body mass index (BMI) ${\geq}25kg/m^2$ (n=34). Results: The mean age was $25.9{\pm}5.7$ year and mean AMH level was $10.1{\pm}5.4$ ng/mL. The BMI ($kg/m^2$) was higher in group 1 ($24.2{\pm}6.3$) than in group 2 ($21.9{\pm}4.3$, p=0.046) or group 3 ($21.6{\pm}3.3$, p=0.019). There was no difference among the three groups in age, menstrual interval, antral follicle counts, androgens, or other metabolic parameters. The obesity group showed significantly lower AMH ($7.7{\pm}3.9$ ng/mL vs. $10.7{\pm}5.6$ ng/mL), p=0.004) and low-density lipoprotein levels ($93.1{\pm}21.2$ mg/dL vs. $107.5{\pm}39.3$ mg/dL, p=0.031), and showed higher total T ($0.74{\pm}0.59$ L vs. $0.47{\pm}0.36$ ng/mL, p=0.001), free T ($2.01{\pm}1.9$ vs. $1.04{\pm}0.8$ pg/mL, p=0.0001), and free androgen index ($6.2{\pm}7.9$ vs. $3.5{\pm}3.0$, p=0.003). After controlling for age factors and BMI, the serum AMH levles did not show any significant correlations with other hormonal or metabolic parmeters. Conclusion: For PCOS patients under the age 40, serum AMH is not negatively correlated with age. High serum AMH levels can not predict the phenotype of PCOS and metabolic disturbances in PCOS patients in the non-obese group. Further study might be needed to define the relation more clearly.

의료기관의 녹색경영인증기준에 대한 유효성 평가 (The Evaluation of Effectiveness of Green Management Accreditation for Hospitals)

  • 본 연구의 목적은 녹색경영 평가기준을 실제 의료기관에 적용해 봄으로서 향후 의료기관의 녹색경영 인증에 필요한 전반적인 전략도출과 녹색경영평가제도의 개선방향을 제시하는 데 있다. 조사는 전국 44개 병원을 대상으로 실시하였으며, 평가도구는 2012년 보건 의료서비스분야 녹색경영 평가기준을 사용하였다. 분석결과 44개 병원중 11개 병원만이 녹색경영 인증을 받는 것으로 나타났다. 유형별로는 400병상 이상병원, 서울 경기지역병원(${\geq}400$), 사립병원(${\geq}400$)들이 상대적으로 높은 평가점수를 획득하였다. 인증점수의 평균차이의 분석결과 400병상 이상과 미만의 병원군 간 차이만이 통계적으로 유의하였다. 획득점수가 부족한 병원들의 인터뷰 결과, 경영진과 직원의 의식변화, 비전/전략/목표 수립, 비교적 적은 규모의 활동부터 추진, 장기계획 수립 등이 중요할 것으로 판단되었다. 녹색경영평가제도의 개선은 평가영역의 재설정, 배점과 가중치 조정, 인증등급의 신설, 가점항목 조절 등이 필요하다고 판단된다.

불필요한 재입원 비용 추정에 관한 연구 (Estimation of cost by unnecessary readmission of the tertiary hospitals)

  • 입원기간동안 적절하지 않거나 불필요한 처치로 발생될 수 있는 재입원관련 질지표는 질스크리닝 뿐만 아니라 입원환자의 적절하지 못한 지출을 파악할 수 있어서 중요하다. 이 연구는 예방할 수 있는 재입원으로 인해 발생되는 진료비 규모를 파악하고자 한다. 건강보험심사평가원의 2014년도 청구 데이터를 활용하여 종합병원이상 의료기관에 입원한 18세 이상 성인을 대상으로 분석하였다. 입원 유형을 1회 입원군과 재입원군으로 분류하였고, 재입원군은 다시 계획된 재입원과 계획되지 않은 재입원으로 분류하였다. 재입원 간격은 28일 이내로 설정하였다. 암질환, 동반질환, 전문재활 환자의 경우 1회 입원군보다 재입원군의 분포가 더 많았다. 계획되지 않은 재입원 유형 중 동일 기관, 동일 진료과목, 동일 질병군 입원의 경우 재입원에 지출된 총 진료비가 전체 재입원 진료비의 50%가량을 차지하였다. 불필요한 재입원으로 인해 발생되는 진료비는 2014년 1년간 9,990억원 정도였다. 재입원이 잦은 지역, 진료과목, 질병군 등의 패턴을 활용하여 재입원을 줄이려는 노력이 필요하다.

Development of an evidence-based clinical imaging diagnostic guideline for implant planning: Joint recommendations of the Korean Academy of Oral and Maxillofacial Radiology and National Evidence-based Healthcare Collaborating Agency

  • Purpose: This study was conducted to develop an evidence-based clinical imaging diagnostic guideline for implant planning, taking into account efficacy, benefits, and risks. Materials and Methods: The guideline development process employed the adaptation methodology used for Korean clinical imaging guidelines(K-CIG). Core databases(Ovid-Medline, Ovid-Embase, National Guideline Clearinghouse, Guideline International Network) and domestic databases (KoreaMed, KMbase, and KoMGI) were searched for guidelines. The retrieved articles were analyzed by 2 reviewers, and articles were selected using well-established inclusion criteria. Results: The search identified 294 articles, of which 3 were selected as relevant guidelines. Based on those 3 guidelines, 3 recommendations for implant planning were derived. Conclusion: We recommend radiography or cone-beam computed tomography (CBCT) scanning for individual patients judged to require a cross-sectional image after reading of a panoramic X-ray image and a conventional intraoral radiological image. Various steps should be taken to raise awareness of these recommendations among clinicians and the public, and K-CIG should be regularly reviewed and revised.

Model-Based Survival Estimates of Female Breast Cancer Data

  • Background: Statistical methods are very important to precisely measure breast cancer patient survival times for healthcare management. Previous studies considered basic statistics to measure survival times without incorporating statistical modeling strategies. The objective of this study was to develop a data-based statistical probability model from the female breast cancer patients' survival times by using the Bayesian approach to predict future inferences of survival times. Materials and Methods: A random sample of 500 female patients was selected from the Surveillance Epidemiology and End Results cancer registry database. For goodness of fit, the standard model building criteria were used. The Bayesian approach is used to obtain the predictive survival times from the data-based Exponentiated Exponential Model. Markov Chain Monte Carlo method was used to obtain the summary results for predictive inference. Results: The highest number of female breast cancer patients was found in California and the lowest in New Mexico. The majority of them were married. The mean (SD) age at diagnosis (in years) was 60.92 (14.92). The mean (SD) survival time (in months) for female patients was 90.33 (83.10). The Exponentiated Exponential Model found better fits for the female survival times compared to the Exponentiated Weibull Model. The Bayesian method is used to obtain predictive inference for future survival times. Conclusions: The findings with the proposed modeling strategy will assist healthcare researchers and providers to precisely predict future survival estimates as the recent growing challenges of analyzing healthcare data have created new demand for model-based survival estimates. The application of Bayesian will produce precise estimates of future survival times.
