Chitosan과 chitosan-cellulose를 이용한 차폐막의 골조직 재생유도능력에 관한 연구 (Guided bone regenerative effect of chitosan and chitosan-cellulose membranes)

  • Chitosan has been known as a wound healing agent. The purpose of this study was to evaluate the biocompatibility and guided bone regenerative effect of chitosan and chitosan-cellulose membranes. The effects of chitosan and chitosan-cellulose membranes on the growth and survival of human periodontal ligament cells were examined by rapid colorimetric MTT(tetrazolium) assay, and the tissue response and resorption pattern were observed by implanting the membranes into the subcutaneous tissue of the back of rats for 6 weeks. To evaluate the guided bone regenerative potential of membranes, the amount of newly formed bone in the rat calvarial defects(8mm in diameter) was measured by histomorphometry and radiomorphometry 1,2 and 4 weeks after implantation of membranes. Chitosan and chitosan-cellulose membranes showed no adverse effect on the growth and survival of human periodontal ligament cells. When membranes were subcutaneously implanted, inflammatory reaction was observed at 1 week and which gradually subsided 2weeks after implantation. Membranes remained intact throughout the experimental period of 6 weeks. Radiomorphometric analysis of the craniotomy sites revealed that chitosan and chitosan-cellulose membrane implanted sites showed increased radiopacity over control. Statistically significant differences with control were found in chitosan-cellulose membrane implanted group at 2 and 4 weeks, and chitosan membrane implanted group at 4 weeks(P<0.05). Histomorphometric data indicated a pattern of osseous healing similar to radiomorphometric analysis. There was a statistically significant difference between control and chitosan-cellulose membrane implanted group at 4 weeks(P<0.05). These results implicate that chitosan and chitosan-cellulose membrane might be useful for guided bone regeneration.

유방암 환자의 보완대체요법 추구 과정: 치유를 향한 끝없는 심신 다스리기 (Endless Management of Mind and Body for Healing: The Process of Pursuing Complementary and Alternative Medicine in Patients with Breast Cancer)

  • Purpose: The purpose of the study was to explore and describe the experience of pursuing complementary and alternative medicine (CAM) in breast cancer patients. Methods: Ten women with breast cancer participated in the study. Data were collected through individual in-depth unstructured and individualized interviews with each participant from February to July, 2015. Theoretical sampling was used upto the point of theoretical saturation. Data were analyzed using Corbin & Strauss's grounded theory methodology. Results: Through open coding, 22 sub-categories, and 13 categories were identified. Analysis revealed that the core category was 'endless management of mind and body for healing', which consisted of four phases; exploring, applying, grasping, and integrating. Through this process, the participants utilized various action/interactional strategies such as 'dealing with information', 'trying out blindly', 'enduring volitionally', 'experiencing effects on mind and body,' and 'grasping one's own way.' The consequences of these strategies were integrating the habit of health remedy into one's life, becoming a main agent for change, and tolerating one's weak body. Conclusion: In-depth understanding of the CAM pursuing experiences of patients with breast cancer would guide clinical nurses and policy makers to develop effective interventions and policies for better supporting them with regard to the usage of CAM.

부분층화상을 입은 외래 환자에서 이부프로펜 방출성 드레싱 제재(Biatain Ibu®)의 창상부위 통증의 경감 효과에 대한 연구 (Pain Relief Efficacy of Ibuprofen Releasing Foam Dressing (Biatain Ibu®) on Outpatient Patient with Partial Thickness Burn Wound)

  • Purpose: Pain management in burn treatment is important in improving wound healing and quality of life. Ibuprofen is a proven pain relieving agent in patients with partial thickness burn by intraveous injection. The purpose of this study is to evaluate the efficacy of Biatain Ibu® (polyurethane foam containing ibuprofen) in pain control for outpatients with partial thickness burns. Methods: A prospective randomized clinical trial was performed in outpatients with partial thickness burn from August 1, 2017 to July 31, 2018. Acute pain, chronic pain, complications, days for re-epithelialization and patient's satisfaction were compared between Biatain Ibu® and Biatain® groups. Results: A total of 20 patients (Biatain Ibu®, n=10; Biatain®, n=10) were assessed in the trial. On Burn days 3, 5, 7, 11, 13, and 15, the acute pain levels were significantly lower in the Biatain Ibu® group than in the Biatain® group. Complications, chronic pain levels and days for re-epithelialization were not significantly different between the two groups. Patient's satisfaction was not statistically significant but was higher in the Biatain Ibu® group. Conclusion: Biatain Ibu® is effective in relieving pain in outpatients with partial thickness burn without decreasing patient satisfaction, wound healing ability or developing any complications.

