Development of an EMS Thoracic Support Harness for Upper Posture Correction (상부 자세 교정을 위한 EMS 흉추 보호대 개발)

  • This study developed a thoracic support harness designed to contract the lower trapezius muscle through electrical muscle stimulation (EMS) applied to the subscapularis region. Fifteen female office workers in their 30s who were diagnosed with forward head posture syndrome (FHP) participated in experiments evaluating stiffness and rigidity of the upper trapezius muscle. The experiment measured FHP Angle (FHPA), Neck Flexion Angle (NFA), Neck Extension Angle (NEA), Neck Rotation Angle (NRA), and Visual Analog Scale (VAS) for pain perception and comfort. Participants were divided into four groups (A, B, C, D) for sequential testing. Group A served as control, Group B performed upper trapezius stretching (UTS), Group C received EMS to stimulate weakened lower trapezius muscle. Group D underwent both UTS and EMS concurrently. After intervention, cervical angles and VAS scores were compared between groups. Statistical analysis using t-tests revealed significant differences between Group A and Group B, C, or D. ANOVA was used to test mean differences among groups, followed by Tukey-Kramer post-hoc tests to identify specific group differences. Results indicated that stretching or EMS alone had limited effects, while a combination of stretching and EMS showed significant benefits. The comfort level of the harness was rated at an average of 3.86 with a standard deviation of 2.38

Deep neck flexor endurance in university students: normative data and reliability

  • Objective: Deep neck flexor muscle endurance (DNFET) is important to cervical pain patients. However, there is no normative data of the DNFET hold time of Korean university students. The purpose of this study was to provide normative data and the reliability of the DNFET times of Korean university students and to compare the DNFET hold times between male and female subjects. Design: Cross-sectional study. Methods: The participants included 39 male and 39 female students with no cervical pain. Each DNFET was measured while the subjects kept their chin tucked in while in a supine (hook-lying) position and with the head lifted 2.5 cm off the bed. The DNFET was conducted on each subject twice and the mean values were obtained. After each measurement, the participants rested for 5 minutes. Inter-rater reliability was measured by intraclass correlation coefficient (ICC) by three separate evaluators. Results: The inter-rater reliability was good, showing an ICC (2,3) value = 0.785 (95% confidence interval, 0.370-0.942). The DNFET time scores for men and women were $25.14{\pm}9.96$ seconds and $15.23{\pm}6.10$ seconds, respectively, in which the time scores of the men were significantly longer compared to women (p<0.05). Conclusions: Asymptomatic men displayed higher DNFET scores than women. This study can help clinicians test cervical function of patients and set an interventional goal. These findings serve as a basis that insists Korean women to increase their amount of physical activity.

A Case of Intramuscular Abscess in the Sternocleidomastoid Muscle after Intramuscular Stimulation (IMS) (근육내 자극요법 후 목빗근내 농양의 치험례)

  • Purpose: Intramuscular stimulation (IMS) shows good results in the treatment of chronic pain patients who did not respond to other treatments such as oral analgesics, trigger point injection, nerve block and epidural steroid injection. But, IMS procedure especially, patients with diabetes mellitus (DM) has sometimes serious problem. So, we present a very rare case of intramuscular abscess in the sternocleiomastoid muscle after IMS with literature review. Methods: A 66 year old male visited our department 7 days after IMS in the neck. His premorbid conditions and risk factors of deep neck infection was DM and old age. Computed tomographic scans of the head and neck region were performed in this patient: signs of deep neck infection, were seen enhanced abscess in the sternocleidomastoid muscle, cellulitis overlying tissue of the neck, and air bubbles involved muscle. Necrotic wound was excised serially and we treated this with the Vacuum-assisted closure (VAC, Kinetics Concepts International, San Antonio, Texas) system device. After appropriately shaping the sponge in the pockets, continuous negative pressure of 125 mmHg was applied. The VAC therapy was utilized for a period of 12 days. Results: We obtained satisfactory results from wide excision, drainage of the abscess with the VAC system, and then primary closure. The postoperative course was uneventful. Results: We suggest that many of the infectious complications may be preventable by strict adherence to aseptic techniques and that some of the other complications may be minimized by refining the techniques with a clear understanding of the medical disorders of patients. And, the refined technique using the VAC system can provide a means of simple and effective management for the cervical intramuscular abscess, with better cosmetic and functional results.

