Absorbed and effective dose for periapical radiography using portable and wall type dental X-ray machines (이동형 구내방사선촬영기와 벽걸이 구내방사선촬영기로 촬영한 치근단 방사선촬영에서 환자의 흡수선량과 유효선량 평가)

  • Purpose: The purpose of this study was to measure the absorbed dose and to calculate the effective dose for one periapical radiography using the portable and wall type dental X-ray machines. Materials and methods: Thermoluminescent chips were placed at 25 sites throughout the layers of the head and neck of a tissue-equivalent human skull phantom. The man phantom was exposed with the portable and wall type dental X-ray machines. For one periapical radiography taken by portable dental X-ray machine, the exposure setting was 60 kVp, 2 mA and 0.2 seconds, while for one periapical radiography taken by wall type dental X-ray machine, exposure setting was 70 kVp, 8 mA and 0.074 seconds. Absorbed dose measurements were performed and equivalent doses to individual organs were summed using ICRP 103 to calculate effective dose. Results: In the upper anterior periapical radiography using portable dental X-ray machine and in the lower posterior periapical radiography using both machines, the highest absorbed dose was recorded at the mandible body. The effective dose in upper anterior periapical radiography using portable and wall type dental X-ray machines was $4{\mu}Sv$, $2{\mu}Sv$, respectively. In the lower posterior periapical radiography, the effective dose for each portable and wall type dental X-ray machines was $6{\mu}Sv$, $2{\mu}Sv$. Conclusion: It was recommended that the operator use prudently potable dental X-ray machine because that the effective dose in the periapical radiography using wall type dental X-ray machine was lower than that in the periapical radiography using portable dental X-ray machine.

Influence of the implant-abutment connection design and diameter on the screw joint stability

  • PURPOSE. This study was conducted to evaluate the influence of the implant-abutment connection design and diameter on the screw joint stability. MATERIALS AND METHODS. Regular and wide-diameter implant systems with three different joint connection designs: an external butt joint, a one-stage internal cone, and a two-stage internal cone were divided into seven groups (n=5, in each group). The initial removal torque values of the abutment screw were measured with a digital torque gauge. The postload removal torque values were measured after 100,000 cycles of a 150 N and a 10 Hz cyclic load had been applied. Subsequently, the rates of the initial and postload removal torque losses were calculated to evaluate the effect of the joint connection design and diameter on the screw joint stability. Each group was compared using Kruskal-Wallis test and Mann-Whitney U test as post-hoc test (${\alpha}$=0.05). RESULTS. The postload removal torque value was high in the following order with regard to magnitude: two-stage internal cone, one-stage internal cone, and external butt joint systems. In the regular-diameter group, the external butt joint and one-stage internal cone systems showed lower postload removal torque loss rates than the two-stage internal cone system. In the wide-diameter group, the external butt joint system showed a lower loss rate than the one-stage internal cone and two-stage internal cone systems. In the two-stage internal cone system, the wide-diameter group showed a significantly lower loss rate than the regular-diameter group (P<.05). CONCLUSION. The results of this study showed that the external butt joint was more advantageous than the internal cone in terms of the postload removal torque loss. For the difference in the implant diameter, a wide diameter was more advantageous in terms of the torque loss rate.

Effect of denture cleansers on Candida albicans biofilm formation over resilient liners

