Retrospective Drug Utilization Review Study on the Therapeutic Duplication in Patients with Anxiety Disorders (불안장애 환자에서의 치료약제 중복사용에 대한 후향적 의약품사용평가 연구)

  • The present study was aimed to examine the occurrence and influencing factors therapeutic duplication (TD) of medications for anxiety disorders by analyzing the relevant prescription data. In this study, the prescription data issued on March 19, 2008 in domestic medical institutes were utilized. TD was defined as more than two medications under the same therapeutic classification per prescription based on the Anatomical Therapeutic Classification (ATC) code. The assessment of TD was performed based on the number of cases and on the ratio determined. To identify the influencing factor of TD, the variables related to the differences in the TD ratio were analyzed based on the results of the Chi-Square test conducted with the variables; patients, medical institutes, diseases, and treatments. The number and ratio of TD were determined to be 1,333 out of the total of 19,219 anxiety disorder cases, and 6.94%, most cases involving benzodiazepine derivatives, respectively. The TD ratio was found to be higher in relation to males than to females. Patients with national health insurance benefits have a higher TD ratio compared to the medical-aid beneficiaries. The TD ratios were highest in clinics, psychiatry divisions, and Gyeongsang district. The TD ratio of the cases with more than two anxiety disorders was found to be higher than that of the cases with only one anxiety disorder. As the number of medications per prescription increased, the TD ratio was shown to have become gradually higher. In conclusion, in order to prevent TD, the concurrent DUR system should be implemented. The prescribers and pharmacists must be educated regarding duplicated medications to promote the safe and effective use of medicines, without unnecessary TD.

Consciousness on the Korean Traditional Food of School Food Service Dietitians (한국 전통음식에 대한 학교급식 영양사의 의식 조사)

  • The purpose of this study was to investigate the consciousness of dietitians who provide the basic data for the utilization of traditional food in school food service. The conclusions of this study are as follows. The rate of recognition and awareness of traditional Korean food as the pursuit of learning about socio-demographic was generally high, and was remarkable in the elderly dietitians who have worked for a long term. The way how they learned about traditional Korean food was through media, school education, books, and home education, in order. Also, the dietitians who are relatively older or have worked for a long term have pride in traditional food because it suited their taste and was our traditional food. However, it was difficult to provide the food to school food service because the cooking process is complex, and students don's prefer it. Accordingly, improvements of recipe with use of traditional Korean food in school food service are urgently required. The dietitians in Gwangju City in Jeonnam province felt the need to make a standard traditional Korean food recipe, and the demand was especially the strongest among dietitians who have worked from 6 to 10 years. They suggested that the standard recipe should be made by the Korean Dietetic Association, dietitians working at school, Ministry of Education & Human Resource Development, professors majoring in Food & Nutrition, and Ministry of Health & Welfare in order.

The Survey of Dental Hygiene Student's Consciousness about the Disabled (치위생과 학생들의 장애인에 대한 인식도에 관한 조사연구)

  • The purpose of this study was to investigate consciousness about the disabled on dental hygiene students and search for course of dental hygiene education as oral health care with the disabled. The subjects in this study were 538 dental hygiene students from three college in Jeonnam province. For statistical analyses of collected data, the descriptive analyses and chi-square were adopted. The obtained results were as follows. Seniors showed positive attitude about the disabled and dental service of the disabled(p<0.05). The common types of contact the disabled were physical Cerebral palsy and inner disabled were rarely. The problems are difficulty in communication, short of clinical experience in dental service of the disabled, noncooperation of the disabled, short of knowledge in dental service of the disabled. To dental service of the disabled, demand as necessity of education was 87.5%, theory and practice at the same time was 68.0% in pattern of education. Finally, 49.5% of respondents observed special dental clinic to utilization pattern of dental service of the disabled.

Successful Schooling Rate and Satisfaction of the Inpatient Hospital School Participants among the Child and Adolescent Psychiatric Inpatients in the Closed Ward (정신건강의학과 안정병동 내 병원학교교실 참여자의 학교복귀율과 만족도)

  • Objectives : The objective of this report is to identify the utilization of hospital school service during hospitalization among patients in their childhood and adolescence with psychiatric disorders. Methods : We retrospectively reviewed the medical record of child and adolescent psychiatric who were hospitalized during March 2009 through October 2012. We compared the one-year successful schooling and outpatient follow up rate between users and nonusers of the inpatient hospital school service. The hospital schooling experiences of the users were investigated upon follow-up visits to the outpatient clinic. Results : Sixty-three students received hospital school service during hospitalization among total 122 child and adolescent inpatients. Hospital school participants showed a significantly higher school reentry rate (61.9%) than non-participants (40.7%). However, there was no difference on follow up rate between the two groups. More than 60% of the 22 interviewed participants expressed an above-average level of satisfaction about hospital school service. Conclusion : Many patients with mental illness experience difficulty in receiving school education during treatment. That induces deterioration in disease, academic failure, poor social skills, low self-esteem, economic difficulties, and future job opportunities. The results of this study emphasize the importance of hospital school service and offer useful guidance for hospital school operation.

