A Study on the Occurrences and Preventive Strategies of Accident in Children (초등학생의 안전사고 실태 및 예방전략에 관한 조사 연구)

  • Accidents are important causes of death and disability in children. They also have enormous financial implications. Young children become an victim of accidents easily because of their physical fragileness and their coping behavior being vulnerable to any actions taken by accidents. Once they have a accident, the children whose not fully developed, suffer from devastating long-term after-effects. Lee, Lee, Kang and Han(1995) reported that ninety percent of accidents can be prevented. But there is no national system to manage, evaluate and analyse the information about child accidents, even though it is necessary for accident prevention policies and health promotion of the general public. The purpose of the study was to determine how often children have accidents and define the accident prevention strategies in children. The investigator conducted a descriptive study by performing the surveys, interviews, and workshops for the 2,458 young children, 10 teachers, and 1,494 parents. The data collection for the study began on September 2000 and completed on April 20, 2001. The analysis of the data was done with Window SPSS 10.0 for descriptive statistics. Among those children, 1,298 children(52.8%) injured from accidents. The children who had accident answered that they injured from traffic accident(27.3%), inside the home(26.3%), on the playground(17.0%), during playtime(13.6%), in the school(5.9%) and food poisoning(7.1%). To define accident prevention strategies for the school children, the parents and the teachers who had a special interest in this topic formed a special task force under the guidance of the investigator. The team was charged to prepare the basis of content materials by identifying the problems, setting standards for the program, prioritizing the process, and selecting the methods of implementation and evaluation. Eight issues and concerns identified by the team were: 1)allowing young children to learn undesirable habits and behaviors that would bother others without knowing; 2) not guarding young children from car accident; 3)unattended accident at playground; 4) considering home places safe; 5)unattended accident at school. These issues were found to be coinciding with the actual child accident cases occurred recent years in Korea. Greater efforts are required to reduce unnecessary deaths and disability from childhood accidents. This study gave a very useful and important data to prepare accident prevention educational program and to prepare accident prevention strategies.

Suggestions on Time-saving Processes of Receiving Medicines at the Outpatient Pharmacy in a University Hospital (일개 대학병원의 투약대기시간 단축방안)

  • Background : Many patients have been frequently complaining that they have to spend couples of hours in hospital on visiting outpatient clinic. Among several steps, two major time consuming steps were waiting to see a doctor and/or waiting at pharmacy to get medicine. Therefore not only to provide the proper guidance for medication or counseling on health affairs but also to make waiting time short is very important for the better hospital services. The aim of this study is to validate several time-saving processes to reduce waiting time at outpatient pharmacy and its efficacy. Methods : We surveyed the time interval actually taken to receive medicine after issuing prescription by doctors, and analyzed the data on the bases of relevant or possible causative factors. Then following processes were given to reduce waiting time and resurveyed and compared both data to validate efficacy of those processes : 1. No work-off on Monday and Tuesday 2. Work hour shift to start 30 minutes earlier 3. Changeable work shift between outpatient pharmacy and ward pharmacy according to work load 4. Use of pre-made medicines prescribed more frequently by certain doctors at certain time 5. Cooperation with doctors to use set prescriptions. Results : Before the process, mean waiting time at pharmacy was 29.2 minutes and most time consuming period was from noon to 1 PM, 3 to 4 PM, 1 to 2 PM in order of frequency. Only 37.7 % of patients could get the medicine within 20 minutes. Three times of surveys after process showed mean waiting time at pharmacy were 18.1 minutes, 19.0 minutes, and 17.6 minutes, respectively. And 72.7 %, 81.3%, and 82.2% of patients could get the medicine within 20 minutes. Conclusion : The mean waiting time was markedly reduced with above mentioned processes which applied intradepartmently event hough with little cooperation from other department. Consequently, the complaints of patients were decreased with increasing the satisfaction degree. In conclusion, those suggestions were recommanded to improve the degree of satisfaction of patients.

