베일리영유아발달검사 제3판(Bayley-III)의 미국 규준 적용의 문제: 미숙아 집단을 대상으로 (The Clinical Utility of Korean Bayley Scales of Infant and Toddler Development-III - Focusing on using of the US norm -)

  • 베일리영유아발달검사 제3판(Bayley-III)의 미국 규준이 영유아의 발달수행을 과대추정하고 발달지연율을 과소추정 한다는 논란이 있어서, 본 연구는 발달지연의 고위험군인 미숙아를 대상으로 미국 규준 적용시 Bayley-III의 임상적 유용성을 살펴보고자 하였다. 본 연구는 생후 1개월부터 24개월(교정원령)의 미숙아(재태기간 37주 미만)와 만삭아 93명을 대상으로 Bayley-III와 한국형베일리영유아발달검사 2판(K-BSID-II)을 실시하였다. 연구결과는 다음과 같다. 첫째, 미숙아/만삭아 각 93명을 대상으로 혼합설계 분산분석을 실시한 결과 재태기간(경증미숙아, 중증미숙아, 극중증미숙아, 만삭아)에 따른 Bayley-III의 발달지수 및 척도점수 간에 유의미한 차이가 있었다. 특히 경증미숙아 집단 내에서는 발달영역(인지, 언어, 운동)에 따라 유의미한 차이가 있는 것으로 나타냈다. 둘째, Bayley-III와 K-BSID-II을 모두 실시한 미숙아(n=79)를 대상으로 환산점수 평균 간의 차이를 살펴본 결과, Bayley-III의 발달지수는 K-BSID-II의 발달지수에 비해 유의미하게 높은 것으로 나타났으며 발달지연률은 크게 감소하는 것으로 나타났다. 마지막으로 ROC 분석을 통해 발달지연 변별에 대한 Bayley-III의 민감도와 특이도를 분석하고 Bayley-III의 임상적 절단점을 탐색한 결과, Bayley-III의 하위 척도 모두 우수한 수준의 변별정확도를 나타냈으나, 미국 규준 적용시 절단점은 더 높게 나타났다. 본 연구는 미국 규준을 사용하여 Bayley-III를 임상 현장에 적용할 때 점수 해석의 주의를 알리는 실증적인 자료로서, 한국 규준 마련시 표집 문제와 관련된 논의를 제공하였다.

A Multicenter Study to Identify the Respiratory Pathogens Associated with Exacerbation of Chronic Obstructive Pulmonary Disease in Korea

  • Background: Although respiratory tract infection is one of the most important factors triggering acute exacerbation of chronic obstructive pulmonary disease (AE-COPD), limited data are available to suggest an epidemiologic pattern of microbiology in South Korea. Methods: A multicenter observational study was conducted between January 2015 and December 2018 across 28 hospitals in South Korea. Adult patients with moderate-to-severe acute exacerbations of COPD were eligible to participate in the present study. The participants underwent all conventional tests to identify etiology of microbial pathogenesis. The primary outcome was the percentage of different microbiological pathogens causing AE-COPD. A comparative microbiological analysis of the patients with overlapping asthma-COPD (ACO) and pure COPD was performed. Results: We included 1,186 patients with AE-COPD. Patients with pure COPD constituted 87.9% and those with ACO accounted for 12.1%. Nearly half of the patients used an inhaled corticosteroid-containing regimen and one-fifth used systemic corticosteroids. Respiratory pathogens were found in 55.3% of all such patients. Bacteria and viruses were detected in 33% and 33.2%, respectively. Bacterial and viral coinfections were found in 10.9%. The most frequently detected bacteria were Pseudomonas aeruginosa (9.8%), and the most frequently detected virus was influenza A (10.4%). Multiple bacterial infections were more likely to appear in ACO than in pure COPD (8.3% vs. 3.6%, p=0.016). Conclusion: Distinct microbiological patterns were identified in patients with moderate-to-severe AE-COPD in South Korea. These findings may improve evidence-based management of patients with AE-COPD and represent the basis for further studies investigating infectious pathogens in patients with COPD.

