Satisfaction Determinants of Rehabilitation Care Group Inpatients in Rehabilitation Long-term Care Hospitals (재활요양병원의 재활군 환자의 만족도 요인에 관한 연구)

  • Objectives : The aim of this study was to investigate the satisfaction determinants of rehabilitation care group inpatients in rehabilitation long-term care hospitals. Methods : A survey was conducted with 262 patients from 6 rehabilitation long-term care hospitals located in Busan Kyungnam. Data were collected from June 2016 to July 2016 with a structured questionnaire and analyzed with SPSS 23.0. Results : The significant factors affecting the overall satisfaction score were disease group and 5 quality indices of medical services(Medical service quality, Rehabilitation services quality, Hospital reputation, External physical environment, and Internal physical environment). Additionally, the significant factors affecting intention to revisit were age, disease group, length of hospital stay and the 5 quality indices of medical services. Conclusions : The results of this study indicate that rehabilitation long-term care hospitals should try to improve the overall satisfaction of inpatients, especially focusing on the internal physical environment and the quality of the rehabilitation services and medical services, which will lead to a high level of intention for patients to revisit.

Evaluation of Hospital Information System Based on the Performance Reference Model (병원정보화 평가를 위한 PRM 기반의 체계 개발 및 적용)

  • The purpose of this paper was to evaluate performance of information system for one national university hospital in order to identify the factors influencing performance of information system. KPIs were collected for 181 users of information system (41 doctors, 104 nurses, and 11 medical supporting staffs, and 25 administrative staffs) from August 10 to 24, 2010. The results were as follows: Average performance score for input layer was 3.16; average performance score for process layer was 3.35; and average performance score for business layer was 3.57. Scores for input layer was lowest for nurses and scores for process and business layer were lowest for doctors. Results from the path analysis showed that system quality, demographic characteristics, and security significantly influenced management process but these factors except demographic characteristics influenced user satisfaction; and management process also significantly influenced user satisfaction.

How to Sustain Smart Connected Hospital Services: An Experience from a Pilot Project on IoT-Based Healthcare Services

  • Objectives: This paper describes an experience of implementing seamless service trials online and offline by adopting Internet of Things (IoT) technology based on near-field communication (NFC) tags and Bluetooth low-energy (BLE) beacons. The services were provided for both patients and health professionals. Methods: The pilot services were implemented to enhance healthcare service quality, improve patient safety, and provide an effective business process to health professionals in a tertiary hospital in Seoul, Korea. The services to enhance healthcare service quality include healing tours, cancer information/education, psychological assessments, indoor navigation, and exercise volume checking. The services to improve patient safety are monitoring of high-risk inpatients and delivery of real-time health information in emergency situations. In addition, the services to provide an effective business process to health professionals include surveys and web services for patient management. Results: Considering the sustainability of the pilot services, we decided to pause navigation and patient monitoring services until the interference problem could be completely resolved because beacon signal interference significantly influences the quality of services. On the other hand, we had to continue to provide new wearable beacons to high-risk patients because of hygiene issues, so the cost increased over time and was much higher than expected. Conclusions: To make the smart connected hospital services sustainable, technical feasibility (e.g., beacon signal interference), economic feasibility (e.g., continuous provision of new necklace beacons), and organizational commitment and support (e.g., renewal of new alternative medical devices and infrastructure) are required.

Differences of Organizational Health and Customer Orientation by the Hospital Ownership (병원설립주체별 간호조직건강성이 고객지향성에 미치는 영향)

  • Objectives : This study was conducted to verify the influence organizational health has on customer orientation by hospital ownership. Methods : This survey was conducted on a public hospital, private hospital, and religious hospital, respectively, during March 2014 and a total of 210 questionnaires were used in the analysis. ANOVA and regression analysis were used to analyze the collected data. Results : The management environment factor of organizational health was statistically significant in the public hospital compared to the private hospital. The reliability factor of customer organization was statistically significant in the public hospital and religious hospital respectively compared to the private hospital. For reactivity, the religious hospital was statistically significant compared to private hospital. A positive influence was shown in the vitality factor at the public hospital, and community orientation at the private hospital, while both community orientation and career showed effects at the religious hospital. ICU had a negative influence on customer orientation at the private hospital. Conclusions : The reason vitality had a relatively more important effect on customer orientation in the public hospital was due that age group of 20~29 was the dominant socio-demographic factor. Furthermore, the influence of community orientation on customer orientation in the private hospital and religious hospital may be related to the nurses' career.

