Determinants of Contingent Workers' Ratio in Public Health Centers (보건소 비정규직 고용 비율에 영향을 미치는 요인)

  • Objectives : This study investigates the determinants of contingent workers' ratio in public health centers. Since the economic crisis in 1997, there have been many studies on contingent workers in Korea. But, previous studies have been not conducted focusing on public health center. Methods : This study used 253 public health centers, installed and operated since December 31, 2008. in Korea as units of analysis. To examine the determinants of contingent workers' ratio, this study uses Pearson correlation and multiple regression analysis. Results : The following appeared as significant variable affecting contingent workers' ratio in public health centers; degree of the local government's financial independence(p<0.001), rate of increase/decrease in ages 65 and over(p<0.001), rate of increase/decrease in basic livelihood security recipients(p<0.01) and rate of increase/decrease in registered disabled persons(p<0.01). In contrast, internal organizational environment characteristics related variables were not statistically significant. Conclusions : Contingent workers' ratio in public health center is significantly affected by financial vulnerability of the local government and increase in demand of health care services.

A Proposal of a Nursing Model Applicable to Group Occupational Health Service (보건관리대행 간호모형의 제안)

  • This study was conducted to propose a model for nursing performance in group occupational health service. To achieve such a goal, the writer choose and analysed two kinds of journals which were thought to be the most suitable for that purpose and authoritative on community health nursing: The Journal of Korean Community Nursing and the Journal of Korean Academic Society of Industrial Nursing. The total number of the articles analysed in this study was 16 and in the analysis of them the writer was specially concerned with the following questions: 1) Have there been any different performances in occupational health management between staying nurses and visiting ones? 2) What are the levels of knowledge, attitude, and practice of workers and how can we, on the basis of them, develop the program that workers are able to participate positively in ? the obtained conclusions are as follows. 1) Different performances are revealed between staying nurses and visiting ones: Common symptom management frequently appeared in staying nurses. Counseling, health education and follow-up care after medical examination were more frequently revealed in visiting nurses. 2) As for the lovel of knowledge, attitude, and practice of occupational health, workers have average one respectively. The major factors influencing the level of KAP of workers were area, age, sex and status. Counseling, health education, follow-up care after medical examination, and health promotion should be reinforced as core programs in group occupational health services. And for the part of visiting nurses, it is necessary to put more emphasis on efficiency training using the method of nursing process.

The Application of the Health Manager to Return-to-work of Workers Injured by Industrial Accidents (산재근로자 직장복귀를 위한 사업장내 보건관리자 활용방안)

  • Purpose: The Purpose of the study was to suggest how the health manager use to be easy return-to-work of injured workers. Method: The data were collected by the health managers working at the 103 companies over medium size in Incheon, Gyoung-gi and Seoul through interview and report by themselves and analyzed by SAS V8 through t-test. Result: 1. The health managers are consisted of 2 kinds, one is health manager such as physician(10%) or nurse(81%) and the other is safety manager. The former works at the manufacturing company(62.9%), the latter at the service one(42.4%). 2. Management and counseling of occupational and non-occupational diseases, and high risk workers, health education, emergency care, worksite rounding, guidance of personal protector use, and health promotion services were highly performed by health managers. Comparing to these, safety managers performed guidance of job safety, safety management. The difference of two kinds of manager was significant statistically not only the aspect of general job but also related job to the workers of return-to-work after accident. Conclusion: This result shows that health manager can function as a care manager to the workers after return-to-work for adaptation to their job and rehabilitation bio-psycho-socially. But health managers don't have any regulations of encouraging injured workers to get their job again officially.