표피성장인자가 치주인대 섬유 모세포의 증식과 부착에 미치는 영향 (THE EFFECTS OF EGF ON PROLIFERATION AND ATTACHMENT OF HUMAN PERIODONTAL FIBROBLASTS)

  • 손상 받은 치주 조직의 치유과정은 치주인대 섬유 모세포의 세포 활성도에 영향을 받는다. 또한 치유과정 중 치주인대 섬유 모세포간의 재부착이 이루어져야 한다. 본 연구의 목적은 표피성장인자가 치주인대 세포의 증식과 부착에 미치는 영향을 알아보는 것으로 이를 바탕으로 향후 완전 탈구된 치아의 보관용액이나 재식전 처리제로서의 표피성장인자의 효용성을 고찰해 보기 위함이다. 발치된 사람의 제 1소구치에서 치주인대 섬유모세포를 채취 및 배양한 후, 세포독성을 및 세포의 최고 활성도를 평가 하기 위해 MTT assay를 시행하였다. 표피성장인자가 첨가된 실험군과 대조군의 세포증식의 차이를 비교하였고, western blot을 통해서 세포 부착에 관여하는 섬유결합소의 발현을 실험군과 대조군을 통해 비교하여 다음의 결과를 얻었다. 표피성장인자는 치주인대 섬유모세포에 대하여 세포독성을 보이지 않으며, 최고의 활성도를 보이는 농도는 10ng/ml였다. 또한 치주인대 섬유모세포의 배지내 증식정도는 10ng/ml의 실험군에서 대조군에 비해 유의하게 높았다. 세포간 부착에 관여하는 섬유결합소의 발현율이 실험군에서 대조군에 비해 유의하게 증가하였다. 본 연구를 통해 표피성장인자는 치주인대 섬유모세포의 재생을 촉진하며 따라서 완전 탈구된 치아의 보관용액이나 재식전 처리제로 사용될 수 있음을 시사한다.

한국과 일본의 웰니스관광정책 동향 분석 및 정책적 함의 (Trend Analysis and Policy Implications of Wellness Tourism Policy in Korea and Japan)

  • 본 연구는 웰니스관광 수요가 증대되고 있는 가운데 관련 문헌연구와 온라인 면접조사를 통해 우리나라와 정책적 유사성을 가진 일본과의 웰니스 관련 관광정책 동향을 비교 분석하여 국내 웰니스관광에 대한 정책적 시사점을 강구하였다. 조사 결과, 웰니스관광 관련 개념은 한일 양국 간 큰 차이는 없으나, 일본의 경우 치유(힐링)관광보다 헬스투어리즘이나 웰니스관광 개념사용이 보편화되어 있는 것으로 나타났다. 웰니스관광정책은 정책주체의 유연성과 정책의 효율성, 관련 인증 및 인력양성 분야는 일본이, 정책의 추진력과 제도적 정비는 우리나라가 보다 체계적인 것으로 분석되었다. 우리나라도 일본의 NPO법인 헬스투어리즘진흥기구나 산림테라피소사이어티 사례와 같이 웰니스관광협의체를 구축하고, 웰니스관광의 체계적 인증 및 관련 인력양성의 질적 관리를 통해 정책의 효율성을 제고할 필요가 있다고 판단되었다. 또한 웰니스관광의 산업화를 위해서는 지역별 특화된 웰니스자원을 유기적으로 연계함으로써 웰니스관광의 치유적 효능과 상품적 가치를 제고시킬 필요가 있으며, 포스트 코로나 시대를 대비한 웰니스관광정책의 방향과 대응전략에 대한 심도있는 분석과 웰니스관광지에 대한 실증적 연구가 필요할 것으로 판단된다.

새로운 프로톤 펌프 억제제, IY-81233의 항위염과 항궤양작용 (Antigastric and Antiulcerative Action of a New Proton Pump Inhibitor (IY-81233))

  • This study was designed to determine the effect of newly synthesized antiulcer agent, 5-pyrrolyl-6-halo-2-(pyridyl-2-methylthio)benzimidazole derivatives (IY-81233), on various experimental ulcers and on the secretion of prostaglandin $E_2(PGE_2)$ into the gastric lumen of rat. IY-81233 was previously reported to have a strong inhibitory effect on $H^+/K^$-ATPase and on gastric acid secretion in rats. Oral administration of IY-81233 at concentrations of 0.2, 2.0, and 20 mg/kg inhibited gastric lesions and duodenal ulcer induced by indomethacin, HCI-ethanol, water-immersion stress, cysteamine, and acetic acid in a dose dependent manner. Their IC$IC_{50}$ values were 3.4, 1.4, 0.8, 1.3, and 1.2 mg/kg, respectively. These results indicate that IY-81233 is a potent antiulcer agent although it is slightly less potent than omeprazole in healing of gastritis and ulcers. The secretion of $PGE_2$ into gastric lumen was also investigated in relation to the cytoprotective effect by IY-81233 in rats. The $PGE_2$ level was not changed significantly by an oral administration of IY-81233, suggesting that IY-81233 has little effect on the gastric protection. Therefore, it can be concluded that IY-81233 exerts prominent antiulcer activity by suppressing gastric acid secretion via an inhibition of a proton pump and not by protecting the gastrointestinal mucosa against various ulcerative stimuli.