  • PURPOSE. The purpose of this study was to analyze the effect of denture cleansers on Candida albicans biofilm formation over resilient liners and to evaluate compatibility between resilient liners and denture cleansers. MATERIALS AND METHODS. Acrylic resin (Lucitone 199$^{(R)}$) and 3 resilient liners (COE-SOFT$^{TM}$, GC RELINE$^{TM}$ and SOFRELINER TOUGH TOUGH$^{(R)}$) were incubated in denture cleansers (Polident$^{(R)}$ and Cleadent$^{(R)}$) for 8 hours a day and in unstimulated saliva for 16 hours a day (n=25/gp) for 60 days. Two-way and three-way repeated measures ANOVA were performed to compare the surface roughness (Ra), pH and C. albicans binding level by radioisotope (${\alpha}$=0.05). The statistical significance of the relation between Ra and adhesion was evaluated by correlation analysis. RESULTS. The degree of Ra was significantly decreased in the following order: COE-SOFT$^{TM}$, acrylic resin, GC RELINE$^{TM}$ and SOFRELINER TOUGH$^{(R)}$. The immersion in denture cleansers significantly increased Ra of resilient liners, except for SOFRELINER TOUGH$^{(R)}$ in Cleadent$^{(R)}$. No significant differences in pH curves were observed among groups immersed in distilled water and denture cleansers. The binding levels of C. albicans were significantly decreased in the following order: COE-SOFT$^{TM}$, GC RELINE$^{TM}$, SOFRELINER TOUGH$^{(R)}$, and acrylic resin. The immersion in Cleadent$^{(R)}$ seemed to decrease C. albicans binding level on GC RELINE$^{TM}$ and SOFRELINER TOUGH$^{(R)}$. CONCLUSION. Based on the C. albicans binding levels results, it is not recommended to immerse COE-SOFT$^{TM}$ in denture cleansers, and GC RELINE$^{TM}$ and SOFRELINER TOUGH$^{(R)}$ should be immersed in Cleadent$^{(R)}$.

The Result and Attentiveness of Reconstructive Surgery by Anterolateral Thigh Perforator Free Flap (전외측 대퇴부 천공지 유리피판술의 실패 원인과 합병증)

  • Purpose: Anterolateral thigh (ALT) perforator free flap is commonly used because of its various benefits. The author reports important factors of preoperative and perioperative ALT perforator free flap and causes of failure. Methods: 84 patients who were treated with ALT perforator free flap from December 2004 to 2008, and February 2010 to April 2010 were studied. 61 patients were male and 23 were female. The mean age of patients was 51.1. The main cause was neoplasm and the main reconstructive areas were head and neck area. The size of flap was various from $3{\times}4$ to $12{\times}18$ cm. 6 patients received split thickness skin graft at donor site. Preoperative angiography was checked to all patients. Results: Among the 84 patients, partial necrosis of flaps occurred in 4 patients because of atherosclerosis, varicose vein, or inattention of patient, etc. And total flap necrosis in 5 patients because of abnormal vessels of recipient area or delay of operation, etc. One case of serous cyst was found as the complication of donor area. Two cases of skin graft on donor site were done because of suspected muscle compartment syndrome, 4 cases of that because of large flap. Septocutaneous perforators were found in 7 cases. The author couldn't find reliable perforator in 3 cases, ipsilateral anteromedial thigh perforator and contralateral ALT perforator and latissimus dorsi musculocutaneous free flap were done instead of ALT. There was no case which needed reoperation because of the impairment of blood supply, and 3 cases were revised by leech because of the burn injury by a lamp or venous congestion. Conclusion: Although ALT perforator free flap is widely used with its various merits, many factors such as preoperative condition of donor or recipient area, morphology of defect and operating time need to consider to prevent flap necrosis. And operators should need careful technique because septocutaneous perforator is uncommon, and musculocutaneous perforator is common but difficult to dissect.

Towards Routine Clinical Use of Radial Stack-of-Stars 3D Gradient-Echo Sequences for Reducing Motion Sensitivity

  • Purpose : To describe how a robust implementation of a radial 3D gradient-echo sequence with stack-of-stars sampling can be achieved, to review the imaging properties of radial acquisitions, and to share the experience from more than 5000 clinical patient scans. Materials and Methods: A radial stack-of-stars sequence was implemented and installed on 9 clinical MR systems operating at 1.5 and 3 Tesla. Protocols were designed for various applications in which motion artifacts frequently pose a problem with conventional Cartesian techniques. Radial scans were added to routine examinations without selection of specific patient cohorts. Results: Radial acquisitions show significantly lower sensitivity to motion and allow examinations during free breathing. Elimination of breath-holding reduces failure rates for non-compliant patients and enables imaging at higher resolution. Residual artifacts appear as streaks, which are easy to identify and rarely obscure diagnostic information. The improved robustness comes at the expense of longer scan durations, the requirement for fat suppression, and the nonexistence of a time-to-center value. Care needs to be taken during the configuration of receive coils. Conclusion: Routine clinical use of radial stack-of-stars sequences is feasible with current MR systems and may serve as substitute for conventional fat-suppressed T1-weighted protocols in applications where motion is likely to degrade the image quality.