사진분석법을 활용한 경북 일부지역 대학생의 식생활 실태 분석 (Diet status of college students evaluated by applying the photographic analysis method)

  • 지금의 3040세대를 비롯한 젊은 층이 부모보다 노화가 빨리 진행되는 가속노화 우려가 지적된 시점에서 3040세대 진입을 앞둔 대학생 시기부터 올바른 생활태도 및 건강습관 배양과 건강증진을 위한 구체적인 전략이 요구된다. 이에 대학생들의 식생활 실태를 분석하여 올바른 식습관 형성을 위한 방안 마련 시 활용할 수 있는 기초자료를 제공하고자 하였다. 본 연구는 대구가톨릭대학교 재학생 67명을 대상으로 사진활용 3일 식사기록을 통한 식사조사를 활용하여 식생활실태를 분석하였다. 조사대상자는 남자 37.3%, 여자62.7%로 평균 연령은 23.25 ± 2.05세, 평균 신장은 165.17 ± 9.04 cm, 평균 체중은 65.45 ± 13.69 kg, 평균 체질량지수는 23.85 ± 3.92 kg/m2이었다. 조사대상자 전체의 73.6%가 아침을 결식한다고 나타났으며, 학교식당보다는 주로 배달이나 외부 음식점, 편의점을 이용하는 것으로 조사되었다. 식품군 섭취패턴과 권장섭취 횟수에 따른 식품섭취 평가 결과, 과일군과 유제품군을 섭취하지 않은 대상자 (grain, meat, vegetable, fruit, dairy and sweet [GMVFDS] = 111001)가 38.3%로 가장 많았으며, 대상자 모두 모든 식품군의 섭취가 부족한 것으로 나타났다. 채소류와 과일류에서 여자가 남자보다 섭취비율이 높아 유의한 (p < 0.05) 차이를 나타냈다. 영양소 섭취 상태를 분석한 결과, 에너지 필요 추정량에 비해 적게 섭취하고 있었다. 중복분석 (%redundancy analysis, %RDA)으로 분석한 결과, 인은 권장량 이상 섭취, 그 외 모든 영양소는 권장량 이하로 섭취하는 것으로 나타났다. 남자가 여자보다 인 (p < 0.01), 철 (p < 0.001), 나트륨 (p < 0.05)의 섭취비율이 높았고, 여자가 남자보다 식이섬유 (p < 0.05)의 섭취 비율이 높았다. 식품섭취횟수와 영양소섭취상태에서 비만도에 따른 차이는 없는 것으로 나타났다. 22가지 식품분류에 따른 식품섭취평가 결과에서도 비만도에 따른 차이는 없었으며, 곡류에서는 남자가 일반곡류 (p < 0.05), 여자가 혼합곡류 (p < 0.01)의 섭취가 높았고, 단백질식품류에서는 남자는 고기류 (p < 0.01), 여자는 달걀류 (p < 0.05)와 콩류 (p < 0.05)의 섭취가 높았다. 과일류에서는 남자가 주스류의 섭취비율이 높아 유의한 (p < 0.05) 차이를 나타냈다. 이상의 결과에서 대학생들의 영양소 섭취상태가 불량하고 식품 기호도에 대한 차이가 뚜렷한 것을 확인함에 따라 식생활 개선 프로그램과 함께 식품 기호도를 고려한 메뉴 제공 등을 통해 대학생들의 식태도 변화를 유도하고 스스로 실천하도록 대학교 급식에 대한 관심과 적극적인 지원이 필요할 것으로 사료된다.