Designing optimized food intake patterns for Korean adults using linear programming (I): analysis of data from the 2010~2014 Korea National Health and Nutrition Examination Survey (선형계획법을 이용한 한국 성인의 최적 식품섭취패턴 설계 (I) : 2010~ 2014년 국민건강영양조사 자료 분석)

  • Purpose: The aim of this study was to design optimized food intake patterns that meet the nutritional recommendations with minimal changes from the current food intake patterns among Korean adults using linear programming. Methods: Data of a one day 24-hour dietary recall from the 2010 ~ 2014 Korea National Health and Nutrition Survey were used to quantify the food items that Korean adults usually consumed. These food items were categorized into seven groups and 24 subgroups. The mean intakes and intake distributions of the food groups and the food subgroups were calculated for eight age (19 ~ 29, 30 ~ 49, 50 ~ 64, and over 65 years old) and gender (male and female) groups. A linear programming model was constructed to minimize the difference between the optimized and mean intakes of the food subgroups while meeting the Dietary Reference Intakes for Koreans (KDRIs) for energy and 13 nutrients, and not exceeding the typical quantities of each food subgroup consumed by the respective age and gender groups. Results: The optimized food intake patterns, which were a set of quantities of 24 food subgroups, were obtained mathematically for eight age and gender groups. Overall, major modifications of current diet were required to increase the intake of vegetables and milk/dairy products and decrease the Kimchi intake. The optimized intake of seasonings, including salt, was calculated to be 0 g for all the age and gender groups. Conclusion: The optimized food intake patterns designed using linear programming in this study lack feasibility because they suggest a seasoning consumption of 0 g. Modification of intake goal for sodium is needed to obtain optimized food intake patterns with improved feasibility.

  • Objectives: The purpose of this study was to investigate the food, nutrient intake, and diet quality of postmenopausal women at high risk of osteoporosis (OP) and cardiovascular disease (CVD) compared with those of control subjects. Methods: A total of 1,131 post-menopausal women aged over 45 years, who took the 2010-2011 Korean National Health and Nutrition Examination Survey (KNHANES), were included for analysis. These participants were classified into the following groups: the OP group, with a risk of OP (n=135); the CVD group, with a risk of CVD (n=373); the OP+CVD group, with a risk of OP and CVD concurrently (n=218); and the control group (n=405) according to bone mineral density (BMD) and CVD risk. Anthropometric measurements, blood profiles, dietary intake, and dietary quality indices were measured and compared among the four groups. Results: Waist circumference, total body fat percentage, blood pressure, fasting plasma glucose, total cholesterol, triglyceride, and LDL-cholesterol were higher, and HDL-cholesterol and BMD were lower in the OP+CVD group than in the control group. In the food frequency questionnaire, the OP+CVD group had significantly higher frequencies of grain (except for multi-grain) and lower frequencies of fruit and dairy product. The frequency of consumption of red meat, processed meat, and carbonated beverages was higher in OP+CVD group. In nutrient density analysis, proteins and vitamin $B_2$ levels were significantly lower in the OP+CVD group than in the control group. The nutritional quality index (INQ) values of calcium were in the order of 0.63, 0.58, 0.56, and 0.55 in each group, and it was urgent to improve the dietary intake for calcium in postmenopausal women. In addition, vitamin $B_2$ was inadequately consumed by all groups. Conclusions: These results suggest that it is necessary to increase the intake of vitamin $B_2$ and calcium and decrease the frequency of intake of red meat, processed meat, and carbonated beverages in postmenopausal women with the risk of OP and CVD.

