Effect of sodium bicarbonate supplementation on two different performance indicators in sports: a systematic review with meta-analysis

  • [Purpose] Sodium bicarbonate shows ergogenic potential in physical exercise and sports activities, although there is no strong evidence which performance markers show the greatest benefit from this supplement. This study evaluated the effects of sodium bicarbonate supplementation on time trial performance and time to exhaustion in athletes and sports practitioners. [Methods] A systematic review was conducted using three databases, including 17 clinical trials. Among these clinical trials, 11 were considered eligible for the meta-analysis according to the criteria for the assessment of methodological quality using the PEDro Scale. Time to exhaustion was assessed in six studies, while time trial performance was evaluated in five studies. [Results] A significant beneficial effect of supplementation on time to exhaustion was found in a random effects model (1.48; 95% confidence interval [CI], 0.49 to 2.48). There was no significant effect of supplementation on time trial performance in a fixed effects model (slope = -0.75; 95% CI, -2.04 to 0.55) relative to a placebo group. [Conclusion] Sodium bicarbonate has the potential to improve sports performance in general, especially in terms of time to exhaustion.

  • A research on accident loss calculation for polyol process without safety management activities, and safety cost estimation using process risk assessment has been implemented. In order to estimate a magnitude of loss, accident scenarios were made by combining result made from HAZOP Study method with accident possibility analysis results implemented with FTA. Also effect assessment was implement for accident consequence of each scenario. And minimum possible loss cost has been calculated when safety investment do or not. Result from cost-benefit analysis was shown as approximately \335 billion(=USS44,000 billion), as cost after subtracting safety management cost from minimum possible loss cost.

  • 본 연구는 단축형 OHIP14 도구를 통하여 노인요양병원에 입원중인 ADL환자들을 대상으로 그들의 삶의 질올 측정한 결과 다음과 같은 결론을 얻었다. 1. 성별, 배우자, 틀니만족, 통증, 재활자기시도에 따른 OHIP14와의 관계에서는 틀니만족과 통증유무가 통계적으로 유의한 의미가 있는 것으로 나타났다. 2. 장애기간, 주관적 구강상태, 치료필요성, 경제적수준, 입원결정, 질병명, 결혼상태, 종교 및 학력에 따른 OHIP14와의 관계에서는 주관적 구강상태, 치료필요성, 입원결정, 질병 명, 결혼상태, 학력에 따라 통계적으로 유의한 의미가 있는 것으로 나타났다. 3. OHIP14에 유의한 영향을 미치는 요인으로서는 치료 필요성이 47.1%로 가장 높게 설명력을 나타내었으며, 처치 치아수, 주관적 구강상태, 배우자, 의도의 순으로 영향을 끼치는 것으로 나타났다.

  • The Japan National Committee for the Union for International Cancer Control (UICC) and UICC-Asia Regional Office (ARO) organized an international session as part of the official program of the 72nd Annual Meeting of the Japanese Cancer Association to discuss the topic "What is cost-effectiveness in cancer treatment?" Healthcare economics are an international concern and a key issue for the UICC. The presenters and participants discussed the question of how limited medical resources can be best used to support life, which is a question that applies to both developing and industrialized countries, given that cancer treatment is putting medical systems under increasing strain. The emergence of advanced yet hugely expensive drugs has prompted discussion on methodologies for Health Technology Assessment (HTA) that seek to quantify cost and effect. The session benefited from the participation of various stakeholders, including representatives of industry, government and academia and three speakers from the Republic of Korea, an Asian country where discussion on HTA methodologies is already advanced. In addition, the session was joined by a representative of National Institute for Health and Care Excellence (NICE) of the United Kingdom, which has pioneered the concept of cost-effectiveness in a medical context. The aim of the session was to advance and deepen understanding of the issue of cost-effectiveness as viewed from medical care systems in different regions.

Does the Use of Asthma-Controller Medication in Accordance with Guidelines Reduce the Incidence of Acute Exacerbations and Healthcare Costs?