토지피복변화에 따른 금강 상류 댐 유역 산림 경관의 구조적 변화 분석 (Analysis of Spatial Changes in the Forest Landscape of the Upper Reaches of Guem River Dam Basin according to Land Cover Change)

  • 유역 내 산림은 생태계 유지에 있어 중요한 역할을 맡고 있으며 생태네트워크 체계를 구성하는 주요 기반 환경이다. 그러나 지난 수십여 년간 행해진 무분별한 개발사업으로 인해 산림 파편화 및 토지이용 변화가 가속화되었으며 본래의 기능을 상실하게 되었다. 산림 생태계를 파악하는 데 있어 산림의 구조적 패턴은 생태적 과정과 기능에 직접적인 영향을 미치기 때문에 변화패턴을 파악하고 분석하는 것은 중요한 인자라 할 수 있다. 이에 본 연구는 금강 상류 댐 유역을 대상으로 FRAGSTATS 모델을 통해 시계열적인 토지피복변화에 따른 산림 경관의 구조적 변화를 분석하였다. 토지피복 변화탐지를 통한 금강 상류 댐 유역 내 토지피복변화는 1980년대부터 2010년대까지 산림 33.12km2(0.62%), 시가화건조지역 67.26km2 (1.26%) 증가하였고 농업지역 148.25km2(2.79%) 감소하였다. 유역 내 산림 경관분석결과 No sampling 분석에서는 경관백분율(PLAND), 면적가중근접지수(CONTIG_AM), 평균 중심지 면적(CORE_MN), 인접지수(PLADJ)가 증가하였고 패치수(NP), 경관형태지수(LSI), 응집지수(COHESION)가 감소하였다. Moving window 분석을 통해 구조적 변화패턴을 파악한 결과, 경상북도 상주시, 충청북도 보은군, 전라북도 진안군 내 산림 경관은 상대적으로 잘 보전되어 있었으나 충청북도 옥천군, 영동군 그리고 충청남도 금산군 사이의 경계부와 전라북도 무주군과 장수군 인접 지역의 산림 경관에서는 파편화가 진행되고 있었다. 결과를 토대로 추후 해당 지역의 산림 관리전략 수립 시 파편화 지역을 대상으로 조림사업을 수립할 필요가 있을 것으로 사료된다. 본 연구를 통해 산림 경관의 파편화가 예상되는 지역을 도출할 수 있었으며, 유역 산림의 건전성 평가 및 관리계획 수립을 위한 기초자료로써 활용될 가능성을 기대할 수 있다.

노인 보행 시 하지 근 활동 양상과 관절의 안정성이 낙상에 미치는 영향 -후향성 연구- (Effects of Muscle Activation Pattern and Stability of the Lower Extremity's Joint on Falls in the Elderly Walking -Retrospective Approach-)

  • 이 연구의 목적은 여성 노인들을 대상으로 낙상자와 비낙상자 간 보행 시 하지의 국부적 안정성과 근 활동 양상을 후향성 접근을 통해 규명하고자 했다. 이를 위해 보행 시 낙상 경험이 있는 20명과 낙상 경험이 없는 20명이 선정되었다. 트레이드밀 보행 시 3차원 운동 캡쳐 시스템과 무선 근전도 장비를 이용해 운동학적 좌표와 근전도 신호를 수집했다. 운동좌표를 이용해 무릎과 발목 관절의 3차원 각 변위를 산출했으며, 이들의 국부적 안정성을 Lyapunov 지수를 통해 계산했다. 근 활성도는 근전도 신호를 이용해 대퇴직근과 대퇴이두근, 전경골근과 비복근을 살펴봤으며, 또한 이들의 동시 수축 지수를 정량화했다. 연구 결과 무릎 관절의 내·외전 움직임의 국부적 불안정성은 비낙상자 집단이 낙상자 집단보다 컸으며(p<.05), 전경골근의 근활성도는 낙상자 집단이 비낙상자 집단보다 적게 나타났다(p<.05). 또한 비복근과 전경골근의 동시 수축 지수는 낙상자 집단이 비낙상자 집단보다 컸다(p<.05). 그 밖에 낙상자 집단에서 비복근과 전경골근의 동시 수축 지수와 발목 관절의 굴신 움직임에 대한 국부적 안정성과는 유의한 상관관계를 보였다(p<.05). 따라서 보행 시 낙상을 예방하기 위해서는 무릎 관절의 내·외전 움직임에 관여하는 근들을 강화하고, 발목 관절의 굴신에 작용하는 비복근과 전경골근을 균형적으로 강화할 필요가 있다고 판단된다.

