Unexpected postoperative atlantoaxial rotatory subluxation after excision of melanocytic nevi of the head and neck in older children: two case reports and literature review

  • Postoperative atlantoaxial rotatory subluxation (AARS) is a rare complication that develops almost exclusively in children following oropharyngeal and otologic surgeries, proposing that oropharyngeal inflammatory responses and excessive head rotation are responsible factors. However, there have been no reports of AARS after excision of a nevus on the head and neck. Here, we present two cases of AARS following limited head rotation during simple nevus excision. Patient 1, a 9-year-old girl, complained of neck pain and limited range of motion after excision of the nevus on the neck. After 2 months, computed tomography and magnetic resonance imaging finally revealed AARS with a ruptured transverse atlantal ligament. A month of halo traction was required for the treatment. Patient 2, an 11-year-old girl, presented with immediate pain and limited neck extension after tissue expander insertion under the upper chest and excision of the nevus on her left cheek. The diagnosis was promptly made using cervical spine radiography. A cervical collar was applied for 1 month. Both patients recovered without any complications after treatment. This report highlights the importance of suspicion for AARS after surgery regardless of surgical duration or amount of head rotation.

A case of retropharyngeal hematoma induced by whiplash injury (편타성 상해로 인한 후인두혈종 1례)

  • Whiplash injury commonly seen in automobile accident. This type of acceleration-deceleration injury may rarely lead to unfavorable outcomes as in the case of retropharyngeal hematoma. Because this lesion has the possibility of compressing the potential airway acutely or gradually, not only the rapid assessment and treatment but also closed observation are needed. We report a case of a 20-year-old man, who sustained a whiplash injury from contact with the headrest of his seat after his car was involved in a rollover. He had no symptom except posterior neck pain initially, but complained odynophagia and acute dyspnea after 10 hours of admission. The diagnostic work-up comprising lateral radiograph, CT imaging disclosed the rare constellation of a retropharyngeal hematoma with cervical spine fractures. Medical treatment and close observation were sufficient for the patient because he had no progressive dyspnea or major dysphagia.

Correlation Analysis between Cervical-Vertebra Angle and Neck Range of Motion, Muscle Strength, and Sternocleidomastoid Thickness (전방머리자세 대상자의 머리-척추각과 목 운동범위, 근력, 목빗근 두께와의 상관관계 분석)

  • Background: This study was to investigate effects of Correlation Analysis between Cervical-Vertebra Angle and Neck Range of Motion, Muscle Strength, Sternocleidomastoid Thickness of Patients with Forward Head Posture Design: Correlation Analysis. Methods: The subjects of this study were a total of 54 people in the forward head position and their ages were between 30 and 50 years old. The subjects cranio-vertebral angles, neck extension, neck flexion, neck rotation angles, neck flexor strength, neck extensor strength, sternocleidomastoid thickness were evaluated through measuring instruments. The thickness of the sternocleidomastoid muscle was measured using an imaging ultrasound diagnostic device (ultra sound, Versana Premier, GE Medical systems, China). CVA was measured by measuring the side photo of the subject was taken with a camera and evaluated.. neck joint range of motion was measured through digital inclinometer for extension, flexion, and neck rotation. neck muscle strength was measured by measuring the using a digital sthenometer. Data analysis in this study was statistically processed using SPSS version 26.0 (IBM SPSS Inc., USA). Correlation analysis was used and the statistical significance level was set at 0.05. Results: The results neck extension(r= 0.70**), neck flexion(r= 0.67**), neck rotation(r= 0.56**), neck extensor muscle strengt(r= 0.85**), neck flexor muscle strength(r= 0.66**), sternocleidomastoid thicknes(r= -0.81**) It indicates that there is a correlation. Conclusion:These results improve the Cervical-vertebra angle of patients with forward head posture should include a program to improve the thickness of the SCM. In the future, study can be used as an evidentiary material for treatment interventions to improve the Cervical-vertebra angle of patients with forward head posture.

  • Respiratory epithelial adenomatoid hamartoma (REAH) in the head and neck is a rare benign lesion containing glandular tissue covered with ciliated respiratory epithelium. In the head and neck, REAH of the nasal cavity, paranasal sinuses, and nasopharynx have been reported in literature. Due to rareness of REAH and insufficient knowledge of its imaging features, the diagnosis can be challenging when we encounter a non-specific cystic mass at an uncommon site in the head or neck. Here, we report the case of a pathologically confirmed REAH showing a cystic mass centered at the buccal space (retromaxillary fat pad) with CT and MRI findings.

