  • The purpose of this study was to select the manageable diseases which are prevalent in Korea for periodic health examinations and to evaluate the sensitivity and specificity of periodic health examination in detecting these diseases. The data was derived from reimbursement data of the Korea Medical Insurance Corporation over a two year period (April 15, 1986 to April 14,1988) and interviews with Yonsei Medical School professors. This study demonstrated that: 1. The manageable diseases which were selected for periodic health examinations are pulmonary tuberculosis, viral hepatitis, diabetes mellitus, syphilis groups, essential hypertension, renal disease groups, and iron deficiency anemia. 2. The sensitivity of the health screening for each disease was as follows: pulmonary tuberculosis 51.5%, viral hepatitis 60.3%, diabetes mellitus 64.7%, syphilis groups 63.3%, essential hypertension 49.9%, renal disease groups 44.0%, and iron deficiency anemia 80.9%. We conclude that peiodic health esaminations should focus on the manageable diseases that we have defined. The number of items in periodic health screening tests and hospital quality control should be increased for the diseases with a demonstrated low sensitivity.

  • Objectives The aim of this study is to evaluate radiation exposure resulting from the comprehensive health examinations of selected university hospital programs and to present basic data for research and management strategies on the health effects of medical radiation exposure. Methods Radiation-based diagnostic studies of the comprehensive health examination programs of ten university hospitals in Seoul, Korea, as introduced in their websites, were analyzed. The medical radiation studies of the programs were reviewed by radiologists. Only the effective doses of the basic studies were included in the analysis. The optional studies of the programs were excluded. Results Among the 190 comprehensive health examination programs, 132 programs (69.5%) included computed tomography studies, with an average of 1.4 scans. The average effective dose of radiation by program was 3.62 mSv for an intensive program for specific diseases; 11.12 mSv for an intensive program for cancer; 18.14 mSv for a premium program; and 24.08 mSv for an overnight program. A higher cost of a programs was linked to a higher effective dose (r=0.812). The effective doses of the examination programs for the same purposes differed by as much as 2.1 times by hospital. Inclusion of positron emission tomography-computed tomography was the most critical factor in determining the level of effective dose. Conclusions It was found that radiation exposure dose from comprehensive health exam programs targeted for an asymptomatic, healthy public reached between 3.6 and 24 times the annual dose limit for the general public. Relevant management policies at the national level should be provided to minimize medical radiation exposure.

  • The purpose of this study is to develop a healthcare service based on standard protocol and information communication technology for mother's sustainable postpartum care. The developed service was consisted of a client area where mothers measure, manage and transmit their vital signs using their own smartphone and personal health devices, and a server area that manages and shares with the received mother's vital signs and the results of examination results and personal health records. The client area collects vital signs through the IEEE 11073 Personal Health Device (PHD) using the m-health application of the previous study and Continua Health alliance certified personal health devices and transfers to Health Level Seven (HL7) V2.4, Continuity of Care Record (CCR) and Continuity of Care Document (CCD). The server area consists of a mobile web that manages and shares the HL7 Fast Healthcare Interoperability Resources (FHIR)-compliant personal health records to ensure interoperability of examination results, and a mobile web where the postpartum caregiver enters and manages the results of the mother's examination results and provides it to the mother. In this way, the healthcare service of this study securing continued exchanges between the mother and postpartum caregiver improves the quality of life of the mother not only to satisfy the needs of the mother who was discharged but also through self-management and postpartum. In the future, we will conduct a study applying mothers and postpartum caregiver after approval of a clinical trail at a university hospital to evaluate developed healthcare services.