Comparison of Health Care Utilization Patterns and Length of Stay Determinants between Fracture Patients with Workers' Compensation Insurance and National Health Insurance (건강보험 환자와 비교한 산재환자의 의료이용 행태와 재원일수 결정요인: 일부 골절환자를 중심으로)

  • Objectives : This study investigated the health care utilization patterns and length of stay (LOS) determinants of fracture patients with workers' compensation insurance (WCI) and national health insurance (NHI). Methods : The discharge summary data of 4,394 WCI fracture patients were compared to 28,874 NHI patients. Health care utilization characteristics were compared with a logistic regression analysis, and the LOS determinants were identified with linear regression analysis. Results : The average LOS of the WCI fracture patients was 2.4 times higher than that of the NHI patients. WCI patients used more small or medium sized hospitals, and were more frequently admitted through the emergency room. Females, older patients and patients admitted through emergency room had a significant positive impact on LOS. Conclusions : Healthcare utilization by the WCI fracture patients may include medically unnecessary hospital stays. Therefore, policy makers need to respond appropriately to the inefficient use of health resources by WCI patients.

Review on Prevalence, Risk Factors, and Research Advancements on the Use of Medical Gloves Concerning Hand Dermatitis Among Health Care Workers

  • The COVID-19 pandemic has led to a significant surge in glove usage, as recommended by the World Health Organization. Despite efforts to ensure the quality and safety of gloves, glove-associated skin diseases such as hand dermatitis have become ubiquitous, particularly among health care workers. This review discusses the prevalence, causes, and risk factors of hand dermatitis, as well as research efforts in medical gloves in the past decade to overcome glove-related hand dermatitis. Research papers from 2013 to 2022 were reviewed, selecting only 49 relevant papers from the Ovid, PubMed, and Scopus databases. The average prevalence of hand dermatitis among health care workers increased from 21.08% to 37.24% upon the impact of the COVID-19 pandemic. The cases are likely due to allergies to latex proteins, rubber additives, and accelerators commonly found in gloves. Using alternatives to latex gloves, such as accelerator-free and latex-free glove options, can help reduce allergy-induced hand dermatitis. Strict hand hygiene practices, such as frequent hand washing and the use of sanitizers, are also contributing factors in contracting hand dermatitis. Over the past decade, glove research advancements have focused mainly on reducing or immobilizing latex proteins. These include the use of biodegradable dialdehyde, sodium alginate, arctigenin, bromelain, papain, UV-LED, prototype photoreactors, and structure-modified nanosilica with silane A174. Two effective hand dermatitis preventive measures, i.e. an additional layer of glove liners and the use of gentle alcohol-based hand sanitizer, were recommended. These advancements represent promising steps towards mitigating hand dermatitis risks associated with glove usage.

Status and Needs for Nutrition Services for Infants and Preschoolers among Public Health Center Workers and Infants Mothers (보건소 영유아 영양사업 실태와 보건소 종사자와 영유아모의 영양사업 요구도)

  • This study was carried out to investigate the present status of nutrition services for infants in public health centers and the need for nutrition services of health workers and infants mothers. The study subjects were 146 health workers and 197 infants mothers. The results were as follows : At present, the only major nutrition services for infants were vaccination and dental care. Proper nutrition management services were available to infants. Nutrition knowledge scores were 16.8 for health workers and 15.3 for mothers out of 20 possible points. Health workers strongly demanded a well-organized nutrition education program, government support, audio-visual materials and the employment of a community nutritionist. The public health workers, in particular, demanded the development of education programs for breastfeeding and weaning. The infants mothers demanded services of nutrition information and teaching of cooking and menu planning. Based on this, the results suggest that the employment of a community nutritionist and the development of practical nutrition service programs for infants are needed very urgently for public health centers.

Oral Health of Stone Mine Workers of Jodhpur City, Rajasthan, India

  • Background: Occupational injuries cause major health problems, which the developed, developing, and underdeveloped nations worldwide are facing today. The present study aimed to assess dental caries, periodontal health of stone mine workers, and the relationship between wasting diseases and the years of working experience. Methods: The study population comprised 510 men, selected based on the stratified cluster sampling procedure. Clinical oral examinations were carried out, and periodontal disease, dental caries, and wasting diseases were recorded. Results: Workers were in the age group of 17-56 years; the prevalence of dental caries in the workers was found to be 74%, with a mean decayed, missing, filled teeth index of 2.89. A periodontal pocket of more than 6 mm was observed in 6% of the workers. Conclusion: The oral health of mine workers is in a poor state; steps should be taken so as to provide basic medical and dental care facilities.