Using Implementation Science to Advance Cancer Prevention in India

  • Oral, cervical and breast cancers, which are either preventable and/or amenable to early detection and treatment, are the leading causes of cancer-related morbidity and mortality in India. In this paper, we describe implementation science research priorities to catalyze the prevention and control of these cancers in India. Research priorities were organized using a framework based on the implementation science literature and the World Health Organization's definition of health systems. They addressed both community-level as well as health systems-level issues. Community-level or "pull" priorities included the need to identify effective strategies to raise public awareness and understanding of cancer prevention, monitor knowledge levels, and address fear and stigma. Health systems-level or "push" and "infrastructure" priorities included dissemination of evidence-based practices, testing of point-of-care technologies for screening and diagnosis, identification of appropriate service delivery and financing models, and assessment of strategies to enhance the health workforce. Given the extent of available evidence, it is critical that cancer prevention and treatment efforts in India are accelerated. Implementation science research can generate critical insights and evidence to inform this acceleration.

Down Regulation of miR-34a and miR-143 May Indirectly Inhibit p53 in Oral Squamous Cell Carcinoma: a Pilot Study

  • Background: Aberrant microRNA expression has been associated with the pathogenesis of a variety of human malignancies including oral squamous cell carcinoma (SCC). In this study, we examined primary oral SCCs for the expression of 6 candidate miRNAs, of which five (miR-34a, miR-143, miR-373, miR-380-5p, and miR-504) regulate the tumor suppressor TP53 and one (miR-99a) is involved in AKT/mTOR signaling. Materials and Methods: Tumor tissues (punch biopsies) were collected from 52 oral cancer patients and as a control, 8 independent adjacent normal tissue samples were also obtained. After RNA isolation, we assessed the mature miRNA levels of the 6 selected candidates against RNU44 and RNU48 as endogenous controls, using specific TaqMan miRNA assays. Results: miR-34a, miR-99a, miR-143 and miR-380-5p were significantly down-regulated in tumors compared to controls. Moreover, high levels of miR-34a were associated with alcohol consumption while those of miR-99a and miR-143 were associated with advanced tumor size. No significant difference was observed in the levels of miR-504 between the tumors and controls whereas miR-373 was below the detection level in all but two tumor samples. Conclusions: Low levels of miR-380-5p and miR-504 that directly target the 3'UTR of TP53 suggest that p53 may not be repressed by these two miRNAs in OSCC. On the other hand, low levels of miR-34a or miR-143 may relieve MDM4 and SIRT1 or MDM2 respectively, which will sequester p53 indicating an indirect mode of p53 suppression in oral tumors.

Retrospective Analysis of 498 Primary Soft Tissue Sarcomas in a Single Turkish Centre

  • Background: Soft tissue sarcomas (STS) must be managed with a team involving pathologists, radiologists, surgeons, radiation therapists and medical oncologists. Treatment modalities and demographic charasteristics of Turkish STS were analysed in the current study. Material-Methods: Primary adult STS followed between 1999-2010 in Cukurova University Medical Faculty Department of Medical Oncology were analzied retrospectively Results: Of the total of 498 patients, 238 were male and 260 female. The most seen adult sarcomas were leomyosarcoma (23%). Localization of disease was upper extremity (8.8%), lower extremity (24.7%), head-neck 8.2%, thoracic 8%, retroperitoneal 5.6%, uterine 12.4%, abdominal 10%, pelvic region 3.6 and other regions 10%. Some 13.1% were early stage, 10.2% locally advanced, 8.2% metastatic and 12.2% recurrent disease. Patients were treated with neoadjuvant/adjuvant (12%) or palliative chemotherapy (7.2%) and 11.4% patients did not receive chemotherapy. Surgery was performed as radical or conservative. The most preferred regimen was MAID combination chemotherapy in the rate of 17.6%. The most common metastatic site was lung (18.1%). The overall survival was 45 months (95%CI 30-59), 36 months in men and 55 months in women, with no statistically significant difference (p=0.5). The survival rates were not different between the group of adjuvant and palliative chemotherapy (respectively 28 versus 18 months) (p=0.06), but radical surgery at 37 months was better than 22 months for conservative surgery (p=0.0001). No differences were evident for localization (p=0.152). Locally advanced group had higher overall survival rates (72 months) than other stages (p=0.0001). Conclusion: STS can be treated successfully with surgery, chemotherapy and radiotherapy. The survival rates of Turkish people were higher in locally advanced group; these results show the importance of multimodality treatment approach and radical surgery.

