Effects of a Ventilator-associated Pneumonia Prevention Program on Incidence Rate and Endotracheal Colonization (인공호흡기 관련 폐렴 예방 프로그램이 폐렴 발생률과 기관내 균집락화에 미치는 효과)

  • Purpose: This study examined the effects of a program designed to prevent ventilator-associated pneumonia (VAP) on VAP rate and endotracheal colonization. The program focused on aspiration prevention and oral care. Methods: A nonequivalent control group post-test only design was utilized. One hundred patients admitted to a medical intensive care unit (MICU) or coronary care unit (CCU) were assigned to either a experimental group (n=50) or a control group (n=50). The participants were selected 48 hours following an endotracheal intubation. VAP prevention program given to the experimental group includes keeping the head of the bed to $30^{\circ}{\sim}45^{\circ}$ high, maintaining continuous endotracheal cuff pressure at 25 cm $H_2O$, performing endotracheal suction before change position, and providing oral care with 0.1% chlorhexidine every four hours. The control group received usual care. Data were analyzed using t-test, $x^2$ test, Mantel-Haenszel $x^2$ and Cox proportional harzard regression model. Results: The experimental group showed a lower VAP rate than the control group although the difference was not statistically significant ($x^2=0.79$, p=.375). The experimental group showed lower colonization in tracheal secretion than the control group ($x^2=14.59$, p<.001). Conclusion: Results showed that a VAP prevention program is effective in reducing colonization of tracheal secretion. Therefore, VAP prevention programs are recommended as an ICU nursing intervention.

A COMPARATIVE STUDY OF ROENTGENOGRAPHIC APPEARANCE OF PERIODONTAL OSSEOUS LESIONS (치조병소에 대한 X-선학적 비교연구)

  • The purpose of this article is to re-examine the roentgenographic appearance of alveolar osseous in an effort to determine the value of the orthopantomogram in the diagnosis of periodontal disease. A total of 158 osseous lesions from 13 human dry skulls were studied. 14 Ultra-speed periapical films, 2 bitewing films and Kodak panoramic film are used to obtain radiographs of all defects. The bisecting technic was used, with a target-film distance of. 8 inches and exposure factors of 70 kVp, 10MA and 0.4 sec. at anterior teeth, 0.6 see at posterior teeth in exposure time. For orthopantomogram, Panoura Eight-C was used with a exposure factors of 90kVp, 10MA. and 15sec. exposure time. All films were developed in a light. tight darkroom at 68°F for 4½ minutes. Comparison of orthopantomogram and intraoral films on the view-box was carefully studied in relation to the types of osseous defect visually evident; Proximal intraosseous defects, Interproximal craters, Interproximal hemisepta, Furcal defects on multirooted teeth, and Facial or Lingual one-walled defects. The results obtained were as follows; 1. Proximal osseous defects throughout the dental arches and furcal defects on facial and lingual surfaces of multirooted teeth can be identified with a high degree of accuracy from their orthopantomographic appearances. 2. Lesions on facial or lingual surface of the alveolar arches are rather difficult to locate or recognize on the dental radiographs. 3. In determining whether the proximal and furcal lesions are located facially or lingually, Orthopantomogram is superior to the conventional film Orthopantomogram obtained with standardization of head in proper position revealed the complete visualization of alveolar bone without showing occlusal surface of molars and proximal superimposition of teeth. Thus, on the standardized orthopantomograms, The roentgenographic characteristics of each defect were determined.

A Study on Shu Points Selection of the Four Seas in Lingshu Hailun (『영추(靈樞)·해론(海論)』의 사해(四海) 수혈(輸穴) 선정에 대한 고찰)

  • Objectives : To study the shu points selection of the Four Seas in the 「Hailun」 chapter of 『Lingshu』. Methods : The effects and main indications of each shu point, their anatomical position, characteristics along with similarities of disease patterns according to excessiveness and deficiency of the Four Seas were examined. Results : The selection of shu points of the Four Seas were deeply related to the effects and main indications, and the common similarity among conditions due to excessiveness and deficiency was the presence of psychological aspects. Also, when the Four Seas are connected to the Jing-Qi-Shen and Blood, marrow sea[髓海] is connected to Jing & Shen, qi sea[氣海] is connected to Qi, blood sea[血海] is connected to Blood, and sea of water and grain[水穀之海] replenishes Jing-Qi-Shen and Blood. When connected to the Four Qi Intersections, the marrow sea is connected to 'head qi with intersection', qi sea is connected to 'chest qi with intersection', sea of water and grain or blood sea is connected to 'stomach qi with intersection', otherwise sea of water and grain is connected to 'shin qi with intersection'. Conclusions : The Four Seas are designated to the shu points due to their accumulation of Qi when the main elements of Jing-Qi-Shen and Blood are deficient or excessive, allowing for the most convenient management and manipulation of the condition of these main elements. In clinical practice, the shu points of the Four Seas will likely increase treatment efficacy for conditions that include psychological aspects.

