Physical therapist perception survey for muscle re-education through visual feedback obtained from rehabilitative ultrasound imaging

  • Objective: This study surveyed the perceptions of physical therapists on muscle re-education through visual feedback obtained from rehabilitative ultrasound imaging (RUSI). Design: Survey. Methods: For this study, 500 physical therapists who participated in a refresher training held by the Seoul City Association in March 2015 were selected for a questionnaire-based survey. Subjects were randomly selected targets physiotherapists who participated in a refresher training.The questionnaire had 21 items in total. Questions 1 to 15 could be answered by everyone. However, questions 16 to 21 could be answered only by people who used RUSI. Results: The majority of respondents were aged 20 to 30 years. Respondents in their twenties, thirties, forties, and fifties accounted for 32.4%, 40.2%, 21.9%, and 5.6%respectively. Therapists with careers spanning one to 5 years accounted for 27.8%, while those with careers spanning 5 to 10 years and 10 to 15 years accounted for 34.6% and 17.0%, respectively. Those with careers over 20 years accounted for 9.2%. The types of work have not been various including work related to the nervous system (49.0%), the musculoskeletal system (41.5%), sports (0.7%), juvenile physical therapy (4.2%), and others (4.6%). Conclusions: In this study, we examined the perceptions of physical therapists on rehabilitation ultrasound imaging used in muscle re-education. We also examined how to use this technique. Many therapists who participated in the refresher training were found to be unaware of RUSI. In the future, further investigations on RUSI for muscle re-education are required through refresher training or training lectures at the national level.

Process Evaluation of a Mobile Weight Loss Intervention for Truck Drivers

  • Background: In a cluster-randomized trial, the Safety and Health Involvement For Truck drivers intervention produced statistically significant and medically meaningful weight loss at 6 months (-3.31 kg between-group difference). The current manuscript evaluates the relative impact of intervention components on study outcomes among participants in the intervention condition who reported for a post-intervention health assessment (n = 134) to encourage the adoption of effective tactics and inform future replications, tailoring, and enhancements. Methods: The Safety and Health Involvement For Truck drivers intervention was implemented in a Web-based computer and smartphone-accessible format and included a group weight loss competition and body weight and behavioral self-monitoring with feedback, computer-based training, and motivational interviewing. Indices were calculated to reflect engagement patterns for these components, and generalized linear models quantified predictive relationships between participation in intervention components and outcomes. Results: Participants who completed the full program-defined dose of the intervention had significantly greater weight loss than those who did not. Behavioral self-monitoring, computer-based training, and health coaching were significant predictors of dietary changes, whereas behavioral and body weight self-monitoring was the only significant predictor of changes in physical activity. Behavioral and body weight self-monitoring was the strongest predictor of weight loss. Conclusion: Web-based self-monitoring of body weight and health behaviors was a particularly impactful tactic in our mobile health intervention. Findings advance the science of behavior change in mobile health intervention delivery and inform the development of health programs for dispersed populations.

전산화 인지재활과 전통적 인지훈련이 경도 외상성 뇌손상 환자의 작업기억과 실행기능에 미치는 영향 (The Effects of Computer-Based Cognitive Rehabilitation and Traditional Cognitive Training on the Working Memory and Executive Function in Patients with Mild Traumatic Brain Injury)

  • 본 연구의 목적은 전산화 인지재활과 전통적 인지훈련이 경도 외상성 뇌손상 환자의 작업기억과 실행기능에 미치는 영향을 탐색하기 위함이다. 본 연구는 재활병원에서 재활치료를 받는 경도 외상성 뇌손상 환자 20명이 참여하였다. 대상자들은 실험군 10명과 대조군 10명으로 할당되었다. 실험군은 전산화 인지재활과 전통적인 인지훈련을 30분씩 60분을 받았으며, 대조군은 전통적인 인지훈련을 60분 수행하였다. 두 군은 하루 60분, 주 5회, 4주 동안 배정된 훈련을 수행하였다. 작업기억과 실행기능을 평가하기 위하여, 숫자외우기 검사(정방향, 역방향)와 MVPT-3의 시각기억, 그리고 선추적 검사와 스트룹 검사 A와 B를 중재 전과 후에 실시하였다. 두 군 간 변화량 비교에서 실험군은 대조군보다 시각기억과 스트룹 검사 A에서 유의하게 더 큰 향상을 보였다. 이러한 결과는 전산화 인지재활과 전통적 인지훈련이 전통적인 인지훈련보다 작업기억과 실행기능에 긍정적인 변화를 제공할 수 있음을 시사한다.