Neurocysticercosis: Clinical Characteristics and Changes from 26 Years of Experience in an University Hospital in Korea

  • The prevalence of human taeniasis has decreased in Korea. The stool egg positive proportion decreased from 1.9% in 1971 to 0% in 2004 in nationwide surveys. The neurocysticercosis (NCC) is also presumed to decrease. However, detailed information regarding the recent status of NCC in Korea is lacking. We retrospectively reviewed NCC cases from 1990 to 2016 at Asan Medical Center, a 2700-bed tertiary referral hospital in Korea. We identified patients based on clinical symptoms, brain imaging, pathology and serological assay. The cases were classified as parenchymal, extraparenchymal, and mixed NCC. Eighty-one patients were included in the analysis. The mean age was 54.5 years, and 79.0% were male. The number of NCC cases was highest from 1995 to 1999, and continuously decreased thereafter. Forty (49.4%) patients had parenchymal NCC, while 25 (30.9%) patients had extraparenchymal NCC, and 16 (19.8%) patients had mixed NCC. The seizure and headache were most common symptom of parenchymal NCC and extraparenchymal NCC respectively. Hydrocephalus was more common in extraparenchymal NCC, and patients with extraparenchymal NCC were more likely to require a ventriculoperitoneal shunt. Cases of NCC are decreasing accordingly with human taeniasis and lesion location was the most important determinant of clinical presentation and outcome of NCC in Korea.

Cardiovascular Risk Factors in Diabetic Patients according Duration of Diabetes Mellitus: The Sixth Korea National Health and Nutrition Examination Survey (당뇨병 유병기간에 따른 당뇨병 환자의 심혈관 위험 인자: 국민건강영양조사 6기 자료 이용)

  • The purpose of this study was to provide information on the treatment and management of diabetic patients using all the available data of the Korea National Health and Nutrition Examination Survey (KNHANES, 2013-2015) on the serological and complication patterns of cardiovascular disease (CVD) according to the duration of diabetes mellitus (DM). KNHANES consists of health questionnaires, screenings, and nutrition surveys. The study subjects were 1,316 persons who were aged between 18 and 80 years and who answered the duration of DM. DM was classified as 0-5, 6-10, 11-15, 16-20, and over 20 years according to duration of DM. The longer the duration of DM, the older the BMI, the lower the smoker, the higher the HbA1c and the lower the glomerular filtration rate. Total cholesterol, LDL-C and triglyceride levels were low and HDL-C levels were similar. The proportion of patients receiving insulin and oral hypoglycemic agents increased. The longer the duration of DM, the worse blood glucose control and the increased risk of microvascular and macrovascular complications. To prevent these complications, intensive care and monitoring should be used to control the risk factors.

Impact of Knowledge and Attitude of HIV/AIDS among individuals in their 20s to 40s : An Analysis of the Community Health Survey (20-40대의 HIV/AIDS 관련 지식과 태도에 미치는 영향 : 지역사회건강조사 자료 활용)

  • This descriptive research investigates the knowledge of and attitude toward HIV/AIDS among individuals in their 20s to 40s. This study also analyzes the results of the 2011 and 2013 community health surveys to identify the factors influencing their knowledge and attitude. The study subjects were 20 to 49 years old, and the final analysis included 182,315 subjects. The collected data were analyzed through t-tests, ANOVA, correlational analysis, and multiple regression analysis. The survey results showed a high level of knowledge of HIV/AIDS, with 69.4% correct answers. Most individuals lacked knowledge regarding routes of infection, and those in their 20s had the lowest percentage of correct answers. As for attitude toward HIV/AIDS, 44.3% showed a negative attitude. The scores for knowledge and attitude toward HIV/AIDS were higher among males, unmarried individuals, individuals with a higher income, individuals with a higher level of education, individuals working in agriculture, forestry, or fishery, managers, professions that were not homemakers/unemployed, and professional soldiers. A positive correlation (r=0.27, p<0.001) was observed between knowledge and attitude. The factors influencing AIDS knowledge and attitude were gender, marital status, income level, occupation, and education level. This study confirmed that there is a difference by age group. As such, educational programs customized by age and other characteristics are expected to be more effective in enhancing knowledge and in developing a more positive attitude towards HIV/AIDS.

