U-healthcare Service Management Scheme for Big Data of Patient Infomation (환자 정보를 빅 데이터화 하기 위한 유헬스케어 서비스 관리기법)

  • Recently the disease by eating of the modern prevention, management, and trends in the u-healthcare service that provides healthcare services including health promotion is changing rapidly. However, u-healthcare service is a healthcare information that provides users of the disease can not be analyzed even if the service is stored or not stored in the management server status is giving the inconvenience caused to users of the health services. In this paper, we propose a management method of health care services and a big data formation information that provides users of the disease to facilitate the users of health care services through the use magazine big data information regardless of time and place. The proposed method has the user's bio-information and the measured health information and transmits data through a wired or wireless communication to the medical institution and the user's health information data formation by the big user of the analysis of the health information and the disease of the user feedback to the user.

Oral health status of Korean adults with implants according to their use of oral hygiene products: results from a nationwide population-based study (2013-2015)

  • Purpose: Many home care treatments can be used to promote the health and longevity of dental implants; however, few studies are available to support the concept that self-performed oral hygiene behaviors are an essential tool for improving and maintaining oral health. We investigated age-stratified associations between dental health behaviors related to tooth brushing (TB) and oral hygiene product use in Korean adults with implants. Methods: A total of 1,911 subjects over 19 years of age who had 1 or more implants and who participated in the 2013 to 2015 Korea National Health and Nutrition Examination Survey were reviewed. Periodontal status was assessed using Community Periodontal Index (CPI) scores, and periodontitis was defined as a CPI greater than or equal to 3. The complex sampling design of the survey was utilized to obtain the variance and individual weight of each analyzed factor. A high CPI was the outcome variable, and the main explanatory variables were oral hygiene behaviors, such as TB, dental floss (DF), interproximal brushing, and mouth rinsing. Results: Almost all individuals with a lower CPI brushed their teeth twice or more per day, in contrast to those with a higher CPI, and were likely to use DF. The adjusted odds ratio of not using DF for a higher CPI was 1.83 (95% confidence interval, 1.35-2.49). Conclusions: TB was implemented more than twice a day by patients with good oral health, and the combination of TB and DF significantly reduced the prevalence of a higher CPI. Self-performed oral hygiene practices combining TB and DF were significantly related to a low prevalence of periodontitis in implant patients.

Self-perception of the Amount of Medical Aid Use of Outpatient Overusers in Korea (의료급여 외래 과다이용자의 의료이용량에 대한 자기인식)

  • Limited studies examined Medical Aid recipients' perception for amount of medical use. This study aimed to identify self-perception(optimal, under and overutilization) for amount, and real amount of medical use, and to determine factors associated with the perception. Subjects were 2,489 Medical Aid recipients among top 2% overusers in 2005. 200 case managers(CM) managing them conducted survey. CM interviewed them using 2005 medical claiming data from the Health Insurance Review & Assessment Service and structured questionnaire. Despite of overusers, perception of overutilization was only 26.9% and 23.6% in Class I and Class II, and that of underutilization was 21.4% and 18.7% respectively. In Class I, monthly total outpatient cost per capita of overutilization perception in 2006 was 206 thousand won higher than 150 thousand won of optimal utilization. Amounts of outpatient visit-days and prescribed cases of overutilization perception were higher than those of optimal and underutilization(p <0.0001). In Class II, overutilization perception had more prescribed cases(p 0.004). After adjustment of confounding factors including age and sex, the associated factors(odds ratio) with overutilization perception were hypertension(1.25), arthritis(1.32), depression(1.66), visit of multi medical institutions(3.09), and those of the underutilization were female(1.34), disabled(1.27), no family support(1.49), living in medium and small city(1.48), experience of unabled-visit to medical institution(2.54), frequent visit-recommendation from physician (1.36). In conclusion, education and consult are needed for subjects to improve the reasonable decision of medical use, and the self-care ability to manage diseases and symptoms. We suggest reinforcing the alternative service in community instead of costly medical institution.

A Qualitative Study about the Function and Relationships of Social Community Welfare Centers and Health Family Support Centers for Family Welfare Practice (가족복지실천을 위한 종합사회복지관과 건강가정지원센터의 기능 및 관계에 대한 질적연구)

  • This study searched for the common and different aspects of social community welfare centers and health family support centers for family welfare practice of the community. We also tried to find out the awareness of workers and the performance function for each institution. Through the interviews and review of literature, and comparative analysis of operating status, we looked for the common and different aspects, and the direction of development, which evolved family welfare into the community by way of function and relationships of both institutions. As a result, the participants of this study pursued common purposes. They however made aware of the issues of discrimination about social values and each institution's functions. We suggest that the social community welfare center focuses on individuals and families, in order to integrate the community. A health family support center should play the role to fulfill needs for family welfare according to family life cycle, and have good accessibility for individuals and families in the community.

