Comparison of the Wound Healing Effect of Cellulose and Gelatin: An In Vivo Study

  • Background Many topical hemostatics are widely applied for bleeding control. They can be classified into two categories according to their mechanism of action on the clotting cascade in a biologically active or passive manner. Passive hemostatics include cellulose and gelatin. We performed an experimental study to compare the effect of passive hemostatics in wound healing by applying them to a rectus abdominis muscle defect of white mice. Methods Surgicel is a sterile absorbable knitted fabric prepared by the controlled oxidation of regenerated cellulose. Spongostan is an absorbable hemostatic gelatin sponge. In 30 mice, a $1{\times}1$ cm defect was created on the rectus abdominis muscle and the materials were applied in three ways: control group, cellulose (Surgicel) group, gelatin (Spongostan) group. For the histologic analysis, biopsies were performed at 3 and 28 days. Results After 3 days, the cellulose group showed limited granulation formation with acute inflammatory reactions similar to the control group. At the 28th day, moderate amounts of granulation tissue formation was observed with milder inflammatory reactions than the control group. In the gelatin group, after 3 days, gelatin remnants were observed surrounded by severe inflammatory changes. After 28 days, the same quantity of gelatin remnants could be still observed. Conclusions This study suggests that cellulose is associated with minimal morbidity in wound healing, while the use of gelatin shows severe adverse tissue reactions with delayed wound healing. Consequently, cellulose is better than gelatin when considering wound healing.

  • Sex hormones have long been considered to play an important role in bone turnover rate, periodontal diseases, and wound healing. We have studied the effect of porcine testis steroid extract (PTSE), an extract of porcine testes, which holds a good ratio of 19-nortestosterone (nandrolone), testosterone, androstenedione, $17{\beta}$-estradiol, and estrone, on the healing rate of a standardized full-thickness linear wound on the back of the rat. Skin punch or carbon dioxide ($CO_2$) laser methods were used to create the deep skin injury in two groups of animals. The animals were treated with the PTSE cream, control cream and Vaseline (control) to find out the effect in re-epithelialization, contraction, and formation of granulation and scar tissues. Histological examination after 21 days showed 100, 87.4, and 80.5% recovery of epidermis, dermis, and hypodermis, respectively in the PTSE-treated animals. Similarly, on the 15th day of treatment, complete healing of intact skin was observed in the PTSE cream-treated animals among the laser radiation group. Even though the beginning of re-epithelialization phase and completion of serum crust formation was also observed in the base cream- and Vaseline-treated animals respectively, the complete healing cycle was observed only in the PTSE-treated group. The white blood cell count in the PTSE-treated group showed that PTSE cream is nontoxic to animals.

  • The purpose of this study is to suggest that compassion is used as a mechanism to improve immunity by activating people's parasympathetic nerves. Compassion is pity and heartbreak for the misfortune of others. The instinctive emotion of compassion is the basis for humans to achieve and develop society. This is also linked to the laws of nature and the factors of evolution that Kropotkin, famous for his "mutual assistance (mutualism)" that all things help each other. Compassion is an individual's instinctive emotion and at the same time a driving force for forming and developing society. If the Hopeful World (希望世上) performs Korean traditional music healing at a nursing home, first, it will have a positive healing effect on the elderly in the nursing home, who are the audience. Second, positive healing effects can also be expected from performers. The stronger the compassion, the greater the healing effect. Third, people who watch the performance also enjoy the healing effect. This seems to have brought about a synergistic effect by combining the feelings felt while looking at the excellent behavior felt by seeing the poor person. It seems that this effect can be named the compassion effect that developed the Mother Teresa effect. The Mother Teresa effect refers to a significant improvement in the body's immune function just by volunteering or seeing good things. By expanding this Mother Teresa effect, it can be inferred that a pitying heart, helping behavior, and being with good behavior will all help improve the human immune system. This can also be called the compassion effect. Therefore, we think having compassion activates the parasympathetic nerves, improving your mood, and increasing your immunity.