  • Background: In asthma, consistent control of chronic airway inflammation is crucial, and the use of asthma-controller medication has been emphasized. Our purpose in this study is to compare the incidence of acute exacerbation and healthcare costs related to the use of asthma-controller medication. Methods: By using data collected by the National Health Insurance Review and Assessment Service, we compared one-year clinical outcomes and medical costs from July 2014 to June 2015 (follow-up period) between two groups of patients with asthma who received different prescriptions for recommended asthma-controller medication (inhaled corticosteroids or leukotriene receptor antagonists) at least once from July 2013 to June 2014 (assessment period). Results: There were 51,757 patients who satisfied our inclusion criteria. Among them, 13,702 patients (26.5%) were prescribed a recommended asthma-controller medication during the assessment period. In patients using a recommended asthma-controller medication, the frequency of acute exacerbations decreased in the follow-up period, from 2.7% to 1.1%. The total medical costs of the controller group decreased during the follow-up period compared to the assessment period, from $3,772,692 to $1,985,475. Only 50.9% of patients in the controller group used healthcare services in the follow-up period, and the use of asthma-controller medication decreased in the follow-up period. Conclusion: Overall, patients using a recommended asthma-controller medication showed decreased acute exacerbation and reduced total healthcare cost by half.

  • Our study from Feburary to March, 2003, were done in university office A and B, located in Seoul. This study was carried out to investigate the reduction of the airborne fungi before and after we activate the air cleaner which using ionization. And the method of assessment was done by the CAMNEA method. The result was as follows. 1. In the research office A, the concentration of the airborne fungi was 18(${\pm}$11.3) CFU/㎥ before the ionizing air cleaner system was turned on: whereas three days after this result the concentration decreased to less than 1 CFU/㎥. 2. In the laboratory office B, the concentration was 210.6(${\pm}$5.3) CFU/㎥ before using the air cleaner and was decreased to 32.2(t 10.3) CFU/㎥ after using the air cleaner. Nonylphenol과 Bisphenol A의 지렁이 독성시험 및 토양 중 생태 위해성평가 (Risk Assessment of Soil through Earthworm Toxicity Test of Nonylphenol and Bisphenol A)

  • Earthworm (Eisenia fetida) acute toxicity test was carried out and ecological risk assessment in soil was performed with national monitoring data. 14 day - $LC_{50}$ of nonylphenol and bisphenol A were 288.1 mg/kg and 90.1 mg/kg, respectively. And NOECs of nonylphenol and bisphenol A were 250 mg/kg and 50 mg/kg, respectively. Significant weight decrement was appeared at 70 mg/kg of bisphenol A, however, nonylphenol at concentrations tested did not severe adverse effect on the weight decrement. The environmental monitoring has been carrying out by NIER since 1999. Exposure levels of nonylphenol in soil were ND$\sim$10.55 $\mu$g/kg and those of bisphenol A were ND$\sim$15.50$\mu$g/kg in National Monitoring data which had been performed from 2000 to 2004. The measured soil exposure level was applied to evaluate the environmental risk assessment. The values of PNEC for bisphenol A and nonylphenol were determined as 0.5 mg/kg and 2.5 mg/kg, respectively using the safety factors which were suggested in EU and OECD. The values of HQ (PEC/PNEC) were determined to be below I for bisphenol A and nonylphenol when the maximum exposure levels for bispheol A (15.50$\mu$g/kg) and nonylphenol (10.55$\mu$g/kg) were applied. Conclusively, the environmental risk assessment of bisphenol A and nonylphenol was not critical in soil.

