대만 의료보장개혁과 교훈 (Tawian's Health Care Reform and Its Lessons)

  • Taiwan has experienced rapid economic growth during the past two decades. As a result, the demand for health care in Taiwan has increased rapidly. To meet the rising demand, Taiwan implemented a National Health Insurance (NHI) program on March 1, 1995. This program now covers more than 96 percent of Taiwan's citizens. Implementation of the NHI in 1995 represents fulfillment of a primary social and health policy goals of Taiwan. The goals of the NHI program is to eliminate financial barriers of health care for the citizens, to improve the quality of care. To achieve these goals, the NHI was designed on the following principles: 1. All Taiwan citizens are compul내교 joined the NHI program by law; 2. The NHI program provides comprehensive services; 3. The NHI is run by one single govt' subsidy; 5. The NHI adopt fee-for-services scheme to pay medical expenses and copayment to avoid abouse of medical services. Does Community Participation Work? : A Study on the Impact of Health Care Reform in Chile

  • Community participation in health has been praised as a new way of improving health inequality in developing countries for many decades. This paper is an attempt to evaluate community participation programs in health focusing on two intercultural health hospitals in IX Region of Chile. After exploring the process of program building and its impact on the quality of service, this paper concludes that a community participation program with stronger participation resulted in higher patient satisfaction. The author expects such finding to contribute to more comprehensive understanding of the impact of participation in health programs.

A Comparative Study on Primary Health Care in Republic of Korea and Republic of Uzbekistan

  • Background: Primary health care (PHC) plays a major role to ensure the basic right and equal distribution of the essential health care services. This study presents comparative analyses of PHC in Korea and Uzbekistan, discusses the existing scenario and the challenges, and provides recommendations. Methods: This study reviewed secondary data from Korea's National Statistical Information Service and the State Committee of the Republic of Uzbekistan on Statistic, regulatory legislation, research reports, and policy papers by research and international institutions. We focus on comparing input and outcome health data, PHC structure, and health expenditure. Results: Overall health status of the population in Korea is better than in Uzbekistan; both countries achieved more than 95% immunization coverage. The reforms implemented in both countries provide initial health care service delivery. However, there are several challenges such as the distribution of the staff between urban and rural areas and interest of the graduates on specialization rather than working in PHC system. Conclusion: PHC plays an important role in the provision of medical services to the population, addressing both health and social problems; it is the best tool for achieving universal coverage for basic health needs of the population. The community health practitioners in Korea and nurses in Uzbekistan plays main role in universal coverage through providing essential health care services. Continuous reform of the PHC system should be directed to strengthen the capacity of the PHC staff in health promotion knowledge and activities as well as to encourage population to improve their own health.

의약분업을 둘러싼 갈등 : 협상론의 관점에서 (The Conflict over the Separation of Prescribing and Dispensing Practice (SPDP) in Korea: A Bargaining Perspective)

  • 본 논문은 의약분업의 실시와 이에 따른 의료인의 대규모 파업을 연구 대상으로 하였다. 협상 (bargaining)에 관한 게임이론 (game-theory) 모형을 활용하여 의료인들과 정부의 협상과 정과 결과를 분석하고자 하였다. 특히 제도 변화의 과정에 있어 사회적 행위자들 사이의 분배적 갈등의 역할에 초점을 맞추고 있다 정부에 의한 의약분업의 시행은 의료인들로 하여금 심각한 분배적 결과를 초래할 수 있다는 것이며, 이는 다양한 배경의 의료인들로 하여금 정치적 연합의 가능성을 강화시켰다. 의료인들에 의한 분배적 결과의 인식은 파업과 같은 집합행동의 조직화로 나타나고 정부와의 협상 관계에 있어서도 줄곧 협상력의 우위를 견지하는 모습을 보여주었다. 결과적으로 의료인들은 그들이 원하는 결과를 확보하는데 성공한듯이 보인다. 협상은 제도의 형성과 변화에 있어 행위자들 간 상호작용의 중요한 형태라 할 수 있다. 이 경우 한 행위자의 목적은 그들에 유리하게 어떻게 제도적 규칙을 형성해 나가느냐 이다. 행위자들간 이해관계에 따른 갈등의 분석에 있어 주요한 변수는 당사자들 간 협상력의 차이라 할 수 있다. 힘의 비대칭 (asymmetry of power)현상은 제도 형성의 중요한 요소가 될 수 있다. 본 논문은 비협조 게임 모형 (the battle of sexes game)을 이용하여 협상력의 차이에 따른 균형해(equilibrium outcome)의 차이를 분석하고자 하였다. 특히 정부와 의료인의 협상에 있어 선호의 강도(intensity of preference)와 협상의 결렬시 지불해야 하는 비용 (breakdown cost) 의 차이는 협상력의 차이로 이어지고, 결과적으로 제도 변화의 결과를 가늠케 하는 척도로 작용하고 있음을 알 수 있다.

