Knowledge Based Recommender System for Disease Diagnostic and Treatment Using Adaptive Fuzzy-Blocks

  • Identifying clinical pathways for disease diagnosis and treatment process recommendations are seriously decision-intensive tasks for health care practitioners. It requires them to rely on their expertise and experience to analyze various categories of health parameters from a health record to arrive at a decision in order to provide an accurate diagnosis and treatment recommendations to the end user (patient). Technological adaptation in the area of medical diagnosis using AI is dispensable; using expert systems to assist health care practitioners in decision-making is becoming increasingly popular. Our work architects a novel knowledge-based recommender system model, an expert system that can bring adaptability and transparency in usage, provide in-depth analysis of a patient's medical record, and prescribe diagnostic results and treatment process recommendations to them. The proposed system uses a set of parallel discrete fuzzy rule-based classifier systems, with each of them providing recommended sub-outcomes of discrete medical conditions. A novel knowledge-based combiner unit extracts significant relationships between the sub-outcomes of discrete fuzzy rule-based classifier systems to provide holistic outcomes and solutions for clinical decision support. The work establishes a model to address disease diagnosis and treatment recommendations for primary lung disease issues. In this paper, we provide some samples to demonstrate the usage of the system, and the results from the system show excellent correlation with expert assessments.

  • Background: The recent revision of South Korea's Mental Health Law emphasizes the role of the Mental Health Review Board. For this study, we examined the current status of continuing hospitalization judgement in Mental Health Review Board at the national level and aimed to determine the directions of improvement. Methods: Using a qualitative case study as the research method, we interviewed 30 Mental Health Review Board members and analyzed the results. Results: Each municipality had very different continuing hospitalization judgement methods. In our systematic review, which consisted of document inspection, we identified reliability problems due to limitations in Mental Health Review Board's operating systems, discharge orders, etc. Additionally, continuing hospitalization judgement needs to improve the objectivity, fairness, and effectiveness of their screening examinations. Conclusion: Based on the results of this study, we suggest policy proposals to improve these systems, such as standardizing examination processes, strengthening on-site inspections, increasing the independence and neutrality of judgment in Mental Health Review Board, building community mental health infrastructures, and establishing integrated management systems.

  • In the process of remarkable progress in the medical and technical field and activating the role of technology in health care services and applications, and since the safety of medical data and its protection from security violations plays a major role in assessing the security of health facilities and the safety of medical servers Thus, it is necessary to know the cyber vulnerabilities in health information systems and other related services to prevent and address them in addition to obtaining the best solutions and practices to reach a high level of cybersecurity against attackers, especially due to the digital transformation of health care systems and the rest of the dealings. This research is about what cyberattacks are and the purpose of them, in addition to the methods of penetration. Then challenges, solutions and some of the security issues will be discussed in general, and a special highlight will be given to obtaining a safe infrastructure to enjoy safe systems in return.

  • The National Nutrition Survey in Japan (NNS-J) started in 1945 and has provided information on dietary intake and health status of Japanese citizens to the public and policymakers for more than half a century. We summarized several relevant issues on the survey in this report : the current framework of the NNS-J in accordance with the Nutrition Improvement Law, utilization of the survey for nutrition and health policy in Japan, the Health Promotion Law recently enacted in 2003, the national plan for health promotion and disease prevention (Health Japan 21), and possible measures to improve the survey systems under the new law. We also mentioned implementation structures of regional health and nutrition surveys, because the Health Promotion Law designates an active role of local governments on promoting health for their citizens, which will enhance the needs for appropriate assessment of health and nutrition conditions in each community as well as the monitoring at the national level.   • 한국 노인을 대상으로 외국에서 비교적 높은 신뢰도와 타당도가 입증된 Oral Health Impact Profile 14(OHIP 14)를 한국의 문화적 특성이나 사고방식에 맞게 번역하여 강릉시에 거주하는 노인 만 65세 이상 668명을 대상으로 2005년 1월부터 2월까지 2개월간 OHIP 14 지표의 신뢰도와 타당도를 검증(Kappa index, Cronbach's alpha Correlation & Spearman ranked correlation coefficient)하고, OHIP 14와(구강)건강인식도, 구강건강상태, 전신건강 상태와의 차이를 검증(Kruskal Wallis test, Mann-Whitney U-test)함으로써, 향후 우리나라 노인 구강건강문제를 포괄적으로 파악하기 위한 지표로 활용할 수 있는 지를 검토하고자 하였다. 1. (구강)건강인식도와 OHIP 14는 전신건강인식도, 구강건강인식도, 치과치료필요도, 구강건강만족도, 통증에 대한 시각적 정도, 틀니만족도, 구강건강상태와 OHIP 14는 무치악 여부, 현존 자연치아의 개수, 치아치료필요, 전신건강상태와 OHIP 14는 질병의 개수, 치매, 눈, 귀, 호흡기, 심혈관, 소화기, 비뇨생식기, 근골격계, 신경계, 심리상태가 상관관계가 있는 것으로 나타났다. 2. (구강)건강인식도에 따른 OHIP 14는 전신건강인식도, 구강건강인식도, 치과치료필요도, 구강건강만족도, 통증에 대한 시각적 정도, 틀니만족도, 구강건강상태에 따른 OHIP 14는 무치악 여부, 치아치료필요, 현존 자연치아의 개수, 전신건강상태에 따른 OHIP 14는 치매, 귀, 호흡기, 심혈관, 소화기, 근골격계, 신경계, 심리상태에서 현저한 차이를 보였다. 본 연구는 일부 노인에서 임상지표를 이용한 OHIP 14에서 (구강)건강인식도, 구강건강상태, 전신건강상태가 OHIP 14와의 타당도와 연관성이 있다는 사실을 일부에서 확인할 수 있었다. 이상의 결과로 향후 한국노인의 구강건강문제를 포괄적으로 파악하는데 전통적인 구강건강 상태검사, 전신건강상태검사와 더불어 '구강건강 관련 삶의 질' 지표인 OHIP 14를 활용할 수 있을 것으로 사료된다.

