Experimental Study of Dohongsamul-tang (Taohongsiwu-tang) on Fracture Healing (도홍사물탕(桃紅四物湯)이 골절 유합에 미치는 실험적 연구)

  • Objectives The purpose of this study is to evaluate the bone healing effect of Dohongsamul-tang (Taohongsiwu-tang; DH) on femur fractured mice. Methods Mice were randomly divided into 4 groups (naive, control, positive control and DH). All groups except naive group were subjected to bone fracture on both hind limb femurs. Naive group received no treatment at all. Control group was fed with normal saline, and positive control group was orally medicated with tramadol. DH-treated group was orally medicated with DH. We analysed the levels of BMP2, COX2, Col2a1, Sox9, Runx2, and Osterix genes on 3, 7 and 14 days after fracture. Alkaline phosphatase, aspartate aminotransferase, alanine aminotransferase, blood urea nitrogen, creatinine, total cholesterol, and triglyceride levels were measured for safety assessment. Results In morphological, histological analysis, callus formation process of DH-treated group was faster than the control group. BMP2, Sox9 gene expression were significantly increased at 7 days after fracture compared to the control group. COX2, Col2a1 gene expression were significantly increased at 14 days after fracture compared to the control group. Total cholesterol was significantly increased by DH at 3 days. Triglyceride was significantly decreased by DH at 3, 7 days after fracture compared to the control group. Conclusions Dohongsamul-tang promoted bone healing process after fracture by stimulating the bone regeneration factors. And DH shows no hepatotoxicity, nephrotoxicity and serum lipid abnormality. In conclusion, it seems that DH helps to promote fracture regeneration after bone fracture by regulating gene expressions related to bone repair.

CD44 Expression in Healing Nd:YAG Laser Wounds and Scalpel Incisions of the Rat Tongue Mucosa (Nd: YAG 레이져와 수술칼로 절개한 흰쥐 혀점막에서의 CD44 발현)

  • This study was undertaken to compare and investigate the wound healing process following Nd:YAG laser wounds and scalpel incisions of the rat tongue dorsal mucosa. Laser wounds were made parallel on the right side of tongue dorsum and scalpel incisions were made on the left side with 5mm length, respectively Visual inspection and immunohistochemistry with CD44 antibody after immediately, 6 hours, 1 day, 2, 4, 7, 11, 21 days after surgery were carried out. The results were as follows : 1. There was macroscopically no treat difference between laser wounds and scalpel incisions. 2. Laser wounds appeared as dry, bloodless and irregular grooves with carbonization and whitening of superficial tissues compared to sharp, straight and bleeding incisions using scalpel. 3. In the epithelium of rat tongue mucosa, CD44 expression in healing laser wounds and scalpel incisions were similar. 4. In the lamina propria and muscle layer, CD44 expression in healing laser wounds gradually increased earlier and decreased faster than scalpel incisions. Conclusively, it is suggested that pulsed Nd:YAG laser has more beneficial effect on the healing of wounded oral mucosa than scalpel in knew of CD44 expression.

Effects of Combined Marine Treatment Program on Risk Factors of Metabolic Syndrome, and CRP in Elderly Musculoskeletal and Metabolic Patients (복합 해양치유 프로그램이 근골격계 및 대사성 질환자의 대사증후군 위험인자 및 CRP에 미치는 영향)

  • Purpose : The purpose of this study is to verify the effectiveness of combined marine healing programs by analyzing the physical composition of elderly musculoskeletal and metabolic patients, the risk factors of metabolic syndrome and the effects of the inflammatory factors, the C -reactive protein (CRP). Methods : Individuals with musculoskeletal and metabolic diseases were identified, and marine healing programs were conducted for f our hours each day for two weeks with 11 elderly participants. A one-way RM ANOVA was conducted to determine the differences due to treatment with composite marine healing systems. The results are as follows. Results : After a two-week ocean healing program, weight decreased the most, while BMI also decreased and muscle mass increased. Waist circumference, a risk factor for metabolic syndrome and CRP, decreased, and CRP demonstrated a decreasing trend. Conclusion : The above results show that the two-week marine healing program has a positive effect on the body composition and inflammatory factors of elderly musculoskeletal and metabolic patients.

