Frailty in Geriatric Patients with Head and Neck Cancer and its Implication in Survivorship (노인 두경부암 환자의 생존에 있어 노쇠의 의의)

  • The aging population, particularly those aged 65 and above, is on the rise, with projections indicating a substantial increase in the elderly demographic. This demographic shift brings challenges in managing age-related diseases, including head and neck cancers (HNCs). Frailty, often characterized by physiological decline and vulnerability to stressors, is a crucial factor affecting treatment outcomes of elderly cancer patients. Accordingly, the significance of assessing frailty in elderly HNC patients before their treatment should be emphasized, but current frailty assessment tools may not fully capture the unique challenges faced by HNC patients. Specific indicators, including respiratory and swallowing functions, are proposed for a more tailored assessment. This comprehensive review explores the impact of frailty on various treatment modalities, including surgery, radiation, and chemotherapy, highlighting the need for personalized interventions. Furthermore, it suggests avenues for future research to enhance frailty assessment tools and investigate interventions aimed at improving treatment outcomes in elderly HNC patients.

  • The Mantle cell lymphoma(MCL) is a relatively rare subtype of malignant lymphoma characterized by chromosomal translocation t(11 ; 14)(q13 ; q32), positive response for CD5, and nuclear cyclin D1. It is account for an estimated 3-6% of all non-Hodgkin's lymphoma. The involvement of extra-nodal site is not uncommon, whereas salivary glands are rarely affected. It is more commonly occurred in men and old age and approximately 75% of cases are diagnosed with advanced stage. It is usually characterized by an aggressive clinical course, and the prognosis is poorer than other type of head and neck lymphoma. We recently encountered a 69-year-old female with mass in parotid tail and upper neck, and it was diagnosed as mantle cell lymphoma. We report the unique case with a review of literature.

  • Thyroid hemiagenesis is rare congenital anomaly that one lobe of thyroid fails to develop. It is often asymptomatic and discovered incidentally with other thyroid disease such as hypothyroidism, hyperthyroidism, multinodular goiter, benign adenoma and cancer. Most cases reported are left thyroid lobe agenesis and occurred in female. Compensatory hypertrophy occurs in most cases. Many cases are asymptomatic and detected incidentally, so awareness of its existence can help its detection and proper treatment. Here, we report a case of thyroid hemiagenesis accompanying hypothyroidism with a review of literature.

  • 본방사선 치료 후에 후두 절제술을 시행할 경우 인두피부누공, 감염, 출혈 등 상처부위 합병증의 가능성이 높다. 저자들은 최근에 항암치료와 방사선 치료 후에 재발한 성문상부암에 대해 후두 전적출술을 시행하였고, 수술 66일째 생명을 위협하는 인두의 동맥 출혈을 경험하였기에 문헌고찰과 함께 보고하는 바이다.

  • Salivary gland tumors take possession of almost 5% in head and neck malignancies. Among these, mucoepidermoid carcinoma(MEC) is most common malignany in major salivary glands(12-29%) and the parotid gland is most predilection site. Intra-oral MEC has a tendency to various locations, and the predilection sites are palate, cheek, mandible, lip and tongue in order of frequency. A few cases of MEC are occurred in with retromolar trigone, oropharynx, and ectopic salivary gland. Recently, we experienced a-65-year old woman with retromolar trigonal mass, and she was finally diagnosed as MEC. We report it with review of literature.

  • Low-grade fibromyxoid sarcoma (LGFMS), a soft tissue tumor that has high recurrence and metastasizing potential, rarely occurs in the head and neck region. Therefore, the treatment for LGFMS in the facial area is challenging in terms of cosmetic and functional maintenance. The authors report a case of LGFMS in the posterior nasal cavity. It was completely removed in parallel with the nasal endoscopic and oral approach, but the lower margin was closed by preserving the soft palate. The patient is well without any recurrence or metastasis after 5 years of follow-up.

  • The malignant peripheral nerve sheath tumor(MPNST) is an extremely rare soft tissue sarcoma of ectomesenchyme also known as malignant schwannoma or malignant neurilemoma. It is a highly malignant sarcoma, which is locally invasive, frequently leading to multiple recurrences and eventual metastatic spread. The peak incidence of this disease is known to arise in the seventh decades. In human body, trunk and extremities are the most commonly involved sites, and only 8-14% of all lesions appear in head and neck region. Because immunohistochemical staining is essential in final diagnosis. The authors report a case of malignant peripheral nerve sheath tumor in neck with hypopharyngeal cancer simultaneously on a review of the literature.

  • Neuroendocrine carcinomas are rare neoplasms of the hypopharynx. Neuroendocrine neoplasms are divided into four main types : carcinoid, atypical carcinoid, small cell carcinoma and paraganglioma. The diagnosis is primarily based on light microscopy and should be confirmed by immunohistochemical investigation. Small cell neuroendocrine carcinoma of the hypopharynx is extremely uncommon tumor and surgical results for this tumor have been disappointing. Chemotherapy and radiotherapy currently appear the most effective forms of therapy. We report our case of small cell neuroendocrine carcinoma of the hypopharynx with the brief review of literatures.

  • Carcinosarcoma of the parotid gland is extremely uncommon and is known to be an aggressive malignant mixed tumor in which carcinomatous and sarcomatous elements coexist and metastasize together. We report a case of recurrent parotid gland carcinosarcoma that showed squamous cell carcinoma and chondrosarcoma in a 58-year-old woman.

  • Adenoid cystic carcinoma(ACC) is one of the common malignant tumor of the major and minor salivary glands. ACC arising from the larynx is relatively rare(less than 1% of laryngeal malignant tumors) and only about eighty cases have been reported in the English literature. Definite diagnosis of these lesions is made only from a histological analysis, because findings and symptoms are non-specific. The diagnosis progresses very slowly, therefore it often presents at an advanced stage with regional and distant metastasis. Here, we present one case of ACC of the supraglottic region with a review of literature.