  • A control of the body posture and movement is maintained by the vestibular system, vision, and proprioceptors. Afferent signals from those receptors are transmitted to the vestibular nuclear complex, and the efferent signals from the vestibular nuclear complex control the eye movement. The postural disturbance caused by loss of the vestibular function results in nausea, vomiting, vertigo and loss of craving for life. The purpose of this study is to develop a off-vertical rotatory system for evaluating the function of semicircular canals and otolith organs, selectively, and visual stimulation system for stimulation with horizontal, vertical and 3D patterns. The Off-vertical axis rotator which stimulates semicircular canals and otolith organs selectively is composed of a comportable chair, a DC servo-motor with reducer and a tilting table controlled by PMSM. And a double feedback loop system containing a velocity feedback loop and a position feedback loop is applied to the servo controlled rotatory chair system. Horizontal, vertical, and 3D patterns of the visual stimulation for applying head mounted display are developed. And wireless portable systems for optokinetic stimulation and recording system of the eye movement is also constructed. The Gain, phase, and symmetry is obtained from analysis of the eye movement induced by vestibular and visual stimulation. Detailed data were described.

  • In this paper, we propose a real-time algorithm for estimating the relative position and velocity of a person with respect to a robot using a Kinect sensor and an extended Kalman filter (EKF). Additionally, we propose an algorithm for controlling the robot in the proximity of a person in a variety of modes. The algorithm detects the head and shoulder regions of the person using a histogram of oriented gradients (HOG) and a support vector machine (SVM). The EKF algorithm estimates the relative positions and velocities of the person with respect to the robot using data acquired by a Kinect sensor. We tested the various modes of proximity movement for a human in indoor situations. The accuracy of the algorithm was verified using a motion capture system.

  • This study investigated the size specific dose estimates of difference localizer on pediatric CT image. Seventy one cases of pediatric abdomen-pelvic CT (M:F=36:35) were included in this study. Anterior-posterior and lateral diameters were measured in axial CT images. Conversion factors from American Association of Physicists in Medicine (AAPM) report 204 were obtained for effective diameter to determine size specific dose estimate (SSDE) from the CT dose index volume (CTDIvol) recorded from the dose reports. For the localizer of mid-slice SSDE was 107.63% higher than CTDIvol and that of xiphoid-process slices SSDE was higher than 92.91%. The maximum error of iliac crest slices, xiphoid process slices and femur head slices between mid-slices were 7.48%, 17.81% and 14.04%. In conclusion, despite the SSDE of difference localizer has large number of errors, SSDE should be regarded as the primary evaluation tool of the patient radiation in pediatric CT for evaluation.

A Study on the Exquisite Revolution of the 16th Century Ruff Collar and Ruff Cuffs in the Portraitures of Queen Elizabeth I (엘리자베스 1세의 초상화에 나타난 16세기 러프 칼라와 러프 커프스의 미시적 변화 연구)

  • The portraits created in the age of Queen Elizabeth I show a very exquisite description about the ruff collar and cuffs representing the notable evolution of periodical costumes from a detailed viewpoint. This thesis analyzes the ruff collar and cuffs of the portraits of Queen Elizabeth I. The method used selects 32 pieces from the portraits of Queen Elizabeth I that are considered excellent depictions of a ruff collar and cuffs, to investigate size, form and decorations. Conclusively, the ruff collar tends to be extremely enlarged and thickened (coinciding with the development of glue) while the cuffs denote the aspect of double cuffs coupled with the turn-back cuffs without a change in size. These traits were widely shown from 1585 to 1587, while after 1588 the ruff cuffs completely disappeared in the portraits of Elizabeth I. At the same time, the change of a ruff collar was remarkable, the collar being in the erect position behind the head (with the use of supporters), of a consistent thickness and decoration of elaborate lace. During the transition period from the $16^{th}$ century to the $17^{th}$ century the earliest change was in cuffs followed by collars. This research is a helpful guide to detect the relatively exact date of portraits not definitively identified in the latter part of the $16^{th}$ century and to observe the microscopic evolution of costumes in the latter part of the $16^{th}$ century.