Enamel matrix derivative for replanted teeth in animal models: a systematic review and meta-analysis

  • Objectives: To investigate the effect of enamel matrix derivative (EMD) on periodontal healing of replanted teeth in animal models. Materials and Methods: The authors searched MEDLINE, PubMed, EMBASE, Cochrane Library, Web of Knowledge and Scopus for articles published up to Oct 2012. Animal studies in which EMD was applied in transplanted or replanted teeth with adequate controls and histological data were considered. Normal periodontal healing or root resorption determined by histology after EMD was applied in replanted teeth with adequate controls was used as outcome measures. The following search strategy was used: ('Emdogain' OR 'enamel matrix proteins' OR 'enamel matrix derivative') AND ('avulsion' OR 'transplantion' OR 'autotransplantation' OR 'replantation'). Results: Six animal studies were included in the final review. There was great heterogeneity in study design among included studies. Two studies with similar study designs were identified and analyzed by a meta-analysis. The pooled estimates showed a significantly higher normal healing and surface resorption and significantly less inflammatory and replacement resorption in EMD-treated groups compared with non-EMD-treated groups. Conclusions: With the limitations of this systematic review, the use of EMD led to greater normal periodontal healing and surface root resorption and less inflammatory and replacement root resorption in the presence of periodontal ligaments. However, no definite conclusion could be drawn with regard to the effect of EMD on periodontal healing and root resorption when no periodontal ligaments exist.

  • Negative pressure wound therapy (NPWT) has emerged as a valuable tool for managing complex wounds within the foot and ankle field. This review article discusses the expanding applications of NPWT in this specialized field. Specifically, it discusses the efficacy of NPWT for various wound types, including diabetic foot wounds, traumatic wounds, surgical wounds, and wounds involving exposed bone or soft tissue defects. NPWT demonstrates versatile utility for foot and ankle wound management by promoting healing, potentially reducing the need for secondary surgery, improving diabetic and neuropathic ulcer healing times and outcomes, and optimizing the healing of high-risk incisions. In addition, this review explores the underlying mechanisms through which NPWT might enhance wound healing. By synthesizing current evidence, this review provides a comprehensive overview of the role of NPWT in foot and ankle surgery and offers valuable insights to clinicians navigating the complexities of wound care in this challenging anatomical area.

The pH Value Changes During Wound Healing Process (창상치유 과정에서의 pH 변화)

  • Purpose: For a proper wound care, the correct evaluation of wound is very important. Usually the evaluation of wound was subjective, and as a result, wound care was empirical. There were many attempts to objectify the wound evaluation, and one of them was measurement of the wound pH. The purpose of this study is to observe the wound pH changes during wound healing phase. Methods: From 2005 to 2007, we measured the pH values of 6 acute wounds, which were split thickness skin graft donor sites. In addition, we measured the pH values of 18 chronic wounds, which were 17 pressure sores and 1 tuberculosis ulcer. After pH meter ($SkinCheck1^{(R)}$, Hanna Instruments, Italy) was calibrated, wound pH was checked. Wound was cleansed with saline gauze and dressed with polyurethane foam dressing($Medifoam^{(R)}$, Biopol, Korea). Results: In split thickness skin graft donor sites, the pH raised(mean pH value: $7.45{\rightarrow}7.62$) when the wound was on the process of healing(*p=0.027, analysis of Wilcoxon signed-rank test). If wound became re-epithelialised, the pH value dropped to that of normal skin. However, we could not find a relation between time and the pH values in chronic wound. Conclusion: We could observe the consistent wound pH changes during wound healing phase in acute wound.

The Effect of Curcumin and Human Adipose-derived Stromal Cells on Wound Healing of Lewis Rats (커큐민과 지방 기질세포가 흰 쥐의 창상치유에 미치는 영향)

  • Purpose: On this study, we investigated the effects of curcumin and adipose-derived stromal cells (ADSCs) in wound healing process, especially in the aspect of synergic effects when they were administrated simultaneously. Methods: Curcumin (40 mg/kg) and/or $1.0{\times}10^6$ ADSCs were applied to an $1.5{\times}1.5\;cm$-sized full thickness wound on the backs of male Lewis rats (n=5 in each group). In control group (n=5), saline was administrated instead of curcumin and ASCs. The wound size was followed by computer planimetry in 5, 7, and 14 days, and wounds were harvested for histological analysis in 7 and 14 days. Results: The dimensions of wounds of curcumin, ADSCs, and curcumin-ADSCs group significantly decreased in 5, 7, 14 days compared with those of control group (p<0.05), but there were no significant differences among three groups. The wound sizes were lowest in curcumin-ADSCs group compared with the other groups, but the differences were insignificant (p>0.05). There were infiltration of more epithelization and more precisely organization of extracellular matrix in curcumin, ADSCs, and curcumin-ADSCs group compared with those of control group. Conclustion: The results suggest that curcumin and ADSCs have beneficial effects in the acceleration of wound healing. Although the simultaneous application of curcmin and ADSCs also has beneficial effects on wound healing, there are no significant synergic effects.