  • A combination of positron emission tomography (PET) with $^{18}F$-labeled fluoro-2-deoxyglucose ($^{18}F$-FDG) and computed tomography ($^{18}F$-FDG-PET/CT) has increasingly become a widely used imaging modality for the diagnosis and management of head and neck cancer. On the basis of both recent literature and our professional experience, we present a set of principles with pictorial illustrations and clinical applications of FDG-PET/CT in the evaluation and management planning of squamous cell carcinoma of the oral cavity and oropharynx. We feel that this paper will be of interest and will aid the learning of oral and maxillofacial radiology trainees and practitioners.

A Case of Hemangioma of Nasal Cavity in Von Hippel-Lindau Syndrome (Von Hippel-Lindau 증후군 환자에서 발생한 비강내 혈관종)

  • We report a unique case of hemangioma of the nasal cavity in von Hippel-Lindau (VHL) syndrome. A 26-year-old female with VHL syndrome who had previously undergone surgery for pancreatic and adrenal mass presented with a 4-month history of left-sided nasal obstruction. The patient had an expansile mass lesion in the left nasal cavity and an imaging test demonstrated the mass in the left maxillary sinus extending to the nasal cavity. The tumor was removed with an endoscopic prelacrimal recess approach considering the possibility of not only a benign tumor such as hemangioma but also a malignant tumor of the maxillary sinus and histopathologic examination confirmed cavernous hemangioma. This case is the rarely reported manifestation of the paranasal sinus in VHL disease.

A Case of Solitary Metastatic Deposit in the Orbital Rim from Follicular Thyroid Cancer (단일성 안와 골병변으로 확인된 갑상선 여포암의 원격 전이 1예)

  • Follicular thyroid carcinoma (FTC) is the second most common thyroid cancer, following papillary carcinoma. Metastasis to the orbital rim from FTC is very rare. We recently experienced a case of FTC with metastasis to the orbital rim in a 74-year-old woman, who initially presented with a huge thyroid mass and an asymptomatic solitary orbital rim lesion. The solitary orbital rim lesion was suspected to be a separate disease entity such as lymphoma from the preoperative imaging, but bone metastasis from FTC was finally confirmed after orbital rim resection and total thyroidectomy. During follow-up, the patient presented multiple bone metastasis, so the solitary orbital rim lesion was considered a clinical sign of systemic metastasis from FTC. Therefore, we present this unique case with a review of the literature.

Transient osteoporosis of the hip with a femoral neck fracture during follow-up: a case report

  • We report a case of transient osteoporosis of the hip with a femoral neck fracture found during follow-up. A 53-year-old man presented with left hip pain without trauma. The pain did not improve after 2 weeks and he was brought to our hospital by ambulance. Magnetic resonance imaging (MRI) of the left hip joint showed diffuse edema in the bone marrow, which was identified by low signal intensity on T1-weighted images, high signal intensity on T2-weighted images, and increased signal intensity on short tau inversion recovery. This edema extended from the femoral head and neck to the intertrochanteric area. He was diagnosed with transient osteoporosis of the left hip. Rest gradually improved his pain; however, 3 weeks later, his left hip pain worsened without trauma. X-ray, computed tomography, and MRI results of the hip joint demonstrated a left femoral neck fracture, and osteosynthesis was performed. Differential diagnoses included avascular necrosis of the femoral head, infection, complex regional pain syndrome, rheumatoid arthritis, leukemia, and other cancers. Transient osteoporosis of the hip generally has a good prognosis with spontaneous remission within a few months to 1 year. However, a sufficient length of follow-up from condition onset to full recovery is necessary to avoid all probable complications such as fractures.

A Case of Lymphoepithelial Cyst Mimicking Cervical Plexus Schwannoma (경부신경총에서 기인한 신경초종으로 오인된 림프상피성낭종 1예)

  • Neck masses can have various origins and diverse presentations, making accurate diagnosis challenging. Schwannomas and lymphoepithelial cysts are commonly encountered neck tumors, and it is hard to get differential diagnosis. A 50-year-old woman presented with a progressively enlarging right neck mass discovered five years ago. Upon examination, a 3 cm firm, non-tender, and mobile tumor was found in the right neck level II region. Imaging studies suggested a tumor originating from the cervical sympathetic chain. The patient underwent a resection of the tumor under general anesthesia. Postoperative follow-up showed no complications or signs of recurrence. However, contrary to the initial suspicion of a schwannoma, the final pathological examination revealed a lymphoepithelial cyst. In this paper, we present a case of a cervical lymphoepithelial cyst misdiagnosed as a schwannoma, aiming to compare the clinical and histological characteristics of these two tumors and provide insights into appropriate diagnosis and management.