  • The purpose of this article is to argue that Arirang, Korean traditional music, could be used for healing purposes for Koreans. Music may be a medicine for curing both the body and mind. That is the soul of folksong. Arirang is a representative folksong in Korea. Koreans thought that listening to Korean traditional songs and singing them have healing powers because it makes people happier. When Koreans listen to Arirang, slowly as if they are mass hypnotized, Koreans calmed down because they think of mother's bosom while listening to Arirang. Also, Koreans find comfort in listening and singing to Arirang. The song's tune is catchy and its lyrics are moving. The song of Arirang was sung from long ago by Koreans. Therefore, it will continue forever as long as Koreans exist.

The Effects of Jawoongo plus Rehmanniae Radix on The Wound Healing of Rats

  • Herbal medicines are popular in Korea communities and used for a variety of diseases, and most herbal medicines has been used an oral administration for treating diseases, but some herbal medicines included Jawoongo has been used by applying on the skin. Jawoongo, a traditional oriental medicine ointment, have been used for treating various dermatitis associated with skin diseases such as tinea manuum, eczema and chilblain, etc. by promoting blood flow(活血), dispersing wind(散風) and moistening(潤燥). Jawoongo used in this study is derived from Yungigo supplemented with Adeps Suillus(豚脂). The aim of this study is to investigate the effect of Jawoongo plus Rehmanniae Radix(生地黃) on the healing of incisional skin wound in rats. After inducing an inflammation on the rats, we observed the amount of Leukocyte, the level of Cortisol and CRP(C-reactive protein), the histological parts and have obtained the healing effect of wound by using Jawoongo plus Rehmanniae Radix(生地黃).

A Study on the Different Point of View in Healing Environment between the East and the West (동·서양의 치유환경 관점의 차이에 관한 연구)

  • Purpose: The definition of healing and treatment is different between the East and the West. I suppose that the different definitions of them affect the notion of healing environment in two cultural area. Therefore the aim of this study is to consider the different perspective in healing environment between the East and the West. Methods: Relevant literature review around domestic and foreign reference in fields of medicine and architecture has been conducted to consider the purpose. Results: First, the perspective that the cause of disease stems from inside of body approaches the body from standpoint of healing for recovery. Healing is the way to conquer imbalance by entering into a connection with nature around body. The concept of healing could analyze that it come from relevant perspective. Second, the healing environment in the East is the place where the flow of Qi between, human and human, and, human and environment, keep a balance. The notion of healing environment in the East is the filed of relations. In the West, the notion of healing environment is the place where arouse fine emotions through sensory stimulations. Implications: Recently the studies of healing environment focus on deducing sensory stimulation factors scientifically in the perspective of the West. But the healing environment couldn't be possible without the notion of place of relations. In the future, the notion of healing environment as the field of relations in the East has to be concerned.

Comparison of Human Bone Marrow Stromal Cells with Fibroblasts in Cell Proliferation and Collagen Synthesis (골수기질세포와 섬유아세포의 세포 증식과 교원질 합성능 비교)

  • It has been established that a graft of fibroblasts is able to improve wound healing. However, there has been no research on the effect of a graft of bone marrow stromal cells on wound healing. The wound healing process requires cell proliferation and production of extracellular matrix and various growth factors. The purpose of this study was to compare the abilities of human fibroblasts and bone marrow stromal cells, which contains mesenchymal stem cells, to proliferate and to produce collagen. Human bone marrow stromal cells and fibroblasts were isolated from bone marrow and dermis of the same patients and grown in culture respectively. Cell proliferation and production of type I collagen by human bone marrow stromal cells and dermal fibroblasts were examined by MTT method and by ELISA of cell culture media on day 1, 3, and 5 days post-incubating. The human bone marrow stromal cells showed 11-17% higher cell proliferation than fibroblasts at each time interval. The levels of type I collagen in the human bone marrow stromal cell group was also significantly higher than those in the fibroblast group. The results indicate that the grafts of human bone marrow stromal cells can show more promising effect than that of fibroblasts for healing of chronic wounds.