A Skin Fixation Method for Decreasing the Influence of Wound Contraction on Wound Healing in a Rat Model

  • Background The elasticity of the back skin of the rat reduced the tension around wounds during the wound healing process in that region, and thus activates wound contraction. The authors proposed two skin fixation methods using readily available materials to decrease the influence of wound contraction on wound healing and designed an experiment to determine their effects. Methods The authors made 36 skin wounds on the backs of 18 rats, and they divided them into three groups. Each group was treated with three different kinds of dressing materials, each with different skin fixing characteristics. Group A was a control group. Group B and group C were dressed by the first and the second skin fixation method. We measured the areas of the wounds post-surgically and calculated the wound area reduction rates. Results The two skin fixation methods both reduced the effect of wound contraction compared to the control group. Each of the two methods had different outcomes in reducing wound contraction. Conclusions The experiment demonstrated significant differences among the wound areas and the wound area reduction rates of the three groups as a result of differences in the degree of wound contraction. To obtain accurate results from wound healing experiments, appropriate skin fixation methods must be adopted.

Comparative study of wound healing in porcine urinary bladder with CO2 laser and scalpel incisions (CO2 Laser와 Scalpel을 이용한 절개 시 돼지 방광에서의 창상치유 평가)

  • This study compared the instrument performance and tissue healing of a steel scalpel with a $CO_2$ laser in an animal urinary bladder surgery model. Landrace and Yorkshire mixed breed pigs were used. Two symmetrical incisions were made in urinary bladder of each pig. One incision was made on the left side of ventral aspect on urinary bladder using a steel scalpel, while the other incision was performed on the right side using a $CO_2$ laser with an 8W output power. Each instrument was evaluated clinically for speed, ease of incision, and extent of bleeding. At 7 and 21 days after initial wounding, each wound was taken for histological observations. The scalpel was an easier instrument to use in the confines of the urinary bladder tissue, compared with the laser. However, there was no significant difference between the two groups. The amount of bleeding was less in the laser group but the time of the incisions was shorter with the scalpel. Scalpel incisions showed complete restoration of the epithelium and muscularis. On the other hand, the laser incisions showed incomplete restoration of the epithelium and muscularis. However, most of wound healing in the laser incisions was accomplished according to the time lapse. Although the scalpel produced less damage to the urinary bladder tissue and was easier to handle than the $CO_2$ laser, it did not provide hemostasis that was helpful for use on highly vascular tissue. The $CO_2$ laser provided good hemostasis, but delayed wound healing. In conclusion, the $CO_2$ laser provided better hemostasis and better surgical field than the scalpel. The $CO_2$ laser was used effectively in urinary bladder incision.

Effect on Tenascin Expression of Low Power Generating Laser Irradiation during Wound Healing Process (저출력 레이저가 창상치유과정에서 Tenascin 발현에 미치는 영향)

  • The purpose of this paper was to observe the influence of Ga-As semiconductor-low power generating laser on she appearance and actions of tenascin, extracellular matrix, as healing process of intentional wound on the experimental animals is taking place. 35 rabbits were divided into control and experimental group. ; and on each, 3mm-long and 2mm-deep, surgical wounds were created on buccal oral mucosa and thoracodorsal portion of skin. Ga-As laser was applied to the experimental group starting a day of the day the wounds were created , the laser was applied for 5 minutes every other day. Tissue samples were taken after the 2, 4, 7, 10, and 14 days after wound formation. Then tile healing process of experimental and control groups were observed and compared, using light microscope. Afterwards, the samples were immunohistochemical stained and again observed tenascin by quantitative measuring. The following results were obtained : 1. Tenascin was observed prevalently on epithelial cells, border area of dermis, and interstitial matrix between connective tissue layers in both experimental and control groups. 2. In oral mucosa, the experimental group showed significant increase in the appearance of tenascin after 4 days compared to the control group, but after 10 days, it decreased to a point which is even less than the control group. 3. In the skin samples, the pattern of appearance of tenascin was the same in both groups, but there was some difference concerning when the peak period was shown, In the experimental group, the peak period of tenascin expression was the 7 days after wound formation in epithelium and connective tissue. In the control group, the peak period was 10 days after. 4. In both the experimental and control groups, tenascin first appeared in the epithelium near the wound area and submucosa, and then spread on the underlying connective tissue. In conclusion, appearance of tenascin is closely related to regeneration of epithelium and development of granulation tissue : therefore, low power laser, which fastnes appearance of tenascin, is sure to faciltate healing process of oral mucosa.
