Assessing the Public's Interest in Orofacial Pain Specialists: A Google Trends Analysis

  • Purpose: To assess Google Trends (GT) search behavior regarding orofacial pain (OFP) and headaches. Methods: GT scores for OFP and headache specialists between February 2013 and December 2022 were analyzed. Statistical tests such as Poisson regression analyses, mean differences, and Cohen's D were used to assess the score change over time. Results: The top three search words for OFP specialists were "temporomandibular joint (TMJ) specialist," "TMJ doctor," and "TMJ dentist," whereas the top three search words for headache specialists were "Headache specialist," "Headache doctor," and "Migraine specialist." Here, TMJ is temporomandibular joint. The GT scores for OFP specialists increased significantly (p<0.05) for all years except 2017, with the highest mean difference in 2020. The scores for headache specialists showed similar trends but gradually. Conclusions: The interest in OFP and headache specialists expressed by Google searches has increased over the years. More awareness is needed regarding the OFP scope of practice, and the use of GT may serve as an indicator.

The relationship between primary headache and constipation in children and adolescents

  • Purpose: Many patients presenting with headache also complain of constipation; the relationship between these two symptoms has not been explored in detail. The aim of this study was to investigate the association between primary headache and constipation. Methods: This retrospective study included all children who attended the Inje University Sanggye Paik Hospital complaining of headache, and who had been followed up for at least 100 days. Patients were divided into 2 groups: group A, in whom the headache improved after treatment for constipation, and group B, in whom headache was not associated with constipation. Results: Of the 96 patients with primary headache, 24 (25.0%) also had constipation (group A). All 24 received treatment for constipation. Follow-up revealed an improvement in both headache and constipation in all patients. Group B contained the remaining 72 children. Comparison of groups A and B indicated a significant difference in sex ratio (P=0.009, chi-square test). Patients with probable tension-type headache were more likely to be in Group A (P=0.006, chi-square test). Conclusion: Resolution of constipation improves headache in many patients diagnosed with primary headache, especially those with probable tension-type headache. We suggest that either constipation plays a key role in triggering headache, or that both constipation and headache share a common pathophysiology.

  • 두통은 대부분의 사람들이 평생 한번씩은 경험하는 가장 흔한 신체증상이다. 심각한 상황을 야기하는 뇌종양과 같은 일부의 원인을 제외하고, 두통의 대부분은 편두통이나 긴장성 두통과 같은 일차성 두통으로 알려져 있다. 두통의 정확한 진단은 임상의들의 자세한 병력 청취와 신체 검사로부터 시작된다. 본 종설에서는 두통의 세부진단과 진단과정에 대해 고찰하고, 효과적인 약물치료 전략에 대해서 논의하고자 한다.

Case studies on prophylactic ayurvedic therapy in migraine patients

  • Ayurveda is a nearly 3000 years old traditional medical system of India. Most of the time, people turn to ayurvedic physicians in desperate conditions. Here clinical practices of Ayurveda were initially found effective in the management of migraine among few patients. Later, it was developed as an ayurvedic treatment protocol (ATP) which consists of four herbo-mineral formulations (HMF), three meals and three snacks in a day with eight hours sleep at night. ATP brought significant relief in reducing the frequency, intensity of pain and associated symptoms in the migraine patients. IHS diagnostic criteria was followed to establish the diagnosis of migraine and uniform ATP was prescribed to each patient who were primarily treated by the ayurvedic physicians at their respective clinics. Such observations were presented at appropriate international forums. In an effort to validate the above, the present study carries the details of nine migraine patients who were first diagnosed and treated for migraine by a leading headache expert at Mumbai in India and were then referred to receive ATP. A total number of nine subjects volunteered to this program. Out of those, seven subjects completed 120 days of ATP. Five subjects reported significant improvement in overall symptoms of migraine. All subjects were followed up periodically for four years. No Grade II side effects were observed in any treated case. HMF has also been proved to be safe in experimental studies. Further pharmacological and randomized controlled clinical studies are in progress at the respective departments of a premier medical institute in India.