Influence of social network service(SNS) information on intention to dental office use (SNS 정보 활용이 치과의료기관 이용의사에 미치는 영향)

  • Objectives: The purpose of the study is to investigate the effect of social network service(SNS) information on intention to dental office use. Methods: A self-reported questionnaire was completed by 410 users of social network service between twenty and thirty years old in Seoul and Gyeonggido after receiving informed consent from July 1 to August 3, 2015. The questionnaire consisted of general profiles of the subjects(6 items), use of dental office(3 items), use of SNS(3 items), use of dental SNS(4 items), use of general SNS(6 items), application of dental SNS(6 items), and intention to dental office use(2 items). Except 8 incomplete answers, 402 data were analyzed using SPSS 18.0 program. Results: The first choice of dental office was recommendation by the acquaintances. There were close correlations between the use of SNS information and the use of general SNS information according to sex(p=0.016) and the frequency of SNS(p=0.012). The use of SNS for dental information showed a meaningful correlation with sex(p=0.003). The intention to use was influenced by sex(p=0.016) and the use of SNS for dental information(p<0.001). Conclusions: The important factor of visit intention to dental service was recommendation by the acquaintances through SNS. The appropriate advertisement of visit intention is very important to improve dental health care.

The Impact of Weekend Admission and Patient Safety Indicator on 30-Day Mortality among Korean Long-Term Care Insurance Beneficiaries

  • Background: This study investigates the impact of weekend admission with a patient safety indicator (PSI) on 30-day mortality among long-term insurance beneficiaries. Methods: Data were obtained from the National Health Insurance Service-Senior claim database from 2002 to 2013. To obtain unbiased estimates of odds ratio, we used a nested case-control study design. The cases were individuals who had a 30-day mortality event after their last medical utilization, while controls were selected by incidence density sampling based on age and sex. We examined the interaction between the main independent variables of weekend admission and PSI by categorizing cases into four groups: weekend admission/PSI, weekend admission/non-PSI, weekday admission/PSI, and weekday admission/non-PSI. Results: Of the 83,400 individuals in the database, there were 20,854 cases (25.0%) and 62,546 controls (75.0%). After adjusting for socioeconomic, health status, seasonality, and hospital-level factors, the odds ratios (ORs) of 30-day mortality for weekend admission/PSI (OR, 1.484; 95% confidence interval [CI], 1.371-1.606) and weekday admission/PSI (OR, 1.357; 95% CI, 1.298-1.419) were greater than for patients with weekday admission/non-PSI. Conclusion: This study indicated that there is an increased risk of mortality after weekend admission among patients with PSI as compared with patients admitted during the weekday without a PSI. Therefore, our findings suggest that recognizing these different patterns is important to identify at-risk diagnosis to minimize the excess mortality associated with weekend admission in those with PSI.

Changes in the Health Behaviors and Eating Habits of University Students Due to the COVID-19 Pandemic (COVID-19로 인한 대학생의 건강행태 및 식생활 변화)

  • This study investigated alterations in the health behaviors and eating habits due to the coronavirus disease 2019 (COVID-19) pandemic. An online survey was administered to 270 university students from September 22, 2021 to October 26, 2021. While the frequency of eating at home had increased during the COVID-19 pandemic compared to before the pandemic, there was a decrease in the frequency of eating out, drinking, and eating vegetables. However, the frequency of eating delivery, takeout, and convenience foods and the utilization of online shopping and delivery apps had considerably increased. In addition, when selecting menus, considerations of health, hygiene, and convenience were more important during the COVID-19 pandemic than before the COVID-19 pandemic. Physical activity more than 3 days a week had decreased, whereas sedentary time showed a proportionate increase. The percentage of people who perceived their health status to be worsening was determined to have increased during the COVID-19 pandemic. To prepare for the era of infectious diseases, future research needs to identify health behaviors and dietary problems by administering surveys that include a large sample size and participants of various ages. Moreover, health promotion and nutrition management plans should be prepared accordingly.