적혈구(赤血球) Protoporphyrin과 철분대사(鐵分代謝)에 관(關)한 연구(硏究) (A Study on Red Cell Protoporphyrin Concentration and Iron Metabolism)

  • The relative state of human iron storage may be ascertained more reliably through determination of the serum iron, iron binding capacity, transferrin saturation and absorption of radioactive iron in conjunction with studies of red cell morphology than from the study of red cell morphology alone. Recent investigations have shown that there is an increase in red cell protoporphyrin concentration in iron deficiency anemia. The significance of the red cell protoporphyrin has been discussed greatly during the years since its discovery. Two of the main factors which appear to influence the amaunt of protoporphyrin are increased erythropoiesis and factors interfering with the utilization of iron in the synthesis of hemoglobin, and iron deficiency. Recently Heller et al. have described a simplified method for blood protoporphyrin assay and this technique could be used assess nutritional iron status, wherein even minor insufficiencies are detectable as increased protoporphyrin concentrations. Based on the evaluation of the relationship between nutritional iron status and red cell protoporphyrin as an index suitable for the detection of the iron deficiency is described in this paper. RESULTS 1. Hemoglobin Concentrations and Anthropometric Measurements. The mean and standard deviations of the various anthropometric measurements of different age and sex groups are shown in table 1. There measurements have been compared with the Korean Standard. In the absence of local standards for arm circumference and skin-fold thickness over triceps, they have been compared with the standard from Jelliffe. Table 2,3, and 4 give anthropometric measurements and frequency (%) of anemia in children surveyed. The mean height of the children studid was 10 to 20 percent; below the Korean Standard. The distribution of height below 80 percent of the Standard was 21.2 percent, however, among anemic group this percentage was 27.7 percent. In general, the mean weight of the children was 10 to 15 percent below the Korean Standard. The percentage of children with weight less than 80 percent of the Standard was about 35 percent. But in the anemic group of the children, this percentage was 44 percent. The mean arm circumference was about 15 percent lower than the Jelliffe's standard. 61.2 percent of the children had values of arm circumference below 80 percent of the standard. Children with low hemoglobin levels, this percentage was 80 percent. The mean skinfold thickness over the triceps of the children studied was about 25 Percent lower than the Jelliffe's standard and 61.2 percent of the children had the value less than 80 percent of the standard. Among anemic children, this percentage was 70.8%. As may be seen from table 5, the mean hemoglobin concentration of the total group was 11.3g/100ml. Hemoglobin concentration was less than 11.0g/100ml. in 65(36.5%) of the 178 children. The degree of anemia in most of these children was mild with a hemoglobin level of less than 8.0g/100ml. found in only one child. In general, the prevalence of anemia was high in female children than male and decreased its frequency with increasing age. Relatively close relationship was observed between hemoglobin level and anthrophometric measurements especially high between arm circumference and skinfold thickness and hemoglobin but very low in height and low in weight and hemoglobin level, estimated by chi-square value. II. Serum iron, Transferrin saturation (1) Serum iron, and transferrin saturation Serum iron, transferrin saturation and red cell protoporphyrin concentrations were estimated in sub-sample of 84 children from 1 to 6 years and 24 older children between 7 and 13 years of age. The findings are presented in table 6. The mean serum iron concentration of the total group was 59ug/100ml. However, the level incrased with age from 36.6ug/100ml. (1-3years) to 80.8ug/100ml. (7-13 years). 60 percent of these children had a serum iron level less than 50ug/10ml. in the 1-3 years age group and 31.4 percent for 4-6 years group. These contrast with the finding of 12.5 percent anemic children in the 7-13 years age group. The mean transferrin saturation for the total group was 18.1 percent and frequency of anemia by transferrin saturation was observed same pattern as serum iron concentration. (2) Red cell protoporphyrin concentrations. (a) Red cell protoporphrin levels of children: Red cell protoporphyrin and other biochemical data are shown in table 4. The mean concentration in red cell of all children was fround 46.3ug/100ml. RBC. and differences with age groups were observed; in the age group 1-3 years, the mean concentration was $59.5{\pm}32.14$ ug/100ml. RBC; 4-6 years $44.1{\pm}22.57$ ug/100ml. RBC. and 7-13 years, $39.0{\pm}13.56$ ug/100ml. RBC. (b) Normal protoporphyrin values in adults: It was observed that in 10 normal adult males studied here the level of protoporphyrin in red cell ranged from 18 to 54 ug/100ml. RBC. and the mean concentration was $47.5{\sim}14.47$ ug/100ml. RBC. Other biochemical determination made on the same subjects are presented in table 8. (c) Red tell protoporphyrin concentration of occupational blood donors: The results of analyses for red cell protoporphyrin as well as serum iron, transferrin saturation and hemoglobin in the 76 blood donors are presented in table 7 and 8. In this experiment, donors were selected at random, however, most of them bled repeatedly because of poor economic situation, I doubt. Table 9 shows the distribution of red cell protoporphyrin concentration and hemoglobin concentration of occupational donors. The mean hemoglobin value for the total was 11.9 g/100 ml. When iron deficiency anemia is defined as a transferrin saturation below 15%, prevalence of anemia was 47.4 percent and the mean serum iron was 27.1ug/100ml. and red cell protoporphyrin, 168.3ug/100ml. RBC. However, mean serum iron and protoporphyrin concentration of above 15% transferrin saturation were 11.6 ug/100 ml. and 58.8 ug/100 ml. RBC. respectively. The mean Protoporphyrin concentration of non-anemic (above 15% transferrin saturation) donors was slightly higher than the results of normal adult males.