Assessment of Xenogenic Bone Plate and Screw using Finite Element Analysis

  • The aim of this study was to evaluate the biomechanical behavior of xenogenic bone plate system (equine bone) using a three-dimensional finite element ulna fracture model. The model was used to calculate the Von Mises stress (VMS) and stress distribution in fracture healing periods with metallic bone plate and xenogenic bone plate systems, which are installed while the canine patient is standing. Bone healing rate (BHR) (0%) and maximum VMS of the xenogenic plate was similar to the yield strength of equine bone (125 MPa). VMS at the ulna and fracture zones were higher with the xenogenic bone plate than with the metallic bone plate at BHRs of 0% and 1%. Stress distributions in fracture zone were higher with the xenogenic bone plate than the metallic bone plate. This study results indicate that the xenogenic bone plate may be considered more beneficial for callus formation and bone healing than the metallic bon plate. Xeonogenic bone plate and screw applied in clinical treatment of canines may provide reduced stress shielding of fractures during healing.

The Preclinical Study of Hyeolbuchugeo-tang (Xuefuzhuyu-tang) on Bone Healing in Rats with Rib Fracture (골절 유발 Rat에 대한 혈부축어탕(血府逐瘀湯)의 전임상 연구)

  • Objectives The purpose of this study is to evaluate the healing effect of Hyeolbuchugeo-tang (HC) in rats with rib fracture. Methods Rats were randomly divided into 5 groups (naive, control, positive control, HC-L and HC-H). All groups except naive group were subjected to bone fracture of rib. Naive group received no treatment at all. Control group was fed with phosphate buffered saline. Positive control group was orally medicated with tramadol. Experimental group was orally medicated with HC extract (50 mg/kg for low concentration [HC-L], 100 mg/kg for high concentration [HC-H]). X-ray and micro-computed tomography (micro-CT) were conducted to assess the effect of HC. We analysed the level of 2) transforming growth factor-β1 (TGF-β1), Ki67, alkaline phosphatase (ALP), receptor activator of nuclear factor kappa-β, runt-related transcription factor 2 (Runx2) and tartrate resistant acid phosphatase (TRAP) on 7 and 14 days after fracture. ALP, alanine aminotransferase, aspartate aminotransferase, blood urea nitrogen, creatinine was measured for safety assessment. Results X-ray and micro-CT, showed HC enhance bone repair process. Callus formation was increased in experimental group at 7 days after fracture, but decreased at 14 days after fracture. 7 days after fracture, the level of TGF-β1 in experimental group was decreased. The level of Ki67, Runx2 in HC-H, TRAP in HC-L was increased. 14 days after fracture, the level of Ki67 in HC-L and HC-H was decreased. The level of ALP, Runx2, BUN in HC-L, TRAP in HC-L and HC-H was increased. Conclusions Taken together the results, HC promoted healing of bone fracture. In conclusion, HC has a potential to promote healing of bone fracture.

Comparative study of wound healing in canine stomach with CO2 laser and scalpel incisions (CO2 레이저와 scalpel을 이용한 위 절개 시 개에서의 창상 치유 평가)

  • The objective of this study was to compare wound healing in stomach with $CO_{2}$ laser and scalpel incision by measuring the extent of bleeding, the ease of gastric incision, incision time, degree of adhesion and wound healing degree in dogs. Sixteen healthy dogs were used. Two symmetrical incisions were made in ventral aspect of the stomach between the greater and lesser curvatures were made with scalpel and 0.2 mm spot diameter $CO_{2}$ laser (8W, continuous wave) in sixteen dogs. And then each wound was closed with absorbable suture in a two-layer inverting seromuscular pattern. At 3, 7, 14 and 21 days after initial wounding, each wound was taken for histological observation. On surgery, the extent of bleeding, the ease of incision and incision time showed significant differences between the groups. The $CO_{2}$ laser provided better hemostasis (p < 0.05) and smaller postoperative adhesion compared with the scalpel. However, the scalpel produced faster speed of incision and was easier to handle than the $CO_{2}$ laser (p < 0.05). Although there was no considerable difference between the two groups in histological observation, necrosis and calcium deposit tended to be larger in the $CO_{2}$ laser than in the scalpel.
