Validation of Self-Administered Dietary Assessment Questionnaires Developed for Japanese Subjects : Systematic Review

  • Several self-administered dietary assessment questionnaires have recently been developed, validated, and used in nutritional epidemiological and clinical studies in Japan. This article describes recent evidence on development and validation of them. After extensive search of published articles both in English and Japanese languages, we identified 25 articles on 13 questionnaires of which validation studies have existed. Number of foods/menus assessed varied from 31 to 169 according to questionnaires. Eleven questionnaires were food frequency type, either with fixed portion size or semiquantitative, and two diet history types. All the 13 questionnaires were validated against intakes assessed with dietary record or 24-hour recall, and only two with biomarkers. Number of subjects used in the studies was between 23 and 350. All the studies used adult subjects. In the studies with dietary record or recall, the correlation coefficient for or orgy intake was between 0.22 and 0.65 (median = 0.44). Median correlation coefficient for nutrients was between 0.21 and 0.61. In the studies with biomarkers, serum marine-origin n-3 polyunsaturated fatty acids and carotenes, and urinary potassium seemed useful biomarkers. In conclusion, recent progress of this field in Japan is remarkable. But more research is needed for validation studies with biomarkers, and the development and validation of questionnaires for children and elderly subjects. (J Community Nutrition 5(2) : 83∼92,2003)

The Process Quality Assessment of Health Promotion Programs at Public Health Centers (보건소 건강증진사업 수행과정의 질 평가)

  • This study purports to assess the process quality of health promotion programs at public health centers. The sample used in this study consisted of 242 public health centers in South Korea. Data was collected with self-administered questionnaires between September. 3rd to September. 23rd in 2002. The 95 returned questionnaires were analyzed by SPSS 11.0 version. The major findings are as follows; There is a need to improve (1) a procedure for reviewing and analyzing strategic planning, (2) the quality of the staff in charge of program management, (3) the function of planning and training for monitoring and evaluation, (4) the ability of program managers in advising and consulting with clients, and (5) a procedure for organizing resources and information. In conclusion, in order to improve the process quality of health promotion programs at public health centers, integrated support from various institutions such as public health centers, community, regional and national health authorities and administrative departments is required.

The Study of Developing a Diagnostic Assessment Tool for Church Health Through The Korean Baptist Church (교회 건강성 측정도구 개발에 관한 연구: 한국침례교회를 중심으로)

  • This study was about developing a diagnostic assessment tool for the health of Baptist Church. To enable this, the definition of healthy Baptist churches and health factors was examined, diagnostic questions by church health factors was developed. Literature review and research of Delphi were used for research methods. Based on the quarter sampling method, the main survey was conducted on 395 people about factor analysis and needs analysis. Through this study, a healthy Baptist church is defined as a community of believers who glorify God by performing the functions of the church well, having the attributes of the church and the identity of the Baptist church as a body member of Christ. Factors of the healthy Baptist churches are community, loyalty of function, identity of Baptist church, and the essential qualities of the church. It was confirmed that there are 17 factors such as community of faith, worship, priesthood of all believers, and apostolicity. For measuring healthy Baptist Church, it consists of 199 diagnostic assessment questionnaires which can be reduced to 53 or 23 condensed diagnostic assessment questionnaires.

Perception and use of gait measures among physical therapists in South Korea

  • Objective: The purpose of this study was to investigate the physical therapists' perception of the use of gait measures, the frequency of the gait measures used, and also to identify the barriers that limit the use of these assessment tools. Design: Cross-sectional study. Methods: Physical therapists from the Seoul, Gyeonggi area from March to July 2016 were included in the study. Over the course of 18 weeks, a cross-sectional study was conducted with a self-report questionnaire. A total of 700 questionnaires were distributed and 350 questionnaires (50%) were collected, however with the exclusion of 140 questionnaires due to non-consent, a total of 210 questionnaires (30%) were analysed. Results: Out of the 10 standardized assessment tools, the therapists showed the highest perception for the timed up and go test (TUG [n=153, 72.9%]) and they also had high perception for the 10 meters walk test (10MWT [n=149, 71.0%]), and 6-minute walk test (6MWT [n=123, 58.6%]). The respondents answered that the TUG (n=116, 55.2%), 10MWT (n=100, 47.6%), and 6MWT (n=51, 24.3%) was used the most often. On the contrary, only four (1.9%) therapists have used the Chedoke-McMaster stroke assessment and the Rivermead Mobility Index. The lack of time was considered as the most important barrier to the use of assessment tools in clinical practice. Conclusions: Through this study, it has been shown that the domestic physical therapists used the TUG and the 10MWT mainly due to high recognition and evaluation status; however, the lack of time was the greatest impediment to the clinical application of the gait assessment tools.