Empowering Rural Housewives in Iran: Utilizing the Transtheoretical Model to Increase Physical Activity

  • Objectives: Rural housewives are integral to household management and family care, yet their sedentary lifestyles present significant health risks. This study used the transtheoretical model (TTM) to investigate strategies that encourage and maintain regular exercise habits among rural housewives. Methods: A semi-experimental study was conducted in 2021 with 114 housewives aged 30 to 59 who attended rural health centers in Gorgan, Iran. Participants were randomly assigned to 1 of 2 groups. Data collection involved a validated questionnaire that gathered demographic information and constructs of the TTM. The intervention group participated in a comprehensive educational program, which included four 60-minute sessions. Data were collected again 6 months post-intervention and analyzed using descriptive and inferential statistics in SPSS version 21. Results: The study encompassed women with an average age of 39.75±6.05 years, the majority of whom had educational levels below a diploma, and over 90% were married. We observed strong correlations between the processes of change, self-efficacy, and decisional balance. At the outset, there were no significant differences in demographics or model structures between the 2 groups. However, 6 months post-intervention, the intervention group exhibited statistically significant differences in the mean scores of model structures, stages of change, and body mass index (p<0.05). Conclusions: This study highlights the importance of physical activity training for rural housewives. The findings suggest that the educational intervention, which utilized the TTM, significantly impacted the participants' model structures and their stages of change.

Education and Counseling of Pregnant Patients with Chronic Hepatitis B: Perspectives from Obstetricians and Perinatal Nurses in Santa Clara County, California

  • Background: This study aimed to better understand the barriers to perinatal hepatitis B prevention and to identify the reasons for poor hepatitis B knowledge and delivery of education to hepatitis B surface-antigen-positive pregnant women among healthcare providers in Santa Clara County, California. Materials and Methods: Qualitative interviews were conducted with 16 obstetricians and 17 perinatal nurses in Santa Clara County, California, which has one of the largest populations in the United States at high risk for perinatal hepatitis B transmission. Results: Most providers displayed a lack of self-efficacy attributed to insufficient hepatitis B training and education. They felt discouraged from counseling and educating their patients because of a lack of resources and discouraging patient attitudes such as stigma and apathy. Providers called for institutional changes from the government, hospitals, and nonprofit organizations to improve care for patients with chronic hepatitis B. Conclusions: Early and continuing provider training, increased public awareness, and development of comprehensive resources and new programs may contribute to reducing the barriers for health care professionals to provide counseling and education to pregnant patients with chronic hepatitis B infection.

Effects of Underwater Treadmill Gait Training on Gait, Balance, and Pulmonary Function in Stroke Patients

  • The purpose of this study was to compare the effects of underwater treadmill gait training (UTGT) and overground treadmill gait training (OTGT) on the gait, balance ability and pulmonary function of stroke patients. Twenty subjects were recruited for this study. The subjects were randomly assigned to two groups: UTGT ($n_1$=10) and OTGT ($n_2$=10). The 10 m walk test (10 MWT), Berg Balance Scale, Timed Up and Go (TUG) test, center of pressure, pulmonary function of forced vital capacity (FVC), forced expiratory volume after 1 sec (FEV1) and FEV1/FVC were measured before and after 4 weeks of training. Both groups undertook the gait training for 30 min a day, 3 times a week, for 4 weeks, and rating of perceived exertion of the groups were measured and compared. All the studied variables were significantly improved in both groups (p<.05) at the end of the study, except in the FEV1 of OTGT (p>.05). There was significant between-group difference in all of the variables, except in the 10 MWT (p>.05). These findings suggest that UTGT is more effective than OTGT in improving the balance and pulmonary functions of stroke patients.