A Clinical Review on 143 Cases of Basal Cell Carcinoma (143례의 기저세포암에 대한 임상적 고찰)

  • Purpose: Basal cell carcinoma is one of the most common cancers in the western population and the annual incidence rate is still on an increasing course. In Asian countries such as Korea, the incidence of basal cell carcinoma is reported to be remarkably low but is estimated to be in a steep increase nowadays. This study was to analyze the recent clinical trends of basal cell carcinoma in Korea by reviewing a single institution's experience. Methods: Throughout an 11-year period, the surgical excision of 143 cases of basal cell carcinoma was performed in our department. General data of these cases such as the primary site of cancer, age and sex of the patient, operative methods, recurrence rate were reviewed retrospectively. Results: Among the 143 patients included in this study, 82 patients were men and 61 patients were women. The ages ranged from 27 to 89 years with a mean age of 64.0 years. The head and neck region was the most frequently involved primary site for the cancer as 95.1% of the total cases occurred in this area. Especially the nose and perinasal area were the most frequent region, which was followed by the periorbital area, cheek, and perioral area. During this study period, 9 cases showed recurrence of the cancer as the overall recurrence rate was 6.3%. Conclusion: Gradual increase in the incidence of basal cell carcinoma was demonstrated in this study. Basal cell carcinoma showed high incidence in the old-age population as 68.5% of the total patients were more than 60 years of age. High recurrence rate was noted in the nasal region especially after local flap reconstruction. A more cautious approach is to be required when handing such high risk lesions. Multiple factors such as extended life span, increased outdoor leisure activity and exposure to sunlight, higher accessibility to medical services and increased understanding of the public about skin cancer are assumed to be the main reasons for this increase of basal cell carcinoma.

The Effect of Optokinetic Stimulation with Smooth Pursuit Eye Movement to Chronic Hemispatial Neglect: A Pilot Study (완곡 추적 안구운동을 동반한 시운동자극이 만성 편측무시에 미치는 영향 : 예비연구)

  • Objective: This study aim is to identify whether 30minutes optokinetic stimulation(OKS) with smooth pursuit eye movement can alleviate chronic hemispatial neglect. Methods: We applied optokinetic stimulation with smooth pursuit eye movement to 2 chronic hemispatial neglect patients. experimental duration was total 4weeks - 1week baseline, 2weeks OKS intervention, 1week 2nd. baseline. The intervention was 10 OKS sessions (30min each, 1session daily, from Monday to Friday) over a period of 2weeks. The neglect test carried out 3 times a week every other day. The OKS was provided on the screen when patient sat in front of the screen(13.5 inch). The OKS video that the 24 yellow squares moving coherently from the right to the left side. Patients were instructed to perform smooth pursuit eye movement without head and neck movement. Results: As a result of the OKS for 2weeks, the degree of neglect trends to decrease on both 2 cases compared to baseline A. The degree of the neglect of subject 1 tended to increase the at the baseline A. In contrast, the subject 2 showed that tendency that the degree of the neglect declined additionally at the baseline A. Conclusions: We identified that 30 minutes optokinetic stimulation with sooth pursuit eye movement is effective intervention method for chronic hemispatial neglect.