해조의 식용분말화에 관한 연구 (A Study on the Nutritive Value and Utilization of Powdered Seaweeds)

  • I. Subject of the study A study on the nutritive value and utilization of powdered seaweeds. II. Purpose and Importance of the study A. In Korea the shortage of food will be inevitable by the rapidly growing population. It will be very important study to develop a new food from the seaweeds which were not used hitherto for human consumption. B. The several kinds of seaweeds have been used by man in Korea mainly as side-dishes. However, a properly powdered seaweed will enable itself to be a good supplement or mixture to certain cereal flours. C. By adding the powdered seaweed to any cereals which have long been staple foods in this country the two fold benefits; saving of cereals and change of dietary pattern, will be secured. III. Objects and scope of the study A. Objects of the study The objects will come under four items. 1. To develop a powdered seaweed as a new food from the seaweeds which have been not used for human consumption. 2. To evaluate the nutritional quality of the products the analysis for chemical composition and animal feeding experiment will be conducted. 3. Experimental cocking and accepability test will be conducted for the powdered products to evaluate the value as food stuff. 4. Sanitary test and also economical analysis will be conducted for the powdered products. B. Scope of the study 1. Production of seaweed powders Sargassum fulvellum growing in eastern coast and Sargassum patens C.A. in southern coast were used as the material for the powders. These algae, which have been not used for human consumption, were pulverized through the processes of washing, drying, pulverization, etc. 2. Nutritional experiments a. Chemical composition Proximate components (water, protein, fat, cellulose, sugar, ash, salt), minerals (calcium, phosphorus, iron, iodine), vitamins (A, $B_1,\;B_2$ niacin, C) and amino acids were analyzed for the seaweed powders. b. Animal feeding experiment Weaning 160 rats (80 male and 80 female rats) were used as experimental animals, dividing them into 16 groups, 10 rats each group. Each group was fed for 12 weeks on cereal diet (Wheat flour, rice powder, barley powder, potato powder, corn flour) with the supplementary levels of 5%, 10%, 15%, 20% and 30% of the seaweed powder. After the feeding the growth, feed efficiency ratio, protain efficiency ratio and ,organs weights were checked and urine analysis, feces analysis and serum analysis were also conducted. 3. Experimental cooking and acceptability test a. Several basic studies were conducted to find the characteristics of the seaweed powder. b. 17 kinds of Korean dishes and 9 kinds of foreign dishes were prepared with cereal flours (wheat, rice, barley, potato, corn) with the supplementary levels of 5%, 10%, 15%, 20% and 30% of the seaweed powder. c. Acceptability test for the dishes was conducted according to plank's Form. 4. Sanitary test The heavy metals (Cd, Pb, As, Hg) in the seaweed powders were determined. 