공중보건의의 고혈압 진단 및 치료과정 평가 (Quality Evaluation for the Diagnosis and Management of Hypertensives by Pubilc Health Doctors)

  • Background : Little work has been carried out regarding quality assessment research in a primary care setting, comparing with that of hospitals. This study aims to evaluate the process of diagnosis and management of hypertension by public health doctors on the basis of pre-established clinical guideline, and to identify several modifying factors associated with them. Methods : Hypertension was selected as the target disease, because it is a chronic disease which is of great public health importance. Self-administered questionnaires were mailed to public health doctors practicing at health centers and health subcenters across the nation. The response rate was 20.9%. The questionnaire included the diagnosis and management process such as measuring blood pressure, history taking, physical examinations, and treatment approches and potentially modifying factors such as level of training, duration of practice as a public health doctor, and education on management of hypertension. Results : Public health doctors pay little attention in measuring BP, hypertension related history taking, performing physical examination and laboratory examination. But they devoted much effort in diagnosing hypertension exactly and giving nonpharmacological treatment. Among various antihypertensive drugs, calcium-channel blockers were the most preferred agent(50.9%). Level of training, duration of practice ad a public health doctor, and education on management of hypertension made no difference on quality of care(p>0.05). Conclusion : These public health doctors showed poor compliance with the pre-established clinical guidelines, which leaves much to be desired in diagnosing and managing hypertensive patients by public health doctors. This study might be able to contribute to develop some strategies, such as educational programs, which would be able to improve the process of care in hypertensives.

뇌졸중 후 환자의 일상생활활동 및 손 기능 개선에 대한 가상현실 훈련의 효과 : 예비 연구 (The Effects of Virtual Reality Training on Improving Hand Function and Activities of Daily Living in Patients Living Post-Stroke: A Pilot Study)

  • 가상현실 훈련은 신체 기능을 개선하기 위한 중재로 제안되었다. 그러나 급성 뇌졸중 환자의 ADL 개선에 초점을 둔 가상현실 훈련의 효과는 명확하지 않다. 본 연구의 목적은 급성 뇌졸중 환자의 손 기능 및 일상생활활동(ADL)에 대한 가상 현실 훈련의 효과를 조사하는 것이다. 급성 뇌졸중 환자 16명이 본 연구에 포함되었다. 실험 군(VRA군)은 각 세션마다 ADL에 중점을 둔 30분의 가상현실 훈련을 받았으며, 통제군은 30분의 보편적인 가상현실 훈련을 받았다. 피험자의 손기능과 ADL을 조사하기 위해 JTHFT(Jebsen-Tylor Hand Function Test)와 K-MBI (Korean Modified Barthel Index)를 각각 사용했다. 두 그룹 모두 중재 후 마비측과 비마비측의 손기능 및 K-MBI 총점에서 유의미한 개선을 보였다. 실험군은 통제군보다 중재 후 K-MBI의 자가관리 영역에서 유의하게 큰 향상을 보였다. 이러한 결과는 급성 뇌졸중 환자에서 ADL에 중점을 둔 가상 현실 훈련이 기존의 가상 현실 훈련보다 자기관리에 더 나은 영향을 줄 수 있음을 시사한다.

  • This purpose of research is to offer basic materials for analyzing organizational structure in an effort to develop a standardized model that considers regional characteristics with the ultimate goal being the establishment of a National Health and Family Support Center. Research methods include aliterature survey data. The research target is 94 centers run by the National Health Family Support Center training. These are assessed in terms of their regional characteristics. The objectives of this paper are as follows: 1. To look at the present National Healthy Family Support Center's yearly conditions investigate the regional characteristics. 2. To analyze the National Healthy Family Support Center's annual organizational managements and operations characteristics. 3. To analyze the National Healthy Family Support Center's regional characteristics (Urban-only-, Urban-Rural Integration, Rural-only-) in terms of its organizational management and operations. First, at the national, county, and ward levels, Health and Family Support Center can have family intervention purpose. Regional Center should be operated to keep pace with custom of different regions. Standardization can also be beneficial, including considerations such as agricultural needs and a type center.