  • This study is to investigate the effects of personal characteristics and perceived interactivity on acceptance of mobile health information service by applying Technology Acceptance Model (TAM). Also it examine whether the perceived interactivity, which is a key variable in the previous research studying the intention of PC-based health information service, works on the mobile environment equally. As a result of the analysis, the relationship between the health hypochondria of personal characteristics and perceived interactivity is negative, and the health concern and perceived interactivity is positive. Also, personal characteristics influence perceived usefulness and perceived ease of use of mobile health information service through perceived interactivity. Therefore, the perceived interactivity in the health information service of web and mobile environment is an important variable to mediate or directly affect other variables.

Analysis of Status and Success Factor of Referral and Return of Patients to Clinics: Focusing on Patients with Endocrinology and Cardiology at a General Hospital in Goyang (진료회송 사업 현황 및 성공요인 분석: 고양시 소재 종합병원급 내분비내과와 심장내과 환자를 중심으로)

  • Background: This study aimed to identify the characteristics of the referral and return of patients to clinics in the endocrinology and cardiology departments at the National Health Insurance Service Ilsan Hospital to evaluate the "referral and return of patients to clinics" program and reduce the rate of returning patients. Methods: From May 2018 to December 2020, we identified the number of visits to referral hospitals and hospital usage status at Ilsan Hospital after returning to clinics. We also identified the patients who returned to Ilsan Hospital within 6 months, defined as "failure to transport," among those recommended to be transported to clinics of the Medical Cooperation Center. Additionally, we evaluated the characteristics of the "failure to transport" patients. Results: Among the returning patients, the rate of visiting Ilsan Hospital within 6 months was higher in cardiology than in endocrinology (25.1% vs. 16.7%). Older age, more severe disease, and more number of visits to the department were associated with a high rate of failure to transport. The rate of failure to return was low in cases diagnosed with hyperlipidemia/lipoprotein metabolism disorder. With respect to diabetes, the rate of failure to transport differed according to each type of diagnosis of diabetes. Conclusion: The success rate of the "referral and return of patient to clinics" program differed based on each patient's characteristics, department of visit, and diagnosis. Individualizing according to the visit department and diagnosis is required to ensure successful transfers, and infrastructure expansion and institutional arrangements must be facilitated.

The Utilization of Health Service by Psychiatric Outpatients (정신과 외래환자들의 의료이용에 관한 연구)

  • Objective of the study represents experiences of medical utilization of Psychiatric Outpatients. This research draws on information obtained from the 2008 National-wide sampling study of the Korean Healthcare Panel(KHP) conducted. The results of our study suggest the significance of variables such as the gender(p<.001) in the social demographical characteristics, the medical security type(p<.016), medical institution utilization(p<.012) treatment type(p<.004) in the utilization factors. In the case of medical utilization cost, the financial support factor(p<.018) showed a significant relationship. Depending on the particularities of gender, medical security type, financial support, medical institution utilization, treatment type were determined through a multiple logistics regression analysis. Gender showed that, among Psychiatric outpatient age of 30-59 level was 5.358 times and 60 years and older, their the second medical examination level was 4.490 times higher than Psychiatric outpatients under the age of 29. Health insurance type showed for the others medical allowance was 6.712 times higher than job health insurance and the other treatment was 0.395 times lower than drug treatment. Psychiatric outpatients utilization are rooted in the thoughts and factor levels of the Psychiatric patients and must be understood from the Psychiatric outpatients's perspective. The point is not only to gauge the patients research during Psychiatric medical utilization. The important of social community mental healthcare services levels goes beyond the goal of enhancing healthcare.