A study on the applications and prospects of big data in the field of digital dental healthcare (디지털 덴탈 헬스케어 분야에서의 빅데이터 활용 전망에 대한 연구)

  • Purpose: The purpose of this study is to investigate the applications and prospects of big data in digital dental healthcare. Methods: The study included 30 participants in the dental field (dentists, technicians, professors, and graduate students). From June 25 to 30, 2023, the contents of the study were thoroughly explained, consent was obtained from the research subjects, and a questionnaire was administered via an internet service. The questionnaires of 28 participants who responded completely were used for analysis. The collected data were statistically processed using IBM SPSS Statistics ver. 22.0 (IBM). Results: The use of big data in digital dental healthcare, digital dental health system, mobile dental health, dental health analysis, and telehealthcare were all heavily surveyed, with an average score of 3.97 or higher on a 5-point Likert scale. The areas where big data can be utilized in digital dental healthcare are as follows. The utilization rate for three-dimensional digital product development via linkage with big data systems and industrial field manufacturing technology was found to be 4.11±0.67, and the analysis of trends by age in the occurrence of various oral diseases was found to be 4.00±0.98. Conclusion: In the future, research into the viability of big data's success in the medical data field, which is directly related to human life, is needed. Additionally, social policies and regulations regarding big data-related information and standards in dental healthcare are necessary.

Analysis of Medical Use and Treatment Costs of Hepatocellular Carcinoma Patients Using National Patient Sample Data (환자표본자료를 이용한 간세포암종 환자의 의료이용 특성 및 치료별 의료비용 분석)

  • Background: With increasing economic evaluation studies on the treatment of or screening tools for liver diseases that cause hepatocellular carcinoma (HCC), interest in the analysis of the medical utilization and costs of HCC treatment is increasing. Therefore, we aimed to estimate the medical utilization and costs of HCC patients, and calculate the cost of main procedures for HCC treatment, including liver transplant (LT), hepatic resection (HR), radiofrequency ablation (RFA), and transarterial chemoembolization (TACE). Methods: We analyzed claim data from January to December 2018 from the Health Insurance and Review and Assessment Service-National Patient Sample (HIRA-NPS-2018) dataset, including data of patients diagnosed with HCC (Korean Standard Classification of Diseases code C22.0) who had at least one inpatient claim for HCC. Results: A total of 715 HCC patients were identified. In 2018, the yearly average medical cost per HCC patient was ₩18,460K (thousand), of which ₩14,870K was attributed to HCC. Among the total medical costs of HCC patients, the inpatient cost accounted for the largest portion of both the total medical and HCC-related costs. The major procedures of HCC treatment occurred most frequently in the order of TACE, RFA, HR, and LT. The average medical cost per treatment episode was the highest for LT (₩87,280K), followed by HR (₩10,026K), TACE (₩4,047K), and RFA (₩2,927K). Conclusion: By identifying the medical costs of HCC patients and the costs of the main procedures of HCC treatment, our results provide basic information that could be utilized for cost estimation in liver disease-related economic evaluation studies.

Development of a Hospital Service-based Costing System and Its Application (병원서비스별 원가분석모형의 개발과 적용)

  • The managerial environment of hospitals in Korea characterized by low levels of medical insurance fees is worsening by increasing government regulations as to the utilization of medical services, rising costs of labor, material, and medical equipments, growing patient expectations concerning the quality of services, and escalating competitions among large hospitals in the market. Hospitals should seek for their survival strategies in this harsh environment and they should have information about costs of their products in doing so. However, it has not been available due to the complexity of the production process of hospital services. The objectives of this study were to develop a service-based cost accounting model and to apply the developed model to a study hospital to obtain cost information of hospital services. A model commonly used for the job-order product cost accounting in the manufacturing industry was modified for the use in hospitals in Korea. Actual costs, instead of standard costs, incurred to produce a unit of services during a given period of time were estimated in the model. Data required to implement the model included financial information, statistics for the allocation of supportive cost center costs to final cost centers, statistics for the allocation of final cost center costs to services, and the volume of each services charged to patients during a study period. The model was executed using data of a university teaching hospital located in Seoul for the fiscal year 1992. Data for financial information, allocation statistics fo supportive service costs, and the volume of services, most of them in electronic form, were available to the study. Data for allocation statistics of final cost center costs were collected in the study. There were 15 types of evaluation and management service, 2, 923 types of technical service, and 2, 608 types of drug and material service charged to patients in the study hospital during the fiscal year 1992. Labor costs of each of seven types of pesonnel, material costs of 611 types of drugs and materials, and depreciation costs of 212 types of medical equipments, miscellaneous costs, and indirect costs incurred in producing a unit of each services were estimated. Medical insurance fees for basic services such as evaluation and management of inpatients and outpatients, injection, and filling prescriptions, and for operating procedures were found to be set lower than costs. Infrequent services which use expensive medical equipments showed negative revenuse as well. On the other hand, fees for services not covered by the insurance such as CT, MRI and Sonogram, and for laboratory tests were higher than costs. This study has a significance in making it possible for a hospital to obtain cost information for all types of services which produced income based on all types of expenses incurred during a given period of time. This information can assist the management of a hospital in finding an effective cost reduction strategy, an efficient service-mix strategy under a given fee structure, and an optimum strategy for within-hospital resource allocations.