건강보험수가제도 개선을 통한 의료전달체계 확립방안 (Establishment of Healthcare Delivery System through Improvement of Health Insurance System)

  • Establishing a healthcare delivery system is key to building a cost-effective healthcare system that can prevent the waste of healthcare resources and increase efficiency. Recently, the rapid increase in the national medical expenditures due to the aging of the population and the increase in chronic diseases has raised the question about the sustainability of the healthcare system including the health insurance system. This is why we need to reform the medical delivery system, including the function setting of medical institutions. Accordingly, gradual and practical efforts based on the recognition of reality are needed for solving the problems and improving the medical delivery system. The first effort is to secure policy measures to establish functions and roles of medical institutions which are the basis of the healthcare delivery system, and a systematic medical use system for appropriate medical use. This approach can be achieved through a reasonable health insurance schemes. Without reasonable reform efforts, it will be difficult for Korea's health care system to develop into a system that can provide cost-effective and high-quality medical services that the people want.

의료서비스경험자의 보건의료제도 인식도와 영향 요인 (Health Care System Recognition and Influential Factors of Health Care Experiences)

  • Objectives : This study was performed to investigate health care system recognition and influential factors using the data from the "2017 Health Care Experience Survey". Methods : Data on 7,000 participants in the Health Care Experience Survey were drawn and statistically examined using a t-test, ANOVA, and multiple regression analysis. Results : First, the significant factors of health care service satisfaction were education, income, region, chronic diseases, unmet medical needs, satisfaction with doctors and institutions, and the health care system's reliability and importance. Second, the influential factors of willingness to pay additional health insurance premium were age, occupation, income, health status, chronic diseases, unmet medical needs, satisfaction with health care institutions, limit to utilization of medical services, necessity of health care reform, and the health care system's reliability, satisfaction, importance. Conclusions : Since the additional burden for improving the health care has been negative to the socially disadvantaged, there should be efforts to provide stable health care funding for financial stability of the health insurances by considering public opinions and reaching social consensus.

고령사회에 대응한 보건의료체계 개편의 시급성 (The Urgency of Reforming the Healthcare System for the Aged Society)

  • Korea, which entered the aged society in 2018, is accelerating the progress of the aged society due to the rapid decrease in birth. Although the low birth rate is the important issue, comprehensive measures to cope with the aged society should be established. In particular, the reform of the healthcare system in response to the aged society is very urgent. The reorganization of the healthcare system in response to the aged society should focus on improving the functional abilities of the elderly and should be transformed into integrated older person-centered health service. The benefit package of National Health Insurance should be diversified and appropriate payment of each benefit package should be applied. The ageism should be overcame, and it is important to recognize that the measures for the older person are investments in future society. The reform of healthcare system for older person are very urgent in Korea, which is advancing into a rapidly aged society.

농어촌 개발과 의료보장 개혁에 따른 보건진료원의 책임 (The role and responsibility of community health practitioner based on the rural community development and the reform of health care system)

  • This study was conducted by community survey of 215 community health practitiner's posts and literature review of official documents. The result was as follows ; 1. The role and responsibility of community health practitioner's post must be studied by the community health practitioner and the community health leader, and on the basement of community health needs, community development plan, and reforom of health care system. 2. Comprehensive health care of community is very important role and responsibility of community health practitioners. However, it was supervised by the senior community health practitioner in provincial government. 3. The community health practitioner must be trained by formal inservice educational program focused on comprehensive health care. 4. The community health practitoner must be the health guider and health leader as the member of community.