Unplanned Reoperation Rate at a Government-Designated Regional Trauma Center in Gangwon Province

  • Purpose: Determining appropriate ways to assess health care quality within the National Health Insurance System is of interest to both the Korean government and the medical community. However, in the trauma field, the number of indicators used to evaluate surgical quality is limited. Using data collected over 5 years at Wonju Severance Christian Hospital Trauma Center in Korea, this study aimed to determine whether the unplanned reoperation rate in the field of trauma surgery could be used to assess the quality of an institution's surgical care. Methods: In total, 665 general surgical procedures were performed at the Trauma Center in 453 patients with abdominopelvic injuries from January 2015 to December 2019. Data were collected from the Trauma Center's data registry and medical records, and included information regarding patients' demographic characteristics, the type of index operation, and the reason for unplanned reoperations. Results: A total of 453 index operations were evaluated. The proportion of patients with an Injury Severity Score (ISS) >15 was 48-70% over the 5-year period, with an unplanned reoperation rate of 2.1-9.3%. Patients had an average ISS score of 17.5, while the average Abbreviated Injury Scale Score was 2.87. Unplanned reoperations were required in about 7% of patients. The most common complications requiring reoperation were recurrent bleeding (26.9%), wound problems (26.9%), intestinal infarction (15.4%), and anastomosis site leakage (7.7%). The procedures most frequently requiring unplanned reoperations were bowel surgery (segmental resection, primary repair, enterostomy, etc.) (24.5%) and preperitoneal pelvic packing (10.6%). Conclusions: The proportion of reoperations was confirmed to be affected by injury severity.

A Study on the Functional Health Status of Living-alone Elderly (독거노인의 건강기능상태에 관한 연구 -광주광역시를 중심으로-)

  • The degree of nursing service requirement can be determined by functional health status and a predictive explanation for the status on the living-alone elderly is essential to plan home-care nursing intervention. The purpose of this study is to explore the functional health status of living-alone elderly. Especially the main purpose is to contrast the differences of the status between the residential living-alone elderly and institutional living-alone elderly. This study was conducted through the interview by the students of nurse college who were previously trained during the months of Oct. and Nov. 1996. Sample participants located in Tong-Gu and Seo-Gu of Kwangju City. To contrast the functional health status. this study tried to utilize the Activity Inventory(AI). Among the AI's this study adopted the Physical Activity of Daily Living(PADL) and Instrumental Activity of Daily Living(IADL). Results of the study show that the home-care nursing intervention for the living-alone elderly is urgent more for the residential than for the institutional. It seems that the residential living-alone elderly suffer from inability in Physical Activity of Daily Living. As the residential elderly who have some physical lacks prefer the institution. it is recommended to support physical lacks for the institutional. And more mental-emotional support is recommended for the residential living-alone elderly. The degree of the residential's IADL is higher than that of the institutional. It seems that it was caused by the reason that the residential's needs for the minimum level of life is greater than that of the institutional. So the polypharmacy in elderly nursing-home residents must be considered as sigificant.

Disaster Mental Health Research Ethics Review (재난 정신건강 연구윤리 고찰)

  • Although disaster research participants are in a more vulnerable state than general research participants, various ethical issues to be considered in the study may be overlooked due to the special situation of disaster. Therefore, research ethics should be considered to reduce damage to study participants and maximize benefits. In addition, from the perspective of researchers, ethical considerations should be applied in the disaster research process, so research ethics awareness should be established. In addition, at the health care institution and national level, it is necessary to prepare research ethics that reflect the Korean situation while meeting international standards in consideration of the characteristics of local communities. In Korea, after the Ferry Sewol accident in 2014, social interest in disaster mental health increased and the National Trauma Center was established in 2018, raising the need for disaster mental health ethics guidelines. Therefore, this review aims to discuss the significance of six items: specificity of the study, prior consent and autonomy, community participation, confidentiality and feedback provision, risk minimization, and research support. So far, the experience of disaster mental health research is not sufficient in South Korea. Therefore, the current guidelines are required to be continuously revised through practical experience in the future.

CARE OF TRISMUS AND OROCUTANEOUS FISTULA BY ODONTOGENIC INFECTION IN A DISABLED PATIENT (장애환자에서 치성감염에 의한 아관긴급과 구강피부누공의 관리)

  • Some odontogenic infections erode into fascial spaces directly and spread toward lymphatic tissues and blood streams. The principal maxillary primary spaces are the canine, buccal, and infratemporal space, the next secondary spaces are the masseteric, temporal and pharygeal space. As a result of the infection, trismus and orocutaneous fistula may be occurred. Trismus is owing to conditions not associated with temporomandibular joint itself and may be of myogenic, neurogenic, or psychogenic nature. Muscular trismus is due to infection adjacent to the elevator muscles of the jaw. The four principles of treatment of infection are as follows: (1) removal of the cause, (2) establishment of drainage, (3) institution of antibiotic therapy, and (4) provision of supportive care, including rest, nutrition and physiotherapy. Jaw physiotherapy is necessary to increase the amount of mouth opening and regain normal muscle tone. If proper care of odontogenic infection could be attained, the orocutaneous fistula will heal and close spontaneously by wound contraction mechanism of natural homeostatic response. This is a case report of the care of trismus and orocutaneous fistula due to fascial space abscess by advanced odontogenic infection in a physically disabled patient.