Review of Exposure Assessment to Obtain Exposure Factors of Consumer Products (소비 생활용품의 노출계수 조사 방법의 고찰)

  • Background: Exposure factors for consumer products have been developed since the 1980s. Such exposure factors are important in risk assessment. Since the exposure/use patterns of consumer products in a country may reflect its respective cultural, meteorological, and socio-economic circumstances, unique Korean exposure factors for consumer products are needed. Such exposure factors are required for newly-enacted chemical regulations in Korea. For this review, published papers and survey reports on exposure factors of consumer products were examined for their assessment methodologies. Results: Investigation into exposure factors for consumer products used a variety of methods: home visit, online, telephone, and mail surveys using questionnaires; face to face interviews; modeling using a constructed database; and direct measurement. To collect more accurate exposure information or to check the reliability of the sampling method, some studies were repeated using the same questionnaire, in-home observation, direct measurement of usage, and videotaping. In Korea, nationwide exposure surveys were conducted five times over five years to obtain Korean exposure factors. However, with the exception of the $5^{th}$-year study, the surveys were online questionnaires and only the $5^{th}$-year study validated the accuracy of exposure information by re-visits and direct measurement. Conclusion: Accurate exposure factors are an essential part of risk assessment to assure safe use of consumer products. For better and safer management of consumer products, accurate exposure factors in Korea should be assessed for various exposure pathways.

Effects of Direct Practice of Newborn Health Assessment on Students' Nursing Clinical Competence and Self-Efficacy (신생아 건강사정 직접실습이 간호학생의 간호수행능력과 자기효능감에 미치는 효과)

  • Purpose: This study was done to examine the effect of direct practice of newborn health assessment on nursing student's clinical competence and self-efficacy and to propose effective strategies for clinical education on newborn care. Methods: Design was a nonequivalent control group quasi-experimental study. The direct practice program was composed of a lecture, demonstration, drill and feedback using a manikin, and repeated direct practice regarding newborn health assessment. Participants were 65 student nurses taking the pediatric nursing practicum in the nursery room at M hospital. The experimental group (n=33) participated in the direct practice program for newborn health assessment and the control group (n=32) received the traditional practice method. Nursing clinical competence was assessed by two nurse investigators and structured questionnaires were used to measure self-efficacy. Results: The experimental group's clinical competence was significantly higher than that of the control group (t=-4.82, p=.000). However no significant difference was found between the two groups for self-efficacy (t=1.264, p=.211). Conclusion: These findings indicate that the direct practice program is effective in improving nursing student's clinical competence, but it was not effective in increasing self-efficacy. Direct practice in various clinical education settings is recommended and longitudinal effects be evaluated.

Prioritisation of Medical Procedure for Health Technology Assessment (전문의 대상 설문조사를 이용한 보건의료 기술평가 대상 시술의 우선 순위 선정 - 이비인후과와 흉부외과를 중심으로 -)

  • Background & Objectives : Korea is face with the social need for health care technology assessment so that it is urgently needed to found principles and methodology in technology assessment in health care. As a groundwork for health care technology assessment, we tried to prioritize medical technology for assessment. Among medical technologies, procedure is somewhat difficult to assess, compared to drug or equipment. In this study, we aimed at the prioritisation of medical procedure to be assessed, in terms of efficay, safety, and adequacy. Method : For the standardized classification of medical procedure, ICD-9-CM(International Classification of Diseases 9th edition - Clinical Modification) was used. Among the list the procedures coming under otorhinolaringjology and thoracic surgery were selected by three family physicians. The list of procedure was mailed to the board certified surgeons of both disciplines, with the question asking about the necessity for assessment in terms of efficay, safety, and adequacy. Replied questionnaires were analyzed in each procedure. Results : Of 560 otorhinolaryngologist and 480 thoracic surgeon, 114 surgeons replied. Of otorhinolaryngological procedure, incision, excision, and destruction of inner ear : fenestration of inner ear : stapedectomy and its revision were the most urgent technology to assess in the aspect of safety. For adequacy, operations on Eustachian tube: fenestration of inner ear: incision, excision, and destruction of inner ear were highly ranked in necessity, and for efficary, operations on Eustachian tube; external maxillary antrotomy; fenestration of inner ear. Thoracic surgeons replied thoracic procedures, lung transplantation; heart transplantation; implantation of heart assist system [pump] are most important for evaluation in terms of safety; and heart transplantation; Lung transplantation; Implantation of heart assist system [pump] in terms of adequacy, and surgical collapse of lung [Artificia니 pnemothorax or pnuexoperitoeum]; lung transplantation; periarterial sympathectomy in terms of efficacy. As a whole, surgeons regard safety evaluation is more urgent than adequacy or efficary. In addition, otorhinolaryngological surgeons regard evaluation of their procedures more urgent than thoracic surgeons regard theirs. Conclusion : By the questionnaire to board certified physicians, we get some preliminary data for prioritisation of technologies to assess. Through the questionnaire like this, much information would be gathered for technology assessment, especially for medical procedure, if not enough. In the near future, well structured expert opinion gathering research, such as modified Delphi or nominal group technique, should be done succeedingly.