An investigation of the relationship between cutaneous allodynia and kinesiophobia, gastrointestinal system symptom severity, physical activity and disability in individuals with migraine

  • Background: To investigate the relationship between cutaneous allodynia (CA) and kinesiophobia, gastrointestinal system (GIS) symptom severity, physical activity, and disability, and to determine whether CA, pain, and disability were influencing factors for kinesiophobia, GIS symptoms, and physical activity in individuals with migraine. Methods: The study included 144 individuals with migraine. CA, kinesiophobia, GIS symptoms, physical activity level, and migraine-related disability were evaluated with the Allodynia Symptom Checklist, the Tampa Kinesiophobia Scale (TKS), the Gastrointestinal Symptom Rating Scale (GSRS), the International Physical Activity Questionnaire-7, and the Migraine Disability Assessment Scale (MIDAS), respectively. Results: The CA severity was only associated with TKS (r = 0.515; P < 0.001), GSRS-total (r = 0.336; P < 0.001), GSRS-abdominal pain (r = 0.323; P < 0.001), GSRS-indigestion (r = 0.257; P = 0.002), GSRS-constipation (r = 0.371; P < 0.001), and MIDAS scores (r = 0.178; P = 0.033). Attack frequency (P = 0.015), attack duration (P = 0.035) and presence of CA (P < 0.001) were risk factors for kinesiophobia. Attack frequency (P = 0.027) and presence of CA (P = 0.004) were risk factors for GIS symptoms. Conclusions: There was a relationship between the CA and kinesiophobia, GIS symptoms, and disability. CA and attack frequency were found to be risk factors for kinesiophobia and GIS symptoms. Migraine patients with CA should be assessed in terms of kinesiophobia, GIS, and disability. Lifestyle changes such as exercise and dietary changes and/or pharmacological treatment options for CA may increase success in migraine management.

만성두통환자의 침치료효과에 대한 임상적 고찰 (Clinical study of Acupuncture effect on Chronic Headache)

  • Chronic headache is one of the most common symptoms of outpatients. There are many approaches to treat chronic headache, but it is difficult to be resolved completely. So analgesics-addicted patients are increased in numbers. Acupuncture and ear-acupuncture have been used for relief headache. But it is not sufficient to compare the effect of acupuncture on several chronic headache groups. Thus we investigate the effect of acupuncture and ear-acupuncture on several chronic headache groups and analgesics-abused groups. We researched 36 patients who were treated at Department of Acupuncture & Moxibustion, Kyung Hee Medical Center from August 1998 to July 1999. The chronic headache patients were classified into tension-type headache group, migraine group and unclassified headache group by Guide of IHS. Each group was treated with acupuncture and ear-acupuncture. The effect was assessed through questionnaires and self-rating headache index.

만성편두통 치료를 위한 측두 부위의 보툴리눔 독소 주사 자입점 제시 (A proposal of injection points of botulinum toxin into temporal region for chronic migraine)

  • 보툴리눔 독소 주사는 이마, 눈가 주름치료 등 미용성형분야 뿐만 아니라 만성 편두통(chronic migraine), 근육긴장이상(dystonia), 경직(spasticity), 측두하악장애의 치료 등에 사용되어 왔다. 특히 보툴리눔 독소 주사는 만성편두통환자의 예방적 치료요법으로 현재까지 유일하게 승인된 요법이다. 기존에 잘 알려진 운동신경에서의 마비효과와는 달리, 편두통에 대한 작용기전은 감각신경에서 말초감작과 신경원성염증과 관련되는 substance P, CGRP, glutamate 등 비콜린성 신경전달물질의 유리를 차단하여 통증신호를 차단하는 역할을 한다는 가설이 제기되고 있다. 본 논문에서는 보툴리눔독소가 갖는 진통효과에 대한 고찰과 함께 이를 통하여 추후 만성편두통환자에 대한 주사법 개발에 대한 방향성을 제시하고자 한다.

According to headache position, the distribution of acupuncture point $Yangmy\check{o}ng\;Ky\check{o}ng$ and Governer vessel was used to frontal headache, Soyang $Ky\check{o}ng$ to migraine, Taeyang $Ky\check{o}ng$ and Governer vessel to occipital headache. 6. LI4(合谷) and LU7(列缺) was used to general headache as basic acupuncture point.