  • Stroke is the second cause of death worldwide, although the survival period is increasing after the occurrence of stroke, severe physical disability is caused with aftereffect. Oral inflammation is not limited to the oral cavity, it can cause malignant changes in other tissues and organs. In previous studies, we confirmed the relationship between tooth loss and stroke due to periodontal inflammation. The purpose of this study was to investigate the relationship between oral health and stroke such as oral hygiene behavior, tooth loss and periodontal disease among Korean adults over 40 years of age. This study was analyzed using the 6th Korea National Health and Nutrition Examination Survey (KNHANES) data. A total of 3,389 adults over 40 years of age were analyzed as final subjects. Socioeconomic statuses and oral health status was analyzed using a complex sample analysis technique. Logistic regression was used to analyze the relationship of oral health and stroke, and 95% confidence intervals were computed using SPSS. When the prevalence of stroke according to oral hygiene behavior was checked, the prevalence of strokes was lower in subjects who had a lot of brushings per day and subjects who used oral hygiene products (p<0.05). The risk of stroke was 2.17 times (95% confidence interval, 1.43~3.28) higher in the group with less than 19 remaining teeth, but it was not statistically significant as a result of adjusting for age and sex, income level, education level, drinking and smoking (p>0.05). Loss of teeth was found to be associated with the risk factor of stroke. Therefore, loss of teeth due to periodontal disease is an additional issue that should be considered as a risk factor for stroke.

  • Background: Breast cancer is one of the most common cancers and a major public health problem in developing countries. However, early detection and treatment may be achieved by breast self-examination (BSE). Despite the importance of BSE in reducing the incidence of breast cancer and esultant deaths, the disease continues to be the most common cause of cancer death among women in Iran. This study aimed to determine the effects of self-care education on performance of BSE among women referring to health centers in our country. Materials and Methods: This quasi-experimental interventional study with pretest/posttest control group design was conducted on 168 women referred to health centers. The data were collected using a validated researcher-made questionnaire including demographic variables and trans-theoretical model constructs as well as a checklist assessing BSE behavior. The instruments were administered to groups with and without self-care education before, a week after, and 10 weeks after the intervention. Then, the data were entered into the SPSS statistical software (version 19) and analyzed using independent sample t-tests, paired sample t-test, repeated measures ANOVA, Chi-square, and Friedman tests (p<0.05). Results: The results showed an increase in the intervention group's mean scores of trans-theoretical model constructs (stages of change, self-efficacy, decisional balance, and processes of change) and BSE behavior compared to the control group (p<0.001). Conclusion: The study confirmed the effectiveness of aneducational intervention based ona trans-theoretical model in performing BSE. Therefore, designing educational interventions based on this model is recommended to improve women's health and reduce deaths due to breast cancer.

  • BACKGROUND/OBJECTIVES: This study was conducted to evaluate the feasibility of a sodium reduction program at local restaurants through nutrition education and examination of the health of restaurant owners and cooks.SUBJECTS/METHODS: The study was a single-arm pilot intervention using a pre-post design in one business district with densely populated restaurants in Seoul, South Korea. The intervention focused on improving nutrition behaviors and psychosocial factors through education, health examination, and counseling of restaurant personnel. Forty-eight restaurant owners and cooks completed the baseline survey and participated in the intervention. Forty participants completed the post-intervention survey. RESULTS: The overweight and obesity prevalences were 25.6% and 39.5%, respectively, and 74.4% of participants had elevated blood pressure. After health examination, counseling, and nutrition education, several nutrition behaviors related to sodium intake showed improvement. In addition, those who consumed less salt in their baseline diet (measured with urine dipsticks) were more likely to agree that providing healthy foods to their customers is necessary. This study demonstrated the potential to reduce the sodium contents of restaurant foods by improving restaurant owners' and cooks' psychological factors and their own health behaviors. CONCLUSIONS: This small pilot study demonstrated that working with restaurant owners and cooks to improve their own health and sodium intake may have an effect on participation in restaurant-based sodium reduction initiatives. Future intervention studies with a larger sample size and comparison group can focus on improving the health and perceptions of restaurant personnel in order to increase the feasibility and efficacy of restaurant-based sodium reduction programs and policies.