Worker's Health Management and Health Promotion of General Hospitals in Korea (우리나라 종합병원 사업장의 근로자 건강관리 현황 분석)

  • Purpose: This study was conducted to understand the situation of general hospital worker's health management and health promotion. Methods: To investigate the current situation of health management in the hospital, structured questionnaires were sent to 122 occupational health providers by post. About 79% hospitals returned questionnaires. The data were analyzed using descriptive analysis, ${\chi}^2$-test by SPSS 12.0 program. Results: A quarter hospitals responded set up separated health care office for workers, 87.5% provided health educations, and 56.5% operated health promotion projects. In the contents of health promotion program embraced both health behavior practice and disease prevention, musculoskeletal disease control, infection control, smoking cessation, and exercise program were most commonly provided to the workers in order. Occupational health care provider chose the item such as budget limitation, manager's apathy, lack of employee's participation, cooperation provider, and so on as the reason of difficulty to run health promotion program in the hospital setting. Conclusion: Hospital managers need to construct infra to manage and promote worker's health. For example, establishing Industrial safety and health committee in hospital and arranging nurses who being fully responsible to worker's health. And occupational health care provider should advertise health promotion projects both managers and workers actively.

Awareness and educational needs on preventive dental treatment among oral health workers (구강보건인력의 예방치과진료에 대한 인식 및 교육요구도)

  • Objectives: The objective of this study is to provide basic data needed in developing an educational program designed to upgrade capacity and awareness of preventive dental treatment among oral health workers, by analyzing levels of awareness of preventive dental treatment and educational needs among dentists and dental hygienists. Methods: The collected data was analyzed with SPSS program ver. 19.0. The data was under t-test. Results: The frequency level of giving preventive dental treatment to patients among dentists and dental hygienists is below mid-point, 3 on the 5-point Likert scale. In terms of frequency level per item, scaling & polishing was ranked the highest, followed by periodontal maintenance, tooth-brushing instruction, and prescription and instruction of oral care product in descending order. On the questions asking how important preventive dental care they perceive to be, both dentists and dental hygienists perceived it to be highly important. When they were asked to rank those items by the importance of education, they considered periodontal maintenance as the most important one, followed by individual education of oral health, incremental oral health care, scaling& polishing, toothbrushing instruction, and prescription and instruction of oral care product. Respondents pointed out problems in running a preventive dental treatment program as follows: overwork, lack of dedicated workforce, un-fixed costs, and lack of necessary equipment. When they were asked to point out items needed to run such a program, the largest number of respondents indicated dedicated workforce placement, followed by improving awareness of the customer, and improving awareness of the dental workers. Conclusions: In order to effectively run a preventive dental treatment program, it is necessary for oral health workers to clearly understand the concept of it. It is also necessary to develop and operate an education program on preventive dental treatment targeting oral health professionals.

A Convergence Study on the Necessity of General Hospital Workers Hospice Palliative Care Education (일반병원직원의 호스피스 완화의료 교육 요구도에 관한 융합 연구)

  • This study aims to identify the requirement of death, hospice palliative care education targeting nursing assistant and general workers among hospice palliative care private hospital workers convergencely. The survey period is from September 22 to October 21 in 2016, and 158 workers of hospice palliative care private hospitals in Chungnam region have been surveyed. Detailed results of this study are as follows. In terms of importance of theoretical and practical education in hospice palliative care education content, all of the respondents answered that practical had higher importance than theoretical education. The above results of this study are hoped to be helpful in developing customized hospice palliative care education program that is meeting characteristics and requirements of hospice palliative care private hospital workers, especially for nursing assistants as basic data.