Analysis of Factors that will Ensure Effective Health Care Delivery System (효율적인 의료전달체계 확보를 위한 요인 분석)

  • In order to analyze factors that support effective health care delivery system, this study aimed to research general public's perception on the functions of medical institutions(based on the levels of treatment: primary care, secondary care, and tertiary care), choices of medical institution when contracted with an unfamiliar disease, and recognition of diseases based on their severities. We collected data using self-administered survey from 400 general public living in Seoul, S. Korea from April 25 to May 18, 2011. The analysis was conducted using frequency analysis, chi-square test, and t-test; we analyzed the data to see if there are differences based on gender, age, and level of education. The result of both recognition of functional differences of medical institutions and selection of medical institutions when contracted with unfamiliar diseases showed that there were no significant differences based on the gender; however there were significant differences when considering the age and education. Looking at the result of the knowledge of the disease classification based on its severity, there were significant differences in age, gender, and education. In order to provide sustainable and effective health care delivery system, utilization of primary care as well as education and promotion regarding the functional differences of medical institutions and classification of disease based on its severity need to be encouraged.

A Study on Nursing Service of Chronic Diseases by the First Step and Third Step Medical Treatment (1차 및 3차 진료기관 이용 만성질환자의 간호서비스에 관한 연구)

  • It is to be growing up the interest of community health affairs through visiting nursing care. The health medical treatment of Korea has been changed largely on the period. The juvenile population has decreased. This means that is has took the population consensus of advanced national organization to be increased by the old age. The transition of disease has changed from the contagious disease importance to the chronicity disease omportance because the domestic district population has experienced the sudden urbanization circumstance district population has experienced the sudden urbanization circumstance to be growing up $70\%$ of the whole population. When the nursing service has common function to be delivering from all direction to home, this study is getting the great important phase velocity in order to manage the kernel questional adult chronicity disease of health medical institution at the present age. (1) community over system or with people particularity (2) the first of third step medical treatments. The variety of medical treatments organization has quantity of the delivery manpower and specially between consumers and rdlated person. A qualitative difference is showed at the purpose to be seizing. That research related person is use at district health center in Seoul, by foundation on nurse registration book of H collage hospital and public health registration book. According the chronicity disease. age. and sex. nature agree-able standard 54 people took the content analysis on nurse registration book of total 108 people. The results of the study were as follows: 1. General background factors are houses or kind of medical facilities and number of patients in family. The first medical treatment is more patients than third medical treatment organization. The first medical treatment of economic environment os appering to be worse. 2. The chronicity disease frequency have been different speciality according to medical treatment organization. On case of the first medical treatment. Diabetes and High Blood Pressure were good but Cerebrum Vascular Accident(CVA) showed many for bed case. In addition. the number of family is comparative large exception of CVA on according for moving condition and health more than the first medical treatment. However. family condition. whole family percentage is decreasing preferably through the potential resource is increasing by the number of and the construction of family. The ability of real resource is considered to be low. 3. The average percentage of nurse service has appered to be differed two groups by the first step medical treatment(33.72 times) and third step medical treatment(45.70 times). However, the difference (the first step medical treatment and third step medical treatment) is to be limited to issue the medicine at the service. The condition of nurse care was the indirect nursing care. Supportiong area was to be related to volunteer service and administration support. 4. The various nursing care average percentage of the chronicity disease was increased by orders of Diabetes. High Blood Pressure. and CVA in examination result and the medical treatment. The indirect nursing care was also same. At third step medical treatment, orders of chronicity disease were same. The case of other area on service conditions were increased by order of Diabetes. High Blood Pressure, and CVA. However. it is never appearing the difference at bottleneck affairs nursing care. 5. When the visiting nursing care demand particularly. the average percentage of nursing care from the first step medical treatment that the time under a person is many more than the time over two people. However, there was no difference in statistic. Third step medical treatment is $49.81\%$ at the time under a person. The average nursing care service is appeared by more many when the visiting nursing care demand is a few by 12.83 at the time over two people. 6. By visiting nursing care percentage to be frequency that nursing care averaghe percentage and inter-relation are large. The related factor of the first medical treatment is 0.96. However, the related factor of third medical treatment has shown the decreased 0.49 for the condition of relation more than that. Therefore. the nursing care average percentage is related to the visiting times of a nurse. This result is be showing the obvious fact that the first step medical treatment is few more than third step medical treatment.