A Proposal of study Designs and Methods for Evaluating the Adverse Health Effects of Agent Orange among Korean Vietnam Veterans (고엽제의 건강위해성 평가를 위한 연구설계와 방법 제안)

  • Objectives : To propose a feasible, valid and appropriate study designs and epidemiologic methods for evaluating the adverse health effects of Agent Orange-chemical defoliants used in Vietnam- in Korea. Methods : A literature study was peformed on Agent Orange, herbicides, pesticides and dioxins. The study subjects, study design, exposure assessment and health outcomes assessment were examined in each study. The potential data sources for the study subjects, study design, exposure assessment and health outcomes assessment in Korea were investigated. Results and Conclusion : In earlier Korean studies, research subjects for studying the effects of Agent Orange were identified from the patients or persons who claimed to have Agent Orange-related diseases due to the difficulties in identifying the entire population of Vietnam veterans in Korea. In this study, an attempt was made to identify the total number of Vietnam veterans in Korea. As a result, the addresses of 20,000 Vietnam veterans were obtained. It is proposed that a retrospective cohort design on a sample of the total number of Vietnam veterans is a feasible and appropriate study design. Self report questionnaires and military records were proposed to assess the exposure level. It is believed that measuring the plasma or tissue TCDD should be used only for a validation study assessing the level of exposure. For the health outcomes assessment, it is possible to obtain the mortality, cancer frequency, physical examination, screening and medical insurance record data.

The Development of a Risk Management System in the Field of Industrial Safety in the Republic of Kazakhstan

  • Background: The purpose of the work is to develop a system that allows processing of information for analysis and industrial risk management, to monitor the level of industrial safety and to perform necessary measures aimed at the prevention of accidents, casualties, and development of professional diseases for effective management of industrial safety at hazardous industrial sites. Methods: Risk assessment of accidents and incidents is based on expert evaluations. Based on the lists of criteria parameters and their possible values, provided by the experts, a unified information and analytical database is compiled, which is included in the final interrogation questionnaires. Risk assessment of industrial injuries and occupational diseases is based on statistical methods. Results: The result of the research is the creation of Guidelines for risk management on hazardous industrial sites of the Republic of Kazakhstan. The Guidelines determine the directions and methods of complex assessment of the state of industrial safety and labor protection and they could be applied as methodological basis at the development of preventive measures for emergencies, casualties, and incidents at hazardous industrial sites. Conclusion: Implementation of the information-analytical system of risk level assessment allows to analyze the state of risk of a possible accident at industrial sites, make valid management decisions aimed at the prevention of emergencies, and monitor the effectiveness of accident prevention measures.

A Review on Youth Risk Behavior Surveillance System of U.S.A (미국의 청소년 건강위험행위 감시체계에 관한 고찰)

  • Purpose: This study reviews the Youth Risk Behavior Surveillance System (YRBSS) in the United States. This review focuses on the purposes, questionnaires, survey methods, and use of survey results. The author also discussesthe establishment and use of Korean Youth Risk Behavior Surveillance System. Results:The YRBSS was developed in 1990 to monitor priority health risk behaviors that contribute to the leading causes of death, disability, and social problems among youth and adults in the United States. The purpose of YRBSS is to determine the prevalence of health risk behaviors, to assess the trend of health risk behavior prevalence, provide comparable data, monitor progress toward achieving the Healthy People 2010. The questionnaires inquire about health-related behaviors, including tobacco use, unhealthydietary behaviors, inadequate physical activity, alcohol and drug use, sexual activity, behaviors that contribute to unintentional injuries and violence. The results of YRBS have been used to estimate the prevalence of risk behaviors among youth, to set goals for school health programs, for training and development of health program personnel, for the development of health education curriculum, to support enactment of health-related regulations, and to support funding for futher research. Conclusions: The YRBSS represents a potentially valuable program that can be applied in Korea. Assessment of risk behaviors among Korean youths will provide invaluable insight for many potential purposes.