  • Objectives: Dental caries and periodontitis are major oral bacterial infections associated with dental plaque. Infection and inflammation play a role in carcinogenesis, and a significant link has been found between some highly prevalent oral and dental diseases and some types of cancer. Therefore, the aim of this study was to evaluate the relationship between cancer and oral health among Korean adults. Methods: Data from the 6th Korea National Health and Nutrition Examination Survey were used to analyze the incidence of cancer according to oral health. Demographic and socioeconomic statuses and oral behavior of the participants was analyzed using complex chi-square tests. Logistic regression was used to analyze the relationship between a high DMFT index and cancer by calculating the 95% confidence intervals. Results: In total, 6,450 case-control subjects were included. Significant difference was observed in the DMFT index, but not in periodontal diseases, between the cases and controls. The odds ratio for cancer was 1.80 (95% confidence interval: 1.18 to 2.73) in the high-risk group. However, no significant difference was observed after adjusting for age, education level, and income. Conclusions: The results provide potential evidence of a significant association between cancer and oral disease.

  • Purpose: The purpose of this study was to identify knowledge, attitude, and practice of middle-aged women breast self-examination, and to develop a breast self-examination promotion program for them. Method: The subjects of the study were 365 middle-aged women in Seoul, Busan, Daegu, Daejeon, Gyeongbok, Gyeongnam, Jeonbuk, Gangwon, and Kyonggi. The instruments used in this study included knowledge (l6items), attitude (22items) by Choi (1996) and practice (1item). The data were collected from September 15 to October 31, 2001 by using self-administered questionnaire. The data were analyzed by using descriptive statistics, Pearson correlation coefficient, $x^2$ test, t-test, ANOVA and Scheffe test with SPSS program. Results: 1) 78.4% of the subjects were educated for breast self-examination. The most common source of knowledge for the breast self-examination was mess media. 2) The average score of knowledge on breast self-examination was 8.52 points. The average score of attitude toward breast self-examination was 3.08 points. 58.3% of the subjects practiced breast self-examination. But only 9.4% of the subjects practiced breast self-examination with a monthly basis. 3) Knowledge on breast self-examination was correlated with demographic variables including residence (F=2.923, p=0.004), education (F=5.145 p=0.000), number of children (F=5.125 p=0.002), family income (F=6.128 p=0.002), and menopause (t=-2.330, p= 0.020). Attitude toward breast self-examination was correlated with demographic variables including residence (F=3.611, p=0.000), age (F=2.731 p= 0.029), education (F=4.480 p=0.004), and family income (F=5.963 p=0.003). Practice of breast self-examination was correlated with demographic variables including residence ($x^2$=51.342, p=0.000), education ($x^2$=19.896, p=0.000), and menopause ($x^2$=9.841, p=0.000). 4) In terms of correlations among the scores of breast self-examination related knowledge, attitude, and practice, there was a correlation between knowledge and attitude. But there was no correlations between knowledge and practice, and between attitude and practice of breast self-examination. Conclusion: This study suggests that programs about the practice of breast self examination need to be developed in the future.

  • Objectives : This study analyzed the factors that determine the use and type of medical examination services (MES) to develop a model explaining the use behavior of MES which could consequently contribute to policy implications for medical examinations. Methods : Based on Anderson's healthcare utilization model, the MES utilization model was developed by including the perceived needs for MES. The data were collected from an online survey of a population aged 20-39 years and from a telephone survey of a population aged 40 years or older, respectively. Chi-Square tests and hierarchical logistic regression analyses were done with SAS version 9.3. Results : Generally, as health status became lower, the use of MES increased. However, patients with two or more chronic diseases were less likely to use private MES compared to patients with one chronic disease. The perceived needs for MES were only related to the use of service and not to the choice of the MES type. Conclusions : There were different results for the significant determinants between the use of the MES and the choice of the MES type. The healthcare industry needs to aware of consumer needs to provide MES based on empirical findings.