Metformin displays in vitro and in vivo antitumor effect against osteosarcoma

  • Purpose: Patients with unresectable, relapsed, or refractory osteosarcoma need a novel therapeutic agent. Metformin is a biguanide derivative used in the treatment of type II diabetes, and is recently gaining attention in cancer research. Methods: We evaluated the effect of metformin against human osteosarcoma. Four osteosarcoma cell lines (KHOS/NP, HOS, MG-63, U-2 OS) were treated with metformin and cell proliferation was evaluated using 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide assay. Cell cycle progression and apoptosis were evaluated using flow cytometric analysis, and migration and wound healing assay were performed. Fourteen female Balb/c-nude mice received KHOS/NP cell grafts in their thigh, and were allowed access to metformin containing water (2 mg/mL) ad libitum. Tumor volume was measured every 3-4 days for a period of 4 weeks. Results: Metformin had a significant antiproliferative effect on human osteosarcoma cells. In particular, metformin inhibited the proliferation and migration of KHOS/NP cells by activation of AMP-activated protein kinase and consequent inhibition of the mammalian target of rapamycin pathway. It also inhibited the proliferation of cisplatin-resistant KHOS/NP clone cells. Analysis of KHOS/NP xenograft Balb/c-nude models indicated that metformin displayed potent in vivo antitumor effects. Conclusion: Further studies are necessary to explore metformin's therapeutic potential and the possibilities for its use as an adjuvant agent for osteosarcoma.

편측안면경련 환자 5 예에 대한 증례보고(證例報告) (The Clinical Observation on 5 Cases of Patient with Hemifacial Spasm)

  • In the treatment of hemifacial spasm, the operative methods, such as neurovascular decompression, rhizotomy etc. are the most efficacious therapy on its control. But, due to some side effects and complication, pharmacotherapy has been the first attempted by first intention of hemifacial spasm. The cabamazepine is the most commonly used agent, however it is often inefficacious in some cases, and attenuates as the time passes, engages in some symptoms, such as dizziness, nausea, vomiting, and causes an aplastic anemia, it has many deficiencies in becoming the first intention. Acupuncture is a simple, effective and conservative spasm control modality. It is so simple to use in hemifacial spasm. We had observed 5 cases of hemifacial spasm which were treated by acupuncture in our hospital recently. We reported these cases and reviewed related literatures briefly. The results of treatment are as follows : The average grade of spasm intensity classified by Scott's description was 3 before acu-therapy and it became 2 after the therapy. These results suggest that acupuncture and ear-acupuncture were effective as the first intention in healing hemifacial spasm.

  • To compare the effects of various pulp capping agents that are usually applied to human pulp tissue, adult dogs were bred for a certain period and each capping agent was applied experimentally to pulp tissue after vital pulpotomy. Histological observations are as follows. 1) In comparison between methods of vital pulpotomy, one and two appointment method, different courses of healing were observed. In one appointment method, the granulation tissue formation at the amputation sur face of pulp tissue had a tendency to be transformed to scar tissue formation. In two appointment method, more transformation than that of one appointment method from scar tissue to dentin matrix formation were observed. 2) Histologic changes that have appeared in pulp tissue are a) fixation at outer layer b) degeneration at middle layer c) hyperemia and round cell infiltration at inner layer 3) With use of formocresol mixed zinc oxide powder in two appointment method complete formation of dentin matrix were observed. 4) Among the methods and aagents described above formocresol mixed zinc oxide powder in two appointment method appeared to be relatively effective.

A novel L-ascorbic acid and peptide conjugate with increased stability and collagen biosynthesis

  • L-ascorbic acid (Vitamin C) and peptide are both useful compounds for collagen biosynthesis in cosmeceuticals (cosmetic and pharmaceutical fields). The instability of these compounds, however, limit their application in these industries. In this report, we describe the development of a novel compound, Stabilized Ascorbyl Pentapeptide (SAP), which physically is much more stable than L-ascorbic acid in water. The inhibitory effects of this SAP compound on tyrosinase and melanin synthesis is comparable to that of L-ascorbic acid. Importantly, the SAP compound displays no cytotoxicity at a high concentration (5 mM). The ability of SAP to promote collagen biosynthesis is greater than that of L-ascorbic acid or the KTTKS peptide alone. Considering the in vitro stability and functional effects, our data strongly suggest that the SAP compound is a good candidate not only as a cosmetic ingredient, but also as a wound healing agent.