미생물 간이 검사법 실시에 따른 학교급식소의 위생 및 건강관리 개선 효과 (Effects of Microbe Inspections in School Food Service Systems)

  • Objectives: The purpose of this study was to analyze the effects of microbe inspections on the hygienic improvement of school food service systems. Methods: Thirty-three school food service systems in Incheon metropolitan city participated in the study from Sep. 2004 to Apr. 2005. Major items of microbe inspections were cutting boards, knives, kitchen towels, and kitchen staff's hands. The Rodac plate and hand plate were used to measure the surface contamination level of germs such as Staphylococcus aureus, Vibrio species, Salmonella species and E. coli. Results: This study compared the results of the inspections in Sep. 2004 and Apr. 2005. The surface contamination level of Staphylococcus aureus and Vibrio species on the knives and kitchen towels had significantly improved. However, the surface contamination level of E. coli on the hands of the kitchen staff had worsened. Conclusions: This study showed that microbe inspections could control the hygienic level of the school food service systems. In the future, microbe inspections should be actively used to improve sanitary conditions in the school lunch system.

Exploring the Feasibility of Differentiating IEEE 802.15.4 Networks to Support Health-Care Systems

  • IEEE 802.15.4 networks are a feasible platform candidate for connecting all health-care-related equipment dispersed across a hospital room to collect critical time-sensitive data about patient health state, such as the heart rate and blood pressure. To meet the quality of service requirements of health-care systems, this paper proposes a multi-priority queue system that differentiates between various types of frames. The effect of the proposed system on the average delay and throughput is explored herein. By employing different contention window parameters, as in IEEE 802.11e, this multi-queue system prioritizes frames on the basis of priority classes. Performance under both saturated and unsaturated traffic conditions was evaluated using a novel analytical model that comprehensively integrates two legacy models for 802.15.4 and 802.11e. To improve the accuracy, our model also accommodates the transmission retries and deferment algorithms that significantly affect the performance of IEEE 802.15.4. The multi-queue scheme is predicted to separate the average delay and throughput of two different classes by up to 48.4% and 46%, respectively, without wasting bandwidth. These outcomes imply that the multi-queue system should be employed in health-care systems for prompt allocation of synchronous channels and faster delivery of urgent information. The simulation results validate these model's predictions with a maximum deviation of 7.6%.

한국의 진료비 지불제도 현황과 혁신 과제 (A Review of Healthcare Provider Payment System in Korea)

  • This study aims to propose the implementation of innovative payment models in Korea in order to promote the financial sustainability of the national health insurance system by reviewing the current status of the payment system in Korea and examining other countries' experiences with various innovative payment models. Korea primarily uses a fee-for-service payment system and additionally uses various payment systems such as case payment, per diem, and pay-for-performance. However, each payment system has its limitations. Many OECD (Organization for Economic Cooperation and Development) countries have pointed out the limitations of existing payment systems and have been attempting various innovative payment models (e.g., add-on payment, bundled payment, and population-based payment). Therefore, it is essential for Korea to consider innovative payment models, such as a mixed payment model that takes into account the strengths and weaknesses of each payment system, and to design and pilot these models. This process requires stakeholders to work together to build a social consensus on the implementation of innovative payment systems and to refine legal and systematic aspects, develop an integrated health information system, and establish dedicated organizations and committees. These efforts towards innovative payment models will contribute to developing a sustainable health insurance system that ensures the public's health and well-being in Korea.

신 공중보건과 국가공중보건체계 (New Public Health and National Public Health System)

  • The New Public Health(NPH) is a comprehensive approach to protecting and promoting the health status of each individual member and society as a whole. NHP is not so much a philosophy to broaden the understanding of public health as it is an action plan to address current public health system. This paper's objectives include increasing public and professional awareness of the significant changes in the national public health systems of developed countries and contributing to more effective delivery of public health services in Korea. This paper reviews articles and documents concerning NPH and the public health system, and outlines of the achievements in developed countries since NPH movement began. These include the change in the definition and function of public health, expansion of public health networks, strengthening of public health policy, reorientation of public health delivery systems, promotion of workforce capacity, and the implementation of evidence-based management. To overcome the challenges facing the public health system of Korea, we must prioritize the value of population-based approach, expand the notion of a public health system to encompass all sectors that can influence health, promote a "Health in All Policies" approach, focus on an evidence-based health policy and program, develop core competencies for public health workers, and establish performance standards for public health organizations based on the core functions of public health.

Preventing Intra-hospital Infection and Transmission of Coronavirus Disease 2019 in Health-care Workers

  • Coronavirus disease 2019 poses an occupational health risk to health-care workers. Several thousand health-care workers have already been infected, mainly in China. Preventing intra-hospital transmission of the communicable disease is therefore a priority. Based on the Systems Engineering Initiative for Patient Safety model, the strategies and measures to protect health-care workers in an acute tertiary hospital are described along the domains of work task, technologies and tools, work environmental factors, and organizational conditions. The principle of zero occupational infection remains an achievable goal that all health-care systems need to strive for in the face of a potential pandemic.