Hip Range of Motion Estimation using CT-derived 3D Models (CT기반 3차원 모델을 이용한 고관절 운동범위 예측)

  • The success of the total hip arthroplasty is revealed as initial stability, range of motion, and long term pain, etc. Depending upon choice of implantation options such as femoral neck offset, diameter of the femoral head, the lateral opening tilt. Especially the impingement between femoral head component and acetabular cup limits the range of motion of the hip. In this sense, estimation or evaluation of the range of motion before and after the total hip arthroplasty is important. This study provides the details of a computer simulation technique for the hip range of motion of intact hip as well as arthroplasty. The suggested method defines the hip rotation center and rotation axes for flexion and abduction, respectively. The simulation uses CT-based reconstructed 3D models and an STL treating software. The abduction angle of the hip is defined as the superolateral rotation angle from sagittal plane. The flexion angle of the hip is defined as the superoanterior angle from the coronal plane. The maximum abduction angle is found as the maximum rotation angle by which the femoral head can rotate superolaterally about the anterior-posterior axis without impingement. The maximum flexion angle is found as the maximum rotation angle by which the femoral head can rotate superoanteriorly about the medial-lateral axis without impingement. Compared to the normal hip, the total hip replacement hip showed decreased abduction by 60 degrees and decreased flexion by 4 degrees. This measured value implies that the proposed measurement technique can make surgeons find a modification of increase in the femoral neck offset or femoral head, to secure larger range of motion.

  • Ge-Fu(歌賦) means prose and poetry individually, and they both have a meaning of all the rhythmical poetrical compositions making it ease for people to remember the content. All the Ge-Fus used in oriental medicine are made in Yuan(元), Mine(明)and Qing(靑)dynasty, and they have been largely used in most of all the fields of medicine such as Ben-Cao(本草), Tang-Ye(湯液), Zhen-Jiu(鍼灸), Zhen-Duan(診斷). Zhen-Jiu-Ge-Fu(鍼灸歌賦) has about 90 poetries and 10 proses and they contain the names of meridian; courses of meridian streams; accurate positions of acupuncture points; functions; effects; meanings of the name of acupuncture points; usages and effects of special points; manipulations of reinforcing and reducing method; contraindications of acupuncturing; principles of selections and orders of acupuncture points in therapy; and eight diadgoses. Zhen-Jiu-Ge-Fu is subdivided into Jing-Xue(經穴歌), Zhi-Jiu-Ge(刺灸歌), Shu-Xue-Ge(輸穴歌), Zhi-Liao-Ge(治療歌). And In Zhi-Liao-Ge, the most brief and essential Ge-Fu-Si-Zong-Sue-Ge- contains theraputic designs using far apart acpuncture points from the right painful areas in the body. In this study, the author opinionated the Si-Zong-Sue-Ge can be the prototype of the distant needling; the research on this can open the importance of Ge-Fus. On conclusion, 1. "Upper and lower Abdomen - Zu-San-Li($S_{36}$) (肚腹三里留)" means when there are problems and disorders in upper and lower abdomen, distinctively, such as gastric pain, maldigestion, flatulence, abdominal pain, constipation, diarrhea, vomiting, menstrual disorer, knee pain and tonic functioning, 족삼리 can be a right choice for distant needling point for treating. 2. "Face and Eye-He-Gu($Li_4$) (面目合谷收)" means when there are problems and disorders in facial, eye, ear, nose, throat, mouth regions, distinctively, such as facial edema, toothache, headache, sore throat, rhinorrhea, frontal headache, abdominal pain, dizziness, He-Gu can be a right choice for distant needling point for treating. 3. "Upper and lower back - Wei-Zhong($B_{40}$) (腰背委中求)" means when there are problems and disorders in upper and lower back, distinctively, such as upper back pain, lumbargo, hamstring muscle pain, popliteal region pain, lower extremity compartment syndrom, Inguinal region pain, muscle twitch, vomiting and diarrhea, hemorrhoidal bleeding, skin rash, Wei-Zhong can be a right choice for distant needling point for treating. 4. "Head and neck - Lie-Que($L_7$) (頭項審列缺)" means when there are problems and disorders in capital and nuchal area, distinctively, such as migraine, frontal headache, rhinorrhea, asthmatic dyspnea, aphasia, coughing, neck stiffness, occipital headache, upper extremity pain, Lie-Que can be a right choice for distant needling point for treating.

Research on the Subjective Symptoms of Musculoskeletal Disorders for Dental Hygienists in Daegu (대구지역 치과위생사의 근골격계 질환 자각증상 실태에 관한 조사연구)

  • To devise a plan to efficiently control and cope with musculoskeletal disorders, this study conducted a self-administered survey among dental hygienists working at dental clinics in Daegu. The data were collected from December 1, 2013 to February 28, 2014, and drew the following conclusions. For body posture for medical procedures, at lower levels in their careers, the hygienists' head and back were more bent; at higher levels in their careers, the their shoulders, body and hips were more bent; and at higher levels in their careers, their assistance posture in general was more bent. Hygienists with longer years of experience had more pain in the neck and the arms/elbows. Respondents engaged in medical treatment services were more likely to have pain in their arms/elbows, and those engaged in assisting in medical treatment and consulting and insurance services were more likely to have pain in their necks. It is necessary to recognize the seriousness of musculoskeletal disorders, provide specific education through supplementary training and relevant seminars, and avoid repetitive use of the same muscle or joint through environmental improvement of dental clinics, and personal efforts should be made to maintain good posture.