Methodologic Issues in Using Epidemiologic Studies for Quantitative Risk Assessment

  • Although animal studies have been used most often for quantitative risk assessment, it is generally recognized that well-conducted epidemiologic studies would provide the best basis for estimating human risk. However, there are several features related to the design and analysis of epidemiologic studies that frequently limit their usefulness for quantitating risks. The lack of accurate information on exposure in epidemiologic studies is perhaps the most frequently cited limitation of these studies for risk assessment. However. other features of epidemiologic study design, such as statistical power, length of follow-up, confounding, and effect modification, may also limit the inferences that can be drawn from these studies. Furthermore, even when the aforementioned limitations are overcome, substantial uncertainty exists concerning the choice of an appropriate statistical (or biologic) model for extrapolation beyond the range of exposures observed in a particular study. This paper focuses on presenting a review and discussion of the methodologic issues involved in using epidemiologic studies for risk assessment. This review concentrates on the use of retrospective, cohort, mortality studies of occupational groups for assessing cancer risk because this is the most common application of epidemiologic data for quantitative risk assessment (QRA). Epidemiologic data should not be viewed as a panacea for the problems inherent in using animal bioassay data for QRA. Rather, information that can be derived from epidemiologic and toxicologic studies complement one another, and both data sources need to be used to provide the best characterization of human risk.

생태위해성 평가를 위한 독성동태학 및 독성역학 모델 (Toxicokinetic and Toxicodynamic Models for Ecological Risk Assessment)

  • 오염물질에 대한 생태위해성평가(ecological risk assessment)를 위해서는 노출평가(exposure assessment)와 함께 생물영향에 대한 평가(effect assessment)를 수행해야 한다. 노출평가의 경우는 지화학적 과정에 대한 이해를 바탕으로 환경농도를 예측하기 위한 화학평형모델이나 다매체환경거동모델 등 다양한 평가 및 예측모델을 활용해 왔다. 이와 달리 생물영향평가는 실험실 조건에서 제한된 독성자료를 대상으로 외부노출농도에 기반한 농도-반응관계를 통계적 방법을 통해서 추정하는 '경험적 모델(empirical model)'에 주로 의존해 왔다. 최근에 와서 생체 내 잔류량을 기반으로 농도-시간-반응관계를 기술하고 예측하는 독성동태학 및 독성역학 모델(toxicokinetic-toxicodynamic model)과 같은 독성작용에 기반한 모델(processbased model)들이 개발되어 활용되고 있다. 본 논문에서는 여러 종류의 독성동태학 및 독성역학 모델을 소개하고, 이를 통계적 추론에 기반한 전통적인 독성학 모델과 비교하였다. 서로 다른 종류의 독성동태학 및 독성역학 모델로부터 도출된 노출농도-시간 -반응관계식을 비교하고, 동일 독성기작을 보이는 오염물질 그룹 내에서 미측정 오염물질의 독성을 예측할 수 있게 해주는 구조-활성관계(Quantitative Structure-Activity Relationship, QSAR) 모델을 여러 독성동태 및 독성역학모델로부터 유도하였다. 마지막으로 독성동태학 및 독성역학 파라미터를 추정하기 위한 실험계획을 제안하였고, 앞으로 독성동태학 및 독성역학 모델을 생태계 위해성평가에 활용하기 위해서 해결해야 될 연구과제를 검토하였다.

PNF 훈련 프로그램이 건강한 노인의 기능적 평가 측정과 보행에 미치는 효과 (Effect of a PNF Training Program on Functional Assessment Measures and Gait Parameters in Healthy Older Adults)

  • Purpose: The purpose of this study was to examine changes in functional assessment measures (FAMs) and spatiotemporal gait parameters (STGPs) in healthy older adults before and after a 6-week intervention of a proprioceptive neuromuscular facilitation (PNF) training program. Methods: Thirty healthy older adults (mean age: $73.37{\pm}1.21$ age range: 65-79) were randomly assigned either to an experimental group (participating in a 6-week intervention of PNF training) or a control group (only performing daily activities). Participants in the control group did not receive any training program. Performance was assessed by recording changes in the FAMs and STGPs using functional assessment tools and GAITRite. Results: Participants in the PNF group showed significantly improved functional assessment measures and increased stride length, cadence, velocity, and step width (p<0.05). No significant differences in FAMs and STGPs were found in the control group (p>0.05). Conclusion: Participation in a PNF training program improves FAMs and STGPs, thereby increasing the ability of healthy older adults to maintain dynamic balance during functional performance and gait. These findings support the use of PNF training programs as effective fall-prevention programs for the elderly.