The Satisfaction Analysis for Patient Care Service in National University Hospitals (국립대학병원의 환자 만족도 분석 - 7개 국립대학병원의 입원.외래간 비교 -)

  • The objectives of this research are 1) to identify different motivation of choice for each National University hospitals 2) to examine the factors influencing the patients' satisfaction, 3) and to investigate that the patients' satisfaction effects the hospital re-choice and recommendation on seven National University's hospitals. The data for this analysis were collected by questionnaire survey. Totally 852 interviews were conducted. The major statistical methods used for the analysis are paired t-test, factor analysis, x2-test, and multiple regression. We find that satisfaction level is a function of not only the quality of medical service but also non-medical service. The main results of research are follows: 1) The most important factor of hospital choices is 'the trust and fame as National University hospitals'; 2) Overall satisfaction of inpatients is turned to be influenced by the doctor's services, nurse's services, environment, and convenience. And overall satisfaction of outpatients is also found to be influenced by the waiting time for lab test; 3) Highly satisfied patients preferred to revisit and to recommend to others.

The Effect of Reform of New-Diagnosis Related Groups (KDRGs) on Accuracy of Payment (신포괄수가 시범사업 모형 개선 이후의 지불정확도 변화)

  • Background: Korea set up new diagnosis related group (DRG) as demonstration project in 2009. The new DRG was reformed in 2016. The main purpose of study is to identify the effect of reform on accuracy of payment. Methods: This study collected inpatient data from a hospital which contains medical information and cost from 2015 to 2016. The dependent variables were accuracy of total, bundled, unbundled payment, and payment for procedures. To analyze the effect of reform, this study conducted a multi-variate regression analysis adjusting for confounding variables. Results: The accuracy of payment increased after policy reform. The accuracy of total, bundled, unbundled payment, and payment for procedures significantly increased 3.90%, 2.92%, 9.03%, and 14.57% after policy reform, respectively. The accuracy of unbundled payment showed the largest increase among dependent variables. Conclusion: The results of study imply that policy reform enhanced the accuracy of payment. The government needs to monitor side effects such as increase of non-covered services. Also, leads to a considerable improvement in the value of cost unit accounting as a strategic play a role in development of DRG.

Performance of Occupational Health Services by Type of Service (사업장 보건관리사업의 형태별 수행성과 분석 -실적, 수혜도, 영향요인을 중심으로-)

  • Occupational health services in Korea have been operated as dual types: one is operated by occupational health care manager and the other is health care agency without their own personnel. The performance of occupational health service should be different due to the variety of characteristics of health care manager and workplace, qualification of health care manager. This study is to analyze performance of occupational health care services with a particular consideration of job, based on comparing those two types of health care management to show on the basic data for the settlement of more qualitative. health care management system at workplace. For this study, total 391 places in Seoul and Inchon city area: 154 places (39.4%) managed by designated. health care manager and 237 places (60.6%) by the agency with their commission are selected as research samples. Tools for data collection are questionnares have been investigated during the period of 20 September 1993-20 December 1993. Those data are compared with percentiles, mean, standard deviation due to the characterstics of each variable and analyzed for impacting factors with relation to the using multiple regression analysis using SPSS PC program, especially using t -test method in order to compare each type of health care management. Conclusions observed from the tests and each comparison could be summerized as follows : 1. Occupational health care have been accomplished at workplaces with designated people than with agencies people, and coverage rate of the occupational health care services has differences, due to management types. The reason of these results is due to visit only one or two times monthly by the agencies, while their own health care manager obsess, at the workplaces all the times. 2. The common sickness management is the most accomplished item in health care area of occupational health care services, while the preventive care and control for the workers who have serious health problems are insufficient in workers health care area. 3. An insufficient accomplishment of overall health education has been shown because it is difficult to perform health education due to almost no chance of the direct introduction at workplaces. Therefore a strong support system for making and supplying the media is necessary in order to activate indirect health education by means of media. 4. Because health care managers and the agencies managers where take the workplaces for this study are almost nurses who have been comparatively high work site rounding rate about an environmental management at the workplaces, that non-profession can also do it, the activities about the professional area not enough. Therefore, an appropriate referral system should be established in order to complement professional area. 5. Two factors which have an effect on the coverage rate of occupational health care services are : one is those from the workplaces such as type of services, the number of workers, the number of harzadous factors and safety & health committee, the other from health care organization about whether there is its own manager or not.