Location-based Clustering for Skewed-topology Wireless Sensor Networks (편향된 토플로지를 가진 무선센서네트워크를 위한 위치기반 클러스터링)

  • The energy consumption problem in wireless sensor networks is investigated. The problem is to expend as little energy as possible receiving and transmitting data, because of constrained battery. In this paper, in order to extend the lifetime of the network, we proposed a location-based clustering algorithm for wireless sensor network with skewed-topology. The proposed algorithm is to deploy multiple child nodes at the sink to avoid bottleneck near the sink and to save energy. Proposed algorithm can reduce control traffic overhead by creating a dynamic cluster. We have evaluated the performance of our clustering algorithm through an analysis and a simulation. We compare our algorithm's performance to the best known centralized algorithm, and demonstrate that it achieves a good performance in terms of the life time.

Whole brain radiotherapy using four-field box technique with tilting baseplate for parotid gland sparing

  • Purpose: The aim of this study is to evaluate the efficacy and feasibility of four-field box whole brain radiotherapy (FB-WBRT) with tilting baseplate by comparing bilateral WBRT (B-WBRT). Methods and Materials: Between March 2016 and September 2018, 20 patients with brain metastases underwent WBRT using the four-field box technique. WBRT is performed with a dose of 30 Gy in 10 fractions daily. Two computed tomography simulations per person were performed. One was in the traditional supine position for B-WBRT and the other by applying the tilting acrylic supine baseplate to elevate the head by 40° for FB-WBRT. The B-WBRT used the field-in-field technique, which is the most commonly used method in our institution. The FB-WBRT comprised anterior, posterior, and bilateral beams. A wedge was applied in anterior and posterior fields to compensate for skull convexity. Results: The average of Dmean of both parotid glands was 10.2 Gy (range, 3.8 to 17.8 Gy) in B-WBRT and 5.4 Gy (range, 2.0 to 11.7 Gy) in FB-WBRT (p < 0.05). Compared to B-WBRT, FB-WBRT reduced the mean dose of the right and left parotid glands from 10.1 Gy to 4.9 Gy and from 10.4 Gy to 5.8 Gy, respectively (p < 0.05). Further, V5, V10, V15, V20, and V25 for the parotid gland decreased significantly in FB-WBRT (p < 0.05). The Dmax and Dmean of lens decreased according to the dose-volume histogram. Conclusion: Compared to B-WBRT, FB-WBRT with a tilting baseplate is a simple and effective method that takes feature of noncoplanar beam to protect the parotid gland.

An Analysis on Kinematically Contributing Factors at Impact of Forehand Drive Motion in Squash (스쿼시 포핸드 드라이브 동작의 임팩트시 운동학적 주요요인 분석)

  • This study were obtained elapsed time phase-by-phases, displacement, user angle, velocity and angular velocity to analyse kinematically contributing factors at impact of forehand drive motion, on targeting three male players. The results of the study were presented as follows; In the forehand drive swing, the elapsed time by phases was a total of .52 seconds: .30 seconds from backswing to impact and .22 seconds from impact to follow-through, Considering the mean change in locations of COM of each(part$\rightarrow$body segment) at impact, racket head, left shoulder, right wrist and left hip, the left-right directions(X-axis) were showm to be each $.61{\pm}.03$, $1.19{\pm}.08$, $.66{\pm}.03$, $.94{\pm}.06$, and $.45{\pm}.03m$. The displacement differences of COM of each body segment were shown to be -.57, -.05, -.33, and .16m. For the vertical direction(Z-axis), the center of mass was lowest at impact and highest at E3. For the displacement of the right wrist on the left hip, the right wrist moved to .82m to the lower direction without change in the locations of the hip from E1 from E2. When the left hip moved .02m from E2 to E3, the right wrist moved .7m in the upper direction. In respect to the velocity of each body segment, the hip and the shoulder joint accelerated and then the wrist followed. Then the right wrists of all the subjects and their racket heads showed maximum speed, and an effective swing was observed. At the angle of each part, the angle of the right wrist was the smallest at the backswing and the largest at the moment of the impact. Then it increased gradually in the follow-through section. In respect of angular velocity for subject A, the hip moved and the largest change occurred. Immediately before the impact, the subject made a swing using his right wrist, his hip, and the shoulder joint, showing the maximum value, which was judged to be effective.