A Study on VDT Sydrome by Sasang Constitution of High School Students in Su-won City (수원지역 고등학생의 사상체질별 VDT증푸군에 대한 연구)

  • Objectives : This study investigated and made comparative studies of the aspect of VBT syndrome by Sasang constitutions, so that Korean medical doctors can manage and treat people who use the internet. Methods : Sasang constitutions were analyzed by using QSCCII. Questionnaires including VDT syndrome, internet usage trends and QSCCII were distributed to freshmen in a high school in Su-won city, Gyeonggi-do, and 600completed questionnaires were analyzed. Results : There were no significant differences among Sasang constitutions in the indexes of optic and mental problems in VDT syndrome. According to the indexes of total health problems. musculoskeletal problems, 'frequent fatigue', 'vacant head', 'dizziness', 'neck pain' and 'lower back pain' in VDT syndrome, Soeumin had the most problems and Taeumin had the least. In the index of 'back pain', in Soyangin had the most problems. Conclusions : There were some different trends of aspects of VDT syndrome among Sarang constitutions, so it is necessary to use the result of this study to make a plan to care for health problems related to using the internet through Korean medicine with additional studies.

The Sclerotherapy of Plunging with the Use of OK-432 (OK-432를 이용한 몰입성 하마종의 경화치료)

  • Background and Objectives: A plunging ranula represents a mucus escape reaction occuring because of the disruption of the sublingual salivary gland. It is commonly a condition of young adults, although the reported age range is 6-43 years. There is said to be a slightly female preponderance of about 1.9:1(F:M). Surgical incision has been considered the definite treatment, but sometimes complete excision is very difficult. Non-surgical treatment of these lesions has been attempted, but the results have not been satisfactory. In this study, we present our experiences with picibanil (OK-432) sclerotherapy for a plunging ranula. Materials and Methods: We retrospectively reviewed 41 patients who have undergone sclerotherapy with picibanil for plunging ranula. Information was collected on age, sex, a number of injection, post-sclerotherapy side effect and outcome of treatment. Results: 17 patients (41.5%) showed a complete response, and 20 patients (48.7%) showed inter-mediate response. No response was seen in 4 patients (9.8%). As a side effect of intracystic OK-432 injection therapy, fever (26.8%) and pain(39.0%) were observed. However, fever and pain disappeared after several days in all cases. Conclusion: The results are showing that OK-432 injection is an effective and safe treatment for plunging ranula.

The Effects of the Action Observation and Visual Feedback Convergence Exercise on the Alignment, Pain and Function of Forward Head Posture and Round Shoulder Posture (동작관찰과 시각적 되먹임 융합 운동이 머리전방자세와 둥근어깨의 정렬, 통증과 기능에 미치는 영향)

  • The aim of this study was to determine the effects of the action observation and visual feedback on the alignment, pain and function of forward head posture(FHP) and round shoulder(RS). A total of 24 participants with FHP and RSP were randomly assigned to general exercise(GE, n=8), action observation(AO, n=8), and visual feedback(VF, n=8). All subjects were exercised three times a week for four weeks. The groups were assessed for craniovertebral angle(CVA), round shoulder posture(RSP), visual analog scale(VAS), and neck disability index(NDI) before and after exercise, There was a significant difference in CVA in the GE, AO and RSP, VAS and NDI were significantly different in all groups. AO was more effective than GE, VF for VAS. The results of this study suggest that action observation may be effective to improve the FHP and RS.

Hip Resurfacing Arthroplasty after Failure of Tantalum Rod Insertion in Patients with Osteonecrosis of the Femoral Head

  • Purpose: The purpose of this study was to examine the clinical outcomes and efficacy of hip resurfacing arthroplasty (HRA) in patients with osteonecrosis of the femoral head after the failure of porous tantalum rod insertion without rod removal. Materials and Methods: Conversion to hip resurfacing arthroplasty was performed in 10 patients (11 hips) with a mean period of 14.9 months after the primary surgery. The mean follow-up period was 73.7 months. Analysis of pre and postoperative range of motion (ROM), University of California at Los Angeles (UCLA) activity score, modified Harris hip score, and visual analog scale (VAS) pain score was performed. Radiographic analysis of component loosening and osteolysis was performed. Results: The postoperative ROM showed significant improvement (P<0.05), excluding flexion contracture. The modified Harris hip score showed improvement from 65.82 to 96.18, the UCLA score showed improvement from 4.18 to 8.00, and the VAS pain score was reduced from 6.09 to 1.80. All scores showed statistically significant improvement (P<0.05). No component loosening or osteolysis was detected by radiographic analysis. Conclusion: Satisfactory results were obtained from conversion hip resurfacing arthroplasty after failure of porous tantalum rod insertion without rod removal. The findings of this study demonstrate the advantages of HRA, including no risk of trochanteric fracture and no bone loss around the tantalum rod. In addition, the remaining porous tantalum rod provided mechanical support, which reduced the potential risk of femoral neck fracture or loosening. This technique can be regarded as a favorable treatment option.