5. Economical analysis The retail price of the seaweed powder was compared with those of other cereals in the market. And also economical analysis was made from the nutritional point of view, calculating the body weight gained in grams per unit price of each feeding diet. IV. Results of the study and the suggestion for application A. Chemical composition 1. There is no any big difference in proximate components between powders of Sargassum fulvellum in eastern coast and Sargassum patens C.A. in southern coast. Seasonal difference is also not significant. Higher levels of protein, cellulose, ash and salt were found in the powders compared with common cereal foods. 2. The levels of calcium (Ca) and iron (Fe) in the powders were significantly higher than common cereal foods and also rich in iodine (I). Existence of vitamin A and vitamin C in the Powders is different point from cereal foods. Vitamin $B_1\;and\;B_2$ are also relatively rich in the powders.'Vitamin A in ·Sargassum fulvellum is high and the levels of some minerals and vitamins are seemed4 to be some influenced by seasons. 3. In the amino acid composition methionine, isoleucine, Iysine and valine are limiting amino acids. The protein qualities of Sargassum fulvellum and Sargassum patens C.A. are seemed to be .almost same and generally ·good. Seasonal difference in amino acid composition was found. B. Animal feeding experiment 1. The best growth was found at.10% supplemental level of the seaweed Powder and lower growth rate was shown at 30% level. 2. It was shown that 15% supplemental level of the Seaweed powder seems to fulfil, to some extent the mineral requirement of the animals. 3. No any changes were found in organs development except that, in kidney, there found decreasing in weight by increasing the supplemental level of the seaweed powder. 4. There is no any significant changes in nitrogen retention, serum cholesterol, serum calcium and urinary calcium in each supplemental level of the seaweed powder. 5. In animal feeding experiment it was concluded that $5%{\sim}15%$ levels supplementation of the seaweed powder are possible. C. Experimental cooking and acceptability test 1. The seaweed powder showed to be utilized more excellently in foreign cookings than in Korean cookings. Higher supplemental level of seaweed was passible in foreign cookings. 2. Hae-Jo-Kang and Jeon-Byung were more excellent than Song-Pyun, wheat cake, Soo-Je-Bee and wheat noodle. Hae-Je-Kang was excellent in its quality even as high as 5% supplemental level. 3. The higher levels of supplementation were used the more sticky cooking products were obtained. Song-Pyun and wheat cake were palatable and lustrous in 2% supplementation level. 4. In drop cookie the higher levels of supplementation, the more crisp product was obtained, compared with other cookies. 5. Corn cake, thin rice gruel, rice gruel and potato Jeon-Byung were more excellent in their quality than potato Man-Doo and potato noodle. Corn cake, thin rice gruel and rice gruel were excellent even as high as 5% supplementation level. 6. In several cooking Porducts some seaweed-oder was perceived in case of 3% or more levels of supplementation. This may be much diminished by the use of proper condiments. D. Sanitary test It seems that there is no any heavy metals (Cd, Pb, As, Hg) problem in these seaweed Powders in case these Powders are used as supplements to any cereal flours E. Economical analysis The price of the seaweed powder is lower than those of other cereals and that may be more lowered when mass production of the seaweed powder is made in future.