Status and Prospect of the Elderly Living in Small and Medium-sized cities Using the Senior Citizens' Center (중소도시 지역 거주 고령자의 경로당 프로그램 이용 실태 및 활용 전망)

  • Objective : In this study, we wanted to analyzed the current status of domestic senior center programs to collect data for the development of center programs for small and medium cities. Methods : To collect data, the researchers visited small and medium cities (Gyeonggi, Gyeongsang, Chungcheong, Jeolla and Gangwon) between July and September 2019 and conducted interpersonal surveys. The study involved 183 people. Results : The users mainly used senior centers 'because they of their friends' 55% indicated that they used centers four to five times a week. 45.9% were satisfied with their center, and 20.2% indicated that 'various programs' were needed to make the centers attractive. Of the non-users, 43.5% indicated that they did not use the centers for their lack of 'enjoyable' activities, and 46.5% indicated that 'various programs' should be developed to make the centers attractive. Programs favored by both users and non-users included exercise, hobbies and entertainment and health programs. Conclusion : The study show that the general factors influencing the use of senior centers by senior citizens living in small and medium-sized cities in Korea include the purpose of use, satisfaction with the program, and improvement points and limitations. Accordingly, the limitations of the existing center programs will be identified, and the findings can be used as the basis for the development of future programs.

Lifestyle and dietary changes related to weight gain in college students during the COVID-19 pandemic (COVID-19 유행 동안 대학생의 체중증가와 관련된 생활습관 및 식생활 변화)

  • Purpose: This study aimed to assess the weight fluctuations in college students during the coronavirus disease 2019 (COVID-19) pandemic and identify lifestyle and dietary changes related to weight gain. Methods: An online survey was conducted on 270 college students from September 22 to October 26, 2021. A logistic regression analysis was performed to analyze the relationship of weight gain with the general characteristics, lifestyle, and dietary changes of the students. Results: Among the respondents, 42.9% of men and 44.7% of women reported weight gain. The main reasons given for weight gain were reduced activities due to restrictions during lockdown and diet changes, mainly relating to delivered or fast foods. Among the general characteristics and lifestyle factors poor perceived health (odds ratio [OR], 3.97, 95% confidence interval [CI], 1.98-7.96) and being underweight (OR, 0.19, 95% CI, 0.05-0.83) were significantly associated with weight gain. With respect to the diet, increased frequency of eating breakfasts (OR, 4.44, 95% CI, 1.76-11.21), decreased frequency of eating snacks (OR, 0.35, 95% CI, 0.16-0.77), decreased frequency of fruit intake (OR, 3.0, 95% CI, 1.32-6.80), increased frequency of carbonated and sweetened beverage intake (OR, 2.74, 95% CI, 1.26-5.99) and increased frequency of fast food consumption (OR, 2.32, 95% CI, 1.14-4.70) were significantly associated with weight gain. Conclusion: The COVID-19 pandemic affected weight gain and caused lifestyle and dietary changes. Specific health and nutrition management plans should be prepared for handling future epidemics of infectious diseases based on the results of surveys conducted on larger sample size.

Gender Differences in Trajectories of Successful Aging Indicators: Findings from Korean Longitudinal Study of Aging (다층모형 분석을 활용한 한국 노인의 성공적 노화 지표들의 변화궤적 연구: 남녀 차이 검증을 중심으로)

  • The current study aimed to examine the gender differences in trajectories of nine successful aging indicators (chronic disease, depression, activities of daily living, instrumental activities of daily living, mini mental state examination, social activity, personal contact, health satisfaction, and general life satisfaction) with age, controlling the effect of education. The data were from the Korean Longitudinal Study of Aging, which had been conducted biennially from 2006 to 2016. The sample included 822 men and 1,236 women who responded to all of the panel surveys and were 65 years old or above in 2006. Multilevel modeling analyses showed that older men had fewer chronic diseases; lower levels of depression; higher levels of activities of daily living, cognitive function, and social activity; and better perceived health satisfaction and general life satisfaction at age 65 years compared to women. However, both men and women showed increase in the number of chronic diseases and depression level, and decrease in physical, cognitive, and social functions with age. In addition, perceived health and life satisfaction also decreased after the age of 65. The trajectories of most of the indicators were non-linear, and markedly increased or decreased around mid-70s. Study limitations and implications were further discussed.
